Objectives: The study aimed to understand the current treatment patterns in Korean medicine to develop clinical practice guidelines for autonomic dysfunction in Korean medicine. Methods: This study sent an online survey vai text message to 25,900 Korean medicine doctors whose contact information was registered with the Association of Korean Medicine. A total of 1,410 Korean medical doctors completed the online survey. Results: When autonomic treating dysfunction clinically, 77% of the cases included only a description without entering a diagnosis code. The most commonly used information to diagnose o autonomic dysfunction was history-taking and symptoms (79%), and the main symptoms of autonomic dysfunction were palpitations, dizziness, sleeping difficulties, anxiety/nervousness, and depression/lethargy. The most frequently mentioned cause of autonomic dysfunction was mental problems (54%). The most commonly used Korean medicine treatment method for autonomic dysfunction was herbal medicine (70%), and Soyo-san/Gamisoyo-san is the most frequently used herbal medicine preparation. Liver qi depression used to indicate the most often mentioned Korean medicine pattern identification used to indicate autonomic dysfunction (31%). When asked whether cardiac neurosis in Chinese medicine can be considered autonomic dysfunction, opinions for and against it are determined almost equally. Conclusions: Our results serve are a foundation for developing clinical practice guidelines for autonomic dysfunction in Korean medicine and are expected to catalyst promoting future clinical research on autonomic dysfunction.
Objectives : This study was carried out to know the status of clinical utilization of Korean medical knowledge and the Korean medical knowledge system which Korean medical doctors want. Methods : Questionnaires were distributed to Korean medical doctors registered for the Association of Korean medicine via the email and 249 answered questionnaires were analyzed. Results : 50.6 % of Korean medical doctors utilized the textbooks for Korean medicine when they treated their patients. The reason why they did not utilize the textbooks for Korean medicine was mostly that 'it could not be applied directly into clinical application with only contents of textbook because clinical approach (diagnosis and treatment methods) was not clearly described in the textbook'. 48.6 % of Korean medical doctors utilized the EBM materials when they treated their patients. EBM materials that they referred included domestic articles, other EBM materials excluding papers and international academic papers based on PubMed in order. The reasons why they did not utilize the EBM materials were mostly that 'it was difficult to find the evidence' and 'EBM materials were not actually helpful for the clinical practice.' It was found that Korean medical doctors approached the disease in aspects of Korean and Western Medicine concurrently upon the diagnosis of patients and performed the diagnosis and treatment of Korean medicine on the diseases which seemed to be effective with Korean medicine. It was also found that they thought that they needed to know the Western medical knowledge as much as general doctors who have a doctor's license or family medicine specialists who comprehensively treat the general diseases know. Korean medical doctors wanted to have the systematic summary of modern research performance in fields of acupoint, acupuncture, herbal drugs and formula. They thought that constitutional medicine and Sa-am acupuncture therapy were representative characteristics in Korean medicine differentiated from Traditional Chinese medicine and Kampo medicine. Conclusions : Korean medical doctors wanted to establish the Korean medical knowledge system based on evidence focusing on clinical and practical contents. New Korean medical knowledge system should be established based on these requirements in the future.
Objectives : Betula Platyphylla(BP) is a traditional analgesic, anti-fungal, anti-inflammatory herb used in Chinese 1medicine. However, no information is available to explain its action. In this study. we investigated the anti-inflammatory 1effects of BP to elutidate the molecular pharmacological activity in the ethanol extract of BP(BPE). Methods : We performed WTS assay in lipopolysaccharide (LPS) stimulated RAW264.7 macrophages with BPE. Nitrite was measured by Griess assay, prostaglandin E2 (PGE2) by enzyme-linked immunosorbent assay (ELISA) in LPS induced RAW264.7 macrophages with BPE. Inducible nitric oxide synthase (iNOS) and cyclooxygenase (COX-2) were determined by Western blot. Activation of nuclear factor-kappaB (NF-kB) was measured by electrophoretic mobility shift assay (EMSA). Results : BPE significantly suppressed production of nitric oxide (NO) and PGE2 in LPS-induced RAW264.7 macrophages in a dose-dependent manner. The maximal inhibition rate of NO and PGE2 production by BPE was ca. 88.8% and 93% at the concentration of $100{\mu}g/ml$ (non-cytotoxic concentration), respectively. BPE also decreased iNOS protein and COX-2 protein in LPS-induced RAW264.7 macrophages. EMSA demonstrated that BPE inhibited the DNA binding activity of the NF-kB. Conclusions : These results suggest that BPE inhibits NF-${\kappa}B$-mediated gene expression and downregulates inflammatory mediator production in RAW264.7 macrophages.

