The clinical usefulness of fat suppression by chemical shift selective(CHESS) pulse in MRI (MRI에서 화학적 이동 선택(CHESS) pulse에 의한 지방소거의 임상적 유용성)

  • Magnetic Resonance Imaging(MRI) has chemical shift phenomenon between fat and water, and the phenomenon has influence on structure enclosed by fat. Strong signals emitted from fat often generate false artefact, which reflects the importance of fat suppression techniques. There have been a number of researches on fat suppression techniques, but using fat suppression method alone in MRI can cause difficultproblems in diagnosis. This paper aims to study a fat suppression method by Chemical Shift Selective saturation(CHESS). This research describes the theoretical background and the experiment on water and fat phantom with MR instruments. In the experiment, CHESS pulse was designed by utilising Matlap program, and the pulse diagram was generated for the Pre-saturation process. The experiment using water and fat phantom was applied to C-spine, L-spine and Breast, and produced successful fat suppression results. This experiment has proved that the CHESSpulse fat suppression is a very helpful technique in diagnosing medical imaging. [ $T_2$ ]-relaxation Time Measurement of ex vivo $^1H$ MR Metabolite Peaks for Evaluation of Human Stomach Cancer

  • In this study, transverse relaxation time (T2) measurement and the evaluation of the characteristics of the spectral peak related to stomach tissue metabolites were performed using ex vivo proton magnetic resonance spectroscopic imaging (MRSI) at 1.5-T MRI/S instruments. Thirty-two gastric tissues resected from 12 patients during gastric cancer surgery, of which 19 were normal tissue and 13 were cancerous tissue, were used to measure the $T_2$ of the magnetic resonance spectroscopy (MRS) peaks. The volume of interest data results from the MRSI measurements were extracted from the proper muscle (MUS) layer and the composite mucosa/submucosa (MC/SMC) layer and were statistically analyzed. MR spectra were acquired using the chemical shift imaging (CSI) point resolved spectroscopy (CSI-PRESS) technique with the parameters of pulse repetition time (TR) and echo times (TE) TR/(TE1,TE2)=1500 msec/(35 msec, 144 msec), matrix $size=24{\times}24$, NA=1, and voxel $size=2.2{\times}2.2{\times}4mm^3$. In conclusion, the measured $T_2$ of the metabolite peaks, such as choline (3.21ppm) and lipid (1.33ppm), were significantly decreased (p<0.01 and p<0.05, respectively) in the cancerous stomach tissue.

  • $^{19}$ F imaging of mice was carried out. For $^{19}$ F imaging, 5-flouro-uracil (5-FU) was injected into a mouse and in vivo detection of the catabolism of 5-FU to a-fluoro-P-alanine (FBAL) was carried out. The chemical shift selective (CHESS) imaging technique was employed. The 19F spectra and images give temporal and spatial information of the metabolism for 5-FU in mice.

  • MRI의 artifact는 대부분 신호의 부호화 방향에 따라서 방향성을 가지는데, 이를 요약해보면, 위상부호화 방향의 artifact에는 motion artifact, flow artifact, RF noise등이 있고, 주파수 부호화 방향의 artfact는 susceptibility artfact, chemical shift artifact, central line artifact등이 있으며, 양방향 모두 생길수 있는 것은 Aliasing artifact와 Gibb's phenomenon이고, 전체적으로 영샹의 질을 떨어뜨리는 것은 susceptibility artifact, Eddy current, cross talk등이 있다. 이런 artifact는 대부분은 MRI 자체의 물리적 특성에 다소간 기인하므로, artifact가 없는 양호한 영상을 얻기 위해서는 MRI의 설치 단계부터 관심이 필요하고, MRI의 기본원리와 다양한 artifact에 대해 이해함으로써, 제거 가능한 artifact는 제거하여 양질의 영상을 만들고 판독시의 오류를 피할 수 있도록 해야할 것이다.

