인천지역 도축돈에서 돼지호흡기질병복합감염증의 육안적 폐병변과 원인체에 관한 연구 (Study on gross finding of lung lesions and causative pathogens of porcine respiratory disease complex from slaughtered pigs in Incheon)

  • The purpose of this study was to investigate association with gross lesions and causative pathogens of porcine respiratory disease complex (PRDC) including porcine circovirus type 2 (PCV2), porcine reproductive and respiratory syndrome virus (PRRSV), swine influenza virus (SIV), Mycoplasma hyopneumoniae (MH), Pasteurella multocida (PM), Actinobacillus pleuropneumoniae (APP), Haemophilus parasuis (HP) in slaughtered pigs. A total of 1,200 lung samples were collected randomly from slaughtered pigs in Korea during August of 2010 through July of 2011. The gross lesions were classified according to the six stages (0, 1~10, 11~20, 21~30, 31~40 and ${\geq}41$, unit=%) and 48 samples from each stage were selected to detect viral and bacterial pathogens. The results according to the six stages were 100 (8.3%), 259 (21.6%), 326 (27.2%), 213 (17.8%), 144 (12.0%) and 158 (13.2%) cases, respectively. Prevalence of pneumonia according to season was 87.0~96.7% and the highest prevalence was in spring. In detection of pathogens by PCR, 53 samples were not detected any causative pathogens of PRDC. PCV2, PRRSV, SIV, MH, PM, APP serotype 2, APP serotype 5 and HP were positive in 45.5%, 12.5%, 10.4%, 60.1%, 1.7%, 13.9%, 12.2% and 15.6%, respectively. In co-infection, PCV2-MH was the most detected causative pathogens of PRDC. The detection rate of PCV2 and PRRSV was the highest in spring, of SIV, MH and HP was in winter. The detection rate of APP-2 and APP-5 had no seasonal prevalence. The more severe gross lesions increased, the higher the detection rate showed.

급성 중이염의 원인 병원균과 Cefprozil 치료의 임상적 평가 (Causative Pathogens and Therapeutic Assessment of Cefprozil in Acute Otitis Media)

  • 목 적 : 국내에서 급성 중이염의 원인 규명에 관한 전향적 연구는 거의 없고, 합의된 진단 기준에 의한 임상 연구 또한 매우 미흡하다. 이에 저자들은 2000년 미국 CDC에서 합의된 급성 중이염의 임상적 진단 기준에 따른 급성 중이염 환아를 대상으로 원인 병원균의 규명과, 2세대 세팔로스포린계 항생제인 cefprozil을 투여하여 임상적 반응을 평가하고자 본 연구를 실시하게 되었다. 방 법 : 2001년 6월부터 2002년 6월까지 가톨릭대학교 성모자애병원과 성빈센트병원, 인하대학교 부속병원, 중앙대학교 성심병원의 응급실 및 소아과 또는 이비인후과 외래를 방문한 생후 6개월 이상 15세 이하 연령의 급성 중이염 환아 33명에게서 고막천자를 실시하여 원인균 규명을 시행하고, 1일 30 mg/kg 용량의 cefprozil을 10-12일간 투여하여 완치, 개선, 실패의 임상적 반응 기준으로 평가하였다. 동시에 분리된 균주에 대한 항균력 감수성 검사를 cefaclor, cefixim을 포함하여 실시하였다. 결 과 : 19명의 환아에서 원인 병원성 세균으로 S. pneumoniae(10례), H. influenzae(5례), S. aureus(2례), M. catarrhalis(1례), group A Streptococccus(1례)가 분리되었고, 14명에서는 세균이 분리되지 않았다. 원인 병원균이 분리된 환아에서 고막천공이 유의하게 많았고, 균이 분리되지 않은 환아에서 이통이 유의하게 많았다. 또한 병원균이 분리된 환아의 연령이 2세 미만이 대부분인 반면, 균분리가 되지 않은 환아들의 연령은 주로 2세 이상이었다. Cefrpozil의 임상반응 평가에서 병원균이 분리된 환아의 84.2%가 호전되었고, 병원균이 분리되지 않은 환아의 85.7%가 호전되었다. Cefprozil에 중등도 내성을 보인 S. pneumoniae가 분리된 환아의 경우에 임상적 호전이 있었던 반면 고도 내성을 보인 S. pneumoniae가 분리된 환아의 경우에는 치료실패를 보였다. Cefaclor, cefixime의 항균력 감수성 결과 분리된 S. pneumoniae에 1례만 감수성이 있고 모두 고도 내성을 보여 cefprozil의 경우와 뚜렷한 차이를 보였다. 결 론 : 새로운 급성 중이염 진단 기준에 의한 대상 환아 연구에서 급성 중이염의 원인이 60% 정도가 세균성 감염임을 확인하였고, 이 병원균 중 S. pneumoniae가 제일 주된 원인균 임을 확인하였다. 세균 감염에 의한 환아와 세균이 분리되지 않은 환아에서 임상적으로 세균성인 환아에서 중이 내 천공이 많았고, 무균성인 경우에는 이통 호소가 많았으며, 세균성인 경우의 환아 연령이 주로 2세 미만인 반면 무균성인 경우는 대부분 2세 이상인 차이점이 있었다. 급성 중이염 환아에 대한 cefprozil의 감수성 및 임상 평가, 이상반응에 대한 연구 결과는 외국의 경우와 유사함을 알 수 있었고, 중등도 내성 S. pneumoniae에는 좋은 반응을 보였으나 고도 내성 S. pneumoniae는 치료되지 않음을 확인하였다.

