Alzheimer Dementia and Microvascular Pathology: Blood-Brain Barrier Permeability Imaging (알츠하이머 치매와 미세뇌혈관병리: 혈액뇌장벽 투과도 영상)

  • Accumulating evidence suggests that Alzheimer's disease (AD) is not only caused by accumulation of abnormal proteins, including amyloid and tau, but is also closely associated with abnormalities in the microvascular environment including the blood-brain barrier (BBB), both of which lead to neuroinflammation and neurodegeneration. Application of in vivo magnetic resonance imaging (MRI) has recently increased to assess BBB permeability in AD and related diseases. Here, we provide a narrative review of BBB permeability-related pathology in Alzheimer dementia and recent MRI research on BBB permeability changes in AD and related diseases. Furthermore, we briefly introduce the measurement of BBB permeability using MRI and its methodological issues.

Leptomeningeal Enhancement without Thalamic Involvement as an Initial Manifestation of Japanese Encephalitis: A Case Report (시상 침범 없는 연수막 조영증강을 보인 일본뇌염: 증례 보고)

  • Japanese encephalitis (JE) is a common infection caused by the Japanese encephalitis virus in Southeast Asia, which is transmitted to humans through Culex mosquitoes. Magnetic resonance imaging (MRI) is used to diagnose JE, which is often characterized by the presence of bilateral symmetric thalamic involvement. Here, we report a rare case of JE characterized by leptomeningeal enhancement without thalamic involvement. This leptomeningeal enhancement disappeared with the treatment; however, new non-specific multifocal and bilateral high signal intensities in the cerebral white matter were found on follow-up MRI.

  • Hashimoto's encephalopathy (HE) is a rare autoimmune disease characterized by a high serum concentration of antithyroid antibodies without evidence of cerebral disease. Magnetic resonance imaging (MRI) findings in HE patients are nonspecific, although diffuse or focal white matter changes have been reported in several cases. We present a rare case involving a 79-year-old woman with elevated antithyroid antibody levels and abnormal imaging findings similar to meningoencephalitis. Serial MRI initially showed multiple T2 hyperintense lesions with diffuse leptomeningeal enhancement that disappeared after steroid therapy.

  • BOLD T2*-weighted MR images reflects cortical blood flow and oxygenation alterations. fMRI study relies on the detection of localized changes in BOLD signal intensity. Since fMRI measures the very small modulations in BOLD signal intensity that occur during changes in brain activity, it is also very sensitive to small signal intensity variations caused by physiologic noise during the scan. Due to the complexity of movement of various organs associated with heart beat, it is important to reduce cardiac related noise rather than other physiological noise which could be required with relatively simple method. Therefore, a number of methods have been developed for the estimation and reduction of cardiac noise in fMRI study. But, each method has limitation. In this study, we proposed a new estimation method for brain activities influenced by blood pulsation effect using regression analysis with blood pulsation signal and the correspond slice of fMRI. We could find out that the right anterior cingulate cortex and right olfactory cortex and left olfactory cortex were largely influenced by blood pulsation effect for new method. These observed areas are mostly on the structure of anterior cerebral artery in the brain. That is convinced with that our method would be valid and our new method is easier to apply in practice and reduce computational burden than the retrospective method.

  • An 8-month-old 3.15 kg female Cocker-spaniel with history of ataxia referred to the Veterianary Medical Teaching Hospital, Chungnam National University. There were no abnormalities in CBC and serum chemical analysis. Agenesis of dens was found on dorsoventral view in cervical radiography. Compressed cervical spinal cord and enlarged cerebral ventricle were observed in magnetic resonance imaging. It was diagnosed as atlantoaxial instability with hydrocephalus. For conservative therapy, neck brace was applied and diuretics and prednisolone were administered. The dog's ataxia became better gradually.

  • 목적: Writer's cramp를 가진 환자와 정상 성인에서 뇌 활성화를 비교 하고자 한다. 대상 및 방법: Writer's cramp를 가진 1명의 환자와 정상 성인 4명에서 글을 쓰는 과제를 시행하여 관련된 대뇌영역의 활성화를 유도하였다. resting, writering을 반복하는 자극을 시행하였다. 자기공명영상은 3.0T 초전도 자기공명 영상장치에서 EPI BOLD기법을 이용하여 얻었다. 영상 후 처리는 SPM 분석 프로그램을 사용하였다.

  • The present study was planned to investigate the cortical activation correlated with producing morphologically complex Korean verbs by using. fMRI technique. In this study two derivational affixes and two inflectional affixes were selected: pre-final ending and final ending for inflectional affix and passive affix and causative affix for derivational affix. Two Experiment were conducted. The results of two Experiments suggest a possibility that process of pre-final ending is different from final ending.

  • Many adult diseases(cerebral apoplexy, athymiait, etc.) result from hypertension, blood circulation disturbance and increment of HBP. In early diagnosis of these diseases, MRI, X-ray and PET have been used rather aim for treatment than prevention of a disease. Since, cerebral apoplexy and athymiait have been caused to the regular/irregular persons, it is very important to measure HBP which has connection with cerebral blood low state. HBP has more diagnosis elements than that of BP. So, we can diagnose accurate hypertension by measuring of HBP. But, existing sphygmomanometers and automatic BP monitors can not measure HBF, and can not execute complex function(measuring of BP/HBP, blood flow improvement). The purpose of this paper is to develop the system and algorithm which can measure BP/HBP for accurate diagnosis. Also, we extracted diagnosis factors by the correlativity analysis of BP/HBP. The maximum pressure of HBP corresponds to 62% that of BP, the minimum pressure of HBP corresponds to 46% that of BP. Therefore, we developed the multi function automatic blood pressure monitor which can measure BP/HBP and improve cerebral blood flow state.

  • Clinical observation was made on 161 cases of CVA that were confirmed through brain CT, MRI scan and clinical observation. They were hospitalized in the oriental medical hospital of Kyung-Won University from January to December in 1994. 1. The CVA cases were classified into the following kinds: cerebral infarction, cerebral hemorrhage, subarachnoid hemorrhage(SAH), transient ischemic attack, hypertensive encephalopathy, and the greatest in number among them were the cases of cerebral infarction. 2. The frequency of strokes was much the same between male and female cases and most cases were over 50 of age. 3. In the case of cerebral infarction the place of the most frequent occurrences was in the MCA territory, and as for cerebral hemorrhage, in the basal ganglia area. 4. The most ordinary preceding disease was hypertension. The next was diabetes mellitus. 5. Generally it is thought that CVA occurs frequently in winter. But on the contrary this study of observation confirmed that it occurs mostly in summer. 6. The predisposing factors of cerebral infarction were usually initiated during the time of resting and sleeping and those of cerebral hemorrhage chiefly during the time of exercising. 7. As concerns the course of hospitalization, most patients passed through western medical hospitals or oriental medical hospitals. 8. For the patients the condition of whose consciousness was bad at the time of admission, the prognosis in most cases was bad. 9. The common symptoms were motor disability and verbal disturbance. 10. With regard to cerebral infarction, the average time to start physical theraphy was 11.4 days and with cerebral hemorrhage 22.7 days after stroke. 11. The duration of hospitalization was in most cases more than one month. 12. The main complication was urinary tract infection. The next was pneumonia. 13. At the time of admission to hospital, the blood pressure in most cases was high, but it well controlled at the time of discharge. 14. Most cases were given simultaneous treatment in both ways of western and oriental medicine.
