[15O-H2O] 심근 양전자 단층 촬영에서 혈류 분포의 비균일성과 분배계수 (The Heterogeneity of Flow Distribution and Partition Coefficient in [15O-H2O] Myocardium Positron Emission Tomography)

  • O-15 표지 물 동적 PET을 이용하여 비균일 심근조직의 혈류를 정확히 추정하기 위하여 일차구획 모델을 변형한 두 개의 혈류 모델을 고안하였다. 첫 모델에서는 혈류, PTI, 조직회수 분획($F_{MM}$)을 추정하였고, 두 번째 모델에서는 혈류, PTI, 그리고 분배계수를 추정하였다. 비균일 심근조직의 비균일성을 나타낼 지표를 도입하여 여러 종류의 비균일 조직을 모사하고 이 지표로 비균일성을 표현하였다. 우리 모델을 적용하여 PTI가 혈류분포의 비균일성과 상관이 적음을 확인하고 분배계수를 변수로 취급한 두 번째 모델에서 추정한 분배계수가 비균일성을 나타냄을 알았다. 분배계수는 비균일성에 따라 굽은 선형(curvilinear)으로 감소하였다. 분배계수와 함께 추정된 혈류도 비균일성이 커지면 참값보다 작게 추정되었다. 추정된 분배계수로 혈류 추정값의 과소평가플 보정하여 추정값의 바이어스를 바로잡을 수 있었다. O-15 표지 물 동적 PET으로 심근혈류를 측정할 때 분배계수를 변수로서 혈류와 함께 추정하여 비균일성을 나타내는 지표로 쓰고 동시에 혈류 추정값을 참값에 가까운 값을 얻는데 쓸 수 있다고 본다. 혈류분포의 비균일성 정도를 수치적으로 표시할 수 있는 지표를 임상에 적용하면 허혈성 심근 질환의 혈류 비균일성을 해석할 수 있을 것으로 생각한다.

디지털 흉부 촬영에서 구리필터사용에 따른 환자 표면선량 감소효과에 관한 연구 (The Study on the Reduction of Patient Surface Dose Through the use of Copper Filter in a Digital Chest Radiography)

  • 의학적으로 방사선을 사용 시 가장 중요한 점은 환자의 병을 진단 가능하되 피폭선량을 최소화시켜야 한다는 점이다. 진단용 X-선 중 장파장부분은 대부분 흡수에 관여하게 되어 환자의 피폭선량을 증가시키므로 불필요하다. 이러한 장파장영역을 제거하기 위한 방법으로 가장 효과적인 방법은 여과 판의 사용이다. 이에 부가여과가 없는 경우와 0.3mmCu filter 사용 시의 화질과 피부입사선량에 대해 실험을 해 보았다. ROC 신호용 지름 4mm, 두께 3mm의 아크릴 디스크를 chest phantom에 랜덤하게 위치시키면서 non filter 및 Cu filter시 각 각 20매씩 총 40매의 신호+잡음영상을 촬영하고, 또한 신호가 없는 영상(잡음만의 영상)을 각 각 20매씩 40매 촬영하여 총 80매의 영상을 준비한 후 5명의 방사선종사자가 판독 후 P(S/s), P(S/n) 구하여, ROC곡선을 그리고, 감도와 특이도 및 곡선하면적을 구해 평가하였다. 또한 AAPM(American Association of Physicists in Medicine by the American Institute of Physics)권고에 따른 ANSI chest phantom과 Contrast-detail phantom을 이용하여 non filter 또는 Cu filter사용 시 시각적으로 관찰 가능한 병소의 갯수를 파악하였으며, 또한 두 경우에서 피부입사선량을 측정하였다. 그 결과 Cu filter사용 시에 ROC곡선이 좌상방향에 위치하고, 감도, 곡선하면적 및 CD phantom 병소의 갯수에서 모두 좋은 결과를 나타내었다. 또한 피부입사선량은 감소가 되어 여러 측면에서 부가필터의 사용을 검토해 볼 필요가 있으리라 사료된다.