ESKAPE Pathogens in Oral and Maxillofacial Infections

  • Purpose: Most cases of oral and maxillofacial infections are usually easily treated by proper diagnosis, elimination of causative factors, and antibiotic therapy. However, the emergence and the increase of multidrug-resistant bacteria make treatment challenging. "ESKAPE" pathogens are the most common opportunistic organisms in nosocomial infections and have resistant to commonly used antibiotics. There are many medical reviews of ESKAPE pathogens, but few in dentistry. This study focuses on oral and maxillofacial infection especially with ESKAPE pathogens. The purpose of this study is to prepare feasible data about tracing and treatment of infection related to pathogens that may be beneficial to clinicians. Methods: A total of 154 patients with oral and maxillofacial infections were reviewed by analyzing retrospectively hospitalized data in the Department of Oral and Maxillofacial surgery, Chosun University Hospital, Korea, past 5 years from January 2014 to December 2018. Based on the medical records and microbiological tests, the results were divided into two groups: infections with ESKAPE pathogens and other bacteria. Results: A total of 22 species were isolated from 154 patients. The proportion of ESKAPE pathogens among all bacterial isolates collected from infected patients was 39.6%. Causative factors, especially in post-operative infection, showed a statistically significant correlation to ESKAPE infections (29 cases). And average of treatment period in ESKAPE group was longer than non-ESKAPE groups. Overall, Klebsiella pneumoniae (60.7%) was the most frequently isolated ESKAPE pathogen. And high antibiotic resistance rates had been detected in the ESKAPE during the five-year period. Conclusions: Infections with ESKAPE pathogens are now a problem that can no longer be overlooked in Dentistry. Based on results of this study, ESKAPE pathogens were highly associated with post-operative or opportunistic infections. Clinicians should be careful about these antibiotic resistant pathogens and use appropriate antibiotics to patients while having dental treatments.

Early-onset sepsis in a neonatal intensive care unit in Beni Suef, Egypt: bacterial isolates and antibiotic resistance pattern

  • Purpose: To identify the frequency of bacterial isolates in early-onset neonatal sepsis (EONS) and their antimicrobial resistance pattern. Methods: A retrospective study of EONS was conducted at the Beni Suef University Hospital from September 2008 to September 2012. A case of EONS was defined as an infant who had clinical signs of infection or who was born to a mother with risk factors for infection, and in whom blood culture obtained within 72 hours of life grew a bacterial pathogen. Results: Of 673 neonates screened, there were 138 positive blood cultures (20.5%) (confirmed EONS). Of the recovered isolates, 86.2% were gram-negative pathogens. Klebsiella pneumoniae (42.8%), Enterobacter cloacae (22.5%), and Escherichia coli (13.8%) were the commonest isolated organisms. The most common gram-positive microorganism was Staphylococcus aureus accounting for only 12 isolates (8.7%). All Klebsiella isolates and 93% of Enterobacter isolates were resistant to ampicillin. Gram-negative pathogens had the maximum overall sensitivity to imipenem, cefepime, and ciprofloxacin; whereas, gram-positive isolates were most susceptible to vancomycin, imipenem, and piperacillin. Conclusion: K. pneumoniae was the predominant causative bacteria of EONS followed by E. cloacae and E. coli. There was a high resistance to ampicillin. Imipenem had the maximum overall activity against the causative bacteria. Continuous surveillance is needed to monitor the changing epidemiology of pathogens and antibiotic sensitivity.

