Follicular bronchitis/bronchiolitis is pathologically characterized by peribronchiolar lymphoid follicles, which is one of reactive pulmonary lymphoid disorders. It is associated with 1) the result of infections such as mycoplasma, chlamydia etc., 2) immunodeficiency syndromes, 3) connective tissue diseases such as rheumatoid arthritis and Sjogren's syndrome and 4) local or systemic hypersensitivity reaction. And it can be also developed without obvious causes and associated diseases(idiopathic). Radiologically it represents as bilateral interstitial patterns of pulmonary infiltrates. In this case, a 49 year-old woman was presented with intermittent cough and sputum. On chest X-ray and CT, $5\times4$ cm sized mass in right upper lobe and paratracheal lymphadenopathies were detected, by which lung malignancy was suspected. Bronchoscopy, trans bronchial lung biopsy and transthoracic needle aspiration showed non-specific findings only. After right upper lobectomy, we could confirm a case of follicular bronchiolitis which presented as an unusual mass-like radiologic finding.
본 연구에서는 berberine이 세포성 면역에 미치는 효과를 보기위하여 림프구증식과 GM-CSF생성을 보고자 하였다. 마우스 비장세포에 berberine을 작용시켰을 때 저농도에서 세포성장을 촉진함으로서 림프구증식에 자극제로서의 역할을 담당할 것으로 보인다. GM-CSF의 생성은 berberine 단독처리에 의해서 증가됨으로서 GM-CSF생성의 증가로 인한 세포성 면역반응의 상승이 예상되고 LPS나 Con A에 의한 GM-CSF의 생성을 저해함으로서 과도한 GM-CSF생성을 중간에서 차단할 것으로 보인다. 따라서 이러한 결과들은 앞으로의 추가적인 연구결과들이 있게 되면 임상에서의 면역요법과 항염증작용에 berberine이 이용될 가능성을 시사한다 하겠다.
Background: Chlamydophila felis, formerly known as Chlamydia psittaci var. felis, is frequently associated with ocular, respiratory, and occasionally reproduction tract infections. Even though the infection is sometimes asymptomatic, it potentially results in a latent immunosuppressive infection. Objective: This study aimed to identify occurrences of feline chlamydophilosis, rarely reported in cats in Indonesia. Methods: The observation was conducted in three cats with clinical signs of Cp. felis infection, particularly relapsing conjunctivitis. The cats' histories were recorded based on owners' information. Conjunctival swabs were sampled for cytology examination and molecular assay detection. A phylogenetic tree was generated using MEGA-X software to reveal group clustering. A post-mortem examination was performed on the cat that died during an examination. Results: Cp. felis was detected in both cytological examination and polymerase chain reaction assay. The phylogenetic tree demonstrated that the Cp. felis isolated in this study clustered with several other isolates from the other countries. Cp. felis can be isolated from cats with different clinical manifestations and levels of severity. The chronic fatal infection demonstrated interstitial broncho-pneumonia under histopathological examination. Conclusions: Molecular assay of Cp. felis is always recommended to obtain a definitive diagnosis of feline chlamydophilosis since the disease can have various clinical manifestations. Even though it may be subclinical and is often not fatal, an infected cat may be a carrier that could spread the pathogen in the surrounding environment. Serious disease management is suggested to avoid high costs associated with regularly relapsing disease.

Short-chain fatty acids (SCFAs) are metabolic products produced during the microbial fermentation of non-digestible fibers and play an important role in metabolic homeostasis and overall gut health. In this study, we investigated the effects of supplementation with multispecies probiotics (MSPs) containing Bacillus amyloliquefaciens, Limosilactobacillus reuteri, and Levilactobacillus brevis on the gut microbiota, and fecal SCFAs and lactate levels of weaned pigs. A total of 38 pigs weaned at 4 weeks of age were fed either a basal diet or a diet supplemented with MSPs for 6 weeks. MSP administration significantly increased the fecal concentrations of lactate (2.3-fold; p < 0.01), acetate (1.8-fold; p < 0.05), and formate (1.4-fold; p < 0.05). Moreover, MSP supplementation altered the gut microbiota of the pigs by significantly increasing the population of potentially beneficial bacteria such as Olsenella, Catonella, Catenibacterium, Acidaminococcus, and Ruminococcaceae. MSP supplementation also decreased the abundance of pathogenic bacteria such as Escherichia and Chlamydia. The modulation of the gut microbiota was observed to be strongly correlated with the changes in fecal SCFAs and lactate levels. Furthermore, we found changes in the functional pathways present within the gut, which supports our findings that MSP modulates the gut microbiota and SCFAs levels in pigs. The results support the potential use of MSPs to improve the gut health of animals by modulating SCFAs production.
