  • Objectives : The aim of this study was to observe the effect of Bong Chuna manual therapy(BCMT) and acupuncture on the Turtle Neck Syndrome(TNS) by the changes of radiological findings and symptoms. Methods : Total twenty-eight patients diagnosed as TNS were evaluated. Twenty patients in experimental group were treated by the combination of BCMT and acupuncture, eight patients in control group were treated by acupuncture only. We measured VAS(Visual analogue scale) as pain intensity and Neck Pain and Disability Scale(NDI) at pre- and post-treatment. Also the four line Cobb's method and Jochumsen method were assessed for evaluating the radiographical changes, additionally we used 'two line method'. Results : VAS score showed statistically significant reduction in pain intensity in both experimental group and control group after 8 weeks treatment, $-44.05{\pm}14.91$ vs $-23.75{\pm}14.08$ respectively. Also NDI score presented $-11.40{\pm}8.63$ reduction in experimental group, and it was significant statistically, however not in control group, $-8.63{\pm}9.84$ reduction after 8weeks treatment. In radiological findings, the four line Cobb's method, Jochumsen method and two line method were reduced after 8weeks treatment, $-9.30{\pm}10.33$, $-0.65{\pm}1.72$ and $-14.35{\pm}5.68$ in experimental group respectively, $-0.75{\pm}1.91$, $-0.25{\pm}0.71$ and $0.38{\pm}2.20$ in control group respectively. However they were statistical significance only in the four line Cobb's method and two line method in both group. Conclusions : Combination treatment of CMT and acupuncture showed better effect on pain reduction, VAS score and the correction of neck anteversion than acupuncture only in TNS. Two line method seems valuable for evaluating the improvement of radiographical changes in TNS.

  • Cervical spondylotic myelopathy (CSM) is a severe disease that usually needs surgical treatment. We have treated two CSM patients with Korean traditonal medicine, which includes acupuncture, pharmacopuncture, herbal medicine and manual treatment (Chuna). For objective evaluation, we used Nurick's classification, JOA (Japanses Orthopedic Association) score, grip and release test and hand grasping power measurement. Post-treatment analysis has shown that grip and release test score and grasping power of two patients were both remarkably improved, along with the improvements on both Nurick's classification and JOA score. This study showed that Korean traditional medicine could be effective conservative treatment for CSM.

  • Objectives This study aims to comprehensively review research utilizing ultrasonography for assessing manipulative treatment on spinal diseases, with the goal of promoting the wider integration of ultrasound imaging into clinical practice. Methods A systematic search was conducted on three international databases (Embase, PubMed, Cochrane) up to July 23, 2023. The search included key terms such as ultrasonography, manipulation, and skeletal muscle. The inclusion criteria narrowed down the selection to studies specifically related to lumbar and cervical vertebrae. Results Eleven studies were included in the review, with 10 focusing on lumbar vertebrae and one on cervical vertebrae, all employing spinal manipulation treatment. Among the 11 selected studies, nine primarily focused on ultrasound imaging to measure muscle thickness, while two utilized shear wave elastography to assess muscle stiffness. Also, rigorous measures were taken to ensure the reliability of the ultrasonography data. Conclusions This scoping review highlights the limited but growing evidence supporting the use of ultrasonography to assess manipulative treatment for spinal diseases. Despite a scarcity of studies in South Korea, it is crucial to recognize the potential of ultrasonography in becoming a widely used and practical tool for evaluating the effectiveness of manipulative treatments in the near future.

  • Drug-induced dyskinesia is an involuntary muscle movement caused by various dopamine receptor-blocking drug exposure, such as antipsychotics, antidepressants, and antiemetics. Causative drug removal is the main treatment for drug-induced dyskinesia whenever possible because its pathophysiology lacks a universally accepted mechanism; however, the symptoms can persist for years or decades in many patients even after causative drug removal. Herein, we present a case of drug-induced dyskinesia in a 61-year-old female patient who consumed medication for approximately 10 years for her depression, anxiety, and insomnia. Cervical and facial dyskinesia was suggested to be related to perphenazine and levosulpiride administration. The patient received acupuncture, pharmacopuncture, herbal medicine, and chuna treatment for 81 days during hospitalization. The symptoms were evaluated using the Abnormal Involuntary Movement Scale, Toronto Western Spasmodic Torticollis Rating Scale, Tsui's score, and Numeric Rating Scale, which revealed remarkable improvement, suggesting the effectiveness of combined Korean medicine for drug-induced dyskinesia.