바티니에프 기본원리를 통해 본 양식 태극권에 관한 연구 (A study on Yang Shi Tai Chi Chuan in Bartenieff Fundamentals Perspectives)

  • 본 연구는 태극권 움직임에서 신체의 이완방법을 개발하기 위해 바티니에프 기본원리를 적용하여 태극권 움직임의 원리를 분석하였다. 이 연구과정을 통해 태극권과 바티니에프의 신체 움직임이 일맥상통한다는 것을 알 수 있었다. 첫째, 태극권과 바티니에프의 신체 움직임 철학 각도에서 살펴보면 두 기법의 궁극적인 목표는 모두 정신과 신체의 통합이다. 즉 동양의 심신일원론(心身一元論)과 서양의 신체자각(Body Awareness)이 일맥상통하였다. 둘째, 바티니에프가 제시한 호흡지지의 측면에서 살펴보면 두 기법은 모두 호흡을 통해 신체를 자연스럽게 움직이게 하고 각 부위를 이완시킨다. 태극권에서 기(氣)는 생명의 바탕이며 신(身)의 힘이다. 즉, 태극권의 호흡은 몸과 마음(Body- Mind)을 소통, 조화, 융화시킬 수 있는 것이다. 다시 말해서 태극권의 호흡은 정신적인 융합을 통하여 이루어지며 움직임에 영향을 주었다. 바티니에프의 호흡지지도 마찬가지다. 바티니에프의 호흡은 모든 관점에서 움직임에 영향을 주고 호흡은 몸의 내부와 외부의 형태를 모두 변화시킨다고 한다. 셋째, 바티니에프가 제시한 중심부지지의 측면에서 살펴보면 두 기법은 모두 중심을 강조하였다. 중심 지지를 의식하면서 움직이면 몸의 표면적인 근육보다는 좀 더 깊은 근육을 사용할 수 있으며 이를 통해 강하고 유연한 움직임을 가능하게 하였다. 태극권의 기침단전(氣沉丹田)은 의식적으로 복식호흡을 사용하고 힘을 중심으로 모은다. 이러한 운동을 할 때 중심은 더 안정되고 호흡 역시 순조로워진다. 넷째, 바티니에프 기본원리에서 제시한 회전적 요인의 측면에서 살펴보면 모두 회전을 사용한 움직임을 통해 신체 이완이라는 목적을 이루게 된다. 바티니에프의 회전적 요인은 축을 중심으로 3차원적으로 움직이는 관절운동이라는 특성을 인지함으로써 동작을 더욱 쉽게 하고 자유롭게 할 수 있었다. 태극권도 마찬가지다. 태극권은 원형과 나선형(Spiral Movement)의 움직임을 통해서 공간을 최대한 접근하고 매끄럽게 흐름을 전환해서 이완이라는 목적을 이루게 되었다. 다섯 번째, 코헨(Bonnie Bainbridge Cohen)의 Body-Mind Centering Work 이론을 토대로 바티네에프가 정립한 발달 모형의 각도에서 살펴보면 태극권의 움직임의 발전과정과 발달 모형에서 제시한의 호흡, 중심-말초부 연결 / 중앙 반사, 머리- 꼬리뼈 연결 / 척추의 움직임, 상체-하체 연결 / 상응하는 움직임, 신체의 반쪽 연결 / 동종 편측 연결, 교차 측면 연결 / 대측 연결 모두 일맥상통함을 알 수 있었다. 즉 태극권은 호흡을 통해 에너지를 중심으로 모으고, 요추를 통해 상체와 하체를 연결하며 움직임이 발전할 때 동종 편측 연결뿐만 아니라 교차 측면 연결을 할 수 있다. 이러한 연구를 통해 무용의 움직임을 자연스럽게 표현하고, 신체 자각을 토대로 중심축과 관절, 및 균형을 이용한 신체의 움직임 원리를 분석해낼 수 있었다.

