A systematic review of the keystone design perforator island flap in the reconstruction of trunk defects

  • The keystone design perforator island flap can be utilized in the repair of trunk defects. A systematic review was carried out to identify the complication rates of the use of this flap to treat such defects. The MEDLINE, Embase, Cochrane Library, and PubMed Central databases were searched for articles published between January 2003 and December 2018 that reported the use of keystone design perforator island flaps in the repair of trunk defects. Study selection was conducted in alignment with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. Eight articles involving a total of 54 flaps satisfied the inclusion criteria. The most frequently reported cause of trunk defects was oncologic resection (64.4%). The overall complication rate was 35.2%, and complications included infection (11.1%), wound dehiscence (7.4%), delayed healing (7.4%), and partial flap loss (1.9%). The keystone design perforator island flap is associated with a high success rate and low technical complexity. Despite minor complications, keystone design flaps could be a preferred choice for trunk reconstruction.

A Case of Biliary Papillomatosis with Cystic Dilatation of Bile Duct (낭성 담도 확장을 동반한 담도 유두종증 1예)

  • A 61-year-old male who complained of right upper quadrant pain was referred to the authors for evaluation after his computed tomography suggested biliary adenocarcinoma. The lesion consisted of multiple cysts with papillary mass and peri-ampullay mass. The patient underwent an operation due to a clinical suspicion of biliary cystadenocarcinoma, but the pathology confirmed biliary papillomatosis (BP) after diagnosing intrahepatic papillary neoplasm with high-grade dysplasia and invasive adenocarcinoma with papillary neoplasm from the distal common bile duct to the duodenum. BP is a disease characterized by multiple papillary masses. Its cause has yet to be discovered. It commonly manifests as bile duct dilation but rarely as a ductal cystic change. Under computed tomography or magnetic resonance imaging, both the BP and the cystic neoplasm can show bile duct dilation and a papillary mass, which makes their differential diagnosis difficult. A confirmative diagnosis can be made through a pathologic examination. BP is classified as a benign disease that can become malignant and may recur, though rarely. Its treatment of choice is surgical resection. Laser ablation or photodynamic therapy can be used for unresectable lesions. In the case featured in this paper, biliary papillomatosis was difficult to differentiate from cystic adenocarcinoma due to diffusely scattered multiple large cystic lesions in the liver, and it was histologically confirmed to have become malignant with cystic duct dilation after the operation. Reconstruction of Medial Orbital Wall Fractures without Subperiosteal Dissection: The "Push-Out" Technique

  • Background Various surgical methods for repairing medial orbital wall fractures have been introduced. The conventional technique requires total separation of the displaced orbital bones from the orbital soft tissues. However, subperiosteal dissection around the fracture can cause additional damage. The aim of the present study is to introduce a method of reconstructing medial orbital wall fractures without subperiosteal dissection named the "push-out" technique. Methods Six patients with post-traumatic enophthalmos resulting from an old medial orbital wall fracture and 10 patients with an acute medial orbital wall fracture were included. All were treated with the push-out technique. Postoperative computed tomography (CT) was performed to assess the correct positioning of the implants. The Hertel scale and a comparison between preoperative and postoperative orbital volume were used to assess the surgical results. Results Restoration of the normal orbital cavity shape was confirmed by examining the postoperative CT scans. In the old fracture group, the median orbital volume of the fractured side was $29.22cm^3$ preoperatively, and significantly improved postoperatively to a value of $25.13cm^3$. In the acute fracture group, the median orbital volume of the fractured side was $28.73cm^3$ preoperatively, and significantly improved postoperatively to a value of $24.90cm^3$. Differences on the Hertel scale also improved, from 2.13 mm preoperatively to 0.25 mm postoperatively in the old fracture group and from 1.67 mm preoperatively to 0.33 mm postoperatively in the acute fracture group. Conclusions The push-out technique can be considered a good alternative choice for old medial orbital wall fractures with posttraumatic enophthalmos, acute medial orbital wall fractures including large fractured bone segments, and single-hinged greenstick fractures.

