척추기저동맥박리에 의한 소뇌경색 환자 한방치험 1례 (A Case Report of a Patient with Cerebellar Infarction with Vertebrobasilar Artery Dissection)

  • Objective: This study is a report on a case of cerebellar infarction with vertebrobasilar artery dissection which was improved by Korean medicine. Methods: A 63-year-old man diagnosed with cerebellar infarction with vertebrobasilar artery dissection was admitted to hospital for 86 days and treated with Korean medicine (acupuncture and herbal medicine) and rehabilitation treatment. Clinical symptoms were assessed with a Modified Barthel index, functional independent measurement, Berg balance scale, manual muscle test, and a visual analogue scale. Result: After treatment, the clinical symptoms were improved, and the evaluation index scores (modified Barthel index, functional independent measurement, Berg balance scale) increased. Conclusion: Korean medicine may be a meaningful treatment for patients with cerebellar infarction with vertebrobasilar artery dissection.

Cerebellar vermis atrophy로 인한 Spinocerebellar Ataxia환자(患者) 치험(治驗) 1례(1例) 보고(報告) (Case report on Spinpcerebellar Ataxia(SCA) with cerebellar vermis artophy)

  • Objective : In 1983 Autosomal dominent spinocerebellar ataxia(ADCA) has been classified to four types by a useful clinical features according to Harding. Since 1993 ADCA referred to as SCA by molecular genetic characteristics. We focused on the improvement of clinical symptoms in SCA patient through oriental medical treatment. Methods & Results : The assessment included neurophysiological examination and clinical symptoms. For example slow saccade, gaze limitation, upward and lateral, nystagmus, truncal and gait ataxia. The patient(M/30) was clinically charaterized by ocular abnormalities, trunkal and gait ataxia and the MRI showed atrophy of cerebellar vermis. The patient was taken both common acupuncture and Dong-Si venepuncture on Hwasan with herbal medicine. Before the treatment Rt. ocular movement were slowed and a significant eye fixation was observed always in abduction and intermittently presented trunkal and gait ataxia. During the treatment trunkal and gait ataxia has not been presented and ocular obnomalities changed to normal state. This case suggests the possibility of oriental medical treatment on SCA but further observation is needed on this patient.

  • 1. Objectives This study is about three cerebellar infarction patients with dizziness and ataxia. In this study, we evaluated the effects of Sasang constitutional treatment and suggested using the 'Equilibrium Function Test' as scale. 2. Methods These three patients were treated by Sasang constitutional herbal medications based on "Donguisusebowon". Acupuncture & moxa therapy and west medications were limited as much as possible. We evaluated the symptoms through two methods, the Visual Analogue Scale and the Equilibrium Function Test. 3. Results and Conclusions Dizziness and ataxia that were these three patients' chief complaints were improved. This case study shows that Sasang constitutional herbal medications are an effective treatment for cerebellar infarction patients.

반복적인 소뇌 출혈로 발현한 후두와 모세혈관 확장증 - 증례보고 - (Capillary Telangiectasia of the Posterior Fossa Presenting with Repeated Cerebellar Hemorrhage - A Case Report -)

  • Hemorrhage due to capillary telangiectasia is rare. We report a case of capillary telangiectasia presenting with repeated cerebellar hemorrhage in a 38-year-old male. To our knowledge this is the first case of repeated cerebellar hemorrhage from the capillary telangiectasia. At the first operation, we removed hematoma only, but rebleeding occurred repeatedly in the same area. Finally, we evacuated the recurrent hematoma and vascular mass of capillary telangiectasia at the second operation under surgical microscope. Based on the findings of this case and a review of the literature, we conclude that capillary telangiectasia can be the cause of the massive repeated hemorrhage.

  • This study reported on the effect of traditional Korean medicine on ataxia, gait disturbance, tremor, and dizziness with cerebellar atrophy. The patient was treated with traditional Korean medicines, such as acupuncture, moxibustion, and herbal medicine (Kuibiondam-tang-gami). The effectiveness of the treatment was evaluated with the scale for the assessment and rating of ataxia (SARA). After treatment, the symptoms had improved. The SARA score decreased by 18 points after Korean medicine treatment. According to this study, traditional Korean medicine can be effective to treat ataxia, gait disturbance, tremor, and dizziness in patients with cerebellar atrophy.

  • Objectives : Although there have been studies that examine sex differences of the brain structures using magnetic resonance imaging, studies that specifically investigate cerebellar structural differences between men and women are scarce. The purpose of current study was to examine sex differences in structures of the cerebellum using cerebellar template and cerebellum analysis methods. Methods : Sixteen men and twenty women were included in the study. A MATLAB based program (MathWorks, Natick, MA, USA), Statistical Parametric Mapping 5 (SPM5) using the spatially unbiased infra-tentorial atlas template (SUIT) as the cerebellum template, was used to analyze the brain imaging data. Results : There was no significant difference in age between men (mean age=28.1) and women (mean age=27.2). Men showed higher gray matter density than women in two left cerebellar areas including the clusters in the lobules IV and V (a cluster located across the lobules IV and V), and the lobule VIIIb (lobules IV and V, t=4.75, p<0.001 ; lobule VIIIb, t=3.08, p=0.004). Conclusions : The current study found differences in cerebellar gray matter density between men and women. The current study holds its significance for applying the template specifically developed for the analysis of cerebellum.

