History of the Task Force for the Korean Clinical Guidelines of the Developmental Disorders

  • Under the Ministry of Health and Welfare of the Republic of Korea, the National Autism and Developmental Disorder Centers for people with developmental disabilities are gradually expanding. The headquarters of the National Autism and Developmental Disorder Center provides support for education, training, and research, and several centers have been effectively operating since 2020. This study aimed to provide practical recommendations and guidelines for specialists such as clinical psychologists, child psychiatrists, allied professionals, community workers, and related administrators. It was developed as a guideline to promote early diagnosis, provide important information on integrated treatment, and assist people with developmental disabilities in Korea to make the best decisions for their quality of life.

Exploring the Clinical Characteristics and Comorbid Disorders of Borderline Intellectual Functioning

  • Borderline intellectual functioning (BIF) is characterized by cognitive impairment and deficits in adaptive functioning. Despite affecting a significant proportion of the population, BIF still remains underdiagnosed and poorly understood. In addition to cognitive impairments across a range of domains, individuals with BIF face a greater risk of academic failure and often require special educational support. They suffer from emotional problems, such as difficulties with emotional awareness, anxiety, depressed mood, and unhappiness. Individuals with BIF are more likely to have an impairment of social and adaptive functioning. Furthermore, individuals with BIF are at higher risk of physical and mental health problems, often receive inadequate treatment, and have a poorer prognosis. This review aims to enhance the understanding of clinicians, educators, and policymakers by providing an overview of the characteristics of BIF and its associated challenges, ultimately contributing to the improvement of support systems for individuals with BIF.

어머니의 인구사회학적 특성과 양육스트레스가 자녀의 자아개념에 미치는 영향 : 양육 태도의 매개적 역할을 고려하여 (Effects of Maternal Sociodemographic Characteristics and Parenting Stress on a Child's Self-Concept: Parenting Style as a Mediating Factor)

  • Objectives : The aim of this study was to examine the effects of maternal sociodemographic characteristics, especially education and employment, and parenting stress on the child's self-concept. Attention was also paid to the mediating impact of parenting style on the relationship between the maternal variables and a child's self-concept. Methods : A questionnaire and three scales were completed by fifth graders and their mothers, and 270 sets of paired data were analyzed. Results : The results showed that mothers with higher education experienced less child-related parenting stress, were perceived to be more achievement-oriented and rational by their children, and also had children with more positive self-concept. The relationship between maternal education and child's self-concept was mediated by the rationality dimension of parenting style. Maternal employment was not related to parenting stress, parenting style and most aspects of the child's self-concept. Lastly, child-related parenting stress had a negative effect on the child's self-concept, and this effect was mediated by the warmth and rationality dimensions of parenting style. Conclusion : Parenting style had a mediating effect on the relationships between the child's self-concept and maternal education and child-related parenting stress.

행동장애에 있어서 Serotonin계의 개체발생적인 과정과 정신병리와의 상호관계에 관한 연구 (STUDY ON THE RELATIONSHIP BETWEEN ONTOGENY OF SEROTONIN SYSTEM AND PSYCHOPATHOLOGY IN CONDUCT DISORDER)

  • 목 적:행동장애의 생물학적인 원인을 규명하고 연령 및 정신병리와의 상호관계를 이해하기 위하여 혈장내의 5-HT와 5-HIAA 농도를 측정하여 대조군과 비교하고, 정신병리, 연령간의 상호관계를 연구하였다. 방 법:1999년 3월부터 2002년 3월 사이 서울대학교병원 소아 ${\cdot}$ 청소년정신과를 방문한 환아중 행동장애라고 판단되는 소아 또는 청소년 환자 41명을 대상으로 하였다. 공격적인 행동, 규칙위반, 반항행동의 평가에는 DSM-IV 진단기준에 입각한 부모평가척도를 사용하였으며, 혈장내의 5-HT 및 5-HIAA 농도의 측정에는 HPLC를 사용하였다. 결 과:1) 소아형 ${\cdot}$ 행동장애, 청소년형 ${\cdot}$ 행동장애, 그리고 대조군간에 혈장내의 5-HT 및 5-HIAA농도간의 의미 있는 차이는 관찰되지 않았다(5-HT, F=2.37, df 2, 61, p>0.05). 2) 혈장내의 5-HT와 5-HIAA의 농도와 공격성(aggression) 및 규칙위반(rule violation)간에는 의미 있는 상관성은 관찰되지 않았다. 3) 혈장내의 5-HT농도와 반항행동(oppositional behavior)간에는 의미 있는 상관성이 관찰되었다.(소아형, Pearson 상관계수 0.43, p<0.05, 청소년형, Pearson 상관계수 0.48, p<0.05). 4) 행동장애군에서(소아형, 청소년형 모두 포함) 연령과 5-HT 및 5-HIAA농도간의 의미 있는 상관성은 관찰되지 않았다. 대조군에서도 역시 의미 있는 상관성은 관찰되지 않았다. 결 론:이러한 소견은 행동장애의 생물학적 원인 중의 하나로 serotonin계의 기능장애라는 가설은 지지하지 못하는 소견이다. 공격성 또는 규칙위반보다는 반항행동이 더 밀접한 관계가 있다. 개체발생적인 과정에 있어서도 행동장애군에서 상관성은 관찰되지 않았다. 향후의 연구에서는 반항장애와 serotonin 계와의 상관성이 연구되어야 한다.

