경기지역 일부 초등학생의 비만도에 따른 급식 만족도 및 급식을 남기는 행동에 관한 연구 (Effect of obesity on the Satisfaction with School Lunches and the Behavior of Leaving Lunch Food in Elementary School Children)

  • The purpose of this study was to investigate the effects of obesity on the satisfaction with school lunches and school lunch leftovers of elementary school children in the Kyeonggi area. The study was carried out on 235 children(male : 111, female :124) in December of 2000. The results are summarized as follows. A Rohrer index was used to define obesity. The proportions of severe underweight, underweight, normal, overweight, and obese were 0.9%, 16.6%, 57.9%,17.0%, and 7.7% respectively. For the data analysis, the subjects were divided into 3 groups : an underweight group(Rohrer index 109 including severe underweight), a normal group(Rohrer index 110-140), and an obese group(Rohrer index 141 including overweight). With regard to the satisfaction with school lunches, the proportions of children who were satisfied, neutral, and unsatisfied were 27.2%, 59.6% and 13.2% respectively. The proportion of the satisfaction with school lunches was significantly higher in the male group than in the female group(p〈0.05), and the proportion of the dissatisfaction with school lunches was significantly increased with obesity index(p〈0.05). The largest proportion of children who were satisfied with school lunches answered they felt hungry(56.3%) and the food tasted good(42.2%). Children sho were not satisfied with school lunches answered that the food was not tasty(93.5%), they did not have ant appetite(9.7%), and others. The children not finishing school lunches(leftovers) occasionally or always were 47.7% of the subjects. The behavior of leaving lunch foods was not significantly related to the gender or the frequency of eating snacks, wile it was positively related with dissatisfaction with school lunches(p〈0.01), obesity index(p〈0.05), and the decreased frequency of eating meals per day(p〈0.01). The reasons of school lunch leftovers were food dislikes(67.9%), excess of quantity(39.3%), tastelessness(31.3%), poor appetite(13.4%), and other. The children eating meals twice, 3 times, and 4 times per day were 18.7%, 74.9%, and 6.4% of the subjects respectively. The proportions of the obese and the satisfaction with school lunches were not changed significantly according to the frequency of eating meals, while the proportion of children leaving school lunches was significantly higher in the group of eating meals twice per day than in the other groups(p〈0.01). The frequency of eating snacks was not related to the obesity index, the proportion of school lunch leftovers, or the frequency of eating meals, while the proportion of satisfaction with school lunches was significantly higher in the group of eating snacks 1-2 times per day than in the other groups(p〈0.01). In conclusion, it is necessary to have children enjoy school lunches with efforts to improve the food´s taste in the school food services and to reexamine the serving sizes for the children, in addition to the nutrition education for the children to eat a variety of foods.

후향적 연구방법에 의한 초등학교 아동의 비만이행 추이에 대한 고찰 (A Retrospective Study on the Prevalence of Obesity in Elementary School Children)

  • This study was accomplished to investigate the prevalence of obesity among the 229 elementary school children(boys; 115 persons, girls; 114 persons) during 4 years by retrospective study. The results were as follows; 1. The mean height and weight of 229 subjects were 143.2cm(boys;143.4cm, girls; 142.7cm), 38.8kg(boys;40.4kg, girls;36.9kg), respectively. And other anthropometric body index such as the mean BMI, Rohrer Index, WLI and Obesity Index of subjects were 18.8(boys;19.5, girls; 18.0), 1.31(boys;1.36, girls; 1.26), 115.5(boys;121.4, girls; 108.9) and 110.6(boys;114.5, girls; 106.7), respectively. 2. The mean percentile values of height and weight of subjects were higher than the '85 Korean Children's Growth Standard, respectively. In the case of boys, the 75th percentile value of weights was larger than that's 97th's. And in the case of girls, the 75th percentile value of weights was equalled to the that's 90th's. 3. When the subjects were classified into 4 groups such as obese, overweight, normal, and lean groups according to Obesity Index, their percentages were 28.8%(boys; 38.3%, girls; 19.3%), ll.8%(boys; 9.6%, girls; 14.0%), 50.7%(boys; 47.0%, girls; 54.4%) and 8.7%(boys; 5.2%, girls; 12.3%), respectively. And only I person(3.3%) is classified into normal group and 2 persons(6.7%) are classified into overweight group when they become 5 grade among 30 persons(13.1%) who were classified into obese group when they were I grade. 4. Changes in percentage of subjects differentiated by 3 indices during 4 years(from I grade to 5 grade) were as follows; In the case of BMI were 5.2%, 5.2%, 11.8%, 21.0%, 27.5%. In the case of Roher index were 13.1%, 11.8%, 14.4%, 14.8%, 17.8%. And in the case of WLI were 15.7%, 21.6%, 29.5%, 32.2%, 36.7%. 5. There was no significant difference among 4 groups on the skip rate of breakfast(obese;48.5%, overweight;55. mean meal time of subjects took about 10 minutes which was short, and the eating rate of obese group was significantly shorter than that of other groups(p<0.05). There was a tendency that the rate of the obese or overweight group eating together in breakfast and dinner was lower than that of normal or lean group. The mean T.V. watching time and the rate of eating out of subjects were 1-2 hours/day and 1 or 2 times/week. In conclusion, the subjects have been grown remarkably compared to the '85 Korean Children's Growth Standard, especially boy's weight. In order to the elementary school children to be grew as healthful citizen they should recognize the importance of meals of everyday, the balanced meals, and desirable eating habits. In addition to these, we should educate the ways of meal-life to them. We also are in need to develope the methods of education for the students themselves and their parents so that could prevent the increase rate of obesity.

