PURPOSE. The aim of this study was to observe stress concentration in the implant, the surrounding bone, and other components under the pull-out force during the crown removal. MATERIALS AND METHODS. Two 3-dimensional models of implant-supported conventional metal ceramic crowns were digitally constructed. One model was designed as a vertically placed implant ($3.7mm{\times}10mm$) with a straight abutment, and the other model was designed as a 30-degree inclined implant ($3.7mm{\times}10mm$) with an angled abutment. A pull-out force of 40 N was applied to the crown. The stress values were calculated within the dental implant, the abutment, the abutment screw, and the surrounding bone. RESULTS. The highest stress concentration was observed at the coronal portion of the straight implant (9.29 MPa). The stress concentrations at the cortical bone were lower than at the implants, and maximum stress concentration in bone structure was 1.73 MPa. At the abutment screws, the stress concentration levels were similiar (3.09 MPa and 3.44 MPa), but the localizations were different. The stress at the angled abutment was higher than the stress at the straight abutment. CONCLUSION. The pull-out force, applied during a crown removal, did not show an evident effect in bone structure. The higher stress concentrations were mostly observed at the implant and the abutment collar. In addition, the abutment screw, which is the weakest part of an implant system, also showed stress concentrations. Implant angulation affected the stress concentration levels and localizations. CLINICAL IMPLICATIONS. These results will help clinicians understand the mechanical behavior of cement-retained implant-supported crowns during crown retrieval.
상악 완전 무치악에 6개 이상의 임플란트가 식립된 경우 임플란트-지지형 피개의치 제작이 가능하다. 임플란트-지지 피개의치에 사용되는 다양한 부착장치 중에 milled-bar의 경우 의치의 회전을 허용하지 않고, 안정성이 뛰어나며, 여러 개의 임플란트를 연결 고정하는 효과를 가진다는 장점이 있다. 본 증례에서는 전치부에 심한 골소실을 동반한 상악 완전 무치악 환자에서 세 가지 다른 시스템으로 구성된 7개의 임플란트 상부에 CAD/CAM 맞춤형 지대주와 시멘트 접착형의 milled bar를 이용한 피개의치를 제작하여 상실된 심미와 기능을 효과적으로 회복하였다.
Objectives: This study evaluated the solubility, dimensional alteration, pH, electrical conductivity, and radiopacity of root perforation sealer materials. Materials and Methods: For the pH test, the samples were immersed in distilled water for different periods of time. Then, the samples were retained in plastic recipients, and the electrical conductivity of the solution was measured. The solubility, dimensional alteration, and radiopacity properties were evaluated according to Specification No. 57 of the American National Standards Institute/American Dental Association (ANSI/ADA). Statistical analyses were carried out using analysis of variance (ANOVA) and Tukey's test at a significance level of 5%. When the sample distribution was not normal, a nonparametric ANOVA was performed with a Kruskal-Wallis test (${\alpha}$ = 0.05). Results: The results showed that white structural Portland cement (PC) had the highest solubility, while mineral trioxide aggregate (MTA)-based cements, ProRoot MTA (Dentsply-Tulsa Dental) and MTA BIO ($\hat{A}$ngelus Ind. Prod.), had the lowest values. MTA BIO showed the lowest dimensional alteration values and white PC presented the highest values. No differences among the tested materials were observed in the the pH and electrical conductivity analyses. Only the MTA-based cements met the ANSI/ADA recommendations regarding radiopacity, overcoming the three steps of the aluminum step wedge. Conclusions: On the basis of these results, we concluded that the values of solubility and dimensional alteration of the materials were in accordance with the ANSI/ADA specifications. PCs did not fulfill the ANSI/ADA requirements regarding radiopacity. No differences were observed among the materials with respect to the pH and electrical conductivity analyses.

