Objectives : The aims of this study were to examine agreement between parent and teacher ratings of the Korean version Strengths and Difficulties Questionnaire (SDQ-Kr) for the children's psychopathology and to determine whether it is effective in diagnostics as the Korean version of Childhood Behavior Checklist (K-CBCL). Methods : SDQ-Kr and K-CBCL were completed by parents and teachers of 165 children aged 6-12 years. The clinical diagnosis was made by a child and adolescent psychiatrist who did not have knowledge of the outcome. Spearman's correlations were computed for associations between parents' and teachers' ratings on a subscale level. The Mann-Whitney U test was performed for the influence of child's age and sex on the results. Finally, AUC values were calculated for the diagnostic capacity and the difference between informants was tested using the z test. Results : High correlations were observed between parents' and teachers' SDQ-Kr on every scale. Parents were more sensitive to emotional symptoms of girls, while teachers appeared to be more responsive to externalized symptoms of boys, inattention-hyperactivity of young children, and emotional symptoms of seniors. Parents' SDQ-Kr demonstrated the highest prediction of attention-deficit hyperactivity disorder. Conclusion : Teachers' SDQ-Kr can be used in identification of child psychopathology when parents are absent or when their reliability is considered low.

Objectives: Awareness of attention-deficit/hyperactivity disorder (ADHD) in adults has significantly increased; however, clinical data specific to the Korean population are insufficient. Clinical experience of ADHD may differ based on whether psychiatrists have received pediatric psychiatry-specific training. In order to prepare a practice parameter for adult ADHD patients in Korea, we examined questionnaire data to observe how pediatric psychiatry training could affect clinical practice for adults with ADHD. Methods: A questionnaire about the diagnosis and treatment process was distributed to both general psychiatrists (GPsy) and child and adolescent psychiatrists (CAP) at the summer and winter workshop meetings of Korean Academy of Child and Adolescent Psychiatry. Results: In total, 142 psychiatrists participated in the survey (86 GPsy, 56 CAP). GPsy and CAP preferred pharmacotherapy (GPsy 82.40%, CAP 64.30%) as the primary treatment option and answered that the clinical psychiatric interview is the most necessary step in diagnostic assessment (GPsy 22.16%, CAP 19.00%). The GPsy responded with an optimal and average treatment duration that was shorter than that reported by CAP. Conclusion: Identification of the initial presenting symptom as the correct diagnosis and the optimal duration of pharmacotherapy differed between GPsy and CAP in practice, whereas concepts in diagnosis and treatment of ADHD in adults were similar for both groups. These results suggest the urgent need for the Continuing Medical Education program for psychiatrists treating adults with ADHD.
Cardiomyopathy (CMP) is a heterogeneous disease caused by a functional abnormality of the cardiac muscle. CMP is of 2 major types, dilated and hypertrophic, and is further classified as either primary or secondary. Secondary CMP is caused by extrinsic factors, including infection, ischemia, hypertension, and metabolic disorders. Primary CMP is diagnosed when the extrinsic factors of secondary CMP are absent. Furthermore, the World Health Organization, American Heart Association, and European Cardiology Association have different systems for clinically classifying primary CMP. Primary CMP is rare and associated with a family history of the disease, implying that genetic factors might affect its incidence. In addition, the incidence of CMP varies widely according to patient ethnicity. Genetic testing plays an important role in the care of patients with CMP and their families because it confirms diagnosis, determines the appropriate care for the patient, and possibly affects patient prognosis. The diagnosis and genetic identification of CMP in patients' families allow the possibility to identify novel genes that may lead to new treatments. This review focuses on the epidemiology, pathophysiology, diagnosis, and treatment of CMP, with the aim of providing pediatricians with insights that may be helpful in the early identification and management of idiopathic CMP in children.
