Development of the implant surgical technique and assessment rating system

  • Purpose: There has been no attempt to establish an objective implant surgical evaluation protocol to assess residents' surgical competence and improve their surgical outcomes. The present study presents a newly developed assessment and rating system and simulation model that can assist the teaching staffs to evaluate the surgical events and surgical skills of residents objectively. Methods: Articles published in peer-reviewed English journals were selected using several scientific databases and subsequently reviewed regarding surgical competence and assessment tools. Particularly, medical journals reporting rating and evaluation protocols for various types of medical surgeries were thoroughly analyzed. Based on these studies, an implant surgical technique assessment and rating system (iSTAR) has been developed. Also, a specialized dental typodont was developed for the valid and reliable assessment of surgery. Results: The iSTAR consists of two parts including surgical information and task-specific checklists. Specialized simulation model was subsequently produced and can be used in combination with iSTAR. Conclusions: The assessment and rating system provided may serve as a reference guide for teaching staffs to evaluate the residents' implant surgical techniques.

Development of a Voluntary Hazard Assessment Model for Small- and Medium-Sized Ship-building Plants (중소규모 조선업 사업장을 위한 자율 위험성 평가 모델의 개발)

  • Industrial accident frequency in small- and medium-sized ship-building plants is much higher than that of large-sized ones so that safety management activities should be different. In that sense, voluntary hazard assessment would be helpful for small- and medium-sized plants. However, conventional hazard assessment items and methods had some problems that discouraged voluntary participation of plants concerned. This study aimed to develop a new model for small- and medium-sized ship-building plants that can promote and encourage voluntary hazard assessment activities. For that purpose, ship-building process was assumed as a sequence of phases, and accident characteristics were compared with them. From that result, relative weights of accident factors including ship-building phases, accident types, occupational category, accident-induced objects, and hazardous items were determined with accident frequency data and with the help of expert groups. Therefore, for web-based integrative computer programming, a plain but accident data-dependent model was developed, with an additive function for related agencies that want to collect assessment results. It is expected that this model would help small- and medium-sized ship-building plants that wanted not only simple checklists but also effective assessment and management guidelines.

The Effect of Behavior Based Safety Program on Safe Behaviors of Bus Drivers and Passengers: A Field Case Study (행동기반 안전관리(Behavior Based Safety: BBS) 프로그램이 버스 기사 및 승객의 안전행동에 미치는 효과 검증: 현장 사례 연구)

  • This study examined the effect of Behavior Based Safety(BBS) program on safe behaviors of bus drivers and passengers. Four male bus drivers working at a H transportation company situated in Seoul participated in this study. BBS program consisted of education, prompts, and self-monitoring with goal setting. Dependent variables were the percentage of two safe driving behaviors of bus drivers (departure after stopping for 3 seconds, announcement for holding bus handles) and the percentage of one safe behavior of passengers getting on the busses those drivers drove (holding bus handles). A primary observer and two trained assistant observers measured two safe behaviors of the bus drivers with behavior checklists by riding on the busses and the passengers' safe behavior was observed by CCTV installed on each bus. An ABC multiple baseline design across participants was adopted. After baseline(A), education and prompts(B) and self-monitoring with goal setting(C) were introduced sequentially to each participant. The results showed that BBS program was effective to increase both bus drivers' and passengers' safe behaviors. Especially self-monitoring with goal setting was more effective in improving safe behaviors of bus drivers than education/prompts. These results suggest that education/prompts and self-monitoring with goal setting would be an alternative treatment technique to improve safety for lone workers such as bus drivers.

An Audit of 204 Histopathology Reports Over Three Years of Carcinoma of Cervix: Experience from a Tertiary Referral Centre

