Antioxidative properties of traditional herbal medicines and the application of comet assay on antioxidative study

  • Traditional Chinese medicine (TCM) in single herb or formula prescription has been used for thousands of years. Many of them possess antioxidant activity and the activity may contribute the therapeutic effect. This paper would review the relationship of traditional herbal medicine and antioxidant with particular reference to ginseng. This medicinal herb has been used worldwide with extensive tonic effect. The comet assay, a technique for DNA protecting and damaging investigation would be introduced and the application of comet assay on TCM would be discussed.

Anti-stress effects of Gastrodia elata on catecholamine pathway in rat

  • Enzymes involved in catecholamine synthesis are present in the highest concentration in the adrenal medulla, however they were found also in other, mainly nervous tissues. Increased transcription of genes for catecholamine biosynthetic enzymes is an important mechanism to increase the capacity for epineprine/norepinephrine biosynthesis with stress. Gastrodia elata(Chinese name: Tienma), are very important Chinese herbal medicines used for the medical treatment of headaches, migraine, dizziness, epilepsy, rheumatism, neuralgia, paralysis and other neuralgic and nervous disorders. Immobilize stressed rat markedly increased tyrosine hydroxylase (TH) mRNA and dopamine-${\beta}$-hydroxylase (DBH) mRNA transcriptior level more than control group. But treated Gastrodia elata extracts in immobilized stressed rat slightly increased TH mRNA and DBH mRNA transcription level more than normal group. In addition, we are obtained identical results in PC12 cell line. Decrease of transcription level of TH mRNA and DBH mRNA is indicating that Gastrodia elata have a anti-stress effects which decrease the transcription level of TH and DBH mRNA on catecholamine biosynthesis pathway.

소아 거식증 (Infantile Anorexia)의 한의학(韓醫學)적 치료에 관한 고찰 - 중의학(中醫學) 저널을 중심으로 - (A Literature Study on Treatment of Infantile Anorexia Based on Chinese Medical Journals)

  • Objectives The purpose of this study is to find a literature study of infantile anorexia clinical cases with Traditional Chinese Medicine (TCM) to utilize with Korean medicine treatment methods. Methods 5 TCM journals with the highest impact factors were searched and selected among the journals listed in www.cnki.net. The search keywords were, "Children's Anorexia", "Infantile Apositia", "Infantile Anorexia", "Anorexia Nervosa", and "child of anorexia". he search was limited to the publication date from 2000 to 2012. Results Total 20 studies were selected: Acupuncture (5), Herbal medicine (9), Complex Treatment (3), External Application (2) and Moxibustion (1). Among the Acupuncture related studies, 1 study tested the effect of Sifeng (EX-UE10, 四縫), 3 studies tested the effect of Sifeng (EX-UE10, 四縫) and other acupoints treatment together, and 1 study tested the effect of combination of other acupoints. In all these studies, the study groups showed better therapeutic effects compared to the control groups. The Herbal medicine related studies showed the improvement in appetite and weight as a result of Herbal Medicines treatment. Among the Complex Treatment related studies, 2 studies tested the effect of Acupuncture and Chiropractic co-treatment, 1 study tested the effect of Moxibustion and Herbal medicine co-treatment. In all these studies, symptoms of Infantile anorexia showed significantly improved. Other papers related to External Application and Moxibustion treatment were reported improving in appetite and had other the clinical effects as well. Conclusions Clinical studies testing the effect of TCM for the treatment of Infantile Anorexia have been conducted in small scales, and all the studies showed a certain level of symptom improvement of the patients in the study groups. These results implicate that the methods in Korean medicine can be highly potential treatment options for the treatment of Infantile Anorexia. Accurate and well-controlled studies in large scale would be required to prove the effect of Korean medicine for the treatment of patients with Infantile Anorexia.

