  • The psychiatric interview is not a random or arbitrary meeting between doctor and patient. It is a systematic attempt to understand the relationship of psychopathology to emotional conflicts in patient, and interviewing is an in rather than a science, a skill that can be acquired but probably not taught. The faculty of the Seoul National University Hospital, Department of Psychiatry, have been providing 12 sessions of seminar on Psychiatric Interview and Psychopathology every year for the first year residents for the past 15 years. The authors presented three cases materials with their understandings of psychopathology and psychodynamics, each of which the authors had performed live interviews for 50 minutes in front of the residents. Those are a young male with conversion disorder whose chief complaints was motor aphasia, a young college girl with paranoid schizophrenia whose chief complaints were persecutory and erotic delusions with auditory hallucination, and a climacteric female in delusional disorder who had a delusion of infidelity. The most frequent questions the residents raised after the presentations were on the significances of the first interview, danger of making the diagnoses with short interviews, and methodology of questionings and not-questionings. In reply the authors discussed the importance of understandings of psychopathology and psychodynamics, the flexibility of the techniques of interviewing, and priority-related matters in which the understanding of psychodynamics proceed to the diagnoses.

The Hoehn & Yahr scale, Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale (UPDRS) were used to assess the overall function of the patients. And the Global Assessment Scale (GAS) was used to assess the change of chief complaints after the treatments. Result and Conclusion The non-motor and motor symptoms of Parkinson's disease improved after Sasang constitutional medicine and western medicine treatments for more than a year. Furthermore these patients' chief complaints improved without increasing of amount of western medicine (levodopa). In conclusion, this study showed that Sasang constitutional medicine with western medicine can be effective treatment for long-term clinical care of Parkinson's disease.

  • Objectives: This study was aimed to report changes in clinical symptoms and signs after treatment with Korean medicine in patients who suffered from calf edema due to deep vein thrombosis (DVT). Methods: A 46-year-old male patient, who discharged home after receiving standardized treatment for acute DVT in the right leg, suffered pain and heat sense due to edema of the affected calf. Oral administration of herbal medicine (Gyejibongnyeong-hwan 4 g, twice daily) and acupuncture therapy were given to him for 6 weeks. The change in chief complaints, Villalta score, the right and left side difference of the circumference and the skin surface temperature of both calves, and blood level of D-dimer were observed before and after treatment. Results: The patient's chief complaints began to significantly improve from 2 weeks after treatment, and disappeared completely within 4 weeks. At the end of the treatment, a substantial decrease in the difference between the right and left calves in the circumference and skin surface temperature was observed. This effect was maintained even at the time of follow-up 3 months after the end of treatment, and the patient completely recovered indoor and outdoor life to the level before onset. Conclusions: This case suggests that Korean medicine treatment, including Gyejibongnyeong-hwan administration and acupuncture therapy, can be a viable option to improve edema and related clinical problems in the affected limbs due to DVT.

  • Background: Acupotomy is a type of acupuncture where a scalpel-shaped needle (miniscalpel needle) is used instead of a normal acupuncture needle to exfoliate adhesion sites or to relax entrapped regions. This study aimed to identify the descriptive characteristics of patients who received acupotomy treatment at a single Korean Medicine Clinic. Methods: This retrospective review analyzed the medical charts of patients who had received acupotomy at least once from August 2017 to December 2019 at a single Korean Medicine Clinic. The demographic characteristics, chief complaints, acupotomy treatment sites, and principal diagnosis codes were analyzed. Results: We identified 551 outpatients; the average age was 52 ± 14.26 years and 49.9% were male. The patients underwent an average of 8.47 sessions of acupotomy. Altogether, 35.91% of the acupotomy treatments were administered to the spinal regions, of which 60.01% were in the lumbar region. The codes related to the lumbar spinal condition/disease which were used most frequently. The chief complaints were dizziness, lumbar spinal stenosis, and Dupuytren's contracture in patients over 60 years of age. Conclusion: This is the 1st analysis of acupotomy treatment patterns in Korea to date. Acupotomy is primarily administered in the treatment of spinal conditions/diseases, especially for those involving the lumbar region. Future studies are necessary to determine the clinical outcomes of patients who receive acupotomy treatment and the safety of this treatment.

  • Objectives The purpose of this study is to report on the collecting status of the prospective clinical data, that is designed to find clinical factors for determining the constitutional prescription. Methods We collected the prospective clinical data of 1208 subjects in the 10 oriental hospitals and 2 oriental clinic, from September 2012 to July 2015. Subjects were diagnosed and prescribed by the Sasang Constitution specialist. We counted the usage frequency of each constitutional herbal formula. Besides we check the main chief complaints on each constitutional herbal formula. Results & Conclusions In TE type, Yeoldahanso-tang, Cheongsimnyeonja-tang were used in high frequency. In SE type, Palmulgunja-tang, Gwakhyangjunggi-san were used in high frequency. In SY type, Hyeongbangjihwang-tang, Yanggyeoksanhwa-tang were used in high frequency. The both of high frequency-prescriptions in TE types were for the Interiorheat disease, expecially liver-heat lung-dry symptomatology. The main chief complaint of high frequency-prescriptions was fatigue in every constitution. We hope that the process of constitutional prescription would be standardized through the subsequent researches.

  • In January 1997, there was a change in agricultural extension system enrolling the provincial and county level extension personnel in local government. They were previously enrolled in central government. Even though the extension agents' status were secured, but there were some complaints from the extension personnels until now. The purpose of this study was to examine the possibility to improve the extension service after the change. Some of the tasks for improvements from the change were as follows; 1) Revision of the laws related to rural development to protect against side effects on the localization, 2) Revision of the provisions to interchange personnels between the central and local extension service, 3) Provisions for organizing county level agricultural extension committee, and 4) Central government's support for the personnel expenses of local extension agents. Though there were such several improvements, some extension personnel still raise objection to the change as following; (1) the right of personnel management mandated partially from the chief of local government to the chief of the extension office, (2) substantial raising of the extension agents' pay, and 3) promotion in rank of extension educators.