  • Background: The aim was to see compliance to minimum data set information in carcinoma cervix histopathology reports from a team of 13 pathologists; and also to analyse the distribution of parameters like tumor size, grade, depth of cervical stromal invasion, lymph node yield and pTNM stage. Materials and Methods: All pathology reports of radical hysterectomy for carcinoma cervix operated in house within a three year duration (2010-2012), (n=204) were retrieved from medical records and analyzed for the above parameters. Results: In 2010- 59 cases, in 2011- 67 cases and in 2012- 78 cases of carcinoma cervix underwent operations in our hospital. The median age was 50.5 years and the maximum T diameter was 2.8 cms in the reports of three years. Squamous carcinoma was the commonest subtype amongst all the tumors. It was noted that 60.8% of cases had cervical stromal involvement more than half the thickness of the cervical stroma. Parametrial involvement was seen in 4.82% of cases. pTNM Staging was not mentioned in 65.06% of the cases. The mean bilateral pelvic lymph node yield count in our study was 16.6 inclusive of all the three years. Conclusions: Compliance with provision of a minimum dataset in our team of 13 pathologists was generally good. Lymph node yield in our hands is reasonable, but constant striving for greater numbers should be made. pTNM staging should be more meticulously documented. Use of proformas /checklists is recommended.

Effects of Simulation-based Training for Basic Life Support Utilizing Video-assisted Debriefing on Non-Technical and Technical Skills of Nursing Students (비디오 디브리핑을 이용한 기본소생술 시뮬레이션 교육이 간호학생의 비기술적 술기와 기술적 술기 수행능력에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of simulation-based training (SBT) for basic life support (BLS) utilizing video-assisted debriefing (VAD) about non-technical skills (NTSs) and technical skills (TSs). The goal of the proposed study is the evaluation of a teaching method about the correct application of cardiopulmonary resuscitation (CPR). Methods: The study design was a control group pre- and post-test non-synchronized experimental design. The sample included twelve teams of 36 nursing students. Both the experimental and the control groups received the SBT for BLS. Only the experimental groups received VAD where as the control groups had a verbal debriefing. Raters who used checklists for TSs and NTSs evaluated both groups. Data were analyzed by the SPSS 20.0 using Cronbach's ${\alpha}$, Intraclass Correlation Coefficient (ICC), Mann-Whitney U test and Willcoxon signed rank test. Results: The experimental groups scored higher than the control groups in both TSs (p=.004) and the NTSs (p=.008). Conclusion: The findings of this study suggest that NTSs are an important factor that lead CPR successfully, so VAD can be used as an efficient teaching-learning strategy in the SBT for BLS for nursing students and nurses.

Overview of Methodological Quality of Systematic Reviews about Gastric Cancer Risk and Protective Factors

  • Background and Objective: A comprehensive overall review of gastric cancer (GC) risk and protective factors is a high priority, so we conducted the present study. Methods: Systematic searches in common medical electronic databases along with reference tracking were conducted to include all kinds of systematic reviews (SRs) about GC risk and protective factors. Two authors independently selected studies, extracted data, and evaluated the methodological qualities and the quality of evidence using R-AMSTAR and GRADE approaches. Results: Beta-carotene below 20 mg/day, fruit, vegetables, non-fermented soy-foods, whole-grain, and dairy product were GC protective factors, while beta-carotene 20 mg/day or above, pickled vegetables, fermented soy-foods, processed meat 30g/d or above, or salty foods, exposure to alcohol or smoking, occupational exposure to Pb, overweight and obesity, helicobacter pylori infection were GC risk factors. So we suggested screening and treating H. pylori infection, limiting the amount of food containing risk factors (processed meat consumption, beta-carotene, pickled vegetables, fermented soy-foods, salty foods, alcohol), stopping smoking, avoiding excessive weight gain, avoidance of Pb, and increasing the quantity of food containing protective components (fresh fruit and vegetables, non-fermented soy-foods, whole-grain, dairy products). Conclusions: The conclusions and recommendations of our study were limited by including SRs with poor methodological bases and low quality of evidence, so that more research applying checklists about assessing the methodological qualities and reporting are needed for the future.

