구명뗏목에서의 비상대응 심폐소생술의 효과 분석 및 개선에 관한 연구 (A Study on the Effect Analysis and Improvement of Cardiopulmonary Resuscitation on Life-rafts)

  • 선박, 해양플랜트, 해상풍력 등과 같은 해양에서의 근무 환경은 고립되어 있고 거친 해상에 바로 노출되어 있어 안전사고의 높은 위험성을 안고 있다. 이에 해양에서 근무를 하는 모든 종사자에게는 비상상황에 대한 대처 능력이 요구되고 있고 비상대응업무를 수행하는 자에 대해서는 STCW협약, OPITO Safety Training Standard 등의 관련 규정에 근거하여 자격 증명이 이루어지고 있다. 특히, 고립된 상황에서 해양종사자들의 응급처치기술은 필수 요건 중의 하나이다. 그러나 선박을 포함한 해양구조물에서 비상탈출 시 사용하게 되는 구명뗏목에서의 심폐소생술은 구명뗏목의 바닥이 고무재질로써 충분한 가슴압박이 힘들어 응급처치자의 심폐소생술에 대한 피로도와 정확도에 영향을 미칠 것으로 추정된다. 본 논문은 조파장치가 설치되어 있는 안전훈련센터에서 15명의 응급처치강사를 표본으로 바다와 유사한 환경을 조성하여 구명뗏목에서의 심폐소생술 실험을 하였다. 실험 결과, 강의실에서의 심폐소생술의 정확도 평균은 99.6 %였으나 구명뗏목의 여러 환경에서의 심폐소생술의 정확도는 84 %였다. 두 장소의 심폐소생술 정확도에 대한 t 값의 절대값이 임계값의 절대갑보다 크기 때문에 구명뗏목의 심폐소생술의 정확도가 낮아짐을 검증하였다. 파도를 0.3미터 형성한 경우에는 구명뗏목의 심폐소생술의 정확도가 77 %로 낮아짐을 확인하였다. 이에 본 논문을 통해 구명뗏목에서의 심폐소생술은 2인 1조로 실시할 것을 제시하며 최근 활용되고 있는 흉부 압박 자동화 장비를 활용할 것을 추천한다.

병원 내 심폐소생술의 자세별 피로도와 가슴압박 정확도 비교 분석 (Comparative analysis of cardiopulmonary resuscitation accuracy and fatigue by posture in hospitals)

  • Purpose: This study aimed to identify effective cardiopulmonary resuscitation methods by comparing the fatigue of rescuers according to various positions in cardiopulmonary resuscitation situations conducted on beds in hospitals. Methods: An experimental study of students in the department of emergency medical service in H University, G Metropolitan City was conducted in four positions for applying chest pressure on mannequins on beds. Results: As a result of measuring the muscle fatigue of four muscle attachments according to the four positions conducted on the bed, the average was 3.4%, the P was significant at 0.001, and the fatigue difference was confirmed to occur depending on the attachment. An analysis of pressure depth by pose revealed that P1, P2, P3, and P4 have a depth of 58.3, 55.1, 56.4, and 56.3 mm, respectively, with P4 having the deepest depth. Conclusion: Among the various postures of the rescuer during cardiopulmonary resuscitation performed on the bed in the hospital, P1 is thought to be the most tiring, although its associated CPR quality is good.

Chest compression quality, exercise intensity, and energy expenditure during cardiopulmonary resuscitation using compression-to-ventilation ratios of 15:1 or 30:2 or chest compression only: a randomized, crossover manikin study

  • Objective Our aim was to compare the compression quality, exercise intensity, and energy expenditure in 5-minute single-rescuer cardiopulmonary resuscitation (CPR) using 15:1 or 30:2 compression-to-ventilation (C:V) ratios or chest compression only (CCO). Methods This was a randomized, crossover manikin study. Medical students were randomized to perform either type of CPR and do the others with intervals of at least 1 day. We measured compression quality, ratings of perceived exertion (RPE) score, heart rate, maximal oxygen uptake, and energy expenditure during CPR. Results Forty-seven students were recruited. Mean compression rates did not differ between the 3 groups. However, the mean percentage of adequate compressions in the CCO group was significantly lower than that of the 15:1 or 30:2 group ($31.2{\pm}30.3%$ vs. $55.1{\pm}37.5%$ vs. $54.0{\pm}36.9%$, respectively; P<0.001) and the difference occurred within the first minute. The RPE score in each minute and heart rate change in the CCO group was significantly higher than those of the C:V ratio groups. There was no significant difference in maximal oxygen uptake between the 3 groups. Energy expenditure in the CCO group was relatively lower than that of the 2 C:V ratio groups. Conclusion CPR using a 15:1 C:V ratio may provide a compression quality and exercise intensity comparable to those obtained using a 30:2 C:V ratio. An earlier decrease in compression quality and increase in RPE and heart rate could be produced by CCO CPR compared with 15:1 or 30:2 C:V ratios with relatively lower oxygen uptake and energy expenditure.

