A Cause Analysis of the Construction Incident Using Causal Loop Diagram : Safety Culture Perspective (인과지도를 활용한 건설 안전사고 원인 분석 : 안전문화 관점)

  • Unlike research focused on existing technologies and individual errors to analyze the causes of incidents, this study approached them from an organization and culture. And this study is not a one way study but cyclical study what can track cause down using causal loop diagram methodology. Four diagnostic criteria for the negative state of the safety culture : secretive, blame, failure to learning, and incremental learning, combine literature study and expert opinion to derive 41 variables. Connecting these variable make 4 causal loop diagrams and total causal loop diagram. Case accumulation in secretive, accident report in blame, knowledge accumulation in failure to learning, near miss discovery in incremental learning are the main variables. Safety incident is the objective variable by classifying them into 4 stages in total loop, leading track as the most affect is case accumulation, and Step 4 as you can see accident report and near miss discovery are the result of tracking down the cause. This study can be used as a basis for improving the management priority and the system in incident prevention.

  • This paper analyzes the pitting's cause of elevator's worm gear type traction machine. To find a cause of pitting problem, we analyzed vibration, a proper design allowance and lubrication. We brought a conclusion that the cause of pitting is not a simple vibration problem, such as misalignment of worm reducer and rail, but mostly related to a designed allowance. In this case, the allowance is tight. In general, the allowance of traction machine and lubrication is varied by manufacturers. When the allowance is tight, a proper lubrication can diminish the pitting problem.

  • The relationship between R&D Polices(R&D subsidy, tax incentives) and firm R&D expenditures is analyzed through firm's profit maximization function. As a result, the relationship between R&D policies and fmn R&D expenditures is determined by the relationship between firm R&D expenditures and market price. In case of major innovation which cause the fall of market price, the elasticity R&D subsidy and firm R&D expenditures is negative(substitution). In case of minor innovation which cause the rise of market price, the elasticity R&D subsidy and firm R&D expenditures is positive(complement). Tax incentives is bring about the increase of firm R&D expenditures. R&D subsidy and tax incentives are substitutively influenced at firm R&D expenditures.

  • This paper describes a case study on the failure cause analysis of power cables and their fittings. For more accurate analysis, electrical and chemical analysis techniques were employed from the macrography stage for the exterior of the cables to the entire disassembly stage. As a result it was confirmed that the power cables failed due to improper fabrication by which electric field was concentrated at the deadend of a shrink tube on the cable.

  • Subclavian artery aneurysms are quite rare and they are known to be as only 1% of all peripheral aneurysms. Atherosclerotic disease is the most common cause of subclavian artery aneurysms. These aneurysms can cause rupture, thrombosis, embolisms or symptoms by local compression. Surgical operations are generally performed as treatment. We report here on a case of successful surgical treatment for a right proximal subcalvian artery aneurysm, and the surgery was peformed via the medical exclusion method.

  • Noroviruses have been recognized as the leading cause of epidemic and sporadic gastroenteritis since the advent of molecular diagnostic technique. They have been documented in 5-31% of pediatric patients hospitalized with gastroenteritis. Although norovirus gastroenteritis is typically mild and self-limited, it causes severe, but sometimes fatal, conditions in the vulnerable population such as immunocompromised patients, young children, and the elderly. Bowel perforation due to norovirus infection is rare. We report a case of small bowel perforation with norovirus gastroenteritis in the infant with Down syndrome during the hospitalization with pneumonia. Severe dehydration may cause bowel ischemia and could have triggered bowel perforation in this case. Physicians should be alert to the potential surgical complications followed by severe acute diarrhea, especially in high risk groups.

  • Laryngeal saccular cyst may develop when obstruction of the orifice of the saccule occurs with resultant dilatation of saccule. It is classified into anterior saccular cyst and lateral saccular cyst. When saccular cyst occurs in infancy, it usually may cause respiratory distress, inspiratory stridor, cyanosis and dysphagia. In adults, it may cause hoarseness, dyspnea, dysphagia, pain and neck mass. The laryngeal saccular cyst may be treated by its removal with endoscopic or external approach, depending on the classification of saccular cysts, its size and individual patient factor. Recently, we have experienced a case of laryngeal saccular cyst so we report our clinical experience with brief review of literature.

A Case of Kimura's disease (경부에 발생한 기무라씨병 1례)

  • Kimura's disease is a chronic inflammatory disease which often presents as a tumor like swelling in the head and neck region. This lesion is benign but it may easily be mistaken for a malignant tumor. Kimura's disease has been confused with angiolymphoid hyperplasia with eosinophilia(ALHE). The cause is unknown, but theories include autoimmune, allergic, neoplastic, and infectious cause by insect bites and parasites. The treatment of choice is surgical removal. Recently we experienced a case of Kimura's disease occuring in the both side of posterior neck triangle area of a 37 year old male patient who was treated with surgical excision and systemic corticosteroid administration.

  • Glomus tumors are small vascular tumors that are usually benign and rarely occur. They originate from glomus bodies and present in the reticular dermis. They are clinically distinguished by their small size and their ability to cause extreme pain. Most of these tumors are subungually located. However, atypical locations of the tumors sometimes cause misdiagnosis, particularly when the lesion is rarely reported. Therefore, we report a case of glomus tumor which presented with chronic abdominal pain, found in the abdominal wall that has never been reported before.

Severe Hemolysis after St. Jude Medical Valve Replacement in the Aortic Position -A Redo Case Report - (판막치환술 후 심한 용혈 현상으로 재치환한 경험)

  • Intravascular hemolysis occurs in the majority of patient with mechanical valve prosthesis. The primary cause is mechanical trauma to red cells from turbulent blood flow through the prosthesis. Degree of hemolysis is dependent upon the type, size and material of valve and aggravated by paravalvular leakage. Clinically important hemolytic anemia is required medical management or consideration of reoperation. In severe hemolysis, reoperation is recommended without delay when seems to be renal failure. In this case, postoperative severe mechanical hemolysis was developed immediately after aortic valve replacement with St. Jude medical valve in a 13 year-old male patient. Neither significant paravalvular leakage nor valvular dysfunction was found through redo, but the mechanical valve was strongly suspected the cause of severe hemolysis. The St. Jude Medical valve was changed with Ionescu-Shiley bioprosthesis and any significant clinical problems were not noted through the postoperative course.
