A Study on the Nutrient Intake, Health Risk Factors, Blood Health Status in Elderly Korean Women Living Alone (일부지역 저소득층 독거노인의 영양소 섭취, 영양위험도 및 생화학 지표에 관한 조사연구)

  • This purpose of this study was to investigate the nutrient intake, the health status as determined self-assessment checklist, biochemical indicators of elderly Korean women. We interviewed and 55 female subjects living alone in the Urban aged over 65 years. Information on their dietary intake was collected by 24-hour Recall method. Their health status was determined by a NSI checklist. Biochemical indicators were performed in whole blood and plasma of subjects. Except for protein, Fe, all of the elderly subjects belonged to over moderate nutritional risk. The average daily nutrient intake of the elderly was below the level of the recommended dietary allowances (RDA) for Koreans. A relationship between their health risk score and nutrient intake was observed negatively (not significantly). They had a risk of anemia as hemoglobin and hematocrit of subjects were under the normal value. Therefore, the reason that health risk score and health status badly was thought for lower nutrient intake.

  • Being rapidly increased in population of the elderly, the welfare of the elderly is no longer family's and personal problems. It is a social issue that have to be solved by the community and the government. Daejeon city government faces the same social issue and should release the solution to the community in the near future. Based on the new trend of deinstitutionalization and community care for the elderly who live in normal home rather than for those in protection systems, the solution could be released to the elderly. This paper is assessed as the first stage research of normalization of the old people's living. By the comparition analysis of the existing Senior Welfare Center in Daejeon city and Japan, the facility space improvement method was considered. And through the above investigation and comparition analysis, the method that extend and improve the welfare facility space of Senior Welfare Center is proposed.

A Study on Decision Factors Affecting Utilization of Elderly Welfare Center: Focus on Gimpo City (노인복지관 이용 결정요인에 관한 연구: 김포시 노인을 중심으로)

  • The purpose of this study is to learn about the decision factors affecting utilization of elderly welfare center of the elderly living in Gimpo city. The reason of the study is that the elderly welfare center as a provider of general welfare services could not only thinking about the state policy but also need to consider about the inherent role and function of the elderly. Especially for these elders living in rural areas, although the number of elderly welfare centers of the whole country has greatly increased in last 10 years, the effect and function of the facility are almost the same and they are still lack of leisure activities. This issue become a serious problem nowadays. For the above reasons, this article conducts a social survey of 360 elderly people over the age of 65 who lives in the Gimpo city which is a rural-urban type city. The research method is to examine the relationship between the predisposing factors, enabling factors and need factors of Andersen's behavior model with binary logistic regression analysis and the decision tree analysis. The result of binary logistic regression shows the most of factors of Andersen's model is significant. The factors of age, gender, education level in predisposing factors; monthly income in enabling factors and the reserve for old life, the preparation of economic activity for old life in need factors are significant. Then the result of decision tree analysis shows the interaction between factors; when the education level in predisposing factors is higher, the possibility of using of elderly welfare center becomes bigger. Also as the level of healthy promoting preparation in the need factors gets lower, the possibility of using of elderly welfare center still becomes bigger. Although differences were found in the interpretation of the results of regression analysis and decision tree analysis, the results of this study can still provide support for the necessity of elderly welfare centers providing integrated welfare services.

  • The research on the health promotion effect of the elderly through the health care service has been going on for a long time, but there is insufficient research to grasp the needs of elderly people in order to effectively provide health care services. In order to solve these problems, this study suggested the direction of health care service for elderly people by analyzing regional characteristics and demand among rural areas. To this end, the direction of improvement of customized healthcare service model was suggested through the analysis of the health - related program utilization status, health management method, health care service type, and contents demand of the elderly by urban area and rural area.

  • The purpose of this study is to analyze the program according to the local characteristics of the elderly welfare center and to present the development direction of the elderly welfare center in the future. The questionnaire was administered to 344 elderly welfare centers nationwide and 304 questionnaires were used for analysis. Based on the regional characteristics of the elderly welfare center, three types were derived and analyzed. The results of this study are as follows: First, the projects which showed difference according to the regional characteristics of the elderly welfare center were the functional recovery program, counseling, emotional support, and community welfare program. Second, it was found that the elderly welfare centers had different programs according to their areas. The results showed that the elderly welfare centers had different programs, group programs, self-help groups, beauty services, employment support education, economic education, retirement preparation education, home helper service, generation integration program, mobile welfare business and regional cooperation project. Based on these results, it is suggested that the development direction of the elderly welfare center is necessary to develop the program of the elderly welfare center according to the characteristics of the region and it is necessary to guarantee autonomy so that the elderly welfare center can operate the program.

Gait Feature Vectors for Post-stroke Prediction using Wearable Sensor

  • Stroke is a health problem experienced by many elderly people around the world. Stroke has a devastating effect on quality of life, causing death or disability. Hemiplegia is clearly an early sign of a stroke and can be detected through patterns of body balance and gait. The goal of this study was to determine various feature vectors of foot pressure and gait parameters of patients with stroke through the use of a wearable sensor and to compare the gait parameters with those of healthy elderly people. To monitor the participants at all times, we used a simple measuring device rather than a medical device. We measured gait data of 220 healthy people older than 65 years of age and of 63 elderly patients who had experienced stroke less than 6 months earlier. The center of pressure and the acceleration during standing and gait-related tasks were recorded by a wearable insole sensor worn by the participants. Both the average acceleration and the maximum acceleration were significantly higher in the healthy participants (p < .01) than in the patients with stroke. Thus gait parameters are helpful for determining whether they are patients with stroke or normal elderly people.