COVID-19 중의 진료지침에 수록된 예방 처방 분석 (Analysis of Preventive Formulas Included in Guidelines for Traditional Chinese Medical Treatment of COVID-19)

  • Objectives : This study collected and analyzed information related to preventive formulas from continuously published and revised COVID-19 treatment guidelines in various regions of China. Methods : We collected treatment guidelines published in different regions of China and categorized formulas for prevention and medical observation period listed in them according to the editions. The categorized preventive formulas were compared by type and target group. Results : Herbal medicines used for prevention included formulas derived from Korean medical classics, such as Okbyeongpungsan(玉屏風散) and Eunkyosan(銀翹散). The newly composed formulas, totaling over 100, were created by adding, subtracting, and combining formulas such as Sang-gukeum(桑菊飮), Eunkyosan(銀翹散), Sasammaekmundongtang(沙參麥門冬湯), Okbyeongpungsan(玉屏風散), Gwakhyangjeong-gisan(藿香正氣散), and Soshihotang(小柴胡湯). Patent medicines including Huoxiangzhengqi capsule(藿香正氣膠囊), Lianhuaqingwen capsule(連花淸瘟膠囊), Shufengjiedu capsule(疏風解毒膠囊), and Jinhuaqinggan granule(金花淸感顆粒) were frequently used, mainly targeting close contacts. These medicines were used differently depending on the specific population group, such as the general population, the elderly, children, pregnant women, and patients with underlying diseases, and were also applied differently according to the individual's constitution. Conclusion : We were able to identify various background factors contained in the guidelines for the use of preventive formulas presented by TCM group, and understand the social conditions that enabled the group to provide such guidelines. Through this, thorough preparation should be made so that the Korean Medicine can actively respond to another future pandemic.

EBM 기반구축을 위한 평위산 전임상 연구 문헌 분석 (Analysis of Studies on Pyeongwi-san to Establish the Fundament for Evidence Based Medicine)

  • To establish the fundament for EBM of Traditional Korean Medicine, the papers on Pyeongwi-san (Pingwei-san) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. The papers were classified by the registration of domestic or international journals, the year of publishment, experimental fields and the kinds of studies on biological activities. Of total 518 papers on Pyeongwi-san (Pingwei-san), 32 volumes were selected according to selection creteria. 20 volumes were published in domestic journals, 11 in Chinese journal and 1 in Japanese journal. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Pyeongwi-san (Pingwei-san) using HPLC, GC-MS. The papers on biological activities of Pyeongwi-san (Pingwei-san) showed improvement of gastrointestinal activity and water-electrolyte metabolism, immune regulation, anti-oxidant, anti-inflammatory, analgesic, anti-convulsant activities, hypnotic duration, blood pressure regulation, hepatic protection, congestive heart failure, anti-cancer activities. Further studies including gastrointestinal activity need to be preceeded to establish the fundament for EBM of Pyeongwi-san (Pingwei-san).

EBM 기반구축을 위한 보중익기탕 실험 연구 분석 (Analysis of studies on Bojungikgi-tang(Buzhongyiqi-tang) to establish the fundament for Evidence Based Medicine (EBM))

  • Objective : To establish the fundament for EBM of Traditional Korean Medicine, the papers on Bojungikgitang(Buzhongyiqi-tang) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. Methods : The papers were classified by the registration of domestic or international journals, the year of publishment, experimental fields and the kinds of studies on biological activities. Results : Among 2005 volumes of papers on Bojungikgi-tang(Buzhongyiqi-tang), 219 volumes were selected according to selective criteria. 71 volumes were published in domestic journals, 52 volumes were in Chinese journal, 96 volumes were in Japanese journal. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Bojungikgi-tang(Buzhongyiqi-tang) using HPLC method. The papers on biological activities of Bojungikgi-tang(Buzhongyiqi-tang) showed mainly immune regulation, anti-infection, anti-cancer, gastrointestinal activity, neuropsychiatric regulation, anti-inflammation, hepatoprotection, musculoskeletal regulation. Among biological activities, papers on immume regulation were reported mostly. Conclusions : The efficacy of Bojungikgi-tang(Buzhongyiqi-tang) could be related to immune regulation, gastrointestinal activity, anti-fatigue and further study need to be preceeded to establish the fundament for EBM of Bojungikgitang(Buzhongyiqi-tang).

