함몰흉 교정의 변형수기 (A Modified Technique for the Correction of Funnel Chest)

  • Background : The authors have modified the method of Ravitch technique. Material and Method ; This technique was applied to 6 patients out of 18 patients who underwent corrective surgery from May 1987 to July 1999. The technique is quite different from that of Ravitch. We did not divide the intercostal muscle bundles from the laterals of sternum and the Akin's struts were placed retrosternally crossing the chest horizontally to prevent flail motion during immediate post-operative period and retraction of the sternum afterwards. Anterior sternal osteotomy instead of the posterior one was performed for the latest 3 cases which made operative procedure more simple and easy. The struts were removed one year later. Result : Compared to the hospital stay of the patients who received standard Ravitch method that of the six cases who received our modification was definitely shortened from 13.1 days to 8.3 days(p<0.0000). Flail motion was not noted in any patient and chest wall stability was obtained more easily with this technique. Conclusion : Our modification is recommendable for correction of funnel chest in regards to shorter operation time better chest wall stability shorter hospital stay and less complication.

절반 흉골반전법;비대칭 누두흉에 대한 새로운 수술기법의 제안 (One-half Sternal Turnover; New Operative Approach for Asymmetrical Funnel Chest)

  • The sternal turnover has a limited use in an asymmetrical funnel chest. However we tried `One-half sternal turnover` as a new operative approach for an asymmetrical funnel chest. Through the bilateral submammary skin incision, median sternotomy was made from xiphoid process to midsternum and extended horizontally. The segment of ribs were cut at the angle of depression. The en-bloc resected chest wall segment contained one-half sternum as well as a part of ribs and left half of rectus muscle. After turning over the en-bloc segment, reapproximation with wiring was done. Sternotomy wound was closed in layer after placing of substernal drainage tube. Postoperatively, the chest wall was stable and the recovery course was uneventful except left-sided minimal pneumothorax which was cured spontaneaously. The patient was discharged on postoperative 14th day.

  • A 36 years old male patient underwent en bloc excision of the primary chondrosarcoma arising from 8th, 9th and 10th ribs of the chest wall. The tumor was 11.5 x 7.5 x 6.5 cm in size and was 320 gram in weight. After en bloc excision of the tumor, the defect of the chest wall was reconstructed with pedicle muscle flap and using compression dressing for 2 weeks, there happened no respiratory problem. And so we made a case report and review of literature.

Post-traumatic Intramuscular Hemangioma of the Chest Wall

  • Intramuscular hemangioma originated in chest wall is a rare benign tumor, with no relevant reports in Korea. In most cases, the tumor is discovered before the age of 30 years and it is reported that trauma operates as the initiation factor. It is essential to concern the clinical suspicion and conduct a CT scan for diagnosis. The principle of treatment is surgical excision with clear resection margin. The authors of this study report a case of surgical excision for post-traumatic intramuscular hemangioma of the chest wall with review of literature.

  • Extraskeletal Ewing`s sarcoma was first reported by L. Angervall and F.M. Enzinger in 1975. Clinically the tumor affects mainly young adults and children The extraskeletal Ewing`s sarcoma are mainly involved the soft tissues of the trunk and lower extremity. Microscopically. extraskeletal Ewing`s sarcoma is indistinguishable from the Ewing`s sarcoma of bone. We recently experienced a case of extraskeletal Ewing`s sarcoma of the anterior chest wall in a 13-year old female. The patient underwent local wide excision and postoperative radiation and combind chemotherapy were done. This group of tumors has been rarely reported in Korea. We presented a case of extraskeletal Ewing`s Sarcoma in the anterior chest wall with reviw of world literature.

  • 유척삭종(parachordoma)은 팔, 다리, 가슴에 주로 발생하며 천천히 자라는 저등급 악성 종양으로 알려져 있다. 대표적인 감별진단으로는 척삭종(chordoma), 뼈외 점액양 연골육종(extraskeletal myxoid chondrosarcoma) 등이 있으며, 조직학적 검사와 함께 면역조직화학적 검사가 감별진단에 도움이 된다. 저자는 우측 흉벽에 발생한 유척삭종을 광범위 절제 및 흉벽 재건술 후 16개월 동안 재발없이 추적 관찰한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • Pilomatrix carcinoma originates in the matrix cell and is marked by exophytic growing and common local recurrence. There is no established treatment for wide local invasion and metastasis of the pilomatrix besides wide surgical resection. We report a case of rapidly progressive pilomatrix carcinoma, which arose around an Eloesser open window with direct invasion to the adjacent tissue.

  • In chest wall reconstruction after wide chest wall resection, the use of a musculocutaneous flap or prosthetic materials is inevitable for maintaining thoracic movement and a closed pleural cavity. We report a case of a 63-year-old male with a large invasive thymic carcinoma in the anterior mediastinum. The mass measured 6.8 cm and involved the sternum, left side of the parasternal area, ribs, and intercostal muscles. The patient underwent subtotal sternectomy, radical thymectomy, and reconstruction with biological mesh (Permacol). Successful chest wall reconstruction without any other complications was achieved, demonstrating the effectiveness of Permacol.

  • Rhabdomyosarcoma is relatively rare in general pediatric population. Furthermore, the primary site in the thorax is one of the least sites. Because most patient, when first seen, are extensively advanced and bad in prognosis, an adequate work-up prior to any definitive therapy must be undertaken. Author experienced 2 cases of embryonal rhabdomyosarcoma originating from chest wall. Both 2 cases are included in Group III [Rhabdomyosarcoma-Intergroup Protocol Grouping], one case is treated with radical surgery and radiation therapy, and the other case is treated with radiation therapy and chemotherapy. So author present 2 cases of embryonal rhabdomyosarcoma with reference.

종격동내 섬유종증;1례 보고 (Mediastinal Fibromatosis - One Case Report -)

  • Fibromatoses, a broad group of fibrous proliferations of similiar microscopic appearance, show intermediate biological behavior between benign fibrous lesion and fibrosarcoma. they are characterized by the infiltrative growth and the tendency toward recurrence, but they never metastasize. The mediastinal fibromatosis has been extremely rarely reported. We experienced a case of anterior mediastinal fibromatosis ingrowing onto the anterior chest wall in 2-year-old female. A radical surgical resection and anterior chest wall reconstruction was performed successfully.