대전에 거주하는 미취학 아동과 그 어머니의 식행동 및 이들의 관련성: 영양지수를 활용하여 (Relationship between the Dietary Behavior of Young Children and Their Mothers in Daejeon, Korea Using the Nutrition Quotient for Preschoolers and Adults)

  • Objectives: This study examined the association between the dietary behavior and weight status of preschool children and their mothers in Daejeon, Korea. Methods: A total of 301 mother-preschool children (aged 3~6 years) dyads were included in this study. The dietary behavior of children and their mothers was assessed using the Nutrition Quotient (NQ) questionnaire for preschoolers and adults, respectively. The NQ questionnaires were completed by the mothers. The overweight/obesity status of children and their mothers was determined using data on height and body weight reported by the mothers. Multiple logistic regression was performed to examine the relationship between the dietary behavior and weight status of children and their mothers. Results: The mean NQ score was 58.9 ± 9.7 in children and 55.6 ± 9.2 in mothers. The NQ score was higher in boys than girls but did not vary by age. The prevalence of overweight/obesity was 27.5% in children and 46.5% in mothers. The physical activity level of mothers and their NQ scores were positively associated with the NQ scores of the children. After adjustment for covariates, the mothers in the highest tertile of NQ scores showed a lower odds ratio (OR) for the unhealthy dietary behavior of children (OR = 0.24, 95% CI = 0.11~0.53, P < 0.001) compared to those in the lowest tertile. The obese mothers showed a higher OR for children's overweight/obesity (OR = 3.38, 95% CI = 1.68~6.80, P = 0.001) compared to normal weight mothers. Conclusions: The dietary behavior and weight status of young children and their mothers were closely linked. Nutrition education programs targeting mothers are necessary for improving maternal and child nutrition. Specifically, these programs need to be tailored to the socioeconomic characteristics or weight status of mothers.

비만어린이와 정상체중 어린이의 행동 특성에 관한 문헌적 고찰 (Do Obese Children Exhibit Distinguishable Behaviours from Normal Weight Children?-Based on Literature Review)

  • Since obesity has been a crucial issue regarding children's health in Korea, numerous studies have been conducted. However, it is difficult to locate studies that can help researchers and/or medical professionals who want to study behavioral differences between obese and normal weight (non-obese) children. The collected literature about obese childrens behaviors were accumulated by performing 'key word' searches from several web engines: 'obesity' 'children obesity' 'behavior' 'habit' 'eating behavior or habit' 'exercise' 'physical activity' and 'daily behavior'. The information was extracted and the behaviors were reviewed utilizing 30 papers that revealed any differences from normal weight children with at least p .05 level of significance. According to those studies, the obese children tended to skip meals more often as a method of losing weight. They also have an increased appetite, eat larger portions and rarely leave food on their plates. The obese children prefer to eat high fat, high calorie foods and eat faster than their normal weight counterparts. Also, the children usually eat alone and eat at irregular meal times. Beside the behaviors related to the standard three meals a day, obese children have high preferences for snacking. The children reported that with pocket money they usually eat certain types of snacks without parent approval. The children stated that they exercise more on a regular basis. However, when asked about their participation they were not likely to engage actively. Meanwhile, they tend to spend free time doing sedentary activities such as watching TV or playing on the computer. Since these studies used self -reported questionnaires with a small sample, size there may be difficulty in forming a basis about obese children's behaviors. Nevertheless, these findings are in accordance with other studies conducted in local as well as western countries. In conclusion, obese children have different daily behaviors or routines from normal weight children. It is recommended that altering these behavioral patterns to match those of normal weight children should be encouraged.

초등학교 정상체중아동과 비만아동의 건강증진 생활양식 (Health Promotion Life Profile of Normal-Weight and Obese Children in Elementary Schools)

  • Purpose: The purpose of this study was to identify the Health Promotion Life Profile of normal-weight and obese children, and factors related to obesity in elementary school children, and provide basic data for the development an obesity management program for these children. Method: The survey participants were 878 fourth to sixth grade elementary school children in Gwangju. $x^2$-test, t-test, ANOVA and multiple logistic regression with the SPSS program were used to analyze the collected data. Results: The Health Promotion Life Profile showed significant differences between normal-weight and obese groups according to food habits (t=3.305, p= .001) and exercise habits (t=-4.065, p=< .001). The Health Promotion Life Profile of obese children was also significantly different according to interest in weight control (F=3.044, p= .050), regular breakfast (F=3.699, p= .014), regular meals (F=6.868, p= .002). The multiple logistic regression analysis, showed that factors in obesity were significantly different according to gender, interest in weight control, fast eating, overeating and predilection for some food. Conclusion: The results of this study suggest that items on Health Promotion Life Profile such as food habits and exercise habits should be included in education programs to prevent obesity in elementary school children.

