외과적 정출술을 통한 치은 하방 치경부 파절선이 있는 상악 전치부의 심미 보철 치료 (Esthetic restoration of subgingival crown-root fractured maxillary anterior tooth using surgical extrusion)

  • 치은연 하방에 치아 우식증이나 치아 파절이 발생했을 때, 치아를 탈구시켜 즉시 정출시키는 외과적 정출술을 이용한 치관 연장술은 자연치를 보존하고 추가적인 치주적 수술 없이 단기간에 심미적인 보철 치료를 가능하게 한다. 본 환자는 16세 남환으로 본원 보존과로부터 상악 좌측 중절치의 심미 수복을 위해 본원 보철과로 의뢰되었다. 상악 좌측 중절치는 근관치료가 되어 있었으며 치관-치근 파절로 인해 협측 치관 길이는 4mm였으며 구개측 판막을 열었을 때 근심 구개측 치경부 파절 범위는 치은 하방 3-4mm에 위치하였다. 외과적 정출술을 통한 치관연장술을 시행하였으며 경과 관찰 및 임시 치아의 단계를 거쳐 3개월 후에 전부 도재관으로 수복하여 만족할 만한 임상결과를 얻을 수 있었다.

Effect of the shades of background substructures on the overall color of zirconia-based all-ceramic crowns

  • PURPOSE. The objective of this study was to determine the effect of the color of a background substructure on the overall color of a zirconia-based all-ceramic crown. MATERIALS AND METHODS. Twenty one posterior zirconia crowns were made for twenty subjects. Seven premolar crowns and six molar crowns were cemented onto abutments with metal post and core in the first and second group. In the third group, eight molar crowns were cemented onto abutments with a prefabricated post and composite core build-up. The color measurements of all-ceramic crowns were made before try-in, before and after cementation. A repeated measure ANOVA was used for a statistical analysis of a color change of all-ceramic crowns at ${\alpha}$=.05. Twenty four zirconia specimens, with different core thicknesses (0.4-1 mm) were also prepared to obtain the contrast ratio of zirconia materials after veneering. RESULTS. $L^*$, $a^*$, and $b^*$ values of all-ceramic crowns cemented either on a metal cast post and core or on a prefabricated post did not show significant changes (P>.05). However, the slight color changes of zirconia crowns were detected and represented by ${\Delta}E{^*}_{ab}$ values, ranging from 1.2 to 3.1. The contrast ratios of zirconia specimens were 0.92-0.95 after veneering. CONCLUSION. No significant differences were observed between the $L^*$, $a^*$, and $b^*$ values of zirconia crowns cemented either on a metal cast post and core or a prefabricated post and composite core. However, the color of a background substructure could affect the overall color of posterior zirconia restorations with clinically recommended core thickness according to ${\Delta}E{^*}_{ab}$ values.

Six-year clinical performance of lithium disilicate glass-ceramic CAD-CAM versus metal-ceramic crowns

  • PURPOSE. To assess the clinical performance of monolithic CAD-CAM lithium disilicate glass-ceramic (LDGC) crowns and metal-ceramic (MC) crowns provided by predoctoral students. This study also assessed the effects of patient and provider-related factors on their clinical performance as well as patient preference for these types of crowns. MATERIALS AND METHODS. Twenty-five patients who received 50 crowns (25 LDGC CAD-CAM and 25 MC) provided by predoctoral students were retrospectively examined. LDGC CAD-CAM crowns were milled in-house using the CEREC Bluecam system and cemented with either RelyX Unicem or Calibra Esthetic resin cements. MC crowns were cemented with RelyX Unicem cement. Clinical assessment of the crowns and the supporting periodontal structures were performed following the modified California Dental Association (CDA) criteria. Patients' preference was recorded using a visual analog scale (VAS). The results were statistically analyzed using log-rank test, Pearson Chi-squared test and Kaplan-Meier survival analysis. RESULTS. Twelve complications were observed in the MC crown group (9-esthetic, 2-technical and 1-biological). In comparison, 2 complications in the LDGC CAD-CAM crown group were observed (1-technical and 1-esthetic). The 6-year cumulative survival rates for MC crowns and LDGC CAD-CAM were 90.8% and 96%, respectively, whereas the success rates were 83.4% and 96%, respectively. Overall, patients preferred the esthetic outcomes of LDGC CAD-CAM crowns over MC crowns. CONCLUSION. The high survival and success rates, low number of complications, and the high level of patients' acceptance of monolithic LDGC CAD-CAM crowns lend them well as predictable and viable alternatives to the "gold standard" MC crowns.

In-Ceram Spinell Crown의 제작법과 임상적 응용 (THE CONSTRUCTION METHOD OF IN-CERAM SPINELL CROWN AND ITS CLINICAL APPLICATION)

  • By the concerns of esthetic restoration were increased recently. many all ceramic crowns were developed. But they except In-Ceram Alumina were used only single crown. In-Ceram Alumina, developed by Dr. Sadon, was revealed to have high flexural Strength(450MPa). So it could be used not only anterior bridges but also posterior bridges. But In-Ceram Alumina was seen to be opaque, a little green color in transillumination light by high content of alumina oxide(85%). So new all ceramics with high strength and high translucence were needed. Spinell($MgAl_2O_3$) have a high melting point, high flexural strength, low heat conductivity, high light conductivity. In-Ceram Spinell offers glasslike light transmission by using the spinell cores instead of the alumina cores. And they have a high translucency like to natural tooth, an excellent margin integrity and a high strength(350MPa). The purposes of this study are 1) to know about the construction method of In-Ceram Spinell System, 2) to investigate the its clinical possibiliy through patients and literature reviews.