Chronic Exposure of Nicotine Modulates the Expressions of the Cerebellar Glial Glutamate Transporters in Rats

  • Rats were given nicotine (25 ppm) in their drinking water at the start of their mating period in order to study the expressions of glutamate transporter subtypes in cerebellar astrocytes following the chronic exposure of nicotine after mating. After the offspring were delivered, each group was divided into two subgroups. One group, the control group, was given distilled water only and the other group, the experimental group, was given distilled water containing nicotine. The cerebellar astrocytes were prepared from 7 day-old pups at each group. Ten days after the cells were cultured, the expression of the glutamate transporter subtypes (GLAST and GLT-1) was determined using immunochemistry and immunoblotting. During the continuous treatments, the developmental expression patterns of the GLAST and GLT-1 in the cerebellum were also determined from 2, 4 and 8 week-old rats. The expression levels of GLAST in cultured astrocytes of both the pre- or post-natally exposed groups were higher than those of the control group. However, these expression levels of the continuously exposed group were lower than those of the control group. Compared to those of the control group, the GLT-1 expression levels of all the nicotine-treated groups were higher, particularly in the continuously treated group.. According to the results from the immochemistry procedure, the cerebellar GLAST and GLT-1 expression levels of all nicotine-treated groups were lower than those of the control group at each age. However, the immunoblotting procedure showed that the cerebellar GLT-1 expression levels of all the nicotine-treated groups were higher than those of the control group, except for the rats that were continuously exposed for 8 weeks using immunoblotting. These results suggest that the expression of the glial GLAST and GLT-1 are altered differently depending on the initial exposure time and the partcicular period of nicotine exposure. In addition, nicotine exposure during gestation has persistent effects on glial cells.

천막상부 뇌졸중에서 소뇌의 혈역학 변화 -Dynamic Susceptibility Contrast MR 영상을 이용한- (Effect of Supratentorial Stroke on Cerebellar Hemodynamic Parameters - Assessment by Dynamic Susceptibility Contrast MR Imaging)

  • Background & Purpose : Dynamic susceptibility contrast MR imaging, one method of perfusion MRI, was developed to define cerebral hemodynamic status with good anatomical resolution. The authors investigated hemodynamic parameters using this imaging method, in an effort to identify hemodynamic changes on the remote crossed cerebellum of patients with a supratentorial infarct. Methods : Dynamic susceptibility contrast MR imaging was performed in 15 patients with only unilateral supratentorial infarcts. Imaging was obtained at the anatomic level of the cerebellum. rCBF, rCBV, MTT and TP were determined over both cerebellar hemispheres of interest. Results : The rCBF and rCBV values of the contralateral cerebellar hemisphere were significantly more decreased than those of the ipsilateral cerebellar hemisphere in 12 patients(p=0.028, 0.033). MTT and TP values of the contralateral and ipsilateral cerebellar hemispheres didn't reveal any differences(p=0.130, 0.121). Conclusions : The results of this work suggest that the region which are remote from the ischemic brain lesion shows no changes of MTT or TP but show decrease of rCBF and rCBV, mean to diaschisis, it also demonstrates that perfusion MRI is an easily available method to evaluate the hemodynamic status of the brain.

점진적 앉은 자세에서의 PNF운동이 소뇌 손상환자의 동적 균형능력에 미치는 효과 -단일 사례 연구- (Effects of Using PNF in Progressive Sitting Position on Dynamic Balance in a Patient with a Cerebellar Injury -Single Subject Design-)

  • Purpose: The purpose of this study was to investigate the effects of using proprioceptive neuromuscular facilitation (PNF) exercise in the progressive sitting position on the dynamic balance ability of a patient with a cerebellar injury. Methods: The subject had ataxia due to cerebellar injury. The subject participated in a PNF bilateral scapular pattern exercise with stabilizing reversal technique during a progressive sitting position session as well as baseline for 20 minutes a day for 4 weeks. In the first session, PNF exercises were performed at a height of 40 cm for 10 minutes, and in the second session they were performed at 50 cm for 10 minutes from a lower center of gravity (COG) to a higher COG sitting position. We used the Berg Balance Scale (BBS), Five-Times-Sit-to-Stand Test (FTSST), and the Timed Up and Go Test (TUGT) to measure the subject's dynamic balance ability every two days through the entire session. Results: After participating in the program, the subject's dynamic balance ability improved compared to the first baseline, as measured by BBS (2 points increased), FTSST (5.3 sec decreased), and TUGT (2 sec decreased). The increase was also maintained in the second baseline session. Conclusion: PNF exercise using bilateral scapula patterns with a stabilizing reversal technique helps to enhance the dynamic balance ability of a cerebellar injury patient.

Optimized Immunohistochemical Analysis of Cerebellar Purkinje Cells Using a Specific Biomarker, Calbindin D28k

  • Cerebellar Purkinje cells (PCs) play a crucial role in motor functions and their progressive degeneration is closely associated with spinocerebellar ataxias. Although immunohistochemical (IHC) analysis can provide a valuable tool for understanding the pathophysiology of PC disorders, the method validation of IHC analysis with cerebellar tissue specimens is unclear. Here we present an optimized and validated IHC method using antibodies to calbindin D28k, a specific PC marker in the cerebellum. To achieve the desired sensitivity, specificity, and reproducibility, we modified IHC analysis procedures for cerebellar tissues. We found that the sensitivity of staining varies depending on the commercial source of primary antibody. In addition, we showed that a biotin-free signal amplification method using a horseradish peroxidase polymer-conjugated secondary antibody increases both the sensitivity and specificity of ICH analysis. Furthermore, we demonstrated that dye filtration using a $0.22\;{\mu}m$ filter eliminates or minimizes nonspecific staining while preserving the analytical sensitivity. These results suggest that our protocol can be adapted for future investigations aiming to understand the pathophysiology of cerebellar PC disorders and to evaluate the efficacy of therapeutic strategies for treating' these diseases.