단축형 청소년용 정신화 프로그램의 학생정신건강 증진효과 평가 (Efficacy of the Short Form of Mentalization Improvement Program for Adolescent (MIPAdo) for Enhancement of Mental Health in School Children)

  • Objectives : We developed the short form of the Mentalization Improvement Program for Adolescent-Community Model (MIPAdo-CM-S), based on the concept of the mentalization. Methods : The MIPAdo-CM was composed of 6 sessions and was applied to 133 students in the 1st grade of a middle school for six weeks during their regular school hours. After 6 weeks, we compared the endpoint changes of Adolescent Mental Problem Questionnaire-Revised (AMPQ-R), Emotional Empathy Test, Peer Aggression Scale, Peer Bullying Scale, School Adjustment Scale, Test Anxiety Inventory and Visual Analogue Scale between the subject and the control group. Results : On the Visual Analoge Scale, students in the trial classes reported an increased understanding and respect for others, as well as themselves. Conclusion : The efficacy of MIPAdo-CM was subjective improvement of understanding and respect for both others and themselves. To prove objective usefulness of this program, further studies should be administered in the form of long-term, regular and structured courses.

  • Autism spectrum disorder (ASD) is a neurodevelopmental disorder marked by impaired social communication and repetitive, restricted behaviors and activities. The prevalence of ASD has been increasing for the past 2 decades, but evidence-based therapeutic approaches are lacking for patients with ASD. To date, there is no cure for the core symptoms of ASD, and the existing treatments focus on improving the patient's function and adaptation by using behavioral intervention methods. Behavioral interventions have been proven to show the greatest effect when applied before the age of 2 years, for at least 40-60 hours per week. Many clinicians and ASD families are unfamiliar with the treatment methods, and consequently, may seek unproven and potentially hazardous methods. The purpose of this article was to present an extensive and updated review on evidence-based ASD behavioral interventions that are commonly used in clinical settings.

Analysis of the Status and Future Direction for Digital Therapeutics in Children and Adolescent Psychiatry

  • Digital therapeutics based on software, such as artificial intelligence, virtual reality, games, and smartphone applications, are in the spotlight as new therapeutic alternatives in child and adolescent psychiatry. It draws attention to overcoming conventional therapeutics' limitations, such as toxicity, cost, and accessibility, and encourages patients to participate in the treatment attractively. The growth potential of the digital therapeutics market for psychiatric disorders in children and adolescents in Korea and abroad has been highlighted. Clinical studies and Food and Drug Administration approvals for digital therapeutics have increased, and cases approved by the Ministry of Food and Drug Safety have emerged in Korea. As seen above, digital transformation in child and adolescent psychiatry will change treatment paradigms significantly. Therefore, as this new field has just begun to emerge, it is necessary to verify the effectiveness and scope of the application of digital therapeutics and consider preparing a compensation system and institutional arrangements. Accordingly, this study analyzed the development trends and application status of digital therapeutics in children and adolescents and presented limitations and development directions from the perspective of application in healthcare. Further, the study is expected to identify the utility and limitations of digital therapeutics for children and adolescents and establish effective application measures.

Custody Evaluation in High-conflict Situations Focused on Domestic Violence and Parental Alienation Syndrome

  • In a divorced family, child-centered custody evaluation is essential to ensure the child's best interests and healthy adaptation. A mental health professional's role and involvement are required in gaining an in-depth understanding of various environments and dynamics surrounding the child and family. Domestic violence, including child abuse and intimate partner violence (IPV) or parental alienation syndrome (PAS), is often observed in cases of custody evaluation in high-conflict divorced families, sometimes accompanied by allegations. Such cases warrant an extremely careful approach by the evaluator, who needs to be competent in interpreting the familial dynamics based on a reasonable context understanding. Genuine professionalism is a must for a custody evaluator to best help the child and carry out a high-quality custody evaluation process, and evaluators need to be ready for this task through adequate preparation and empowerment. This article is devoted to examining custody evaluation in divorced families in cases of IPV, child abuse, and PAS.

청소년의 자살 가능성에 대한 임상 평가 (Assessing Suicidality in Adolescents)

  • Objectives : To consider which questions are most useful clinically and to find a framework for assessment of suicidality or risk of suicide for adolescents. Methods : Authors reviewed and integrated both clinical works and researches about measures used to evaluate suicidal behaviors in adolescents. Results : The 'cascade approach' for assessment of suicidality was developed; the quality and quantity of 'distress' leads to an entry question to do with the 'quality of life' and whether it is worth living. These two questions lead to examinations of 'thoughts' which may lead to 'plans' which leads to 'threats' which then leads to 'deliberate self injury' and then on to a history of 'past suicide attempts'. Authors also suggested to use a summed score of cascade approach in clinical decisions and showed how to modify it up to different evaluation situations. Focusing on the good things helps not only to qualify suicidality but also to begin the search for solutions. Conclusion : In the clinical assessment process, while scales assist the clinician, there is nothing to supplant a sensitive clinical approach. The 'cascade approach' was proposed as a framework for assessing adolescents for suicidality.

모-자녀 간 역기능적 의사소통 유형의 군집과 특성분노 및 분노대처방식의 관계 (The Relationships Between Clusters of Types of Mother-adolescent's Problems in Family Communication and Adolescent's Trait Anger and Anger Coping Strategies)

  • This study examined natural groupings of mother-adolescents in terms of the sub-factors of problems in family communication. The natural groupings were as follows; the placating type, the blaming type, the super-reasonable type, and the inattention type. In addition, this paper also examined individual differences in trait anger and anger coping strategies patterns by clusters of sub-factors of problem in family communication. The subjects of this study consisted of 406 adolescents. Data were analyzed by means of cluster analysis and one-way ANOVA. The results from cluster analysis with the adolescent sample suggested the presence of four clusters ('placating-suppression', 'mixing up-confusion', 'authoritarian-hostility', 'consistent-repression'). Additionally, these four groups were found to be related to trait anger and anger coping strategies (anger-suppress, anger-out, anger-control).