인공치관용(人工齒冠用) Glass-Ceramics의 결정화(結晶化)에 관(關)한 연구(硏究) (A Study on the Crystallization of Glass-Ceramics for Dental Crown)

  • Glass ceramics for crown were prepared by adding 3$\sim$11 wt% TiO2 to the weight percent composition of 34.7 CaO, 27.8 SiO2, 18.3P2O5, 12.6MgO and 6.6 TiO2. The starting glasses were prepared by melting the powdered batch in alumina crucible at 1350$\sim$1400 for 1 hr and then quenching into a distilled water. The nucleation and crystallization of the crystalline glass ceramics for crown were studied by DTA, SME and X-ray diffraction analysis. Frit containing 9.11 wt% TiO2 had crystallization temperature of 850$\sim$1075 and major crystalline phase was identified by X-ray diffraction as diopside(CaO-MgO-2SiO2). Activation energies for the crystallization processes were obtained from DTA by varing rates for the fits, and were calculated from modified Ozawa and Kissinger equations. Activation energy for the crystallization processes of the S-4 frit was 489.6 KJ/mol.

치과 CAD/CAM용 복합소재를 이용한 치과보철물의 제작에 대한 연구 (A Study on Hybrid material of Making Dental restorations by CAD/CAM System)

  • In recent years, perhaps the biggest driver in new material development is the desire to improve crown and bridge esthetics compared to the traditional PFM or all-metal restorations. As such, zirconia, leucite-containing glass ceramic and lithium disilicate glass ceramic have become prominent in the dental practice. Each material type performs differently regarding strength, toughness, ease of machining and the final preparation of the material prior to placement. For example, glass ceramic are typically weaker materials which limits its use to single-unit restorations. On the other hand, zirconia has a high fracture toughness which enables multi-unit restorations. This material requires a long sintering procedure which excludes its use for fast chair side production. Developed hybrid material of CAD/CAM is contained nano ceramic elements. This new material, called a Resin Nano Ceramic is unique in durability and function. The material is not a resin or composite. It is also not a pure ceramic. The material is a mixture of both and consists of ceramic. Like a composite, the material is not brittle and is fracture resistant. Like a glass ceramic, the material has excellent polish retention for lasting esthetics. The material is easily machined chair side or in a dental lab, polishes quickly to an esthetic finish and if necessary, can be useful restoratives.

Collarless도재소부전장금관의 제작방법에 따른 치경부 변연적합도에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON THE CERVICAL MARGIN FITNESS IN THE COLLARLESS METAL CERAMIC CROWNS FORMED BY DIFFERENT TECHNIQUES)

  • The purpose of this study was to evaluate the cervical margin fitness in the collarless metal ceramic crowns formed by different techniques. Specimens were divided as follows : the metal ceramic crowns with metal butt margin as group I, the collarless metal ceramic crowns formed with resin binder technique as group II, and the collarless metal ceramic crowns formed with shoulder powder mixed with phosphate-bonded investment liquids on a refractory die as group III. Each group was made of five specimens, and their marginal fitness on each epoxy die was evaluated under scanning electron microscope of x200 magnification at three measuring points : mesial, central, distal. The following results were obtained. 1. The metal ceramic crowns with metal butt margin exhibited significantly better marginal fitness than the collarless metal ceramic crowns. The marginal fitness in descending order was group I, III, II. 2. The collarless metal ceramic crown formed with resin binder technique had the worst marginal fitness & showed cervical color variation and dermacation between the corrected porcelain & the dentin porcelain. 3. The collarless metal ceramic crowns formed with shoulder powder mixed with phosphate-bonded investment liquids on a refractory die exhibited significantly better marginal fitness & sharper marginal configuration than the other collarless group.

즉시 Frialit-2 implant 식립, 즉시 임시치관 제작 그리고 IPS Empress 2 crown을 이용한 상악중절치의 심미적 수복 (Esthetic reconstruction of upper central incisor using immediate Frialit-2 implant placement, immediate temporary crown fabrication and IPS Empress 2 crown)

  • During the past several years, significant advances have occurred in the utilization of osseointegrated implants for the treatment of partially edentulous patients. One of the biggest purposes for treating of these patients is the high demand for improved esthetics, especially in the anterior region. For this esthetics, the new trend in dental implants is the immediate placement and immediate superstructure fabrication. The refined surgical technique, the skillful soft tissue management, and the proper prosthetic coordination are the main factors to achieve natural looking of implant supported prosthesis. The customized provisional restoration and the customized impression coping are recommended for the optimal peri-implant soft tissue contour. The basic concept of Frialit 2 system was the immediate replacement of a tooth with root-analog fixture after extraction. This system guarantees an ideal result in function and esthetics. The ceramic abutment system offers improved quality in the respect of esthetics, fitness, translucency, and biocompatibility. In this clinical report, the final restoration made with IPS Empress 2 crown on the CeraBase abutmen of Frialit 2 system allowed the reproduction of the natural vitality of tooth and adjacent gingiva.

Thermo mechanical analysis of a ceramic coated piston used in a diesel engine

  • The aim of this paper is to determine temperature and stress distributions in a ceramic based on Partially Stabilized Zirconia coated steel piston crown by using plasma spraying for improving performance of a marine diesel engine. Effects of coating constituent and thickness on temperature and stress distributions were investigated including comparisons with results from an uncoated piston by means of finite element method namely ANSYS. Temperature developed at the coated surface is significantly higher than that of the uncoated piston. The maximum stress components occur between bond coat and adjacent ceramic layer. Provided that coating thickness is constant as 0.5 mm, when numbers of layers increase, magnitude of the normal stress decrease about 34.1% on the base metal surface according to uncoated piston, but the base metal surface temperature of the steel piston increase about 13.1%.
