The Difference in the Quality of Life of Korean Children With Attention-Deficit/Hyperactivity Disorder Between Before and After COVID-19

  • Objectives: This study aimed to compare the quality of life (QoL) of children with attention-deficit/hyperactivity disorder (ADHD) before and during coronavirus disease-2019 (COVID-19) and to examine how their QoL is affected by emotional and environmental factors during COVID-19. Methods: Participants in the pre-COVID-19 (n=43) and COVID-19 (n=36) groups were recruited from the same university hospital. The Pediatric Quality of Life Inventory (PedsQL) 4.0 Child Self-report, the Children's Depression Inventory (CDI), the Revised Children's Manifest Anxiety Scale (RCMAS), the PedsQL 4.0 Parent Proxy Report, and the Conners' Parent Rating Scale (CPRS) were employed. Independent t-tests, Pearson's correlation analysis, and multiple regression analysis were conducted. Results: Caregivers assessed the children's QoL more negatively than the children themselves in both groups. Children with ADHD evaluated their physical function more negatively and anxiety was significantly higher in the COVID-19 group. In the COVID-19 group, the PedsQL child self-report was significantly predicted by the CPRS, the CDI, and environmental factors (i.e., relation to child and monthly household income). Conclusion: Children with ADHD in the COVID-19 group had a numerically lower QoL and significantly higher anxiety. To improve QoL, it is important to deal with not only depression but also ADHD symptoms and environmental factors.

지역 저소득층 아동의 우울증상과 주의력결핍-과잉행동증상에서 사회경제적 요소의 관련성 (Association of Psychosocial Factors in Developing Childhood Depression and ADHD in a Community Low Income Family Children)

  • Ovjectives: As the attention devoted to children's mental health increases, medical costs and burdens mount as well. In the present study, we evaluated the association between socioeconomic status(SES) and major child psychiatric symptoms. Methods: The subjects of this study were children of recruited from a mental health screening program in the Seoul Sungbuk mental health center over the course of 3 months. To establish the SES of each child, we collected data about each child's medical insurance, years of parental education, household income, family structure and housing. 149 children & parents completed questionnaires including the Childhood Depression Inventory(CDI) and the Korean Attention Deficit Hyperactivity Disorder Rating Scale(K-ARS). Results: The mean K-ARS-P score was $12.1{\pm}11.1$ and the suspected prevalence of ADHD was 20.8%(n=31). The mean cm score was $12.9{\pm}7.9$, and the prevalence of suspected depression was 16.8%(25). Depressive symptoms and ADHD symptoms were both more severe than those observed in a previous epidemiologic study in Korea. Depressive symptoms were more closely associated with family SES status. Conclusion: SES status is one of the most important risk factors in the development of major child psychiatric symptoms. In our study we found that depressive symptoms in particular were most tightly associated with psychosocial factors. Evaluation of the risk factors, early screening and intervention for low SES children would be valuable mental health management tactics to implement in a community mental health system.

한국인 주의력결핍 과잉행동장애와 Dopamine Beta Hydroxylase 유전자의 관련성 : 가족기반 연구 및 환자-대조군 연구 (A FAMILY-BASED AND CASE-CONTROL ASSOCIATION STUDY OF THE DOPAMINE BETA HYDROXYLASE GENE POLYMORPHISM IN ATTENTION DEFICIT HYPERACTIVITY DISORDER)

  • 목적 : 이번 연구는 최근 주목을 받고 있는 dopamine beta hydroxylase 유전자의 Taq I 절단부위 다형성과 주의력결핍 과잉행동장애 발병 간에 어떤 관련성이 있는지를 알아보기 위한 연구이다. 방법 : 주의력결핍 과잉행동장애 아동 106명을 환자군으로 선정하고, 환자군과 성별, 연령별로 빈도짝짓기된 일반 아동 212명을 대조군으로 하는 환자-대조군 연구를 실시했다. 아울러 환자군과 부모를 대상으로 가족기반 연구를 병행했다. 결과 : 환자-대조군 연구에서는 환자군과 대조군 간의 유전자형이나 대립유전자 분포에 있어 유의한 차이를 발견하지 못했다. 그러나 가족기반 연구에서는 대립유전자 Al의 선택 전달이 관찰되었다. 결론 : 이번 연구를 통해 주의력결핍 과잉행동장애와 dopamine beta hydroxylase 유전자의 Taq I 절단부위 다형성 간의 관련성을 부분적으로 확인했으나 이를 뒷받침하기 위해 서는 추가적인 연구가 필요할 것이다.

