Effect of E10 on Relief of Cervical Myalgia; One Group Pre-Post, Pilot Trial Study (E10의 경항부 근육통 완화에 대한 효과; 전-후 비교, 예비 임상 연구)

  • Objectives The purpose of this study is to investigate the efficacy of E10 on the relief of cervical myalgia. Methods This study is a one group pre-post, pilot trial test. After the approval of institutional review board (IRB), we have recruited 25 patients suffering from cervical myalgia. 23 patients (2 patients dropped out) was treated by using E10 on the cervical muscle and acupuncture points, three times a week for a total of two weeks from February 2016 to April 2016. Evaluations of VAS for bothersomeness of neck pain (VASB), VAS for pain intensity (VASP), NDI, SF-36, X-ray, thermography, etc. before and 1 week after treatment were carried out. The primary outcome measure was the VASB, measured 1 week after the end of the therapy. Results VASB scores was significantly decreased from $72.5{\pm}9.9$ to $50.4{\pm}9.9$. VASP scores was significantly decreased from $71.5{\pm}10.5$ to $48.5{\pm}18.3$. Results showed significant improvements in VASB, VASP, NDI, SF-36 from the baseline. However, in X-ray, thermography, there was no singnificant difference. Conclusions This study demonstrates the effectiveness of E10 treatment for relieving cervical myalgia.

Use of Orthopedic Manual Physical Therapy and Home Self-Therapeutic Exercise to Manage Myofascial Temporomandibular Disorder Accompanied by Headache: Case Study (두통을 동반한 근막성 턱관절 장애 환자의 관리를 위한 정형도수치료기법과 가정 자가-치료적 운동의 적용: 사례연구)

  • Purpose: The current case study focuses on identifying the effects of manual therapy and home self-therapeutic exercise including on mouth opening and pain relief in patients with continuous neck pain with myofascial temporomandibular disorders (TMDs) accompanied by headache induced by masticatory myalgia Subjects: The study participant was a 27-year-old woman who was treated a year ago for pain related to TMDs accompanied by a headache. Methods: Manual therapy of the cervical spine with upper cervical spine posterior-to-anterior mobilization (C1~C2), upper cervical spine flexion mobilization (C0~C2), upper cervical spine lateral flexion mobilization (C0~C1), upper cervical spine thrust manual therapy (C1~C2) and manual therapy of the temporomandibular joint and muscles with transverse medial accessory temporomandibular joint mobilization, manual therapies for the temporal, the masseter, and medial pterygoid muscles were performed twice a week for about 30 minutes for 4 weeks. This protocol included 3 sessions in total. The home self-therapeutic exercise was to be performed two to three times a day. Results: The values more improved MMO increased to 41.4 mm, left masseter muscle PPT to 2.9 kgf/cm2, right masseter muscle PPT to 3.1 kgf/cm2, KHIT-6 to 46 points, neck pain intensity (by NRS) to 2 points, headache frequency to per weeks, cervical kyphotic angle to -8.06%, and GCPS to grade 1 (low-intensity pain without pain-related disorder). Conclusion: Manual therapy and home self-therapeutic exercise can be helpful for mouth opening and pain relief in patients with myofascial TMDs accompanied by secondary headaches induced by masticatory myalgia.

Effects of Shoulder Strength Exercise and Cervical Mobilization to Neck and Shoulder Chronic Pain Patients Pain and Muscle Tension (목과 어깨 만성 통증 환자에게 어깨 강화 운동과 목뼈 관절 가동술이 통증 및 근긴장도에 미치는 영향)

  • Background: Approximately 30% of people suffer from chronic neck and shoulder pain. Chronic neck and shoulder pain arise due to the exposure to continuous low loading and monotonous work. This is a common musculoskeletal disorder in a society. As physical therapists, we should give appropriate treatment to these people. Methods: A total of 26 patients with chronic neck and shoulder myalgia were randomly allocated into two experimental groups. First, 13 patients received shoulder strength training, while the other 13 patients received cervical mobilization. Outcome measures included bilateral pressure pain threshold by using an algometer at upper trapezius, visual analogue scale (VAS) to express their pain scale, and muscle tension by using myoton pro device pre-intervention and after the final treatment. Results: The VAS and muscle tension in upper trapezius significantly decreased (p<.05) in both groups; however, no differences between two groups were observed (p>.05). The pressure pain threshold in upper trapezius significantly increased (p<.05) in both groups; yet, again, the differences between the groups did not reach statistical significance (p>.05). Conclusion: As methods of treatment, shoulder-specific strength training and cervical mobilization can be appropriate approaches to the treatment of neck and shoulder musculoskeletal disorder.