PURPOSE. This study evaluated the influence of abutment materials on the stability of the implant-abutment joint in internal conical connection type implant systems. MATERIALS AND METHODS. Internal conical connection type implants, cement-retained abutments, and tungsten carbide-coated abutment screws were used. The abutments were fabricated with commercially pure grade 3 titanium (group T3), commercially pure grade 4 titanium (group T4), or Ti-6Al-4V (group TA) (n=5, each). In order to assess the amount of settlement after abutment fixation, a 30-Ncm tightening torque was applied, then the change in length before and after tightening the abutment screw was measured, and the preload exerted was recorded. The compressive bending strength was measured under the ISO14801 conditions. In order to determine whether there were significant changes in settlement, preload, and compressive bending strength before and after abutment fixation depending on abutment materials, one-way ANOVA and Tukey's HSD post-hoc test was performed. RESULTS. Group TA exhibited the smallest mean change in the combined length of the implant and abutment before and after fixation, and no difference was observed between groups T3 and T4 (P>.05). Group TA exhibited the highest preload and compressive bending strength values, followed by T4, then T3 (P<.001). CONCLUSION. The abutment material can influence the stability of the interface in internal conical connection type implant systems. The strength of the abutment material was inversely correlated with settlement, and positively correlated with compressive bending strength. Preload was inversely proportional to the frictional coefficient of the abutment material.

Purpose: The aim of this study was to measure the peri-implant bone length surrounding implants that penetrate the sinus membrane at the posterior maxilla and to evaluate the survival rate of these implants. Methods: Treatment records and orthopantomographs of 39 patients were reviewed and analyzed. The patients had partial edentulism at the posterior maxilla and limited vertical bone height below the maxillary sinus. Implants were inserted into the posterior maxilla, penetrating the sinus membrane. Four months after implant insertion, provisional resin restorations were temporarily cemented to the abutments and used for one month. Then, a final impression was taken at the abutment level, and final cement-retained restorations were delivered with mutually protected occlusion. The complications from the implant surgery were examined, the number of failed implants was counted, and the survival rate was calculated. The periimplant bone lengths were measured using radiographs. The changes in initial and final peri-implant bone lengths were statistically analyzed. Results: Nasal bleeding occurred after implant surgery in three patients. No other complications were found. There were no failures of the investigated implants, resulting in a survival rate of 100%. Significantly more bone gain around the implants (estimated difference=-0.6 mm, P=0.025) occurred when the initial residual bone height was less than 5 mm compared to the >5 mm groups. No significant change in peri-implant bone length was detected when the initial residual bone height was 5 mm or larger. Conclusions: This study suggests that implants penetrating the sinus membrane at the posterior maxilla in patients with limited vertical bone height may be safe and functional.

Purpose: This study aimed to compare the long-term survival rate and peri-implant marginal bone loss between different types of dental implant-abutment connections. Methods: Implants with external or internal abutment connections, which were fitted at Gangneung-Wonju National University Dental Hospital from November 2011 to December 2015 and followed up for >5 years, were retrospectively investigated. Cumulative survival rates were evaluated for >5 years, and peri-implant marginal bone loss was evaluated at 1- and 5-year follow-up examinations after functional loading. Results: The 8-year cumulative survival rates were 93.3% and 90.7% in the external and internal connection types, respectively (P=0.353). The mean values of marginal bone loss were 1.23 mm (external) and 0.72 mm (internal) (P<0.001) after 1 year of loading, and 1.20 mm and 1.00 mm for external and internal abutment connections, respectively (P=0.137) after 5 years. Implant length (longer, P=0.018), smoking status (heavy, P=0.001), and prosthetic type (bridge, P=0.004) were associated with significantly greater marginal bone loss, and the use of screw-cement-retained prosthesis was significantly associated (P=0.027) with less marginal bone loss. Conclusions: There was no significant difference in the cumulative survival rate between implants with external and internal abutment connections. After 1 year of loading, marginal bone loss was greater around the implants with an external abutment connection. However, no significant difference between the external and internal connection groups was found after 5 years. Both types of abutment connections are viable treatment options for the reconstruction of partially edentulous ridges.