코로나19 의심 또는 확진 환자가 병원 밖 심정지 발생 시 구조자의 개인 보호장비(PPE) 착용에 의한 가슴압박 효과 비교 : 무작위 교차 시뮬레이션 연구 (Effect of level D personal protective equipment on chest compression for pre-hospital arrest patients with suspected or confirmed COVID-19 : A randomized crossover simulation trial)

  • Purpose: The purpose of this study was to determine how the use of level D personal protective equipment (PPE) and cardiopulmonary resuscitation (CPR) feedback equipment affects chest compression (CC). Furthermore, this study provides basic data for developing Korean CPR guidelines that can be applied to patients with suspected or confirmed COVID-19. Methods: This randomized, single-blinded, crossover simulation trial included 26 student paramedics who performed 2-minute chest compressions using three different methods: Method A involved performing traditional CC for two minutes without donning level D PPE, Method B involved performing CC while donning level D PPE, and Method C involved performing CC with a CPR feedback device while donning level D PPE. Results: The use of a CPR feedback device during the 2-minute CC increased the exercise intensity of the subjects, but donning level D PPE didn't affect the quality of CC and the exercise intensity. The results of methods A and B showed changes in the quality of compression 80 seconds after the start of CC. No significant changes occurred in 2-minute CC when using the CPR feedback device. Conclusion: Using a CPR feedback device could prevent deterioration in the quality of CC while donning level D PPE.

COVID-19 대응 심정지에서 가슴압박 교대 인원에 따른 구조자의 피로도 -마네킹 연구- (Measuring rescuer's fatigue by evaluating varying sized groups of rescuers performing chest compressions on a manikin study for suspected COVID-19 patients)

  • Purpose: The aim of the study is to measure the quality of cardiopulmonary resuscitation (CPR) and the fatigue of rescuers wearing PPE (Level D) during a CPR session and to ultimately provide suggestions of safety standards for rescuers. Methods: 36 subjects were enrolled in the study. The subjects were divided randomly into three groups of two-members, three-members, and four-members. Each group performed CPR for 30 minutes. Blood lactate concentration, heart rate, rating of perceived exertion, chest compression depth and rate were measured before experiment and after each cycle. Results: There was a difference in the blood lactate concentration during CPR cycle by member of shifts (p=.014). The blood lactate concentration increased during CPR (p=.000). Subjective fatigue was a significant difference of chest compression in cycles 3, 4, and 5 for the member of shifts during CPR (p=.049, p=.009, p=.015). Depth and rate of chest compression were not different for the member of shifts during CPR. Conclusion: It is necessary to establish standards for the member of shifts during CPR, to reduce the fatigue of rescuers.

시간 경과에 따른 1인 심폐소생술의 질과 피로도의 변화 (The variation of elapsed time on fatigue and quality of single rescuer cardiopulmonary resuscitation)

  • Purpose : The purpose of this study was to investigate the variation of elapsed time in the cardiopulmonary resuscitation (CPR) quality and the fatigue in continuous CPR by single rescuer. This study will provide basic data about the time for the alternation of the CPR providers. Methods : The volunteer students having healthcare provider certification were recruited from the department of emergency medical service. The students performed 30:2 CPR for 20 minutes, and the data were recorded and analyzed. Metrics were based on the 2010 American Heart Association (AHA) Guidelines, and the CPR continued without any feedback. Results : Among the indicators of CPR, the accuracy and the depth of chest compressions decreased after about 5.3 minutes, and the rate increased approximately after 6.8 minutes. Changes in clinical indicators appeared, and fatigue increased after about 3 minutes. According to the increase in fatigue level, the changes in the CPR indicators and clinical indicators showed up, and these results proved to be statistically significant. However, there were no associations among the time, fatigue, and gender. Conclusion : Even though the times of changes in the indicators appeared differently, the times of changes in fatigue and CPR quality were able to be confirmed.

