백서의 가역성 뇌허혈 모형에서 저체온의 효과와 적용시기 (The Time and Effect of Hypothermia in Early Stage of the Reversible Cerebral Focal Ischemic Model of Rat)

  • Objective : We studied to clarify the effective time zone of mild hypothermic neural protection during ischemia and/or reperfusion after middle cerebral artery occlusion. Methods : In a reversible cerebral infarct model which maintained reperfusion of blood flow after middle cerebral artery occlusion for two hours, the size of cerebral infarction, cerebral edema and the extent of neurological deficit were observed and analyzed for comparison between the control and the experimental groups under hypothermia($33.5^{\circ}C$). The temporalis muscle temperature was reduced to $33.5^{\circ}C$ by surface cooling for two hours during middle cerebral artery occlusion for study group I. The following groups applied hypothermia for two-hour periods after reperfusion : group II(0-2 hours), group III(2-4 hours), and group IV(4-6 hours). They were rewarmed to $36.5^{\circ}C$ until sacrified at 2, 4, 6, 12, and 24 hours after reperfusion. Control group was maintained at normothermia without hypothermia. Results : In the experimental groups with hypothermia, the average value of the size of cerebral infarction($mean{\pm}SD$) was $1.97{\pm}1.65%$, which was a remarkable reduction over that of the control, $4.93{\pm}3.79%$. In the control, a progressive increase was shown in the size of infarction from point of reperfusion to 6 hours after reperfusion without further changes in size afterward. Intra-ischemic hypothermia(group I) prevented ischemic injury but did not prevent reperfusion injury. Group II examplified the most neural protective effect in comparison to the control group and group IV(p<0.05). The cortex was more vulnerable to reperfusion injury than the subcortex. Mild hypothermia showed more neural protective effects on the cortex than subcortex. Conclusion : The most appropriate time zone for application of mild hypothermia was defined to be within four hours following reperfusion.

저체온 순환정지를 이용한 흉부 대동맥 수술 시 뇌관류 방법에 따른 수술결과 (Influence of Cerebral Protection Methods in Thoracic Aortic Surgery Using Hypothermic Circulatory Arrest)

  • 배경: 뇌관류 방법은 저체온 순환정지를 이용한 흉부 대동맥 수술 시 가장 중요한 관심사이다. 이 연구는 저체온 순환정지를 이용한 흉부 대동맥 수술 시 뇌관류 방법(역행성 뇌관류법과 전방성 뇌관류 법)에 따른 수술 결과를 비교하였다. 대상 및 방법: 1995년 5월부터 2007년 2월까지 저체온 순환정지를 이용한 흉부 대동맥 수술 중 역행성 뇌관류법이나 전방성 뇌관류법을 이용한 146명의 환자를 연구대상으로 후향적으로 분석하였다. 이 중 역행성 뇌관류법을 사용한 경우가 114예(1군)였으며 전방성 뇌관류법은 32예(2군)에서 사용되었다. 원인별로는 대동맥 박리증이 104명(급성 박리증 94명, 만성 박리증 10명), 대동맥류가 42명(진성 대동맥류 41명, 가성 대동맥류 1명)이었다. 수술 범위로는 대동맥궁 완전치환술 21명, 대동맥궁 부분치환술 5명, 대동맥궁 반치환술을 33명에서 시행하였으며 모든 환자에서 상행대동맥 치환술을 시행하였다. 결과: 두 군간의 수술 전 혹은 수술 특성은 대부분 비슷하였지만 2군에서는 70세 이상 고령 환자가 더 많았으며(34.4% vs. 10.5%), 관상동맥 질환이 더 많았고 (18.8% vs. 4.4%), 대동맥궁 완전치환술을 더 많이 시행하였고 (46.9% vs. 5.3%), 저체온 순환정지 시간이 더 걸었다 ($50{\pm}24$분 vs. $32{\pm}17$분). 수술 사망률은 1군이 4.4% (5/114)였고 2군이 3.1% (1/32)였다. 영구적 뇌손상 발생률은 1군이 5.3% (6/114)였고 2군이 3.1% (1/32)였다. 일시적 뇌손상은 1군이 1.8% (2/114)였고 2군이 9.4% (3/32)였다. 수술 사망률, 수술 후 출혈 및 뇌손상(영구적 혹은 일시적) 발생률은 두 군간 통계적 유의성이 있는 차이는 보이지 않았다. 결론: 저체온 순환정지를 이용한 대동맥 수술의 조기 결과는 양호하였고 뇌관류법에 따른 차이는 관찰되지 않았다. 하지만 저 체온 순환정지 시간이 더 길거나 더 광범위한 대동맥궁 수술에서 전방성 뇌관류법이 이용되었음에도 불구하고 대등한 결과를 보인 점을 고려할 때 저체온 순환정지 시간이 걸어질 것으로 예상되거나 수 술범위가 커질 것으로 예상되는 경우에는 전방성 뇌관류법을 사용하는 것이 바람직할 것으로 사료된다.