Severe childhood asthma is a complicated and heterogeneous disorder with distinct phenotypes. Children with severe asthma have more persistent symptoms despite receiving treatment, more atopy, greater airway obstruction, and more air trapping than those with mild-to-moderate asthma. They also have higher morbidity and substantial airflow limitations that persist throughout adulthood. Identification of the phenotype clusters and endotypes of severe asthma can allow further modulation of the natural history of severe asthma and may provide the pathophysiologic rationale for appropriate management strategies.
Early onset scoliosis (EOS) describes the onset of scoliosis before the age of 10 years and is associated with significant health risks. Compared to adolescent idiopathic scoliosis, studies on the etiology of EOS in children are more common. EOS includes inhomogeneous groups of patients, and the etiology of EOS may be congenital, neuromuscular, syndromic, or idiopathic. The identification of the molecular etiology underlying patients with EOS has been increasing. This could provide valuable information for optimizing the management and care of these children. In this review, EOS was reviewed from a medical genomic perspective.

Human papillomavirus (HPV) types 16 and 18 are the major causes of cervical lesions and are associated with 71% of cervical cancer cases globally. However, public health infrastructures to support cervical cancer screening may be unavailable to women in low-resource areas. Therefore, sensitive, convenient, and cost-efficient diagnostic methods are required for the detection of HPV16/18. Here, we designed two novel methods, real-time ERA and ERA-LFD, based on enzymatic recombinase amplification (ERA) for quick point-of-care identification of the HPV E6/E7 genes. The entire detection process could be completed within 25 min at a constant low temperature (35-43℃), and the results of the combined methods could be present as the amplification curves or the bands presented on dipsticks and directly interpreted with the naked eye. The ERA assays evaluated using standard plasmids carrying the E6/E7 genes and clinical samples exhibited excellent specificity, as no cross-reaction with other common HPV types was observed. The detection limits of our ERA assays were 100 and 101 copies/µl for HPV16 and 18 respectively, which were comparable to those of the real-time PCR assay. Assessment of the clinical performance of the ERA assays using 114 cervical tissue samples demonstrated that they are highly consistent with real-time PCR, the gold standard for HPV detection. This study demonstrated that ERA-based assays possess excellent sensitivity, specificity, and repeatability for HPV16 and HPV18 detection with great potential to become robust diagnostic tools in local hospitals and field studies.
최근 아동들의 온라인 활동에 대한 참여가 활발해짐에 따라 아동을 대상으로 하는 온라인 서비스 역시 빠르게 증가하고 있음에도 불구하고, 아동들은 그들의 보호받아 마땅한 권리를 충분히 보장받고 있지 못하다. 본 연구는 아동이 주로 이용하는 앱서비스를 대상으로 현황 및 사례 조사를 통해 국내 아동 개인정보보호에 관련한 이슈를 유형화하여 분석하는 것을 목적으로 한다. 이를 위해 아동의 개인정보보호에 관한 현황을 살펴보고 국내외 법제도에 근거하여 이슈를 도출했다. 연구 결과, 아동 식별의 이슈, 고지 및 동의의 실효성 이슈, 정보주체로서 아동의 권익 이슈 등이 도출되었다. 현황분석을 통해 살펴본 온라인 환경에서의 아동 개인정보 관련 내용을 바탕으로 정책적 시사점을 도출하고 개선방안 등을 제언하였다.
Pseudoaneurysm of the splenic artery may arise from a vascular erosion by a surrounding inflammatory processes in acute and chronic pancreatitis. Rupture of the pseudoaneurysm may threaten the patient's life. Conservative management for massive hemorrhage may cause 100 percent mortality and even with prompt therapy there is a high mortality. Preoperative detection of bleeding source is desirable because of the difficult identification of the bleeding site at laparotomy. Angiographic identification and embolization of the hemorrhagic vessels in selected cases may obviate the risk of urgent surgery. The authors have recently managed a case of ruptured splenic artery pseudoaneurysm combined with a pancreatic pseudocyst in a 6 years old boy. A bolus enhanced CT scan and angiography confirmed the diagnosis. We managed this child successfully with the urgent transcatheter arterial embolization followed by elective surgery.