심폐소생술(CPR) 및 자동제세동기(AED)에 대한 웹사이트 내용성 분석 (Analysis of Contents of Website Information about CPR and Automated External Defibrillator)

  • The purpose of this study is to give the users, who wants to know the information about CPR and AED on the Internet, more accurate information which is analyzed and considered with the factors to improve the contents of them. The source of the instrument for this study is from the contents of twenty four websites which is able to join in and searched by the Web Search Engine with four keywords: 'Cardiopulmonary Resuscitation', 'CPR', 'Automated External Defibrillator', and 'AED'. According to the results of the study, frequency questions of more than 50% among 53 questions on whether to suggest were investigated as 22 questions from content analysis results of the websites, and it showed that the highest frequency, which were examined to that chest compressions and respiratory rate; 79.2%. The correctness of the contents of the all websites of the study is as low as 36.00 point full marks at 100 points.

3인 CPR 프로토콜 개발 및 타당도 검증 (Development and validation of a three-person cardiopulmonary resuscitation protocol)

  • Purpose: This protocol aims to improve the simplicity of the existing two-person cardiopulmonary resuscitation (CPR) by focusing on chest compressions and artificial respiration, and classifying the roles within the scope that can be observed. Methods: In this study, the protocol was developed by forming an expert group consisting of a professor from the emergency and rescue department, a professor from the nursing department, and a first aid officer from the fire department. In addition, if the number of panels is 15 and the derived content validity ratio (CVR) value exceeds 0.49, a reference point that satisfies content validity at 95% reliability is presented. Results: All 11 performance items exceeded the CVR value of 0.49 to meet the reference point, and the CVR range of this protocol is 0.625 to 1.000. Conclusion: In order to improve the quality of three-person CPR, verification of simulation models and continuous protocol revision should be conducted by individuals specializing in various fields.

  • This study proposes an algorithm for removal of CPR artifact in order that automated external defibrillator (AED) can effectively diagnose ECG rhythm during cardiopulmonary resuscitation (CPR). Current AED required to interrupt chest compression for reliable rhythm analysis to avoid the effect of artifacts produced by CPR. However even temporarily interruption of chest compression during CPR adversely affects the probability of restoration of spontaneous circulation (ROSC) and survival after the delivery of the shock. Therefore, we proposed a method for removal of CPR artifacts using least mean square (LMS) filter. The removal of the CPR artifacts would enable compressions to continue during AED rhythm analysis, thereby increasing the likelihood of resuscitation success. It was tested on 31 segments of shockable and 300 segments of non-shockable ECG signals recorded from three pigs during CPR. In the result, sensitivity (Se) and specificity (Sp) analysis on the test segments showed values of Se = 3.2%, Sp = 66.0% and Se = 96.8%, Sp = 98.7% in the case of unfiltered and filtered signals during CPR. In conclusion, it was shown that the proposed method can be a useful tool to exactly diagnose the ECG rhythm during the CPR.

Comparison analysis of rescuer's neck muscle tone and stiffness according to cardiopulmonary resuscitation skill-level

  • 이 연구의 목적은 가슴압박소생술(cardiopulmonary resuscitation, CPR)의 숙련도에 따른 목근육에 근긴장도와 근경직 차이를 확인하기 위해 실시되었다. 연구대상자는 20대 여학생 30명으로 숙련자군과 비숙련자군 각각 15명이었다. 연구결과, 숙련자군은 가슴압박 수행 후 손잡기(hand grip)의 위손쪽 아랫목근육의 근경직과 위등세모근의 근긴장도가 유의하게 감소되었다(p<.05). 비숙련자군은 가슴압박 후 양쪽 위등세모근의 근긴장도와 근경직이 모두 유의하게 감소되었지만(p<.05) 연구군 간에 유의한 차이는 없었다. 그리고, 비숙련자군은 숙련자군과 비교하여 가슴압박 수행 후 평균압박속도, 총압박횟수, 가슴압박 정확도, 가슴이완 정확도가 유의하게 낮았다(p<.05). 이 연구를 통해 가슴압박은 수행자의 목근육에 근생리학적 영향을 미치지만, CPR 숙련도에 따른 차이는 없으며, 숙련도는 CPR 정확성에 중요한 요소인 것을 확인하였다.
