Comparison of oral hygiene status by clinical dental hygiene care performance (임상치위생관리과정 수행 전·후에 따른 구강위생 상태 비교)

  • Objectives : The purpose of this study was to investigate the oral hygiene status before and after the dental hygiene care performance to patients by the dental hygienists. Methods : Subjects were 55 adults visiting to dental clinics in Chungnam. The patients agreed to undergo a dental hygiene care performance. A skilled dental hygienist provided dental hygiene care performance including periodontal status, halitosis and oral hygiene status. Results : The periodontal pocket depth was compared before and after the performance. The depth dropped from 4.02 mm to 2.81 mm, and the value of halitosis dropped from 45.78 bbv to 35.76 bbv. The O'Leary index of the patients dropped form 49.37 to 32.84, and all the differences were statistically significant. Conclusions : Proper use of oral hygiene supplies and regular dental checkup can prevent periodontal diseases. This study will provide the useful information of the effective application of dental hygiene care performance.

Socioeconomic Inequalities in Preventive Services among the Elderly: Results from Medical Checkup, Cancer Check, and BP Check (노년기 예방검진에서 사회경제적 불평등)

  • Objectives: Due to the assumptions of homogeneity as well as challenges in the socioeconomic position of the elderly, they have been relatively neglected in studies of health inequalities. Therefore, this study was conducted to investigate the social inequalities in preventive services among elderly men and women. Methods: Data were obtained from a nationally representative sample of 342 men and 525 women aged 65 and over collected during the 2001 National Health and Nutrition Examination Survey. Age adjusted proportions and logistic regression were used to identify the social patterning of preventive services among elderly Koreans using various social position indicators. Results: The findings of this study generally supported the presence of social gradients in preventive services among the Korean elderly. The likelihood of using the service becomes progressively higher with social position. Educational level, income, and self-rated living status were significantly associated with increased medical checkups and cancer checks. In addition, logistic regression detected educational inequalities only among older women receiving BP checks. After being stratified based on health status and chronic disease status, social disparities still existed when educational level and self-rated living status were considered. Among unhealthy individuals, place of residence was observed as a barrier to medical checkups. Conclusions: This study demonstrated strong and consistent associations between socioeconomic position and preventive services among the elderly in Korea. The results indicate that public health strategies should be developed to reduce the barriers to preventive services encountered by the elderly.

Relationship between health factor, oral health factor and prevalence of depression in Korean elderly (한국 노인의 건강요인 및 구강건강요인과 우울증 유병여부의 관련성)

  • Objectives: The purpose of the study is to investigate the relationship between health factor, oral health factor and revalence of depression in Korea elderly by data from the 5th Korea National Health and Nutrition Examination Survey(KNHANES) 2012. Methods: The subjects were 10,938 elderly over 65 years old by rolling survey method and data were extracted from the 5th Korea National Health and Nutrition Examination Survey(KNHANES) 2012. Data of 1,421 elderly were finally selected and analyzed using kstrada. The questionnaire consisted of socio-demographical characteristics of the subjects, health factors, oral health factors and depression. The oral health factors included subjective perception of oral health, alcohol consumption, exercise, smoking, and toothache. Results: Health factors influencing on the prevalence of depression were subjective health(p<0.001), stress(p<0.001), and alcohol consumption(p<0.05). Oral health factors related to the prevalence of depression were subjective oral health(p<0.05), annual dental checkup(p<0.01), and speaking problem(p<0.01). Conclusions: It is necessary to develop the oral health promotion program for the elderly and help them maintain good quality of life and mental health.

The relationship between stress perception and oral health status (스트레스 지각과 구강건강의 관련성)

  • Objectives : The aim of this study was to investigate the factors related to stress perception. Methods : A self-reported questionnaire was filled out by 109 people living in Gimpo city from January 16 to April 15, 2013. The data were analyzed using SPSS window ver. 19.0(IBM CO., Armonk, NY, USA) for t-test, one-way ANOVA, and multiple linear regression analysis. Results : Men and group of 59 years old tended to have higher stress perception than women and those who over 60 years old, but the results were not statistically significant. Stress perception was higher among the group with systemic dryness and oral dryness(each${\geq}1$) and the group reporting high difficulty in daily living due to oral problem(${\geq}2$)(p<0.05). Especially, the factor that was most correlated to stress perception was systemic dryness(${\beta}$=0.347) followed by problem of daily living for oral problem(${\beta}$=0.278, p<0.01). Conclusions : Systemic dryness and oral problem can increase the level of stress perception, mouth dryness and oral problems are a part of stress responses. Therefore, the regular health checkup must be done and early treatment to reduce stress perception as well as negative aspects caused by stress. It will contribute to the enhancement of public health and life of quality.