뇌허혈기동안 경동맥으로 냉각 생리식염수 주입이 허혈후 뇌부종에 미치는 영향 (Effect of Intracarotid Cold Saline Infusion during Cerebral Ischemia on Brain Edema in the Rabbit)

  • 건전한 뇌의 수분 함량은 뇌혈류장벽의 투과성, 혈관내 정수압, 혈장의 삼투압 Na-K-APTase pump의 기능에 의해서 결정된다. 뇌허혈에 의해 이러한 기전이 파괴되면 세포 사이의 정상적인 수분이동이 영향을 받게 된다. 그러므로 뇌허혈이 생길 수 있는 수술의 경우에 분리 뇌관류를 하면 뇌허혈시 발생하는 독성 대사물을 발생을 억제하고 뇌혈류의 분포를 개선하여 신경세포의 손상을 줄이고 뇌부종을 감소시킬 수 있으리라 생각되어 토끼에서 분리 뇌관류 모델을 만들어 허혈 유도 후 관류군과 비관류군의 대뇌피질과 해면구의 비중을 정상 토끼와 비교하였다. 뇌허혈에 의해 관류군과 비관류군의 뇌비중이 유의하게 감소하여 뇌부종이 발생하였음을 나타내었고, 비관류군이 판류군보다 유의하게 감소하여 뇌부종이 더욱 현저하였던 것으로 보아 냉각 식염수로 뇌관류한 것이 뇌부종의 발생을 억제한 것으로 생각된다.

공진단이 MCAO모델 흰쥐에서 gliosis 억제에 마치는 영향 (The Effect of Gongjin-dan on Gliosis in Middle Cerebral Artery Occlusion (MCAO) Rats)

  • Objectives : In conditions of brain infarction, irreversible axon damage occurs in the central nerve system (CNS), because gliosis becomes a physical and a mechanical barrier to axonal regeneration. Reactive gliosis induced by ischemic injury such as middle cerebral artery occlusion is involved with up-regulation of GFAP and CD81. This study was undertaken to examine the effect of the Gongjin-dan (GJD) on CD81 and GFAP expression and its pathway in the rat brain following middle cerebral artery occlusion (MCAO). Methods : In order to study ischemic injuries on the brain, infarction was induced by MCAO using insertion of a single nylon thread, through the internal carotid artery, into a middle cerebral artery. Cresyl violet staining, cerebral infarction size measurement, immunohistochemistry and microscopic examination were used to detect the expression of CD81 and GFAP and the effect on the infarct size and pyramidal cell death in the brain of the rat with cerebral infarction induced by MCAO. Also, c-Fos and ERK expression were measured to investigate the signaling pathway after GJD administration in MCAO rats. Results : Measuring the size of cerebral infarction induced by MCAO in the rat after injection of GJD showed the size had decreased. GJD administration showed pyramidal cell death protection in the hippocampus in the MCAO rat. GJD administration decreased GF AP expression in the MCAO rat. GJD administration decreased CD81 expression in the MCAO rat. GJD administration induced up-regulation of c-FOS expression compared with MCAO. GJD administration induced down-regulation of ERK expression compared with MCAO. Conclusion : We observed that GJD could suppress the reactive gliosis, which disturbs the axonal regeneration in the brain of a rat with cerebral infarction after MCAO by controlling the expression of CD81 and GFAP. The effect may be modulated by the regulation of c-Fos and ERK. These results suggest that GJD can be a candidate to regenerate CNS injury.