가시광에서 밝은 1형 활동은하핵의 근적외선 변광 (NEAR-INFRARED VARIABILITY OF OPTICALLY BRIGHT TYPE 1 AGN)

  • 와이즈/네오와이즈 자료를 사용하여 1형 활동은하핵의 근적외선 장기 변광 특성을 살펴보았다. 활동은하핵은 밀리퀘이사 목록을 사용해 가시광에서의 등급 제한을 두어 선정하였고, 여러 번의 다중시점 측광 자료를 확보하기 위해 황도북극과 황도남극에 가까운 대상으로 한정하였다. 이후 와이즈와 네오와이즈 데이터베이스에서 해당 대상들의 측광 자료를 추출하였다. 일부 근적외선에서 검출되지 않은 경우와 관측 횟수가 지나치게 적은 경우를 제외하고 총 73개의 1형 퀘이사 및 활동은 하핵, 140개의 측광학적으로 선택된 활동은하핵 후보에 대해 W1 (3.4 ㎛), W2 (4.6 ㎛) 밴드 광도곡선을 구성하고 이를 이용해 변광 분석을 수행하였다. 변광 여부를 판단하기 위해 초과 분산 값과 변광의 유의확률 Pvar를 계산하였다. 초과 분산이 포아송 오차에 의해 추정되는 오차값보다 큰 경우, 그리고 Pvar이 0.95보다 크거나 같은 경우를 변광으로 판단했는데, 활동은하핵 73개 중 19개, 활동은하핵 후보 140개 중 12개가 W1, W2 밴드 모두에서 변광 대상으로 판단되었다. W1 밴드보다 W2 밴드에서 변광 대상으로 판단되는 숫자가 작게 나타났는데, 이는 긴 파장으로 갈수록 변광의 정도가 작아짐을 시사하는 것으로 보인다. 약 9 ~ 26%의 대상들이 근적외선 변광을 보였다. 감쇠 랜덤워크 모형을 사용하여 변광 폭(σ)과 완화 시간(τ), W1과 W2 밴드 사이의 시간 지연 등을 추정하였다. 변광 폭 및 완화시간은 W1 등급과 큰 상관관계를 보이지 않았으며, 두 변수 사이에도 특별히 두드러지는 상관관계가 나타나지 않았다. 단, 감쇠 랜덤워크 모형을 사용해서 시간 규모를 추정할 때 광도곡선의 분량이 충분한지를 감안하면, 완화시간이 짧은 대상들에 대해서는 변광 폭과 완화시간이 음의 상관관계를 보인다고도 볼 수 있다. 대상의 개수가 통계적으로 유의미한 결과를 제시하기에는 부족하다. X선 광도와 변광 특성을 비교했을 때에는 상관관계를 찾아보기 어려웠으나, 앞으로 얻어질 X선 탐사자료와 전천 스펙트럼 탐사자료와 결합한 추가 연구가 기대된다. 전체 대상 중에서는 시간에 따라 (W1-W2) 색이 변하는 것으로 판단되는 흥미로운 대상이 4개가 존재했는데, 이들의 특성에 대해서는 추후 연구가 더 필요할 것으로 전망한다.

고온 스트레스에 따른 참당귀의 생육 및 유용성분 특성 (Growth and Useful Component of Angelica gigas Nakai under High Temperature Stress)

  • 최근 전 세계적으로 지구온난화가 가속되면서 작물의 생산에 극심한 피해가 야기되는바 극한의 고온 스트레스에 따른 참당귀의 생육특성 및 지표성분을 확인하고 효율적인 참당귀의 재배를 위한 기초자료를 제공하고자 하였다. 2018년, 2019년의 기상관측데이터를 활용하여 실험온도(Control, 28℃, 34℃, 40℃)를 설정하였다. 그리고 식물생장상을 통해 실험온도를 처리하여 실험포지에서 이식 및 생장을 거쳐 생육특성 및 유용성분함량을 분석하였다. 실험이 수행되어진 실험포지는 평균 대기온도 19.38℃, 평균 토양온도 21.34℃, 평균대기습도 81.31 %, 평균 토양습도 0.18 m3/m3, 평균 일사량 162.05 W/m2로 관측되었으며, 토양의 이화학적 특성은 사질식양토에 유기물함량 2.66 %, 유효인산 868.52 mg/kg, 전질소 0.14%, pH 6.65, 칼륨, 칼슘, 마그네슘, 나트륨은 각각 0.95, 7.38, 1.46, 0.15 cmol+/kg로 분석되었다. 그리고 활착율(85 %)과 전체 높이(38.66 cm), 생중량(41.3 g), 건중량(14.24 g) 등과 같은 대부분의 생육특성은 대조구 대비 28℃ 실험구에서 가장 높은 생육을 보였다. 하지만 유용성분함량의 유의성은 확인되지 않았으나 대조구 대비 34℃ 실험구에서 가장 높은 값을 나타냈다. 이러한 결과는 고온의 스트레스로 식물체 조직의 노화 및 광합성량 감소, 생장지연 등과 같은 생육장애 요인에 의해 실험구별 생육의 차이가 발생했으며, 유용성분의 함량 또한 실험구별 생육장애에 의해 동화산물의 축적이 저조해져 발생된 현상이라 판단된다.