Pathogens such as feline herpesvirus, feline calicivirus, Bordetella bronchiseptica, Chlamydia felis, Mycoplasma felis and Pasteurella multocida usually cause feline upper respiratory tract disease (URTD). Real-time PCR was used to analyze the detection and prevalence of the most common respiratory pathogens in cats with (n=69) and without respiratory signs (n=31). Pathogens were detected in 53 cats, divided into 37 (69.8%) with a single pathogen, 15 (28.3%) with two pathogens, and 1 (1.9%) with three pathogens. M. felis had the highest detection rate in 29 (42.0%) cats, P. multocida was detected in 18 (26.1%), FHV in 10 (14.5%), FCV in 7 (10.1%), B. bronchiseptica in 3 (4.3%), and C. felis in 2 (2.9%). M. felis was the most frequently detected pathogen in cats living outdoors without vaccination. Of the 37 cats infected with single pathogen, nasal discharge was observed in 13 (35.1%), ocular signs in 6 (16.2%), drooling in 5 (13.5%), dyspnea in 3 (8.1%), and asymptomatic in 10 (27.0%). In 51 outdoor and 49 indoor cats, pathogens were detected in 35 (68.6%) and 18 (36.7%) cats, respectively. Of the 29 cats infected with M. felis, 22 (75.9%) showed respiratory signs, and 7 (24.1%) were healthy. In the age of the 53 positive cats, 10 (18.9%) were under the age of 1 year, 26 (49.1%) were aged 1~3 years, and 17 (32.1%) were aged 3 years or older. Although the number of cats in the study was small, the results can provide valuable data on the prevalence of URTD in Korean cats.
연구배경 : Moxifloxacin은 광범위한 미생물에 대한 뛰어난 효과, 안전한profile과 유리한 약물동력을 바탕으로 기존의 fluoroquinolones보다 한단계 진보된 약물이라고 할 수 있다. 이러한 효과는 베타-락탐계 혹은 기타 항생제에 내성을 보이는 미생물들에 대해 매우 유효하다. Moxifloxacin은 특히 Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis, Chlamydia pneumoniae, Legionella spp. 그리고 Mycoplasma pneumoniae에 의해 유발되는 호흡기 감염에 특히 유용하다고 알려져 있다. 방법 : 다기관, 무작위, 공개, 비교시험으로 만성기관지염의 급성악화 환자를 대상으로 moxifloxacin 400 mg 1일 1회 7일 투여와 clarithromycin 500 mg 1일 2회 7일 투여 그룹간의 안전성과 유효성을 비교하였다. 결과 : 170명의 환자 중에서 87명이 moxifloxacin 투여군 이었고 83명이 clarithromycin 투여군이었다. Moxifloxacin 투여군종에서 76명(세균학적 효과분석은 35명), clarithromycin 투여군중에서 77명(세균학적 효과분석은 31명)이 치료효과분석에 포함되었으며 모든 환자에서 안전성을 분석하였다. 임상적 성공율은 Moxifloxacin 투여군 76명중에서 70명(92.1%) 이었고 clarithromycin 투여군 77명중에서 71명(92.2%) 으로 유사하였다. 세균학적 Moxifloxacin 투여군이 73.5%, clarithromycin 투여군이 54.8% 으로 Moxifloxacin 투여군에서 높게 나타났으나 통계적으로 유의한 차이는 보이지 않았다(p=0.098). 내원시 시행한 객담의 세균 감수성 검사에서 moxifloxacin 투여군이 clarithromycin 투여군보다 Streptococcus pneumoniae, Pseudomonas aeruginosa, Klebsiella pneumoniae에서 높은 감수성을 보여주었다(p<0.001). 이상반응은 moxifloxacin 투여군 86명중에서 12.8% 에서 나타났고, clarithromycin 투여군 83명중에서 21.7%에서 발생하였다. 두통(4.7% vs 4.8%, 각각 moxifloxacin 투여군 vs clarithromycin 투여군)과 소화불량(2.3% vs 6.0%, 각각 moxifloxacin투여군 vs clarithromycin 투여군) 이 두 군에서 가장 흔한 이상 반응이었다. 결론 : 본 임상시혐의 결과, 만성기관지염의 급성악화 환자의 치료에 있어서 moxifloxacin 400 mg 1일 1회 7일간 요법은 clarithromycin 500 mg 1일 2회 요법과 비교할 때 임상적으로 동등한 결과를 나타냈으며 미생물학적으로 우월한 결과를 보여주었다.
This study explored epidemiological trends in trichomoniasis in Daegu, South Korea. Wet mount microscopy, PCR, and multiplex PCR were used to test for Trichomonas vaginalis in vaginal swab samples obtained from 621 women visiting 2 clinics in Daegu. Of the 621 women tested, microscopy detected T. vaginalis in 4 (0.6%) patients, PCR detected T. vaginalis in 19 (3.0%) patients, and multiplex PCR detected T. vaginalis in 12 (1.9%) patients. Testing via PCR demonstrated high sensitivity and high negative predictive value for T. vaginalis. Among the 19 women who tested positive for T. vaginalis according to PCR, 94.7% (18/19) reported vaginal signs and symptoms. Notably, more than 50% of T. vaginalis infections occurred in females younger than 30 years old, and 58% were unmarried. Multiplex PCR, which simultaneously detects pathogens from various sexually transmitted infections, revealed that 91.7% (11/12) of patients were infected with 2 or more pathogens. Mycoplasma hominis was the most prevalent co-infection pathogen with T. vaginalis, followed by Ureaplasma urealyticum and Chlamydia trachomatis. Our results indicate that PCR and multiplex PCR are the most sensitive tools for T. vaginalis diagnosis, rather than microscopy which has been routinely used to detect T. vaginalis infections in South Korea. Therefore, clinicians should take note of the high prevalence of T. vaginalis infections among adolescent and young women in order to prevent persistent infection and transmission of this disease.