Objective : This study was performed to find out the differential points of the pattern identification for Acne vulgaris and suggest them to the clinic setting Methods : We searched the papers that suggest patterns identification for Acne vulgaris through China National Knowledge Infrastructure(CNKI), Oriental Medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), Korean Studies Information Service System(KISS). We looked through all the papers, excluded unintended papers, and finally chose some papers that are suitable for inclusion. Then, We extracted all the patterns of Acne vulgaris and chose frequently suggested patterns. Finally we extracted symptoms from selected patterns and arranged them in order of frequency. Results and Conclusions : 1. We found 215 papers and chose 17 papers (Korean : 6, Chinese : 11). 2. We extracted total 67 demonstrations and chose Patterns of Wind-Heat, Damp-Heat, Phlegm and Static blood, Disharmony of the Thoroughfare and Controlling vessels. 3. In Acne vulgaris, the pattern of Wind-Heat tends to have a red papule, sometimes pustule, pain with itching, burning sensation, tidal reddening of the cheeks, white head or black head. The pattern of Damp-Heat tends to have a pustule, node, redness and swelling, pain and greasy skin. The pattern of Phlegm and Static blood has a tendency to be a nodule or cystoma that is hard, dark, painful, long lasting and hard to recover and easy to recur with a scar, pimple, pigmentation. The pattern of Disharmony of the Thoroughfare and Controlling vessels tends to be exacerbated before menstruation, have a small papule that is not easily treated under the chin or around the mouth.
Objectives : The glial cell, located in between the blood vessel and nerve cell, takes charge of the cell support, nutrition supply, elimination of body waste, and cell action. GagamSohabhwangwon(GGSH), a chinese traditional medicinal prescription has been used orally for the treatment of seizures, infantile, convulsion, stroke and so forth. This paper examines the effect of the GagamSohabhwangwon(GGSH) essential oil on cell activity and anti oxidation. Methods : MTT assay methods were employed to measure the cell activity based on the amount of the GagamSohabhwangwon(GGSH) essential oil by using primarily cultivated glial cell. In addition, this paper measured a viability of the glial cell after a protein active retarder control to confirm the multiplication of the cell and examined the cell extinction by the active oxygen, an extinction shielding effect with different amount of the GagamSohabhwangwon(GGSH) essential oil to observe anti oxidation. Furthermore, this paper measured a viability of the cell and phosphorylation(phosphorylation) of the protein which affects the multiplication of the glial cell. Results : When controlling the amount of the GagamSohabhwangwon(GGSH), there was a multiplication effect of the primary glial cell, the multiplication of the cell was dependent on the density of the GagamSohabhwangwon. The multiplication power of the primary glial cell was suppressed by PKA inhibiter (H89). In compliance with the active oxygen the extinction of the primary glial cell was dependent on the density of the GagamSohabhwangwon, there was a shielding effect of the cell extinction when GagamSohabhwangwon(GGSH) was preprocessed. When inducing the multiplication of the primary glial cell, phosphorylation of the Akt, BDNF, CREB, ERK and ERM were increased. Conclusions: Based on the results, GagamSohabhwangwon essential oil will have the effect which activates the nervous system cell and protects the cell through anti oxidation.

By using high-performance liquid chromatography-photodiode array detection, a simple and accurate chromatographic fingerprint method was developed for the identification of Radix Codonopsis (roots of Codonopsis) from different sources. Eighteen herbs of Codonopsis at different habitats in China, including roots from Codonopsis pilosula, Codonopsis pilosula var. modesta and Codonopsis tangshen were analyzed by the fingerprint. The amount of lobetyolin was calibrated, which was found to be more consistent in roots of Codonopsis pilosula as compared to that of Codonopsis pilosula var. modesta and Codonopsis tangshen. Having the fingerprint results, hierarchical clustering analyses were performed to classify the eighteen herbs into three groups: Codonopsis pilosula, Codonopsis pilosula var. modesta and Codonopsis tangshen. This clustering analysis agrees very well with the pharmacognostic identification result, and which could be used as a tool in the quality control of Radix Codonopsis.

