Objective : In the present study, we evaluated the effect, safety and radiological outcomes of cervical hybrid surgery (cervical disc prosthesis replacement at one level, and interbody fusion at the other level) on the multilevel cervical degenerative disc disease (DDD). Methods : Fifty-one patients (mean age 46.7 years) with symptomatic multilevel cervical spondylosis were treated using hybrid surgery (HS). Clinical [neck disability index (NDI) and Visual Analogue Scale (VAS) score] and radiologic outcomes [range of motion (ROM) for cervical spine, adjacent segment and arthroplasty level] were evaluated at routine postoperative intervals of 1, 6, 12, 24 months. Review of other similar studies that examined the HS in multilevel cervical DDD was performed. Results : Out of 51 patients, 41 patients received 2 level hybrid surgery and 10 patients received 3 level hybrid surgery. The NDI and VAS score were significantly decreased during the follow up periods (p<0.05). The cervical ROM was recovered at 6 and 12 month postoperatively and the mean ROM of inferior adjacent segment was significantly larger than that of superior adjacent segments after surgery. The ROM of the arthoplasty level was preserved well during the follow up periods. No surgical and device related complications were observed. Conclusion : Hybrid surgery is a safe and effective alternative to fusion for the management of multilevel cervical spondylosis.
Objective: This study aimed to evaluate the effects of the Kaltenborn-Evjenth concept of nerve mobilization combined with intermittent cervical segment traction (ICST) on pain, Neck Disability Index (NDI) scores, range of motion (ROM) and endurance in persons with cervical radiculopathy (CR). Design: Two-group pretest-posttest design. Methods: Thirty subjects participated in this study and were randomly assigned to two groups. The ICST group (n=15) was performed simultaneously with nerve mobilization and cervical traction for the segment with cervical pain at the same time. The intermittent cervical total traction (ICTT) group (n=15) performed nerve mobilization and cervical traction for the whole cervical area at the same time. In this study, outcome measures such as the Visual Analog Scale (VAS), NDI, ROM, endurance (cranio-cervical flexion test), and passive intervertebral motion performed before and 4 weeks after the experiment were compared to investigate the effects of each intervention. Results: In both groups, there were significant differences in the VAS, NDI scores, and endurance, and there were significant differences between the two groups except for endurance (p<0.05). In the ICST group, significant differences were found in all ROM, and in the ICTT group, significant differences were found in only extension, and there were significant differences between the two groups (p<0.05). Conclusions: The ICST group showed more improvement than the ICTT group in pain, NDI scores and ROM. Moreover, our findings show that the ICST could be used as a new strategy for manual therapy in persons with CR.
Purpose : The purpose of this study was to investigate the effects of closed chain breathing exercise on thorax circumference increase and ROM of shoulder rotation for cervical vertebral cord injury patient's. Methods : 12 cervical vertebral cord injury patients volunteer to participate in this study. closed chain breathing exercise group of 4 subjects performed exercise three time a week. The deep breathing exercise group and general exercise group of 4 subjects performed exercise three time a week. We analyaed the descriptive statistics and $3{\times}2$ repeated measures ANOVA by SPSS 12.0 for window. Results : In comparison of rotation of shoulder ROM between pre and post value, the increase of rotation ROM of shoulder was significant in the closed chain breathing exercise group(p<.05). Conclusion : The closed chain breathing exercise helped to increased rotation ROM of shoulder.
Purpose: The aim of this study was to investigate the effect of mandible position on proprioception and range of motion (ROM) during neck stabilization exercise using a sling in healthy adults. Methods: The subjects were randomly assigned to either a sling exercise and mandible open group (n=10) or a sling exercise and mandible closed group (n=12). The sling exercise-mandible open group and sling exercise-mandible closed group took part in an exercise program for 30min, three times per week for 4 weeks. After each training session, head repositioning accuracy (HRA) and the ROM of the cervical spine were measured. Wilcoxon's test was conducted to verify changes within each group, and the Mann-Whitney U test was performed to examine between-group differences. Results: The HRA of the cervical spine was significantly increased during left rotation and extension in the sling exercise-mandible open group. In addition, there were significant differences in both rotations and extension in the two groups. The ROM of the cervical spine increased significantly during both rotations in the sling exercise-mandible closed group. In addition, there was a significant difference in right rotation and extension in both groups. Conclusion: Cervical stabilization exercise using a sling, with the mandible closed increased proprioception and the ROM of the cervical spine.
We report on 4 patients suffering from acute neck pain and limited range of motion (ROM) in the cervical spine who were treated with intradermal acupuncture. The treatment effect was evaluated with numeric rating scale (NRS), ROM, and Neck Disability Index (NDI). After treatment, the NRS and NDI scores decreased and the ROM of the cervical spine increased in all patients. This report shows that intradermal acupuncture treatment can be effective for patients with acute neck pain with limited ROM. However, further controlled studies are needed to confirm the effect of intradermal acupuncture on such patients.