주의력결핍 과잉행동장애(ADHD) 아동 부모의 양육 경험: 정상에 다가가기 (Parenting Experiences of Parents of Children with ADHD : Approaching the Normal)

  • Purpose: The purpose of this study was to explore the experiences of parenting from the parents of children with ADHD and to develop a grounded theory about their experiences. Method: This study was a qualitative research study that used a grounded theory to understand and discuss the parenting experiences of parents of children with ADHD in a greater depth. Grounded data was collected through an in-depth interview from twelve participants with ADHD children. The data was analyzed using the grounded theory method. Results: 'Approaching the normal' emerged as the core phenomenone. Parenting experiences for parents of children with ADHD referred to a process of bringing the children, back on track of normal growth development, and their functioning in the family at a normal level and establishing a new normalcy. The process of approaching the normal involved a basic social-psychological process, such as 'living a day in hell', 'accepting', 'confronting the conflicts', 'lowering expectations', and 'making a new normalcy'. Conclusion: The outcomes of this study, which observed the parenting process of parents of children with ADHD, could enhance nurses' understanding of ADHD and help nurses become major mental health service providers for the mental health of children with ADHD and their families.

주의력결핍 과잉행동장애와 스마트폰 중독, 우울, 불안, 자존감 (Smart-Phone Addiction, Depression/Anxiety, and Self-Esteem with Attention-Deficit Hyperactivity Disorder in Korean Children)

  • Objectives : The current study investigated the risk of smartphone addiction among children and adolescents with or without attention-deficit hyperactivity disorder (ADHD), risk of depression, anxiety, and self-esteem using the Smartphone Addiction Scale Proneness, Kovac's Children's Depression Inventory, State-Trait Anxiety Inventory, and Rosenberg Self-Esteem Scale, commonly used in clinical medicine. Methods : Ninety five students with ADHD who visited psychiatry outpatient clinics completed the questionnaire. At the same time, 592 middle and high school students living in a similar area regardless of ADHD diagnosis, completed the questionnaire as control subjects. Results : Overall, 40.0% of 95 ADHD and 12.8% of 592 control subjects were classified as the smartphone addiction proneness group, 26.3% of the ADHD subjects and 8.3% of the control group were classified as the depression group, and 32.6% of the ADHD subjects and 16.2% of the control group were classified as the anxiety group. Significant differences were observed between the two groups. Conclusion : The results of this study suggest that ADHD subjects are more prone to smartphone addiction, becoming depressed or anxious than those in the control group. From this study, we could suggest that students with ADHD are more easily affected by smartphone addiction than normal control subjects. In addition, we might understand how some psychiatric problems like depression, anxiety, and low self-esteem are related to ADHD and smartphone addiction.

주의력결핍과잉행동 장애아동을 대상으로 제공되는 국내 중재에 대한 고찰 연구 (Effects of Therapeutic Interventions for Children With Attention Deficit Hyperactivity Disorder: A Systematic Review)

  • 목적 : 본 연구는 주의력결핍과잉행동장애아동(Attention Deficit Hyperactivity disorder; ADHD)을 대상으로 국내에서 제공되는 중재 방법을 체계적으로 고찰하여 최신의 국내 치료 경향을 제시하고자 하였다. 연구방법 : 2004년 1월부터 2018년 12월까지 최근 15년간 국내 학회지에 게재된 연구를 대상으로 하였다. 누리미디어(DBpia), 스콜라(Scholar), 학술연구정보서비스(RISS), 한국학술정보(KISS)에서 '주의력결핍과잉행동', '아동', '중재'를 키워드로 검색하였다. 연구대상으로 선정된 연구는 총 14개로, 각 연구는 근거의 질적 수준과 방법론적 질적 수준으로 분석한 후 그 결과를 대상자, 중재, 대조, 결과의 순으로 제시하였다. 결과 : 분석 결과 근거기반 실행의 질적 수준은 수준 IV의 연구가 가장 많았고, 방법론적 질적 수준은 '보통'의 연구가 가장 많았다. 중재는 상호작용식 메트로놈 중재(Interactive metronome; IM)의 빈도가 가장 높았고, 측정된 평가도구는 상호작용식 메트로놈 전체형 검사(Long form assessment of IM), 아동 청소년 행동평가척도(Korean behavior child checklist; K-CBCL)의 사용이 높았다. 중재결과 ADHD아동의 집중력, 과잉행동, 과제 수행, 과다언어 사용, 그리고 타이밍 등에서 유의한 효과가 나타났다. 결론 : 국내 ADHD아동을 대상으로 다양한 중재방법이 제공되었고, 그 결과 ADHD증상에서 효과적인 결과가 나타났다. 따라서 본 연구결과는 임상에서 근거기반 실행을 위한 기초자료로 사용할 수 있다. 앞으로 ADHD아동을 대상으로 한 지속적인 연구가 이루어져야 할 것이다.