Trichomonas vaginalis, a causative agent of trichomoniasis, may trigger symptomatic or asymptomatic non-gonococcal urethritis and chronic prostatitis in men. Despite the availability of highly sensitive diagnostic tests, such as nucleic acid amplification tests, including PCR, few prospective studies present data on male T. vaginalis infection in South Korea. In the present study, the prevalence of T. vaginalis and associated clinical conditions were evaluated in 201 male patients from a primary care urology clinic in South Korea. The prevalence of T. vaginalis infection in our cohort was 4% (8/201) by PCR. T. vaginalis infection was common in men older than 40 years (median age, 52 years). Among the 8 Trichomonas-positive patients, 87.5% (7/8) had prostatic diseases, such as prostatitis and benign prostatic hyperplasia, and 25.0% (2/8) and 12.5% (1/8) were coinfected with Chlamydia trachomatis and Mycoplasma genitalium, respectively. Our results suggest that T. vaginalis infection is not rare in men attending primary care urology clinics in South Korea, especially in those older than 40 years, in whom it may explain the presence of prostatic disease. The possibility of T. vaginalis infection should be routinely considered in older male patients with prostatic diseases in South Korea.
목 적:호흡기 감염의 증상이 없는 소아들을 대상으로 다중 역전사중합효소연쇄반응법(multiplex reverse transcription-polymerase chain reaction; mRT-PCR)을 이용하여 비인두 상주균의 이환율을 알아보고자 하였다. 방 법:2008년 7월 25일부터 28일까지 33명의 소아들을 대상으로 비강 면봉채취법으로 검체를 채취하였으며, 이들은 검체 채취 당시 심각한 호흡기 감염의 증상이 없었다. 모아진 검체에서 DNA를 추출한 후 multiplex primer set ($Seeplex^{(R)}$ PneumoBacter ACE Detection, Seegene, Seoul, Korea)로 PCR을 진행하였다. 증폭된 반응산물은 2% agarose gel과 전기 영동 자동화 시스템인 screen tape system (Lab901, Scottland, UK)에 각각 전기 영동하여 확인하였다. 결 과:전체 33명의 소아 중 남아는 15명 여아는 18명이었으며, 나이는 3.2세에서 16.3세로 중앙값은 8.2세였다. mRT-PCR 결과 30명(90.9%)의 소아들에서 양성을 보였으며(S. pneumoniae, H. influenzae, C. pneumoniae, B. pertussis), 이들 중 13명(39.4%)에서 2가지 이상의 균이 검출되었다. 균의 종류로는 12명(36.4%)에서는 S. pneumoniae와 H. influenzae, 1명(3.0%)에서는 S. pneumoniae, H. influenzae와 C. pneumoniae이 검출되었다.. 결 론:mRT-PCR은 비인두 상주균의 동정에 있어서 민감도가 높은 방법으로 생각된다. 하지만 비인두 상주균에 대한 PCR 결과가 소아들의 임상 양상과 어느 정도 일치할지에 대해서는 더 많은 연구가 필요할 것으로 생각된다.
현재 성병은 성매개 질환(sexually transmitted disease, STD)은 무증상 감염도 포함하는 의미에서 "성매개 감염"(sexually transmitted infections, STI)이라고 표현하고 있다. STI는 다양한 상호 관계를 가지고 있다. 본 연구는 보건복지부 장관이 고시하는 STI로 매독, 임질, 클라미디아 감염증, 연성하감, 성기단순포진, 첨규콘딜로마, 사람유두종바이러스 감염증과 비임균성요도염(non-gonococcal urethritis, NGU)을 대상으로 하였다. 따라서 다발성 및 동시성 STI를 확인하여 요인을 특성화하는 것을 목적으로 하였다. 본 연구는 2014년부터 2019년까지 서울에 위치한 일개 수탁검사기관의 검사실정보시스템(laboratory information system)의 자료를 활용하였다. 본 연구에서 다발성 STI로 두 가지 감염균의 중복 STI는 10 종류, 세 가지 감염균의 다중 STI는 6 종류로 나타났다. 이러한 16 종류의 다발성 STI 중 가장 많은 비중을 차지하는 균은 U. urealyticum (9가지), HSV-2 (8가지), C. trachomatis (7가지), HPV 6, 11 (7가지), N. gonorrhoeae (6가지), T. pallidum (1가지)에 포함되었다. 따라서 본 연구 결과에서 다발성 STI에 가장 비중이 높은 STI 등 상호 관계연구의 추가연구가 필요할 것으로 사료된다.
