『상한론(傷寒論)』 소시호탕증(小柴胡湯證)의 한열왕래(寒熱往來)에 대한 연구(硏究) (The study on the syndrome of Sosihotang's alternate chills and fever in Sang Han Lun)

  • Objectives : Sosihotang is a typical prescription in Sang Han Lun. And the alternate chills and fever is a Sosihotang's typical symptom. Therefore I will study of how the syndrome of Sosihotang's alternate chills and fever caused. Methods : Examine the claims of the syndrome of Sosihotang's alternate chills and fever in Sang Han Lun. The scholar's claims are not all same. Thus, I analysis the claims of many scholars. Results : The syndrome of Sosihotang has two mechanisms. The move of defensive Gi(衛氣) can explain the occurrence of alternate chills and fever. If the defense Gi enter inside the body, it struggle with pathogenic Gi, the fever is occurred. On the contrary to this, if the defense Gi exits the body the chills occurs because of the lack of defense Gi. Conclusions : The syndrome of Sosihotang could be taken ill by tranmission or direct attack of exogenous pathogenic factors(直中). Symptoms chills and fever occurs due to the entrance and exit of defensive Gi.

L-형 사형주조에서 Chill 효과에 관한 수치적 해석 (Numerical Analysis of the Chill Effect in L-shaped Sand Castings)

  • A two dimensional analysis of the solidification phenomena has been simulated by IAD (Implicit Alternating Direction)method for L-shaped castings with external chills. The effect of chills has been studied with the variation of chill size, shape, and their materials, and also with the variation of the pouring temperature. Three kinds of cross-sectional shapes of chills such as triangle, square, and L-shape were chosen, and graphite, cast iron, and copper for their materials. It has been shown by the computation that the hot spot at the junction of L-shaped castings can not be removed by external chills, and that the distance between the inner corner and the hot spot on the line of symmetry, as well as the solidification time of castings depend only on VHC (Volumetric Heat Capacity).

격염구(隔鹽灸)와 사역탕(四逆湯)으로 치료한 상한후(傷寒後) 궐증(厥證) 환자에 대한 증례보고 (A Case Report of Ge-yan-jiu & Herbal Medicine Complex Treatment on Jue zheng (厥證) caused by Wang yang(亡陽))

  • Purpose : The purpose of this case is to report the improvement after acupuncture & moxibustion complex therapy about Jue zheng (厥證) caused by Shang han(傷寒). Methods : We provided acupuncture & moxibustion complex therapy to a patient who suffered from chills and coldness. We used acupuncture technique and Ge-yan-jin in umbilicus (神厥 隔鹽灸). Results & Conclusions : We observed that acupuncture & moxibustion complex therapy decreased symptoms of Jue zheng (厥證) and improved general condition of a patient who suffered from chills and coldness.

Lemierre's Syndrome Originated from the Odontogenic Infection: A Case Report

  • Also called necrobacillosis or postanginal sepsis, Lemierre's syndrome (LS) is an uncommon but potentially lethal complication of odontogenic infection. A 27-year-old male diagnosed with Ludwig's angina was transferred from a local hospital due to continuous fever and chills after incision and drainage under general anesthesia. The swelling of both submental and submandibular area subsided, but the fever and chills persisted. While generalized malaise improved, sepsis developed together with the deterioration of liver function. The chest computed tomography scan revealed multiple cavitations throughout both lungs, which were diagnosed as septic pulmonary embolism. After consulting the department of infectious diseases, the patient was treated with intravenous antibiotics focusing on vancomycin and additional antibiotics. After 3 weeks of treatment, the patient recovered completely. Despite its decreased mortality, dentists are not familiar with LS, and it is difficult to diagnose correctly. In this paper, we report a case and present a review of literature.

방약합편(方藥合編) 중(中) 창출(蒼朮)이 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察) (Studies on application of Rhizoma Atractylodis Japonicae blended prescriptions from Bangyahkhappeun)

  • This report describes the studies as follows through the research into the data of Rhizoma Atractylodis japonicae blended prescriptions from Bang Yahk Hap Peun. The main pathologies of Atractylodis Japonicae blended prescriptions found chills, bowels, malaria, and they are used 18 disease also. In these prescriptions, the symptoms of chills and bowels are the most frequently used. Atractylodis Japonicae main blended prescriptions use in the symptoms of Sang Han Pyo syndrome, Sang Han Yin syndrome, and they are 25 disease also. Through the research, Atractylodis Japonicae main blended prescriptions are used the pathologes of wet, internal disease, the injury of the cold and wind, stomach and spleen. The actual amount of Atractylodis Japonicae blended has a wide capacity from 6 pun to 6 don, 1-2 don is broadlly used. The structures of Atractylodis Japonicae blended prescriptions are Pyong We San, Yi Myo San, Hyang So San, Warl Gook Hwan, Gook Chul Hwan, such as basic prescriptions are most used.