A Study on Obesity-promoting Factors for the Elementary School Chirdren

  • The purpose of this study is to examine the relationships among body composition, dietary intake, exercise, and life style in children(M=80, f=102) of the 5th and 6th grades of elementary school. Anthropometry and multifrequency bioelectrical impedance analysis were conducted to estimate body composition. Dietary intake, exercise, and life style were determined by using questionnaires. When obesity was classified greater than 120% of the ideal body weight, the prevalence rates of obesity were 31.2% for boy and 20.6% for girl. There were significant differences in body composition between nonobese(NO) and obese(OB) groups. Mean fat mass(FM) and lean body mass(LBM) were 8.6kg and 27.7kg for NO group and 16.7kg and 32.3kg for OB group, respectively. Also a significant difference was found in hydration rate(TBW/body weight)between groups( <0.01). No significant difference was found I total calorie intake and nutrient intakes between groups. No difference was found in the frequency and duration of outdoor exercise and indoor activities. Mean sleeping hours was 8-9hours for 62% of nonobese children and for 59% of obese children. However, calorie intake per body weight was significantly lower inobese children than in nonobese. The present study showed that significant differences existed in their body size and composition between NO and OB groups , while no differences existed in daily calorie intake, excercise , and life style. This may indicate that important obesity-promoting factors of early onset obesity may rely on other factors such as hereditary or environmental factors besides factors considered. Further studies are required to understand obesity-promoting factors in children. (Korean J Community Nutrition 2(5) : 680∼686, 1997)

우리나라 양산지역 학동전기 아동에서 비만 진단을 위한 체지방률의 절단값 (The Cut Off Values of Body Fat Percentage for Diagnose Obese among Selected Number of Preschool Children in Yangsan)

  • Objectives The objectives of this study were to evaluate the correlations between the indices of obesity and percentage body fat by bioelectrical impedance analysis(BIA). Also, this study was designed to define the cut off values of percentage body fat by BIA which would represent BMI and degree of obesity in order to detect overweight and obesity in preschool children. Methods The height and weight of 683 children aged four to six years old were measured. Also, percentages of body fat were measured by BIA. Results There were high correlations between the boy's body fat percentages and degree of obesity(r=790). Also, there were high correlations between the girl's body fatpercentages and BMI(r=778). The cut off values of body fat percentage, which would correspond to those of BMI percentiles, were calculated by ROC curve analysis. To classify as an overweight, the correspondent cut off values of body fat percentage were 19.55% in boys and 22.35% in girls. For obesity, the cut off values were 28.80% in boys and 27.60% in girls. The cut off values of body fat percentage, which would correspond to degree of obesity were calculated by ROC curve analysis. To classify as an overweight, the correspondent cut off values of body fat percentage were 19.75% in boys and 22.35% in girls. For obesity, the cut off values were 29.05% in boys and 28.75% in girls. Conclusions According to the BMI criteria by the Korean pediatrics society in 2007, the most appropriate cut off values of body fat percentage in for four to six years old children were follows; for boys, 19.55% considered as overweight, and 28.80% as obese while for girls, 22.35% considered as overweight and 27.60% as obese. According to suggested degree of obesity criteria, the most appropriate cut off values of body fat percentage in for four to six old children were were follows; For boys, 19.75% was considered as overweight, and 29.05% as obesity. For girls, 22.35% was considered as overweight, and 28.75% as obese.

학령기 아동을 위한 멀티에이전트 비만관리 프로그램의 효과 (The Effects of a Multi Agent Obesity Control Program in Obese School Children)

  • Purpose: The purpose of this study was to identify the effects of a multi agent obesity control program in obese school children. This program was composed of strategies to modify diet and exercise habits and to change cognitive behavior variables(stress, coping, and self-efficacy). Method: The subjects were 40 obese school children who participated in our project voluntarily via homepage, TV, newspaper, public paper and school official documents. The program was implemented daily for 4 sessions per day for ten days from August 16 to 26, 2004. The daily program consisted of exercise therapy, dance therapy, cognitive behavior therapy and aroma therapy. The data was analyzed by paired t-test using the SPSSWIN program. Result: There was a significant decrease in children's waist-hip ratio(p=.04) and in children's stress(p=.00) after the program. There was a significant increase in children's self-confidence after the program(p=.02) and a significant decrease in children's diet habit after the program(p=.02). Conclusion: This study provides evidence that a multiagent obese control program is effective in changing waist-hip ratio, stress, self-confidence, and diet habits in obese school children.