다한증의 한약 치료에 대한 연구 : 체계적 문헌 고찰과 메타분석 (Herbal Medicine for the Treatment of Hyperhidrosis: A Systematic Review and Meta-analysis)

  • Objectives : The purpose of this study is to see the effects of herbal medicines on hyperhidrosis treatment. Methods : We searched articles related to oriental treatment of hyperhidrosis through domestic and international databases. Articles were searched for RCT. The results were summarized in tables and graphs, and meta-analysis was performed. Results : As a result of setting the herbal medicine alone treatment as an intervention for hyperhidrosis and western medicine alone as the control group, total 8 articles were selected. We analyzed them and got the results. The efficient rate of the treatment group was statistically significantly higher than that of the control group. In addition, adverse events were significantly less in the treatment group than in the control group. Conclusions : We found that oriental herbal medicine was effective and safe for hyperhidrosis.

External Application of Herbal Medicines for Acne Vulgaris: A Systematic Review and Meta Analysis

  • Aim of the study: The objective of this systematic review is to critically evaluate the evidence of the effectiveness and safety of external application of herbal medicines (EAHM) for acne vurgalis (AV). Methods: English, Chinese and Korean language databases were searched up to May 2018. Randomized clinical trials (RCTs) that reported the effects of EAHM for AV were included and analysed. Results: A total of 10 randomized trials with 656 AV patients were identified. A meta-analysis of two RCTs indicated that EAHM had a significant effect on improving primary outcome 'global assessment' compared with placebo (mean difference (MD) = -2.62, confidence interval (CI) = -4.84 to -0.40, p = 0.02). Furthermore, data extracted from two RCTs showed that EAHM significantly reduce primary outcome 'inflammatory lesion count of acne' (MD = -1.25, CI = -1.68 to -0.83, p < 0.00001) and 'non-inflammatory lesion count of acne' (MD = -1.32, CI = -1.75 to -0.90, p < 0.00001). No significant difference was observed between groups in secondary outcome 'sebum of skin' (MD = -0.21, CI = -0.53 to 0.11, p = 0.20) and 'patient-reported changes in symptom' (relative risk (RR) = 2.56, CI = 0.43 to 15.22, p = 0.30). No severe adverse events (AEs) were found and no treatment was stopped due to AEs of EAHM. Conclusions: EAHM seems to have affirmative effects, but quality of evidence, and non-standardized use of EAHM make our conclusion weak. Our suggestion is rigorously designed RCTs and standardization of EAHM are required in the future.

비만의 체질한약치료에 대한 체계적 문헌고찰 및 메타분석 (A Systematic Review and Meta-analysis on Constitutional Herbal Medicine Treatment for Obesity)

  • Objectives The purpose of this study was to systematically review previous studies on constitutional herbal medicine treatment for the obesity. Methods A total of 11 databases were searched in English, Chinese, and Korean. Standard methodologies for the systematic review were used to search, analyze and synthesize data for undertaking review and meta-analysis for intervention. The risk of bias was assessed by two independent researchers using the Cochrane risk of bias. Results A total of 2 RCTs were selected and analyzed. The results of the meta-analysis showed a significantly positive effect in the body weight and body mass index for the patient group treated with Taeeumjowi-tang compared to the placebo control group. Conclusions Our results suggested that Taeeumjowi-tang could be considered for improvement of symptoms in obese patients, without serious adverse events. Due to the low quality of the clinical evidence in this study, further studies are needed to evaluate efficacy and safety of constitutional herbal medicines for obesity.