전형적(典型的)인 학질의 증례(證例) 보고(報告) (Case study on the Fever pattern of Malaria)

  • This study was performed on the bases of clinical consideration about patient who has malaria. The symptoms of malaria are periodic alternating of chills and fever on everyone, two or three days. The patients feel a chill at the first time of malaria, and have a fever, headache and sweating at the next time. The long cycle of malarial fever such as every 2 or 3 days means that it takes a turn for the worse. In this study, oriental medicine was taken based on the oriental medicine principle such as Dalwonum(達原飮), Sosihotang(小柴胡湯) and Bojungikgitang(補中益氣湯), and the patients take a turn for the better. In the process of treatment if there is no fever in the expected date of the malarial fever, it is sure to have fever in the next expected date. It is favorable that patients lose the chills first and the fever lately in the process of treatment, and we can explain that supporting healthy energy to eliminate evil(扶正祛邪). The long cycle of malarial fever such as every 4, 6 or 9 days means that it is just before the recovery.

산욕기 과보온으로 야기된 산욕기 발한 과다에 대한 증례 보고 (The cases report of postpartum hyperhidrosis caused by keeping overwarm)

  • Purpose : The keeping warm and avoidance of cold are one of important Korean traditional postpartum care, but sometimes overwarm for example over-bundling, an overheated room or a steam bath cause heavy sweats. I observed and treated three patients complained of heavy sweats, chills, fatigue, malaise, cold sensation and pain caused by keeping overwarm for postpartum care. Although the symptoms have aggravated, they have maintained keeping overwarm because of a belief of Korean traditional postpartum care. The purpose of these cases studies are to report postpartum hyperhidrosis caused by keeping overwarm and to make a point of keeping the appropriate room temperature and humidity during puerperium. Methods : I observed and treated three patients complained of heavy sweats, chills, fatigue, malaise, cold sensation and pain caused by keeping overwarm for postpartum care. I found that a cause of the symptoms was hyperhidrosis by keeping overwarm, and administered herbal medicine and was to keep the appropriate room temperature. Results : After medication of Herbal medicine(補中益氣湯加味方) and keeping the appropriate room temperature, they were improved. Conclusion : Postpartum care is not to keep overwarm but to keep the appropriate room temperature and humidity.

유상한(類傷寒)의 내력(來歷)과 그 종류(種類)에 대한 소고(小考) (A study on the History of Yusanghan and the Types)

  • Objectives Yusanghan(類傷寒) means disease which chills and fever manifest like Cold damage(傷寒) but not Cold damage. However, study on Yussanghan is insignificant. The purpose of this study is to examine the origin and development process of Yusanghan. Methods: 1. Terms and disease related with Yusanghan was collected from books covering Cold damage of all time and analyzed. 2. The transition history was researched by analyzing collected terms. 3. Total appearance frequency of all time by collected disease was researched. Results: Yusanghan was appeared in Hwalinseo(活人書), which Ju Geong(朱肱) of the Song Dynasty wrote, for the first time and the definition and type were established. For the types of those, there are four diseases such as Phlegm-Fluid Retention(痰飮), Food Accumulation(食積), Vexation of Deficiency Type(虛煩), Beriberi(脚氣). Since then, the theory about Yusanghan had developed and the number of types had also increased until the Myung Dynasty. Conclusions: In early days, External Contraction Diseases(外感病) was not included in Yusanghan, but since the late Myung Dynasty, there were a lot of doctors who included External Contraction Diseases in Yusanghan, not in Cold damage. There are Summerheat Stroke, Wind-Dampness(風濕), and Warm Disease(溫病) etc. for included External Contraction Diseases. Because of this, the number of types of Yusanhan had significantly increased.

A Clinical Study on the Factors Related with the Sequelae of Facial Palsy

  • Objectives : In order to apply useful data to clinical practice, we undertook this study and tried to find factors related with sequelae of facial palsy in relation with patients' age and gender distribution, past history with the disease, condition before onset, duration of recovery time, existence of remaining sequelae, types of the sequelae, and duration for sequelae to disappear. Methods : We evaluated patients' condition (fatigue, stress, chills, cold and so on), past history (diabetes mellitus, hypertension, stroke, herpes zoster, cancer and so on), duration of recovery time, types of the sequelae, age and sequelae distribution as to when the treatments were started as we examined 473 patients who were diagnosed with facial palsy, and visited the Out-patient Department of Acupuncture and Moxibustion of Bundang Oriental Medicine Hospital of Dongguk University through 2003 and 2004. Results : The sequelae of facial palsy were not significantly relevant to the signs found before facial palsy occurred(fatigue, stress, chills, and cold), or to patients' past history (hypertension, diabetes mellitus, facial palsy, herpes zoster). The duration of recovery time was within 30 days for 45.3% of the patients examined in this study, and within 90 days for 72.6%. Evaluating the existence of sequelae in relation to age, we found more in the group comprised of patients aged 50 and over than under 50. We found more sequelae in the group which is consisted of patients who had not been treated until 6 days after than within 5 days from the onset. Conclusion: Attention to sequelae will be needed for patients aged 50 and over and who were not treated until 6 days after the onset, as they had more sequelae.