Background: Cervical radiculopathy is the result of cervical nerve root pathology that may lead to chronic pain and disability. Although manual therapy interventions including cervical traction and neural mobilization have been advocated to decrease pain and disability caused by cervical radiculopathy, their analgesic effect has been questioned due to the low quality of research evidence. The purpose of this paper is to present the effect of manual therapy on pain, ROM, disability in a patient experiencing cervical radiculopathy. Methods: In this study, 30 participants who met the diagnostic criteria for cervical radiculopathy were randomized into two groups: group I (nerve mobilization group) & group II (Mckenzie exercise group). The experimental group was performed manual cervical traction and neural mobilization technique. The control group was performed manual cervical traction and cervical retraction, extension exercise. Assessments were performed to 30 participants before and after 4 weeks therapy. The components of assessments were pain intensity (PI), cervical rotation ROM (CR) and neck disability index (NDI). Results: After 4 weeks therapy, PI, CR and NDI were significantly reduced in both groups (p<.01). The PI, CR and NDI were no significantly reduced between group I and group II (p>.01). Conclusions: Manual therapy could reduce the symptoms of cervical radiculotpathy.

Background: Neck pain is a common problem in the general population and second only to low back pain in musculoskeletal problems. The aim of this study is to compare three different types of exercise training in patients with chronic neck pain. Methods: This study was conducted on 45 patients with neck pain. Patients were divided into 3 groups: Group 1 (conventional treatment), Group 2 (conventional treatment plus deep cervical flexor training), and Group 3 (conventional treatment plus stabilization of the neck and core region). The exercise programs were applied for four weeks, three days a week. The demographic data, pain intensity (verbal numeric pain scale), posture (Reedco's posture scale), cervical range of motion ([ROM] goniometer), and disability (Neck Disability Index [NDI]) were evaluated. Results: In all groups, a significant improvement was found in terms of pain, posture, ROM, and NDI values in all groups (P < 0.001). Between the groups, analyses showed that the pain and posture improved more in Group 3, while the ROM and NDI improved more in Group 2. Conclusions: In addition to conventional treatment, applying core stabilization exercises or deep cervical flexor muscle training to patients with neck pain may be more effective in reducing pain and disability and increasing ROM than conventional treatment alone.
Objective: Computers and smartphones have become a necessity for modern people, and the use of these things in an inappropriate position has increased the number of people who complain about neck problems. The purpose of this study was to compare the changes of cervical angle, range of motion (ROM) and pain threshold according to the McKenzie stretching and dry cupping therapy. Design: Cross-over design. Methods: We included 12 male and 6 female college students in their twenties, and conducted a pre- and post-test to evaluate the changes of each variable after the application of the McKenzie stretching and dry cupping therapy. Results: Neither the cervical spine angle nor the turtle neck angle showed any change in both the McKenzie stretching and the dry cupping treatment. In the McKenzie stretching, the pain threshold decreased, and the ROM of the cervical spine increased in all directions but there was no significant difference. The pain threshold was increased in the dry cupping treatment, and the ROM of the cervical spine was significantly increased in all directions (p<0.05). Comparisons of the McKenzie stretching and cupping treatment showed that the cupping treatment produced significantly greater pain thresholds and improvements in ROM of the cervical spine than the McKenzie stretching technique (p<0.05). Conclusions: Cupping treatment is more effective in improving ROM of the cervical spine and pain thresholds than the McKenzie stretching technique. In the future, cupping treatment will be one of the treatment options for pain and ROM impairments of the cervical spine.
To identify the effects of joint mobilization on the functional improvement of patients with neck pain, the present research investigated 60 neck pain patients, dividing them into a group doing joint mobilization, a group doing Mckenzie exercise and a group using modalities. This study examined patients degree of recovery from neck pain by comparing their neck pain before and after the treatment, and compared three groups to find difference in the degree of recovery from neck pain. The results of this study are as follows : 1. For the joint mobilization group, the visual analogue scale (VAS) decreased significantly for three weeks treatment, and the range of motion (ROM) of cervical vertebrae increased significantly(p<.05). 2. For the Mckenzie exercise group, the visual analogue scale decreased significantly for three weeks treatment, and the range of motion cervical vertebrae increased significantly(p<.05). 3. For the modality using group, the visual analogue scale decreased significantly for three weeks treatment, and the range of motion of cervical vertebrae increased significantly(p<.05). 4. In the comparison of VAS and ROM of the three groups before and after the treatment, significant differences were found among the three groups in VAS after three weeks' treatment, and in ROM before the treatment(p<.05). 5. In all the three groups, VAS decreased and the ROM of cervical vertebrae increased after the treatment, and in particular, the decrease of VAS and the increase of the ROM of cervical vertebrae were remarkable in the joint mobilization group.
Purpose: The study aimed to determine the effect of proprioceptive neuromuscular facilitation (PNF) lower trapezius muscle strengthening exercise on pain, cervical range of motion (ROM), and neck disability index (NDI) in patients with chronic neck pain. Methods: Following baseline measurements, the subjects (n = 30) with chronic neck pain were randomized into two groups: the PNF group (n = 15) that received PNF strength training of the lower trapezius muscles or a control group (n = 15) that received gentle palpation of the skin. Each group participated in the intervention for 30 min, three times per week for six weeks. The visual analogue scale for pain, ROM, and NDI of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention compared with pre-intervention, and independent t-tests were used to analyze differences in the dependent variables between the two groups. Results: After the six-week intervention, both groups experienced significantly decreased pain and NDI (p < 0.05) and significantly increased cervical flexion, extension, lateral flexion, and rotation ROM (p < 0.05). The PNF group that received PNF strength exercise of the lower trapezius muscles showed greater improvements in pain and NDI and cervical rotation of ROM than those of the control group (p < 0.05). Conclusion: These results suggest that the PNF lower trapezius strengthening exercise reduces neck pain and disability level and enhances cervical ROM in patients with chronic neck pain.