A Case of Systemic Lupus Erythematosus Presenting as Cervical Lymphadenopathy (경부 림프병증으로 발현된 전신홍반루푸스 1예)

  • Systemic lupus erythematosus(SLE) is a multisystemic disorder of autoimmune etiology. SLE can occur commonly in young women, and the early symptoms include fever, myalgia, arthralgia, weight loss, lymphadenopathy and these nonspecific symptoms develop into skin rash, splenomegaly, serositis and encephalopathy. Diagnosis of SLE requires clinical and serologic criteria, and treatment choices are hydroxyquinolone and NSAIDs for mild disease, corticosteroids and immunosuppressant for severe disease. In lupus patient, the prevalence of lymphadenopathy is 12~59%. Although lymphadenopathy is common finding in SLE, it is hard to distinguish in early phase of SLE. A 38-year-old woman visited our hospital for cervical lymphadenopathy with polyarthritis and malaise. Multiple cervical lymph nodes enlargement was found on Neck CT, and serologic laboratory test including ANA, antiphospholipid antibody, and anti-dsDNA was positive. For excluding lymphoma, PET-CT and excisional biopsy were performed. The patient finally diagnosed with SLE, and got regular follow-up without complication.

  • Since the first description of Kikuchi disease(Subacute necrotizing lymphadenitis) in Japan, 1972, this condition has been reported in a worldwide. The etiology remains unknown. This disease shows a marked predilection for young woman, and a few cases in children has been reported. The usual initial clinical manifestation is localized cervical lymphadenopathy with fever, weight loss and myalgia. We experienced a 6 year-old girl of subacute necrotizing lymphadenitis with ANA, therefore report a brief review with the related literatures.

Relationship between Exercise Stress, Tension, Exercise Habits and Sports Injuries of Professional Golf Players (프로 골프 선수의 운동 스트레스, 긴장도, 운동 습관과 스포츠 손상과의 관계)

  • Purpose : The purpose of this study is to know sports injury aspects and relationship between exercise stress, tension, exercise habits and sports injuries. Methods : We surveyed 42 professional golf players who attended Korea Tour during the second half of the year 2010 and have sports injuries with self - assessment questionnaires. Results : According to the types, they had myalgia, tendonitis, ligament sprain in the order. According to the body parts, they had injuries in right thoracic, left cervical, left shoulder region in the order. As a result of the analysis of the sports injury frequency according to the exercise stress and tension, there were no significant differences between the high and low level group. But result of injury regions showed high tension level group showed more damages in right thoracic region (p<0.05). Exercise habits appear subjects had lots of training time, however, they used to skip the muscular strengthening exercise during the on season, but had no significant difference between sports injury frequencies and exercise habit. Conclusion : For the improvement of the athletic performance and prevention of the sports injury, warming-up, cooling-down and muscular strengthening exercise program developments are needed.

  • Histiocytic necrotizing lymphadenitis, which is also commonly referred to as Kikuchi's disease (KD), is a self-limiting disease of unknown etiology. It affects individuals of all ages, although it is usually seen in young women. However, only a few descriptions of this disease are available in the pediatric literature. KD is clinically characterized by cervical lymphadenopathy, high fever, myalgia, neutropenia and, rarely, cutaneous eruptions. Cutaneous manifestations have been reported in 16-40 percent of KD cases. The specific skin changes occurring in cases of KD have yet to be completely characterized. In most of the reported cases thus far, the lesions have been located on the face and upper extremities. In this report, we describe a case of pediatric Kikuchi's disease, occurring in a 9-year-old boy. The boy exhibited transient erythematous maculopapular skin lesions over the entirety of his body, including his lower extremities.

Clinical and Serologic Study of Herpes Zoster in Children and Adolescents in Gwangju-Jeonnam Province (광주·전남 지역의 소아·청소년 대상포진에 대한 임상적 비교 및 혈청학적 의의에 대한 연구)

  • Background: Herpes zoster is a common dermatologic disorder. However, it rarely occurs in children and adolescents in Gwangju-Jeonnam province. Objective: This study aimed to analyze the epidemiological features and clinical characteristics of herpes zoster in children and adolescents. Methods: A retrospective survey was conducted on patients who visited the department of dermatology of Chosun University Hospital in Gwangju-Jeonnam province within the past 8 years. The medical records of 103 patients aged <18 years were reviewed. We analyzed for age, gender, accompanying symptoms, dermatomal distribution, underlying disease, treatment, and complications with serologic test. Results: The male-to-female ratio of the participants was 1.08:1, and their mean age was 13.0 years. Underlying diseases were observed in 3% (3/102) of the patients. The most common dermatomal distribution was thoracic dermatome (34%), followed by trigeminal (26%), cervical (20%), lumbar (15%), and non-skin (6%). The most common accompanying symptoms were headache (10%), fever (3%), and myalgia (3%). No difference was observed between patients who were varicella-zoster virus (VZV) IgM-positive and those who were VZV IgM-negative in terms of dermatome, visual analogue scale (VAS), severity, and body mass index (BMI). Conclusion: In children and adolescents with herpes zoster, the gender ratio and dermatomal distribution were similar to those previously reported, except for the low rate of underlying diseases. The incidence of herpes zoster in children was not significantly associated with immunosuppression and underlying diseases. Higher VZV IgM titer was not associated with dermatomal distribution, higher VAS score, or BMI. This study first compared the serological test results of children.