Parameters Affecting India Ink Artifacts on Opposed-Phase MR Images

  • Purpose: To determine the MR parameters affecting India ink artifacts on opposed-phase chemical shift magnetic resonance (MR) imaging. Materials and Methods: The use of a female Sprague-Dawley rat was approved by our Institutional Animal Care and Use Committee. Using an iterative decomposition of water and fat with echo asymmetry and least-squares estimation (IDEAL) images, which is a modified Dixon method, axial opposed-phase images of the abdominal cavity were obtained with different MR parameters: series 1, different repetition times (TRs; 400, 2000, and 4000 ms); series 2, different echo times (TEs; 10, 50, and 100 ms); series 3, different field of views (FOVs; 6, 8, 16, and 24 cm); series 4, different echo train lengths (ETLs; 2, 4, and 8); series 5, different bandwidths (25, 50, and 85); and series 6, different slice thicknesses (1, 2, 4, 8, and 16 mm). Artifacts on opposed images obtained with different parameters were compared subjectively by two radiologists. For objective analysis, the thickness of the artifact was measured. Spearman's correlation between altered MR parameters and thicknesses of India ink artifact was obtained via objective analysis. Results: India ink artifact was increasingly apparent using shorter TE, larger FOV and ETL, and thicker slices upon subjective analysis. The objective analysis revealed a strong negative correlation between the thickness of the artifact and TE (r = -0.870, P < 0.01); however, strong positive correlations were found between FOV (r = 0.854, P < 0.01) and slice thickness (r = 0.971, P < 0.01). Conclusion: India ink artifact was thicker with shorter TE, larger FOV, and larger slice thickness.

  • MRS is to measure very small metabolite signals, whose resonant frequencies spread over the chemical shift range characteristic of the measured nucleus. The MR signal originates from the excited volume, which is a column of tissue divided into slices by gradient or rf encoding. The parameters that acquired data affected by TE, TR, and other variables. The higher spatial resolution of 3D CSI compared to SVS and its ability to examine regional metabolite variations for brain study.

Proton Magnetic Resonance Chemical Shift Imaging(1H-CSI)-directed Stereotactic Brain Biopsy (양성자 화학적 이동영상기법(1H-CSI)을 이용한 정위적 뇌생검)

  • Objective : To obtain more reliable sample in stereotactic biopsy, authors adopted proton chemical shift imaging ($^1H$-CSI)-directed biopsy. Until now, proton single voxel spectroscopy($^1H$-SVS) technique has been reported as a technique using metabolic information in stereotactic biopsy. The authors performed $^1H$-CSI with a stereotactic headframe in place and evaluated the pathologic results obtained from local metabolic information through $^1H$-CSI. Methods : $^1H$ CSI-directed stereotactic biopsy was performed in four patients. $^1H$-CSI and conventional Gd-enhancement stereotactic MRI was done simultaneously after application of the stereotatic frame. After reconstruction of metabolic maps of NAA/Cr, Cho/Cr, and Lactate/Cr ratios, the focal areas of increased Cho/Cr ratios and decreased NAA/Cr ratios were selected for target sites in the MR images Results : There was no difficulty in performing $^1H$-CSI with the stereotactic headframe in place. In pathologic examinations, the samples taken in area of increased Cho/Cr ratios and decreased NAA/Cr ratios showed the features of increased cellularity, mitoses and cellular atypism, thus facilitated the diagnosis. The pathologic samples taken from the area of increased Lactate/Cr ratios showed prominent feature of necrosis. Conclusion : $^1H$-CSI was feasible with stereotactic head frame in place. The final pathologic results obtained in our samples were concordant with the local metabolic informations from $^1H$-CSI. Authors believe that $^1H$ CSI-directed stereotactic biopsy may provide us advantages in obtaining more reliable tissue specimen in stereotactic biopsy.

Imaging the Enzymatic Reaction of Urease Using Liquid Crystal-Based pH Sensor

  • In this study, real-time and label-free methods for monitoring the enzymatic reaction of urease, which releases ammonia through the hydrolysis of urea in an aqueous solution, were developed using a liquid crystal (LC)-based pH sensor. Nematic liquid crystal 4-cyano-4'-pentylbiphenyl (5CB), doped with 4'-pentyl-biphenyl-4-carboxylic acid (PBA), exhibited a shift in optical appearance from bright to dark when it was in contact with ammonia generated from the enzymatic reaction between urease and urea. This optical change was attributed to the anchoring transitions of LCs caused by hydrophobic interactions between the tails of deprotonted PBA ($PBA^-$) molecules and the LCs at the aqueous/LC interface. This novel technique holds great promise for the sensitive detection of urease along with its substrates and inhibitors.