  • The aim of the present stud)r has been to obtain new and additional data allolwing a better design of sports garments as well as a better choice among some materiales, through measure-ment of body surface changes in the upper trunk in movement, measurement on the effects of local heating on other parts of the body and measurement of the thermal resistance of 6 types of materials applied on a manikin. In the first experiment, the upper trunk was divided in 32 Parts, the surface of which was measured by the tape method for two upper limb positions: extension at $90^{\circ}$ and $180^{\circ}$. In the second experiment, skin temperature, local thermal sensations and whole body thermal sensation were measured every 5 minutes during 40 minutes. The four areas of the shoulder, abdomen, hande and feet were heated with the hot pack at $50^{\circ}C$. In the third experiment, the regional thermal resistance of the various materials selected, in two different cases of clothing ease, have been measured by using a thermal manikin. Resultes of experiments were: 1. Extensions cause the upper front part of the trunk surface to lengthen vertically while the back tends to stretch in width. 2. Skin temperatures of the upper limbs are influenced by the abdomen and shoulder boatings. The correlation between the whole body thermal sensation and the upper trunk thermal sensation is significantly asserted. 3. Ceramic and aluminium coated materiales offer the most effective thermal resistance; ease in clothing increases the thermal resistance at the breast and the abdomen as well as the clo value of the materials.

Conclusion In this essay I considered the relations of records and history, namely the process on the selection and choice from the records to the archives and the historical materials and in the process the role on the archivists and historians and difference and the relation of the facts and theory in the history. Archival science posits that an archives is the whole of the documents made or received in the course of purposeful activity, and of the relationships among those documents. Archival theory posits that it is the primary function of the archivists to maintain unbroken, continuing custody of societal archives, and to protect their integrity by keeping them physically and intellectually uncorrupted. The ultimate purpose of archival endeavors is to hand down to the next generations a reliable, trustworthy, and complete testimony of societal actions so that they can constitute sources of, and foundations for, future decision making. However, historical science searches for the truth in the historical facts, explains the cause and origin of the matters and reflects on th knowledge about the why and how the events happened. And history is firmly rooted on the philosophy. The goal of historian is analysing the nature of meaning of past in the society and tracing up the change and process of the transition. Archival science means historical source studies and other related disciplines. On the other hand archival management is the principle of respect for original order and the importance of original forms and conditions. Thus the relations between archival science and archives management are not one-sided but mutual. Archival science and historical studies, archivists and historians need to link more closely in order to hand down the contemporary memorial, the common memorial of mankind to the future.

  • Tearing of the acetabular labrum is a common cause of chronic hip pain. MR arthrography (MRA) of the hip is the imaging procedure of choice for the evaluation of acetabular labrum. Familiarity with the various imaging findings of MRA of the hip allows recognition of normal variants and differentiation from true pathologic conditions. This article comprehensively reviews the technical aspects and interpretation of MRA of the hip. The appearances of normal and abnormal labra in MRA are discussed. Potential pitfalls in labral tear interpretation, such as sulci around the hip, normal variants of the labrum and plicae, and osseocartilaginous and soft tissue lesions identified on MRA of the hip are also described.

  • This study researches the attitudes of nursing students toward induced abortion. It aims to Identify the structure of subjectivity of nursing students toward induced abortion. As the research method, Q-statements were collected preliminary to the study through in-depth group talking, case study and literature review. For the study, 28 Q-statements were selected. There were 40 students as subjects for the research. The 40 students sorted the 28 statements using the principle of Forced Normal Distribution. The results of the study were as follows : Three attitude types toward induced abortion were derived from Q-Factor analysis by using PC Quanal Program. The first type is called qualified deontology type. The second type is called rational utility type. The third type is called self-centered utility type. 1) Qualified deontology type emphasizes the importance of respect for life. Induced abortion is acceptable within just limit circumstances. 2) Rational utility type has more tolerance limits. There are rationale for induced abortion. Thls type hates becoming the target of criticism because they are single mothers, were rape or had a deformed baby. 3) Self-centered utility type believe that the most important in the world is one's self. This type emphasizes the importance of women's autonomy. And then the woman should be able to decide about abortion, because it is her body, it is her choice. The result of the study suggests that nursing education need more ethical teaching of the importance of human life. Be cause the fundmental concept of nursing is in the preservation of human life and in the respect for life, dignity and the rights of man. Nursing ethics is a essential for nurses.

