The Effect of Support Group Intervention on Various Adaptations of Primary Family Caregivers Caring for Cerebro-Vascular Accident Patients (집단지지 중재가 뇌졸중 환자 가족원의 제 적응에 미치는 효과)

  • The purpose of this research was to examine the effect of support group intervention on the various adaptations of primary family caregivers caring for Cerebro- Vascular Accident patients. The nonequivalent control group pretest-posttest design within the framework of Lazarus & Folkman's stress-adaptation model was used for this experimental study. The subjects were 86 primary family caregivers caring for Cerebro- Vascular Accident patients at K hospital in Taegu, D herbal hospital in Kyung Ju, H hospital in Pohang from March, 1998 to July, 1998. Among 86 subjects, 43 were placed in an experimental group and 43 in a control group. The experimental group was treated by researcher who administered informational and emotional support group intervention once a week over a five weeks period. The data were collected through interviews. Collected data was analized by means of a chi-square test, t-test, ANCOVA, and Pearson correlation coefficient. The results of this research were as follows: 1. Physical, emotional, and social adaptation scores in the experimental group were revealed to be significantly higher than those of the control group. 2. There was significant positive correlation among physical health, subjective burden, depression and objective burden. Accordingly, it is concluded that informational and emotional support group intervention was a useful nursing intervention on the various adaptations of primary family caregivers caring for Cerebro-Vascular Accident patients.

  • 이 연구 대상자는 일개 대학병원 응급의료센터로 소방구급대를 통해 내원한 환자 중 컴퓨터 단층촬영으로 뇌졸중으로 진단된 환자에서 이송 시 환자평가 시행에 영향을 미치는 요인을 분석한 연구이다. 연구결과 환자 평가에 영향을 미치는 요인은 환자분류(응급, 비응급, 지연), 의식상태(AVPU 척도)보다는 구급대원 자격별이 병원 전 뇌졸중 환자 이송 시 환자평가 시행에 영향을 미치는 것으로 확인되었다.

A Study of Caregiver Stress as related to the Level of Self-care in Patients Who had a Cerebro Vascular Accident(CVA) (뇌졸중환자의 자가간호 수준에 따른 보호자의 스트레스)

  • The purpose of this study was to explore the degree of stress in caregivers caring for patients who had had a cerebro vascular accident as the stress is related to the self- care ability of the patient. The subjects for the study were caregivers of 111 CVA patients, hospitalized at two University affiliated hospitals and two general hospitals in Daejon. The survey instruments used in the study were Kang’s ADL Check List and a modified form of Lee’s Stress Inventory. The survey was conducted from July 16th to August 30th, 1992. The survey results were analyzed using the Statisitical Package for Social Sciences (SPSS) and can be sumerized as follows: 1. The level of self- care for the CVA patients was : 1) complete dependence (M=34.7, 31.2%), 2) complete independence (M=14.8. 13.3%), 3) in-complete independence ( M=17.5, 15.8% ), 4) in-complete dependence (M=14.8, 13.3%) and 5) dependence and independence (M=14.1, 12.7%). The items for which there was a high level of self-care were 1) drinking (M=3.640), 2) returning (M=2.351) and 3) eating (M=2.351) : and the items for which there was a low level of self -care were : 1) ascending and descending stairs (M=2.351), 2) dressing and undressing trousers (M=2.514) and 3) dressing and undressing jacket (M :2.532). 2. There was a statistically difference between the paralytic status and the level of self- care accord-ing to their demographic characteristics ( F=24. 7056, p(.001). 3. There was no significant difference in the degree of caregiver stress according to patient's demo-graphic characteristics. 4. There was a statistically significant difference in the degree of caregiver stress according to the following demographic characteristics : age (F=7.4189, p(.001), education level (F=5.8336, P(.01), family structure (t=2.10, p(.05) and their relationship with the patient (F=6.5099, P〈.01). 5. There was no significant difference in the degree of caregiver stress according to the level of patient self - care.

An Epidemiological Investigation on Severity of Cerebro-Vascular Accident Patients in Korea (한국인의 뇌졸중 위중도에 관한 역학적 분석)