Comparison of Depression and Cognitive Function between Elderly Welfare Recipients and Non-Elderly Welfare Recipients in a Public Health Center (한 보건기관에 내원한 기초생활수급노인과 비 수급노인의 우울 및 인지기능 상태 비교)

  • Objectives: This study was performed to provide the fundamental data available in the field of the elderly health of the low-income bracket by researching and comparing related factors for the assessment of the degree of depression and cognitive function between elderly welfare recipients and non-elderly welfare recipients. Methods: The study subjects, 402 elderly person over 65-year-old in Daejeon were interviewed, during the two-month from May to June 2006, about their general characteristics, depression and cognitive function. Results: elderly welfare recipients was higher than non-elderly welfare recipients in degree of depression on the other hand, in cognitive function is lower. Also, the depression and cognitive function were related with not only socio-demographic characteristics like the age, the degree of education, the presence of spouse or not and the health status but also health behavior characteristics like the sleeping time, the drinking and the exercise. Conclusions: The project of health promotion and programs that can improve the related factors to the depression and cognitive function for elderly welfare recipients should be developed and practiced.

Food Preference Test of the Korean Elderly Menu Development (노인식 제공을 위한 일부 노인의 음식 기호도 조사)

  • This study was performed to test food preferences of the Korean elderly for Korean elderly menu development. The elderly subjects preferred steamed rice cooked with 1.2 times water than that with 1.5 times water (p < 0.001) , whole rice grain gruel than the gruel with ground grain (p < 0.05) . Green tea powder gruels developed as a snack, there were no significant differences in the preference of the elderly between gruel with different contents of green tea powder (2, 3, $4\%$). The preferences of the salt concentration of the soup were tested by using soybean sprout soup and soybean paste soup. The elderly preferred 0.7 - $0.8\%$ salted bean sprout soup and $7\%$ soybean paste soup. The elderly preferred Kimchi cut in widths of 1 - 2 cm than that in 0.5 cm. The elderly preferred sweeter grape jam ($67\%$ vs. $50\%$ or $37\%$ sugar content) and were not concerned about chewing seeds. This result could be useful to develop menus for the elderly. Analyzing the Operational Differences of Foodservice Center for Homebound elderly by the Presence of the Dietitian (영양사 유무에 따른 재가노인 급식서비스 제공기관의 실태 분석)

  • The purposes of this study was to analyze the operational difference of foodservice center for homebound elderly by the presence of the dietitian. The questionnaire was developed to measure all variables for menu management and distributed to 103 meal service centers in charge of congregate meal service program and 57 centers for home-delivered meal service program. The data of 160 centers in charge of congregate meal service and home-delivered service centers were usable for analysis. Statistical data analysis was completed using the SAS 8.1 package program for descriptive analysis and chi-square test. Only 21.9% meal service centers had dietitians, what is more, they were not professionals who did menu management but foodservice managers, volunteers, cook or social workers. The current foodservice programs for the homebound elderly were operated without professional. In the part of menu managemet, dietitians were more actively involved in menu planning in the elderly foodservice center in the presence of the dietitians. The performance level of healthcare service was not significantly different, but the nutrition education in the elderly foodservice center with the dietitians was more frequently performed than that without the dietitians(p<0.05). In the food purchasing and food production management, the significant differences were shown that in the elderly foodservice centers in the presence of the dietitians, the proportion of the contract purchasing was significantly higher than that of direct purchasing(p<0.01). In food sanitary management, the significant differences were not shown in the part of management of keeping meal for identifying the cause of food-borne illness and left-over, but the sanitation education for the foodservice employees was performed more frequently by the presence of the dietitians(p<0.01). In conclusion, the foodservice management was more systematically conducted in the elderly foodservice centers in the presence of the dietitians than that without dietitians. The elderly foodservice program has offered the health-related support for homebound elderly. Although there were several problems in elderly foodservice management, the program delivered well-targeted, effective, and efficient nutrition services and wide range of supportive service to the at-risk older population. It needs to be managed by professional for the improvement in the elderly foodservice.

A Qualitative Case Study of the Narrative therapy group work on the elderly with palsy to solve conflict with the elderly with dementia (치매노인과의 관계개선을 위한 중풍노인 이야기치료 집단상담 사례 연구)

  • The purpose of this study is to explore the process for the elderly with palsy to solve conflicts with the elderly with dementia in day care center through the narrative therapy group work. For this, the researcher enacted the Narrative therapy group work on the 12 elderly with palsy for seven sessions, and analysed these data through qualitative case study method. The results of this study are as follows. First, the elderly with palsy became to recognize the elderly with dementia as 'the people suffering from dementia' rather than 'the problem', and tried to care them rather than to blame. Second, they became to recognize themselves as 'component people' rather than 'the victim'. Third, they grew intimate and collaborated with themselves. Fourth, they participated actively in day care center. It was discussed connection between these changes and narrative therapy skills, such as to separate problem from people, to respect client as expert for one's problem, to seek unique outcome. The recommendation is suggested that it is important to enlarge choice to participate for long-term care service consumer and to apply narrative therapy on the elderly.