18F-FDG Whole Body PET/CT 수검자의 거리별 선량 변화에 따른 방사선 작업종사자의 유효선량 고찰: 환자 고유특성 및 응대시간 측면 (The Consideration of nuclear medicine technologist's occupational dose from patient who are undergoing 18F-FDG Whole body PET/CT : Aspect of specific characteristic of patient and contact time with patient)

  • 방사선 안전에 대한 관심과 염려가 전 세계적으로 점차 증가되고 있는 가운데, 의료 실무 현장에 종사하는 방사선 작업종사자의 외부피폭 관리 또한 중요한 이슈로 부각되고 있다. 특히, $^{18}F-FDG$WholeBodyPET/CT검사의 경우 높은 에너지의 방사성동위원소를 사용하므로 검사자의 피폭선량 저감화에 더욱 관심을 기울여야 한다. 따라서, 본 연구는 $^{18}F-FDG$ Whole Body PET/CT 수검자의 거리 별 외부선량률을 측정 및 분석하고, 방사선 작업종사자의 업무 행위 별 누적선량을 확인하여 피폭선량 저감화에 도움이 되는 주요한 요소를 알아보고자 한다. $^{18}F-FDG$WholeBody PET/CT검사를 받은 106명의 환자를 대상으로 검사 종료($75.4{\pm}3.3min$) 후 가슴을 기준 0, 10, 30, 50, 100 cm 거리에서 외부선량률을 측정하였다. 환자측면에서 외부선량률에 영향을 줄 수 있는 개별적 요인을 분석하기 위해 성별, 연령, BMI, 금식시간, 당뇨병 유무, 약물 투여정보, 크레아틴 수치 정보를 수집하였다. 수집된 정보의 통계분석은 ANOVA 분석 및 T-test를 시행하였다. 방사선 작업종사자 측면에서 피폭선량에 영향을 줄 수 있는 요인을 분석하기 위해 주사 업무를 하는 3명의 직원($T_1$, $T_2$, $T_3$)과 스캔 업무를 하는 3명의 직원($T_4$, $T_5$, $T_6$)에 각각 Personal pocket dosimeter를 착용시켜 업무시간 동안 누적된 선량을 기록하였다. 또한 방사선 작업종사자 별 응대시간을 측정하여 분석하였다. 각 거리 별 외부선량은 $246.9{\pm}37.6$, $129.9{\pm}16.7$, $61.2{\pm}9.1$, $34.4{\pm}5.9$, $13.1{\pm}2.4{\mu}Sv/hr$로 산출되었다. 환자측면에서, 근거리에서 성별, BMI, 선량, 크레아틴 수치에 의해 유의미한 차이가 있었지만, 거리가 증가할수록 그 차이는 감소하였다. 그 중 크레아틴 수치의 경우 100 cm에서 집단 간 통계적으로 유의한 차이를 보이지 않는 특징이 있었다. 환자 1명으로부터 받은 선량은 주사 업무를 하는 직원($T_1$, $T_2$, $T_3$)의 경우 0.70, 1.09, $0.55{\mu}Sv/person$이었고, 스캔($T_4$, $T_5$, $T_6$)의 경우 1.25, 0.82, $1.23{\mu}Sv/person$이었다. 응대시간이 상대적으로 적은 $T_4$직원의 경우 $T_3$, $T_5$보다 34% 낮은 누적선량을 확인할 수 있었다. 이를 토대로 환자와의 적정거리 유지와 응대시간 감소가 누적선량에 크게 작용함을 알 수 있었다. 위와 같은 점을 고려했을 때, 환자의 충분한 수분 섭취 및 배뇨, 방사선 작업종사자와 환자 간 적정거리유지(최소 100 cm이상) 및 응대시간 감소를 위해 노력해야 할 것이고, 환자의 video tracking system과 장비의 원격조정 등을 통해 피폭선량 저감화를 위해 노력해야 한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