Correlation between Blood Pressure and Serum Mineral Concentrations in Hypertension Patients with Dehydration (고혈압 환자의 탈수 여부에 따른 혈압과 혈청 무기질과의 관련성)

  • The objective of this study was to analyze the relationship between blood pressure and serum mineral concentration in hypertension patients subjected to dehydration. The data were collected and analyzed for 7,109 patients who visited a hospital for their health checkup from 2011 till 2015. Respondents were classified into four groups depending upon their state of hypertension and dehydration: control group (no hypertension and no dehydrated), dehydrated group, hypertension group, and dehydrated with hypertension group. Hypertension group was found to have higher levels of serum sodium, calcium, and uric acid than the normal group, whereas the dehydrated group was found to have higher levels of serum sodium and potassium than the normal group. In effect, no significant correlation was directly observed between the hypertension group and dehydration group. However, there were significant negative correlations between dehydration of the old-aged group and their serum mineral concentrations, which are reportedly related to hypertension. Thus, dehydration prevention measures in the elderly over 65 could mitigate hypertension problems.

Neuroendocrine Tumor of Unknown Primary Accompanied with Stomach Adenocarcinoma

  • A 67 year old male at a regular checkup underwent esophagogastroduodenoscopy. On performing esophagogastroduodenoscopy, a lesion about 1.2 cm depressed was noted at the gastric angle. The pathology of the biopsy specimen revealed a well-differentiated adenocarcinoma. On performing an abdominal computed tomography (CT) scan & positron emission tomography-computed tomography (PET-CT) scan, no definite evidence of gastric wall thickening or mass lesion was found. However, lymph node enlargement was found in the left gastric and prepancreatic spaces. This patient underwent laparoscopic assisted distal gastrectomy and D2 lymph node dissection. On final examination, it was found out that the tumor had invaded the mucosal layer. The lymph node was a metastasized large cell neuroendocrine carcinoma with an unknown primary site. The patient refused chemotherapy. He opted to undergo a close followup. At the postoperative month 27, he had a focal hypermetabolic lesion in the left lobe of the liver that suggested metastasis on PET-CT scan. He refused to undergo an operation. He underwent a radiofrequency ablation.

  • Sparganosis is a zoonosis caused by the migrating larvae of cestode genus Spirometra. We report a rare form of sparganosis that invades the lung. A 44-year-old man presented with newly appeared pulmonary nodules that were found accidentally on regular medical checkup, and on sequential chest CT, which we checked at an interval of every 2 months, revealed that the pulmonary lesion had migrated. The patient had a medical history of having undergone surgical excisions for sparganosis in muscles and in subcutaneous tissues of the lower abdomen, perianus, thigh, right axilla, and scapula area, several times over 7 years. A right middle lobectomy was performed and the lesion was diagnosed as sparganosis based on the characteristic histological findings.

  • The present study presented the implementation of a remote control/supervision system for guard lamps used in public illumination with little endeavor by far for safe management, which makes possible to supervise the state and to control the functions remotely including electric safety elements. Especially, the developed system adopts the measurement algorithm for detecting resistive leakage current(Igr) flowing based on the phase difference checkable for sensing at a monitor, being allowable for monitoring at MMI and transmitter for data transmittance. To verify reliability about the algorithm to accurately detect Igr leakage current, the laboratory-based functional test was performed.

  • XaaS (Everything as a Service) provides re-usable, fine-grained software components like software, platform, infra across a network. Then users usually pay a fee to get access to the software components. It is a subset of cloud computing. Since XaaS is provided by centralized service providers, it can be a target of various security attacks. Specially, if XaaS becomes the target of APT (Advanced Persistent Threat) attack, many users utilizing XaaS as well as XaaS system can be exposed to serious danger. So various solutions against APT attack are proposed. However, they do not consider all aspects of security control, synthetically. In this paper, we propose overall security checkup considering technical aspect and policy aspect to securely operate XaaS.