저출산 현상이 심화되고 있음에도 불구하고 키즈 패션 시장에서는 시장의 질이 향상되고 있어 브랜드 마케팅의 필요성이 제기되고 있다. 따라서, 본 연구는 자녀를 양육하고 있는 2040 여성 500명을 대상으로 한 온라인 설문을 통해 Aaker(1997)의 브랜드 개성 척도를 기반으로 키즈 패션 브랜드 개성 요인들을 추출하고, 브랜드 충성도를 강화하는 개성 요인의 역할을 살펴보았다. 키즈 패션 브랜드 개성 요인으로 진실함, 유능함, 흥미성이 도출되었고, 각 요인들이 브랜드 충성도에 긍정적인 영향을 미치는 가운데, 진실함과 흥미성의 영향력은 브랜드 동일시에 의해서 충성도에 미치는 영향이 매개되었다. 또한, 자녀의 성별에 따라 매개된 조절효과를 살펴본 결과, 자녀의 성별이 여아로 조사 대상자인 엄마와 성별이 같은 경우에, 유능함이 브랜드 동일시에 미치는 효과가 강화되었고, 자녀가 남아인 경우는 흥미성과 진실성이 강화되었다. 자녀의 성별에 따라서 어머니가 중요하게 생각하는 브랜드 개성 요인이 다르다는 시사점을 가진 본 연구의 결과는 키즈 패션 브랜드의 전략 수립 및 상품 기획 활동에 활용될 수 있을 것으로 기대된다.

본 연구의 목적은 가족의 기본이 되는 부부관계에 초점을 맞추어 부부관계와 부모자녀 관계 사이의 연관성을 알아보고자 1987년 7월부터 9월사이에 소아 정신과에서 치료받은 환아의 부모들과 일반 아동이 부모들을 대상으로 결혼 만족도 검사(Marital Satisfaction Inventory ; MSI)와 부부 적응도 척도(Dvadic Adjustment Scale ; DAS)의 두가지 설문지를 이용하였다. 환아 부모군은 환아의 진단면에 따라 정신증, 신경증, 틱, 자폐증, 정신지연의 5군으로 분류하였고 각군의 결과가 평가되었다. 1) 환아부모군은 대조군보다 결혼만족도가 낮다. 2) 환아부모군은 대조군보다 부부 적응도가 유의하게 낮다(P<0.01). 3) 환아부모군, 대조군에서 여자가 남자보다 역할개념이 더 진보적이며 환아부모군에서 역할부담이 여자에게 편중됨으로써, 양육갈등(conflict in child rearing)은 모성 우월현상과 연관됨을 시사한다. 4) 원래 가족의 고통스러운 가족력(Family History of Distress)는 결혼의 정서적 의사소통, 성생활, 자녀양육과 연관이 깊다. 5) 결혼 만족도 검사(Marital Satisfaction Inventory ; MSI)의 전반적 고통 척도(Global Distress Scale ; GDS)에 의하면 정신증, 신경증, 자폐증, 정신지체, 틱의 순서로 불만족도를 보였다. 6) 부부 적응도 척도(Dvadic Adjustment Scale ; DAS)에 의하면 정신증, 신경증, 틱, 자폐증, 정신지체의 순서로 부적응도를 보였다. 7) 틱군은 자녀양육 갈등 및 양육태도에 문제점을 시사하였고 부부관계 자체는 유의한 장애를 보이지 않았다. 위의 결과로서 부부의 부적응 및 불만족은 자녀양육 및 자녀문제와 밀접한 연관성이 있음을 시사하며 소아의 정신증과 신경증군의 발병요인은 결혼 불만족과 부적응과 관련이 깊으며 자폐증이나 정신지연군의 발병요인은 결혼 불만족과 부적응과 관련이 깊지 않음을 시사한다.