  • A study on severity of cerebro-vascular accident patients in Korea was conduced in order to determine the risk factors affecting the severity of stroke patients. This study was performed by interviewing a total of 477 hospitalized and ambulatory patient of CVA in Seoul, Taejon, and Taegu and Pusan areas from April 1, 1998 through June 30, 1998. The results are as follows; 1. Two hundred and sixty seven $(56\%)$ at Four hundred and seventy seven subjects were manes, md two hundred and ten $(44\%)$ were females. $28.3\%$ of the subjects' ages were 50's and $27.3\%$ were 60's and $18.5\%$ were 70's. $22.6\%$ of the subjects' occupations were home makers, $17.8\%$ were farming. $66.4\%$ of the subjects spent their childhood in urban areas and $33.1\%$in rural areas. $41.7\%$ of the subjects became violin of CVA in Spring, $35.0\%$ in Winter. 2. On physical and mental conditions of the subjects at the occurrence of CVA, $28.7\%$ of the subjects were engaged in physical activities. $22.6\%$ were in a rest stale $19.5\%$ were in sleeping and $18.9\%$ were mentally shocked. $79.4\%$ of the male subjects and $14.8\%$ of female subjects smoked cigarettes. $82\%$ of male subjects drank coffee. $81.1\%$ of the subjects did not exercise regularly. $45.9\%$ of the subjects had the systolic blood pressures in the range of 160 to 199 mmHg and $5.6\%$ of the subjects had hypertension before the occurrence of stroke and $11.7\%$ had diabetes. 3. Of the types of strokes, cerebral hemorrhage was the highest $(49.1\%)$. cerebral infarction was the second $(41.1\%)$. Severe strokes were found in cerebral hemorrhage cases $(52.0\%)$ and cerebral infarction cases $(40.1\%).\;50.9\%$. of the male subjects were moderate cases, $50.9\%$ were severe cases. In females, moderate cases were $72.4\%$, severe cases $15.2\%$. $37.5\%$ of the subjects who had preceding diseases were severe cases, and $15.6\%$ of the subjects without preceding diseases were severe cases. $50.7\%$ of the subjects whose family members had strokes had severe strokes. $34\%$ of the subjects sleeping less than 6 hours a day, $42.4\%$ of the subjects with irregular eating habits, $33.3\%$ of the subjects who liked meat, and $42.3\%$ of the subjects who liked salty foods had severe strokes. $35.9\%$ of the subjects with hot temper, $27.6\%$ of the subjects with moderate temper and $14.5\%$ of subjects with mild temper were severe cases. 4. The correlation coefficient between obesity and blood pressure was 0.094.

Elevated Levels of Serum Lipids Accelerate Biological Vascular Aging evaluated by the Second Derivative of Photoplethysmogram Waveform in the Middle-Age Adults (중년 성인남녀의 혈중 지질농도가 가속도맥파에 미치는 영향)

  • Objective : There is accumulating data that cardiovascular diseases, including hypertension, cerebro-vascular accident, coronary heart disease, relate to atherosclerotic changes. Increased serum levels of lipids could play a role in these circumstances. the SDPTG(the second derivative of photoplethysmogram waveform) is noninvasive methods for evaluating the pulse wave and is correlated with age and other risk factors for atherosclerosis. Methods : To study the effects of serum lipid levels on the SDPTG in the middle-age adults, data on serum lipid levels and the SDPTG were collected in 80 healthy middle age with varying serum lipid concentrations. Biological vascular age was assessed using the SDPTG. the SDPTG consists of an a, b, c, and d wave in systole and an e wave in diastole; an SDPTG aging index(Al) was calculated as (b-c-d-e)/a. Results : Serum triglyceride, total cholesterol, total lipid, phospholipid, HDL cholesterol levels were respectively and significantly correlated with SDPTG Al. Serum lipid levels and blood pressure are age-dependent and the SDPTG is blood pressure-dependent. Restricting analysis to age and blood pressure yield strong positive correlations between serum triglyceride, total lipid levels and SDPTG Al, but no correlations between serum total cholesterol, phospholipid levels and SDPTG Al. Conclusion : The results of this study suggest that preventive acupuncture programs or herbal medications for reducing serum lipid, especially triglyceride, levels in middle-age should include a evaluation of biological vascular aging with the second derivative of photoplethysmogram waveform.

The effects of dual-task training on ambulatory abilities of stroke patients: Review of the latest trend

  • Objective: Dual-task walking deficits impact functional daily life, which often requires walking while performing simultaneous tasks such as talking, thinking or carrying an object. This study is to find out the latest trend of dual-task training's influence on ambulatory abilities of a stroke patient. Design: Systematic review of randomized controlled trials. Methods: This literature review was conducted in Pubmed and Sciencedirect with the follwing key words:stroke, cerebro-vascular accident, hemiplegia, gait, rehabilitation, exercise. 7 studies were chosen in findings by search tool. 3 studies were case study, 3 studies were cross sectional observational study and 1 study was randomized controlled trial. Results: It was found that stroke patients have difficulties in doing 2 motor tasks simultaneously and when they do 2 tasks, one is done in a naturally preferred activity areas. Moreover, when simply applying dual-tasks, the walking speed decreased. Meanwhile, when applying them through training, the speed increased. This showed the improvement of effective task-implementation abilities after dual-task training using task-integration models. Conclusions: In the beginning of the 2000s, dual-tasks were implemented by simply combining walking and cognition or exercise task, and the results of this study suggest that subjects with stroke have difficulty performing dual task. However, the latest trend is to let patients do the dual-task training by combining it with virtual reality. Therefore, dual task training could be performed in a safe in the environment such as virtual reality or augment reality.

