  • Aryl hydrocarbons, environmental contaminants accumulate in tissue and pose potential risk in human health. 2,3,7,8-Tertachlorodibenzo-p-dioxin (TCDD) is known as a most potent toxicant among aryl hydrocarbons. TCDD elicits numerous toxic responses in experimental animals and human, including hepatic carcinoma, pulmonary and skin tumor in adult rodents, craniofacial abnormality during mouse embryogenesis, chloracne, reproductive abnormality, immunotoxicity, endocrine effects in exposed humans.(omitted)

  • 펜타클로로페놀의 현재 직업적 노출기준은 $0.5\;mg/m^3$(TLV-TWA)으로 권고하고 있다. 이 수치는 눈, 점막과 상기도의 염증과 발한, 열, 위장 불만, 시각 장해, 중추 신경계와 심혈관계 장해를 유발할 수 있는 급성 독성의 가능성을 최소로 하는 것을 목적으로 설정하였다. Chloracne는 공업용 펜타클로로페놀을 사용하거나 펜타클로로페놀을 생산하는 근로자들에 대해서 보고하였다. 펜타클로로페놀은 피부를 통해서 즉시 흡수되는데, 이 물질의 조직 독성은 펜타클로로페놀을 포함한 용액이나 펜타클로로페놀로 오염된 의류와 접촉된 사람에게서 보고되었다. 따라서 피부경고주석은 적합한 것으로 판단되었다. 펜타클로로페놀의 발암성 잠재력에 대한 명확한 증거는 간세포성과 신장에 가까운 신생물이 관찰된 설치류 생물 검정에서 발견되었다. 그러므로 본 물질은 인간에게 불명확한 관련성을 가진 동물성 발암물질(A3)로 설정하였다. 충분한 자료가 확보되지 않아 감작성 (SEN) 경고주석 또는 TLV-STEL은 권고되지 않았다. 펜타클로로페놀은 생물학적 노출지수(BEIs)가 권고된 물질이다.

Dioxins and Health: Human Exposure Level and Epidemiologic Evidences of Health Effects (다이옥신과 건강: 인체 노출 수준 및 건강영향에 대한 역학적 연구)

  • General information is summarized, that is necessary to introduce a scientific assessment of the human health and exposure issue concerning dioxin and dioxin-like compound. Scientific literatures were reviewed to assess the background exposures to the dioxin-like compounds for normal residents. Epidemiologic studies were also reviewed to assess malignant and nonmalignant sweets of dioxins. In 1997, the International Agency for Research on Cancer classified 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) as a human carcinogen, primarily based on occupational cohort studies. The US Environmental Protection Agency made the same decision in it's Draft Dioxin Reassessment. Epidemiologic evidences point to a generalized excess of all cancers, without any pronounced excess at specific sites. Reported non-cancer effects included a range of conditions affecting most systems. Among them, chloracne, elevation in gamma glutamyl transferase(GGT), and alterations in reproductive hormones are related to TCDO, Other adverse outcomes, such as lipid concentrations, diabetes, circulatory and heart diseases, immunologic disorders, neurobehavioral effects, and developmental outcomes require further study before their respective relationships to TCDD can be more definitively assessed.

  • Dioxins, especially 2, 3, 7, 8-Tetrachlorodibenzo-p-dioxin (TCDD or dioxin), are ubiquitous environmental contaminants. TCDD is known that it has toxic effects in animals and humans, including chloracne, immune, reproductive and developmental toxicities, carcinogenicity, wasting syndrome and death. TCDD induces a broad spectrum of biological responses, including disruption of normal hormone signaling pathways, reproductive and developmental defects, immunotoxicity, liver damage, wasting syndrome and cancer. Many researches showed that TCDD induces gene expression of transcriptional factors related cell proliferation, signal transduction, immune system and cell cycle arrest at molecular and cellular levels. These toxic actions of TCDD are usually mediated with AhR (receptor, resulted from cell culture, animal and clinical studies). cDNA microarray can be used as a highly sensitive and informative marker for toxicity. Additionally, microarray analysis of dioxin-toxicity is able to provide an opportunity for the development of candidate bridging biomarkers of dioxin-toxicity. Through microarray technology, it is possible to understand the therapeutic effects of agonists within the context of toxic effects, classify new chemicals as to their complete effects on biological systems, and identify environmental factors that may influence safety.

A Study on the Health Status of Korean Vietnam Veterans' Children - A Result of Questionnaire Survey on Vietnam Veterans of Pusan Area Who Diagnosed as Cases by Korean Veteran's Hospital Diagnostic Criteria - (베트남전 참전자 2세의 건강상태에 관한 조사 - 부산지역 고엽제 위해증 환자를 대상으로 한 설문조사 결과 -)

  • Objectives : To propose the referential data to evaluate the health impacts of Vietnam veterans' children whose father were exposed to herbicides in Vietnam War. Methods : Vietnam veterans who visited to Pusan Veteran Hospital for medical care were recruited from April to October, 1998. They were 71 and asked about their own combat history, symptoms and illness, and health status of their 182 children. The informations were collected by direct and phone interview. Exposure estimation was also performed as exposure score depending on year and unit of participation, and personal episodes related to exposure to herbicide in the war. It classified into three groups; lower(<18.0), moderate(18-53), high$(\geq53)$ exposure group. Results : The mean age and the period into the combat of the veterans were 52.8 years and 15.0 months. The mean exposure score was $18.1{\pm}9.9$, and mainly distributed in lower (46.5%) and moderate(52.1%) exposure group. Most(90.1%) of them were diagnosed as sequelae(21 cases) and suspected sequelae(43 cases) of the herbicides by Korean veteran's hospital diagnostic criteria. The major sequelae was peripheral neuropathy 13 cases, chloracne 5 cases, and the major suspected sequelae was hypertension 20 cases, diabetes mellitus 18 cases, liver disease 12 cases, central neuropathy 11 cases, etc. About birth, 42.2% and 16.9% experienced spontaneous abortion and stillbirth, respectively. The mean exposure score was higher in stillbirth experience group(p<0.05). About half of the children(90 cases, 49.5%) hold the abnormal health status: those were skin pigmentation 38 cases, rash 23 cases, congenital anomaly 15 cases, general weakness 12 cases, purpura 8 cases, visual disturbance 8 cases, etc. These health problems had no association with father's exposure level(p>0.05). Conclusions : These results were depend on their own answers, and expectation for compensation did not excluded, therefore, this study may have limitations: inaccuracy of informations due to recall bias and response bias. Nevertheless, through this study, we could image the fundamental aspect for health impacts of Vietnam veterans' children for preparing the national control program and policy. A large scale epidemiologic study with valid exposure assessment on the health impacts of Vietnam veterans' children is recommneded.