ERUPTION AND AUTOTRANSPLANTATION OF A PERMANENT TOOTH RELATED TO DENTIGEROUS CYST IN MIXED DENTITION (혼합치열기의 함치성 낭종 치료 시 영구치의 맹출과 자가치아이식)

  • Purpose: Dentigerous cyst is the most common intra-osseous lesion of the jaw. Dentigerous cysts can cause delays in eruption of the affected permanent tooth in mixed dentition. It has been suggested that the affected permanent tooth could be erupted spontaneously after the dentigerous cyst was enucleated. But in some cases, orthodontic treatment or autotransplantation technique is known to be required. This study reviews previously performed prognoses of affected permanent teeth, which will lead to a more efficient treatment plan. Patients and Methods: With 28 patients who have undergone cyst enucleation and 10 patients who have undergone autotransplantation, the prognosis of permanent teeth was observed. Results: After cyst enucleation, spontaneous eruption of a permanent tooth was observed in 56.3% patients, orthodontic treatment was performed in 25% patients. The success rate of autotransplantation was 60.0%. Discussion: The first choice for treatment of dentigerous cyst in mixed dentition is to guide spontaneous eruption of permanent teeth. For cases without enough eruption space, the orthodontic treatment should be considered. Autotransplantation should be considered when the spontaneous eruption is not expected. It should be considered that the rate of successful autotransplantation is decreased on maxilla anterior area.

Impact of a Breast Health Awareness Activity on the Knowledge Level of the Participants and its Association with Socio-Demographic Features

  • The developing countries have higher mortality rates for breast cancer. A reason for this is presentation at advanced stages due to low levels of public awareness. Activities are arranged by health authorities of developing countries to increase the knowledge of women but their effectiveness has not been evaluated in detail. A multiple choice questionnaire with questions about socio-demographic profile and questions about breast cancer knowledge was designed in local language Urdu, to evaluate the knowledge of the participants before and after an audio visual educational activity in Lahore, Pakistan. Scores of 0-2, 3-5 and 6-8 were ranked as poor, fair and good, respectively. Among 146 participants these scores were achieved by 1%, 55% and 45% before activity and 0%, 16% and 84% after the activity. Overall 66% of participants increased their knowledge score. Younger age, higher education, reliance on television as source of information and being a housewife were associated with better impact of the awareness activity. For the six knowledge related questions 3%, 5%, 11%, 23%, 33% and 44% more participants gave correct answers after the activity. However 6% and 7% fewer participants answered correctly for 2 questions related to the cause and the best prevention for breast cancer. The study indicated that awareness activities are effective to increase the knowledge of women and better impact is associated with higher education and younger age of women. The component analysis showed that the questions and related presentations using medical terms have a negative impact and should not therefore be used. Analysis of activity therefore leads to identification of deficiencies which can be remedied in future.

  • Many patients assume that allergic reactions against foods are responsible for triggering or worsening their allergic symptoms. Therefore, it is important to identify patients who would benefit from an elimination diet, while avoiding unnecessary dietary restrictions. The diagnosis of food allergy depends on the thorough review of the patients's medical history, results of supplemented trials of dietary elimination, and in vivo and in vitro tests for measuring specific IgE levels. However, in some cases the reliability of such procedures is suboptimal. Oral food challenges are procedures employed for making an accurate diagnosis of immediate and occasionally delayed adverse reactions to foods. The timing and type of the challenge, preparation of patients, foods to be tested, and dosing schedule should be determined on the basis of the patient's history, age, and experience. Although double-blind, placebo-controlled food challenges(DBPCFC) are used to establish definitively if a food is the cause of adverse reactions, they are time-consuming, expensive and troublesome for physician and patients. In practice, An open challenge controlled by trained personnel is sufficient especially in infants and young children. The interpretation of the results and follow-up after a challenge are also important. Since theses challenges are relatively safe and informative, controlled oral food challenges could become the measure of choice in children.