The Status of Medical Rehabilitation of the CVA Patients (뇌졸중환자의 재활의료실태)

  • This research tries to find out the methods to enhance the rehabilitation ability of Cerebro Vascular Accident(CVA) patients and their family members. For this purpose, the authors identify the factors that may affect the medical rehabilitation process as well as the rehabilitation itself. Furthermore, the current status of the rehabilitation ability and the desires of CVA patients and their family members are surveyed in this research. The results of this research are as follows: Firstly, the surveyed CVA patients, most of whom are over fifties in the age, have cerebral infection and hemorrhage in a numerical order. The survey showed that the larger cities they live in, the more CVA occur to them. Secondly, the survey for the treatment cost demonstrated that the lower are their living level and their education level, the more pressure is put on the CVA patients and their family. The statistical data proved this conclusion is reasonable. Thirdly, as for the safisfaction for the rehabilitation treatment, the statistical survey showed that it is directly proportional to their financial income. Moreover, those who have lower income generally want the rehabilitation services from a public health center. On the other hand, those who have higher income want the home therapy services.

Comparison in Quality of Life, Relationship with Patients and Family Support, between Caregiver of Patients with Cerebro-Vascular Accident(CVA) in Western and Oriental Medicine (양.한방 뇌졸중 환자 주간호자의 삶의 질, 발병 전 환자와의 관계 및 가족지지 비교)

  • Purpose: This study was a descriptive study to compare of quality of life, relationship with CVA patients, and family support between caregiver of patient with CVA in western and oriental medicine. Method: Total 270 caregivers for CVA patients were selected from western medicine department (114) and oriental medicine department (156) at K hospital in Seoul. The instrument tools utilized in this study were quality of life, Family support, Caregiver's relationship with patients, daily activity. Collected data were analyzed by $X^2$-test, t-test using SPSS Windows 12.0 Program. Result: The result are as follows: 1) There was no significant difference in quality of life between caregivers of patients with CVA in western and oriental medicine(t=.26, p=.792). 2) There was significant difference in relationship with patients(t=2.64, p=.009) and family support(t=2.12, p=.035) before onset of disease between caregivers of patients with CVA in western and oriental medicine. Conclusion: As a result, caregivers of oriental medicine with CVA patients showed closer relationship with patients before the onset of disease and higher family support compare to caregivers of western medicine with CVA patients.

The Relationship between Pulse Wave Velocity (PWV) and Risk Factors of Cardiovascular Disease including Serum Lipid Values (Pulse Wave Velocity(PWV)와 혈청지질수치를 포함한 심혈관 질환 위험 요인들과의 상관관계 연구)

  • Objectives : There are lots of reports that cardiovascular disease, including hypertension, cerebro-vascular accident, and coronary heart disease, is related to atherosclerotic changes. Increased serum levels of lipids could play a role in these changes. This study aimed to investigate the relationship between PWV and risk factors of cardiovascular disease, including serum lipid values. Methods : This study included 261 subjects ($49.32{\pm}11.79$ years, 112 male) who underwent PWV and serum lipid evaluation. We investigated the correlation between serum lipid values, blood pressure, body mass index (EMI) and PWV. Pearson's correlation and partial correlation analysis were applied to examine the relationship between PWV and risk factors of cardiovascular disease. Results : Serum triglyceride, total cholesterol, systolic blood pressure, and diastolic blood pressure were significantly correlated with PWV. Partial correlation coefficient adjusted by age yielded significant correlation between serum triglyceride, systolic blood pressure, diastolic blood pressure and PWV. Conclusion : In this study, it seems that there are significant relationships between PWV, triglyceride and blood pressure. We could suggest that PWV might have some relationships with Dam-eum and blood stasis in oriental theory.

Acoustic parameter delta of an aspirated voice in stroke patients (뇌졸중 환자 대상 흡인 음성의 음향변수 변동)

  • The present study aimed to investigate the changes of acoustic parameters of the aspirated voice in stroke patients. The eighty-eight subjects diagnosed with cerebro-vascular accident were divided into 32 penetration/aspiration (P/A) and 56 Non-P/A groups according to the videofluroscopic swallowing study (VFSS) results, and 26 control subjects participated. All subjects preformed VFSS and vowel /a/ was recorded three times pre- and post VFSS. Since the variation in the acoustic parameters within a single phonation has been observed, we proposed a delta formula for the acoustic parameters which can reflect the temporal changes of the each parameter in an utterance. We measured from the voice data eight acoustic parameters: fundamental frequency (F0), standard deviation of F0 (F0_SD), Jitter, relative average perturbation (RAP), Shimmer, amplitude perturbation quotient (APQ), harmonic to noise ration (HNR), noise to harmonic ratio (NHR). Then we found parameters which show the meaningful biggest temporal change in an utterance using the suggested delta parameter. Among them, the deltas of shimmer and APQ were significantly different pre- and post VFSS. These deltas of the P/A and the control group were increased after VFSS, while those of the Non-P/A group was descended. The variation patterns of the P/A and the control group were similar but the change width of the P/A group was larger. The large variations in an aspirated phonation of the P/A group are thought to be caused by irregular changes in air resistance due to residual food on the vocal cords.