Expression and Significance of MicroRNA155 in Serum of Patients with Cerebral Small Vessel Disease

  • Objective : This study aimed to investigate the changes and significance of microRNA155 levels in serum of patients with cerebral small vessel disease (CSVD). Methods : Thirty patients with CSVD who met the inclusion criteria were selected and divided into eight patients with lacunar infarction (LI) group and 22 patients with multiple lacunar infarction (MLI) combined with white matter lesions (WML) group according to the results of head magnetic resonance imaging (MRI). Thirty samples from healthy volunteers without abnormalities after head MRI examination were selected as the control group. The levels of serum microRNA155 in each group were determined by real-time polymerase chain reaction, and the correlation between microRNA155 in the serum of patients with CSVD and the increase of imaging lesions was analyzed by Spearman correlation analysis. Results : Compared with the control group, the serum microRNA155 level in the LI group, MLI combined with WML group increased, the difference was statistically significant (p<0.05); serum microRNA155 level was positively correlated with the increase of imaging lesions (p<0.05). Conclusion : The change of serum microRNA155 level in patients with CSVD may be one of its self-protection mechanisms, and the intensity of this self-protection mechanism is positively correlated with the number of CSVD lesions.

  • 뇌신경계 인터벤션시술은 장시간의 시술로 인해 피부의 수포 탈모 홍반등의 방사선 피폭으로 인한 위해가 빈번히 보고되어 지고 있다. 이에 CT(computed tomography) scan 시 피부 및 방사선 감수성이 예민한 인체의 전방부에 위치한 표적장기의 방사선 피폭을 감소시킬 목적으로 제작된 bismuth 차폐체를 인체공학적인 설계에 의하여 뇌혈관계 인터벤션 시술에 적용함으로써 방사선 감수성이 비교적 예민한 두피의 방사선 피폭 저감화를 위하여 연구를 제안 하고자 한다.

방풍당귀음(防風當歸飮)이 중대뇌동맥 폐쇄 후 재관류 모델에 미치는 영향 -육안.광학현미경 소견- (The effect of Bangpungdangkwi-eum extracts on reperfusion following the middle cerebral artery occlusion in rats)

  • To evaluate the effect of Bangpungdangkwi-eum extracts on reperfusion following the middle cerebral artery occulsion in Sprague Dawley rats, the neuron protection effect were investigated through examining the size of cerebral infarction, cerebral edema, and the morphologic change of neuron. The results were obtained as follows; 1. The size of cerabral infarction in sample group is significantly decreased compared with that in control group. Sample group has approximately 17% cerebral infarction parts induced by ischemia in cerebrum while control group has approximately 22%. 2. The volume of cerebral edema in sample group is significantly decreased compared with that in control group. The volumn in sample group is increased by approximately 4.4% compared with that in normal group while that in control group is increased by approximately 7.7%. 3. The optical microscopic examination reveals that the damage of neurons in the ischemic parts and CA1 and CA3 region of hippocampus in the same side of the ischemic parts was the most high and the damage in sample group is decreased compared with that in control group.

Apolipoprotein E Expression in Experimentally Induced Intracranial Aneurysms of Rats

  • Objective : An Intracranial aneurysm is an important acquired cerebrovascular disease that can cause a catastrophic subarachnoid hemorrhage. Atherosclerosis is one of possible mechanism, but its contribution to aneurysm formation is unclear. Human apolipoprotein E[apoE] is best known for its arterial protection from atherosclerosis. In this study we observe apoE expression in experimental cerebral aneurysms of rats to elucidate the role of apoE in the process of cerebral aneurysm formation. Methods : Twenty-four male 7-week-old Sprague-Dawley strain rats received a cerebral aneurysm induction procedure. One month[12] and three months[12] after the operation, the rats were killed, their cerebral arteries were dissected, and the regions of the bifurcation of the right anterior cerebral artery-olfactory artery [ACA-OA] bifurcations were examined histologically and immunohistochemically. Results : In the 1 month group [n=12], the ACA-OA bifurcation showed no aneurysmal change in 7 rats and early aneurysmal change in 5 rats. In the 3 months group (n=12), the bifurcation showed no aneurysmal change in 2 rats and an advanced aneurysm in 10 rats. ApoE expression were in 3 specimen in early aneurysmal change, but not in advanced aneurysms. Conclusion : ApoE expression in early aneurysmal wall suggests a possible role for apoE in early events leading to aneurysm formation. Further studios are necessary to elucidate the exact role of apoE in the pathophysiology of cerebral aneurysm.