The Effect of Preceptorship on OJT(On the Job Training) of New Nurses (신규간호사 배치후 교육시 프리셉터쉽 적용 사례연구)

  • The purpose of this study was to examine the effect of preceptorship on On the Job Training of new nurses. The research design was a retrospective survey and one group pre-post test design. The subjects were 49 entry nurses and 49 preceptors. The data were collected from Feburary 1, 1998 to December 1, 1998. One questionnair applied in this study consisted of general characteristics, the responses of new entry nurses and their preceptors and their qualities. The other questionnair consisted of job satisfaction of new entry nurses and preceptors and their competencies. The data were analyzed for frequencies, mean score, standard deviation, t-test, and ANOVA. The results of the study were as follows: 1) Preceptees's responses were that the nurses' affiliation of unit, cooperation of other nurses, and competencies of nursing skills improved after OJT applied preceptorship. 2) Preceptors's answers were that preceptorship had a positive effect on their continuing education. 3) While preceptees's job satisfaction scores were high in interaction, administration, and organization, they were low in autonomy and pay. 4) Pre-competency scores were the same as post-competency scores. 5) After preceptorship, preceptors responded that their nursing skill & communication competency increased, but their nursing administration competency decreased. 6) Finally, preceptorship's other effects were as follows: (1) Checklists for preceptee education at the nursing unit were developed separately. (2) It was proved that 'preceptor interview note' was effective on manage preceptorship. (3) We obtained duty overlap periods; medicalsurgical units-1 month, 5 ICU-2 months, operation room-3 months. (4) We prepared a preceptor program based on the study results.

A Survey on Head Nurses' Perceptions and Performances on Staff Evaluation (수간호사가 활용하는 근무성적 평정에 관한 조사연구)

  • This study was designed and carried out to identify the methodologies of the staff evaluation used by the headnurses and their perception on the purpose of the evaluations, and to measure the degree and type of perceived purpose and the degree of implementation of staff evaluation principles into preactice. A hundred and eleven from four general hospitals with 600 beds and over were conveniently sampled. Data were gathered from August 20, through September 10, 1994 ; demographic data of respondents and 'methodologies used for staff evaluation' by questionaires, level of 'perceived purposes of staff evaluation' and level of implementation of the 'principles of staff evaluation into practice' by Likert-type rating scales developed by the researcher. Raw and percentile scores and means were calculated and data were processed further by t-tests and ANOVA. Results are as follows ; 1. Traditional methodlogies : checklists(90.1%) and ranking methods(45.9%) revealed to be the two mostly prefered and used methods. 2. Of the 20 item-descriptions of the 'purpose of staff evaluation': 10 'control, evaluative' and 10 'non-control, developmental' the first(mean=3.19) to the ninth(mean=2.74) in rank order revealed to be the 'control, evaluative purpose', while the tenth in rank order revealed to be the first among the 'non-control, evaluative' item(mean-1.57). 3. Level of purpose of staff evaluation perceived as 'control ; evaluative'(mean=43.76, S.D.=5.01) revealed to be significantly(t=-3.86, p<.05) highter than 'non-control ; developmental'(mean=40.65, S.D.=3.41). 4. Level of implementation of 'staff evaluation principle into practice' revealed to be moderately high(mean=2.40).

  • Recently, software quality evaluation based on ISO/IEC 9126 and ISO/IEC 14598 has been widely accepted in various areas. However, these standards for software quality do not provide practical guidelines to apply the quality model and the evaluation process of software product=5. And the qualify management in most software projects has been conducted by managing defects without applying the standards for software qualify. Thus, we present a quantitative quality evaluation approach of artifacts in the Component Based Development (CBD). Particularly, our evaluation approach allows most of the standard evaluation process and adopts a quantitative quality model which uses the weights of quality characteristics obtained through carefully selected questionnaires for stakeholder and Analytic Hierarchical Process(AHP). Moreover, we have also examined the proposed evaluation approach with applying the checklists for the artifacts of the CBD to a small-scale software project. As a result, we believe that the proposed approach will be helpful for acquiring the high quality software.

  • This study is the basic design of a P-C-M support system for Agricultural Facilities. By utilizing this system, many users of Agricultural Facilities can easily obtain a large amount of information, ranging from procurement to maintenance. The procurement phase suppliesthe user with agricultural facilities information, economical analysis, and corporation information for procurement decisions. Furthermore, this phase shows the shape or format of the agricultural facilities, the main materials, as well as information on the cultivated crops. The construction phase shows information about agricultural facility-related laws, as well as standard drawings or a construction guide. The maintenance phase shows a guide of maintenance-related checklists. With the results of a basic design of a P-C-M Support System for Agricultural Facilities, the P-C-M support system menu tree is established.