시멘트 유지 임플란트 지지 수복물은 치과 치료에 일상적으로 사용된다. 임플란트와 지대주 나사연결의 안정적인 견고함과 골유합 임플란트의 높은 생존율로 인해 높은 강도를 가진 시멘트의 사용이 점차 늘어나고 있다. 그러나 CAD/CAM을 이용하여 제작한 금속 코핑의 유지력에 대해 이용할 수 있는 임상 자료가 없다. 네 종류의 시멘트를 이용하여 티타늄 지대주와 CAD/CAM을 이용하여 제작한 금속 코핑의 유지력을 평가하는 것과 시멘트 유지력에 대한 sandblasting의 효과를 알아보고자 함이다. 40개의 티타늄 지대주 블록(Innovium, 세라젬바이오시스, 대한민국)을 제작하여 네 그룹으로 나누었다. 40개의 금속 코핑을 CAD/CAM을 이용하여 제작하고 금속 코핑의 교합면 상방부에 유지력 테스트를 위해 구멍을 형성하였다. 네 종류의 시멘트는 Fujicem(Fuji, Japan), Maxcem Elite(Kerr, USA), Panavia F2.0(Kurarary, Japan), Superbond C&B(Sunmedical, Japan)이다. 금속 코핑과 티타늄 지대주를 제조사의 지시대로 시멘트 혼합하여 합착하고 100% 습도 하에 37도에서 24시간 동안 보관 후 유지력 테스트하였다. 유지 실패가 발생하는 힘을 newton으로 기록하고 실패의 양상 또한 기록하였다. 유지력의 평균과 표준 편차를 ANOVA와 Paired t-test 로 통계분석하였다. Panavia F2.0이 Fujicem과 Maxcem Elite보다 통계적으로 유의하게 높은 유지력을 보였다(p<0.05). Sandblasting이 모든 시멘트에서 유의하게 유지력을 증가시켰다(p<0.05). 유지력 실패의 양상은 대부분의 시멘트가 금속 코핑 내부에 남아있는 접착성 실패였다. 본 연구의 한계 내에서 Panavia F2.0이 Fujicem이나 Maxcem Elite보다 통계적으로 유의하게 큰 유지력을 보였고(p<0.05), Fujicem과 Maxcem Elite는 유지력의 차이를 보이지 않았다. Sandblasting처리는 모든 실험군에서 유지력의 향상을 보였다. 따라서 임플란트 지대주에 합착된 금속 코핑의 유지력은 표면 거칠기와 시멘트의 종류에 영향을 받는다.
목적: 슬관절 주위에서 탄력성 있는 골수강 내고정물과 회전경첩 슬관절 전치환물을 이용한 재건술이 종양인공관절과 자가골 경계부위의 골 흡수를 억제할 수 있는지를 35례에서 분석하였다. 대상 및 방법: 재건 술식은 다음과 같다. 1) 종양 절제, 2) 근위부나 원위부의 골수강에 다수의 Ender 정 삽입, 3) Ender 정과 Endo-Link 형 슬관절 전치환물을 강선 과 골 시멘트를 이용하여 결합 결과: 평균 추시기간은 53개월(30~79개월) 이었다. 최종 추시 상 29명의 환자가 가동관절을 유지하였다. 장관골 길이의 40% 이상을 절제한 군에서 종양인공관절과 자가골 경계부위 골질의 비후가 나타났다(p=0.028). 내고정 금속정의 파괴가 8례 있었으며, 조기 및 지연 감염으로 8례의 내고정물을 제거하였다. 본 재건술의 생존율은 6년에 33%였다. Musculoskeletal Tumor Society System 평가 법에 따른 기능적 결과는 26.8(89.3%)였다. 결론: 본 술식의 단-중기적 결과는 양호하였으며 합병증 발생시 재수술이 매우 용이하였다. 본 술식에서 관찰된 종양인공관절-자가골 이행 부위의 골 비후 소견은 추후 더 연구해 볼 문제로 생각된다.