Scutellaria barbata has been used as a traditional Chinese Herb for treating liver, lung and rectal tumors. In the present study, cytotoxic effect of Scutellaria barbata MC fradtion was investigated and it was found to inhibit proliferation of human leukemic U937 cells with an IC50 of approximately 10 μg/ml in a dose-dependent manner. We also demonstrated that Scutellaria barbata MC fraction caused apoptosis in U937 cells. In the flow cytometric assay, the MC fraction-treated U937 cells showed an increase in hypo-diplold Sub G1 DNA contents. DNA fragmentation was observed by TUNEL assay. An increase of Bax:Bcl-2 ratio, activation of caspase-9, caspase-3, and cleavage of poly (ADP-ribose) polymerase (PARP) were demonstrated by western blot analysis. Taken together, these results exerted that the MC fraction suppressed human leukemic U937 cell proliferation by inducing apoptosis via the mitochondrial pathway.
Objectives : Talmyung-san(TMS) has been used for treatment of brain diseases in Chinese traditional medicine. However, little is known about the mechanism by which TMS rescues brain cells from ischemic damages. To elucidate the protective mechanisms of TMS, we execute experiments. Methods : The effects of TMS on ischemia/reperfusion-induced cytotoxicity and generation of nitric oxide(NO) are investigated in primary neonatal myocardial cells and A7rS, aortic smooth muscle cell line. Results : Ischemia/reperfusion itself induces severe myocardial cell death in vitro. However, treatment of the cells with TMS significantly reduces both ischemia/reperfusion-induced myocardial cell death and LDH release. In addition, pretreatment of TMS before reperfusion recovers the lose of beating rates alter ischemia/reperfusion. For a while, the water extract of TMS stimulates myocardial cells to produce NO in a dose dependent manner and it protects the damage of ischemia/reperfusion-induced myocardial cells. Furthermore, the protective effects of the water extract of TMS is mimicked by treatment of sodium nitroprusside, an exogenous NO donor. NG-monomethyl-L-arginine (NGMMA), a specific inhibitor of nitric oxide synthase(NOS), significantly blocks the protective effects of TMS on the cells after ischemia/reperfusion. In addition, on ischemia the water extract of TMS induce NO in A7r5 cell. Conclusions : Taken together, we suggest that the protective effects of TMS against ischemia/reperfusion-induced myocardial damages may be mediated by NO production of myocardial and vascular smooth muscle cell during ischemic condition.
Background: Short-course preoperative radiation (SCRT) with delayed surgery was found to increase pathologic complete response (pCR) rates in several trials. However, there was no clear answer on whether SCRT or long-course chemo-radiotherapy (LCRT) is more effective. Therefore we conducted this meta-analysis to evaluate the safety and efficacy of SCRT versus LCRT, both with delayed surgery, for treatment of rectal cancer. Materials and Methods: The literature was searched from PubMed, EMBASE, Web of Science, Cochrane Library and clinicaltrials.gov up to November, 2014. Quality of the randomized controlled trials (RCTs) was evaluated according to the Cochrane's risk of bias tool of RCT. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) system was used to rate the level of evidence. Review Manager 5.3 was employed for statistical analysis. Pooled risk ratios (RRs) and 95% confidence intervals (CIs) were calculated. Results: Three RCTs, with a total of 357 rectal cancer patients, were included in this systematic review. Metaanalysis results demonstrated there were no significantly differences in sphincter preservation rate, local recurrence rate, grade 3~4 acute toxicity, R0 resection rate and downstaging rate. Compared with SCRT, LCRT was associated with significant increase in the pCR rate [RR=0.49, 95%CI (0.31, 0.78), P=0.003]. Conclusions: In terms of sphincter preservation rate, local recurrence rate, grade 3~4 acute toxicity, R0 resection rate and downstaging rate, SCRT with delayed surgery is as effective as LCRT with delayed surgery for management of rectal cancer. LCRT significantly increased pCR rate compared with SCRT. Due to risk of bias and imprecision, further multi-center large sample RCTs were needed to confirm this conclusion.

Dunong's scholarship is characterized by these: He explained the metaphysical propositions by approaching usual things including oriental medicine practically; He comprised the Confucian concepts intensively to examine the Oriental studies closely; He groped for the outlooks on the world of taking serious view of human being or human body. And his medical thought is characterized by these: He attached importance to the theory of viscera and bowels based on yin-yang and five phase theory; He suggested the concrete yin-yang theory with a viewpoint of qi-blood theory; He considered the concept of 'six' only as mutual rooting of yin and yang and five visceras and six vowels, and denied the traditional six qi concepts; He explained the ghost theories of the metaphysics based on the yin-yang theory to analogize points of sameness between the oriental medicine and the metaphysics; He emphasized restore yang theory. Therefore, we can say that he regarded the human body as small heaven-earth and grafted the abstract propositions into the actuals so as to put the metaphysical propositions in practice.