주의력결핍 과잉행동장애와 노르에피네프린 수송체 유전자 다형성의 연관성 (Case-Control Association Study of the Norepinephrine Transporter Gene Polymorphism in Children with ADHD)

  • Objectives : This study aimed to examine the association between norepinephrine transporter gene (SLC6A2) polymorphisms and attention-deficit hyperactivity disorder (ADHD) and to examine the relationship between the genotypes and allele variants of SLC6A2 and results of the Korean version of the parent ADHD rating scale (K-ARS). Methods : We examined the association between ADHD and norepinephrine transporter gene polymorphism using DNA from 137 Korean children with ADHD and 120 normal controls. We compared the genotype distributions and allele frequencies of SLC6A2 polymorphism between the control group and the ADHD group. Then, we correlated the children's K-ARS mean totals, inattention scores, and hyperactivity/impulsivity scores with the genotypes and alleles for each SLC6A2 polymorphism. Results : There were no significant differences in genotype and allele distribution for each SLC6A2 polymorphism, as shown by the Chi-square test (p>.01). There was a trend toward a difference in allele frequency in rs 5568, but it was not statistically significant after adjusting for multiple comparisons (p=.048). Also, there were no significant differences in K-ARS scores according to the genotypes and alleles for the SLC6A2 polymorphisms. Conclusion : Our study found no significant evidence of an association between SLC6A2 polymorphisms and ADHD.

주의력결핍 과잉행동장애 환아 평가시 지능검사의 유용성에 관한 예비연구 (A Pilot Study of the Usefulness of Intelligence Test in Assessment of Attention-Deficit Hyperactivity Disorder)

  • Objectives:We analyzed the usefulness of intelligence test in assessing attention-deficit hyperactivity disorder (ADHD). Methods:The medical records of 312 patients with ADHD who visited the child psychiatry department in one university hospital in Seoul, Korea, were reviewed. Demographic data and scores on the Korean Wechsler Intelligence Scale for Children-III (K-WISC-III) and Korean-Conners' Parent Rating Scale (K-CPRS) were analyzed. To assess the relationship with comorbidities, AD-HD subjects were classified as; externalizing disorders, internalizing disorders, tic disorders, and no comorbidities. Results:Verbal intelligence quotient (VIQ) was significantly higher than performance IQ (PIQ)($102.6{\pm}14.44$, $99.2{\pm}14.48$, p<.001). Using mean scatter method, subtest scores such as 'information, similarities, digit span and coding' were significantly lower than mean scores of sum of subtests of VIQ or PIQ (p<.05). Regarding comorbidities, children with externalizing disorders exhibited. Lower scores on 'information and vocabulary' than other groups (p=.008). Children with no comorbidities exhibited higher scores on 'similarities and object assembly' than children with externalizing/internalizing disorders (p=.001) and also on 'comprehension' than children with externalizing disorders (p=.006). For subtypes of ADHD, children with hyperactive-impulsive type had higher scores on 'comprehension' than children with inattentive and combined type (p=.004). Conclusion:These results suggested that intelligence test might provide useful information for assessing ADHD.

  • 주의력결핍 과잉행동장애로 진단한 7세 남아에게 주 2회 3개월간의 뉴로피드백 치료를 통하여 일상생활의 산만한 행동의 개선과 단축형 Conner 평가척도, 한국아동인성검사 BASA읽기검사 등에서 개선된 결과 및 주의집중 뇌파 지표가 향상되는 경과를 관찰할 수 있었다. 이후 확대된 임상적용과 연구가 필요하리라 사료된다.

Stimulant Induced Movement Disorders in Attention Deficit Hyperactivity Disorder

  • Stimulants, such as amphetamine and methylphenidate, are one of the most effective treatment modalities for attention deficit hyperactivity disorder (ADHD) and may cause various movement disorders. This review discusses various movement disorders related to stimulant use in the treatment of ADHD. We reviewed the current knowledge on various movement disorders that may be related to the therapeutic use of stimulants in patients with ADHD. Recent findings suggest that the use of stimulants and the onset/aggravation of tics are more likely to be coincidental. In rare cases, stimulants may cause stereotypies, chorea, and dyskinesia, in addition to tics. Some epidemiological studies have suggested that stimulants used for the treatment of ADHD may cause Parkinson's disease (PD) after adulthood. However, there is still a lack of evidence that the use of stimulants in patients with ADHD may cause PD, and related studies are only in the early stages. As stimulants are one of the most commonly used medications in children and adolescents, close observations and studies are necessary to assess the effects of stimulants on various movement disorders, including tic disorders and Parkinson's disease.