연구 목적: 내측연결형 임플란트와 지대주의 연결체에 반복하중을 부여하였을 때 수직 침하를 평가하고자 하였다. 연구 재료 및 방법: 외측연결형 임플란트와 내측연결형 임플란트에 세 종류의 시멘트유지형 지대주를 각각 장착하였다. 즉, 외측연결형 지대주(Ext 그룹), 내측연결형 1-piece 지대주(Int-1 그룹), 내측연결형 2-piece 지대주(Int-2 그룹)를 사용하였으며, 각 그룹마다 7개의 시편을 준비하였다. 임플란트-지대주 연결체에 수직하중을 적용하기 위하여 임플란트 받침대에 고정한 후, 4 Hz의 빈도로 $150{\pm}10N$의 반복하중을 가하였다. 수직침하량은 0, 5, 10, 50, 100, 1,000, 5,000, 10,000회의 반복하중 후에 각각 측정하였다. 반복측정분산분석(RM-ANOVA)를 이용하여 반복하중의 영향을 분석하였으며, 패턴변화를 관찰하기 위하여 선형혼합모형(linear mixed model)을 사용하였다. 유의수준은5% 로 설정하였다. 결과: 10,000회 반복하중 후 수직침하량은, Ext 그룹에서 $0.714{\pm}0.488{\mu}m$, Int-1그룹에서 $5.286{\pm}1.604{\mu}m$, Int-2 그룹에서 $11.429{\pm}1.902{\mu}m$를 나타내었다. 패턴분석에서는, Int-1 그룹 및 Int-2 그룹에서 지속적인 수직침하가 관찰되었으며, Ext그룹에서는 수직침하현상이 관찰되지 않았다. 결론:10,000회 반복하중 후의 선형혼합모형을 통한 분석에서, Ext그룹은 수직침하현상을 보이지 않았으나, Int-1 및 Int-2 그룹은 지속적인 수직침하현상을 나타내었다. 또한, Int-2그룹에서 Int-1그룹보다 더 많은 수직침하량이 관찰되었다.
목적: 임플란트 지지 서베이드 고정성 보철물을 이용한 가철성 국소의치의 합병증을 임상적으로 조사하고, 임플란트 식립위치, 보철물 연결고정여부, 국소의치의 유지장치 종류, 케네디 분류, 대합치에 따른 연관 관계를 조사하고자 하였다. 대상 및 방법: 2000년부터 2016년까지 임플란트 고정성 서베이드 보철물을 이용한 가철성 국소의치로 수복을 받은 환자 11명(남자 7명, 여자 4명)을 대상으로 하였다. 평균 연령은 67.5세였으며, 상악 7개, 하악 4개의 국소의치가 제작되었다. 장착된 국소의치 11개와 이를 지지하는 37개의 임플란트 고정성 보철물의 기계적 합병증, 잔존 자연치아 상태, 주변 연조직 상태 등 모든 합병증과 그와 관련된 요소를 조사하였다. 결과: 장착된 국소의치 11개를 지지하는 총 임플란트는 37개로 국소의치마다 평균 3.4 개의 임플란트 지지 서베이드 고정성 보철물이 이용되었다. 평균 41.5 개월의 추시 기간 동안 발생한 합병증은 임시시멘트 접착 탈락, 대합치 파절/동요, 나사 파절, 나사 풀림, 클래스프 헐거움, 상부 도재파절, 임플란트 주위 골 흡수/동요, 국소의치 인공치아 파절 순으로 발생하였다. 이러한 합병증은 구치부보다 전치부에서, 연결한 보철물 보다 연결하지 않은 보철물에서, 가공선 클래스프를 유지장치로 이용한 보철물에서, 대합치가 의치를 포함하는 보철물보다 자연치열인 경우 합병증 발생이 좀 더 높은 빈도로 나타난 것을 확인 할 수 있었다. 케네디 분류에 따른 합병증 발생 빈도 차이는 유의한 차이를 확인할 수 없었다. 결론: 임플란트 융합 국소의치는 장기간 사용되고 있고 충분히 효과적으로 기능하고 있었다. 하지만 아직까지 이러한 치료의 예후에 대한 축적된 임상 결과는 많이 부족한 실정이므로 좀 더 장기적인 임상 연구가 필요한 상황이다.

회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