흰쥐의 중대뇌동맥 결찰로 유발된 뇌허혈에서 성향정기산과 곽향정기산이 신경세포에 미치는 효과 (The Effects of Sunghyangjunggi-san and Gwackhyangjunggi-san Extracts on Cerebral Ischemia Following the MCA Occlusion in Rat)

  • Objectives : The purpose of this investigation is to evaluate and compare the effects of Sunghyangjunggi-san (SH) and Gwackhyangjunggi-san (GH) extracts on reperfusion following the MCA occlusion in rats. Methods : To evaluate the effect of Sunghyangjunggi-san (SH) and Gwackhyangjunggi-san (GH) extracts on reperfusion following the MCA occlusion, the volume of cerebral infarction and edema were measured and the change of the CA1 pyramidal neurons in the hippocampus were investigated by light microscopy. Results : 1. The infarction volume of the control group was 23.6%, that of the GH group was 23.7%, and that of the SH group was 18.5%. 2. The brain edema volume of the control group increased by 16% compared with that of the normal group, that of the GH group increased by 14%, and that of the SH group increased by 9%. 3. The number of surviving pyramidal neurons in the CAI area of the hippocampus was investigated under light microscopy. In the control group, few surviving pyramidal neurons excisted (mean 6.4) and similarly in the GH group (mean 8.5), but in the SH group, the number of surviving pyramidal neurons was significantly higher, to the mean 18.4. Conclusions : According to the above results, in regard to the damage of neurons following cerebral ischemia, the GH group has little effect of the protection of neurons compared to the control group, but the SH group has a remarkable effect.

청심연자음(淸心蓮子飮)과 성향정기산(星香正氣散)이 Streptozotocin유발(誘發) 당뇨(糖尿)흰쥐의 뇌허혈 손상(腦虛血 損傷)에 미치는 영향(影響) (Effect of Chengsimyeunja-eum (淸心蓮子飮) and Sunghyangjungi-san (星香正氣散) on Streptozotocin-induced Ischemic Damaged Diabetic Rats)

  • Objectives : Chengsimyeunja-eum and Sunghyangjungi-san are prescriptions used for cerebral infarction clinically; it is known that these formulas reduce ischemic damage. According to previous research data, controlling certain types of glucose is considered to decrease the risk of cerebral infarction. Based on this fact, we investigated the effects of Chengsimyeunja-eum and Sunghyangjungi-san extracts on reperfusion following ischemic damage to diabetic rats, the change of c-FOS and Bax positive neurons in the hippocampus and cerebral cortex and protein through immunohistochemical methods, changes of serum glucose level, serum triglyceride level, and hepatic glucokinase activity. Methods : We induced ischemic damaged in diabetic rats, and the rats were administered Chengsimyeunja-eum and Sunghyangjungi-san extracts. Results : Chengsimyeunja-eum demonstrated significant decrease of c-Fos positive neurons in both hippocampus and cerebral cortex as well as a significant decrease of Bax positive neurons in hippocampus after ischemic damage on diabetic rats and decrease of serum glucose level after ischemic damage on diabetic rats. Sunghyangjungi-san demonstrated significant decreases of c-Fos and Bax positive neurons in both hippocampus and cerebral cortex after ischemic damage on diabetic rats. Conclusions : Chengsimyeunja-eum, effect on glucose level control, has a remarkable effect of protection of neurons not effective on glucose level. Sunghyangjungi-san showed neuroprotective effect through preventing neuronal cell death.