괴사성 근막염 환자의 원인 균주에 따른 경과 비교 (Analysis of Necrotizing Fasciitis Patient by Causative Pathogens)

  • Purpose: Necrotizing fasciitis is a rapid progressive, life-threatening disease. Many organisms have been identified for causative pathogens of necrotizing fasciitis. And necrotizing fasciitis caused by Vibrio vulnificus is a matter of grace concern in Southwest seashore of Korea recently. But concrete analysis between these pathogens was not executed yet. Methods: Sixty necrotizing fasciitis patients were included in this study. We divided the patients into 2 groups: Group A was the case which Vibrio vulnificus was cultured for causative pathogen of necrotizing fasciitis, and Group B was the case of other organisms. And we analysed each group for treatment, progression and prognosis. Results: There was no significant difference between two groups in total hospital stay but there was a great difference in ICU stay and progression to septic shock. Also, there was a great difference in mortality within 48 hours but there was no difference in mortality of 48 hours after hospital visit. Conclusion: This indicates that intial management of necrotizing fasciitis cased by Vibrio vulnificus is the key of treatment. So initial medical management with early surgical intervention is necessary for necrotizing fasciitis.

In vitro Antimicrobial Effects of Silver Nanoparticles on Microorganisms Isolated from Dog with Otitis External

  • Silver nanoparticles have marked antimicrobial effects on several pathogens and have been used to control bacterial growth in humans. In the present study, we evaluated the antimicrobial efficacy of silver nanoparticles against the common causative pathogens of canine otitis external through counting of colony forming units. Silver nanoparticles showed significant dose-dependent antimicrobial effects on pathogens. In addition, we conducted antimicrobial susceptibility tests and compared the antimicrobial efficacy of silver nanoparticles. Microorganisms with a high resistance to antibiotics were also resistant silver nanoparticle with low concentration ($5{\mu}g/mL$). However, in high concentration ($15{\mu}g/mL$), almost 100% reduction in the number of CFUs of these pathogens was observed.

  • Bovine mastitis was examined by use of the rolling ball viscosity test in Kwang-ju area. Antibiotic sus-ceptibility of isolated pathogens from raw milk was investigated by the disc method. Positive rate of so-matic cell counts (over 500,000 cells/ml) was decreased from 11.1% in 1991∼1993 to 8.0-% in 1995∼1997. Isolated mastitis-causing-pathogens were identified as Staphylococcus sp. (49.7%) Escherichia coki(23.2%) Streptococcus sp. (10.4%) and Bacillus sp(7.2%) Antibiotic susceptibility of isolated pathogens to almost antibiotics was decreased.

Change Pattern of Species and Antimicrobial Susceptibility of Microorganisms Isolated from Blood Culture during 5 Years: 2008-2012

  • To provide reference data or guideline for empirical treatment of bloodstream infection, we studied a change pattern in causative microorganisms and antimicrobial susceptibility in a general hospital at Gyeonggi province during five years. We retrospectively reviewed the frequency of causative microorganisms and antimicrobial susceptibility results of 5,782 microorganisms isolated from blood culture in a general hospital during the period from January 2008 to December 2012. The most common pathogens were Escherichia coli (14.7%), Staphylococcus aureus (7.5%), Streptococcus viridans group (4.9%), and Klebsiella pneumoniae (4.1%). The multiple microorganisms were isolated in 4.3% of bloodstream infection patients. The average contamination rate of blood culture during five years was 3.0%. Methicillin-resistant S. aureus (MRSA), vancomycin-resistant Enterococcus faecium (VRE), and penicillin-resistant Streptococcus pneumoniae were isolated at 62%, 27% and 11%, respectively. Cefotaxime-resistant E. coli and K. pneumoniae was 20% and 18%, respectively. Imipenem-resistant Pseudomonas aeruginosa (IRPA) and Acinetobacter baumannii (IRAB) was 25% and 66%, respectively. E. coli and S. aureus were most common pathogens isolated from blood culture for five years. The increase of multidrug-resistant microorganisms, such as MRSA, VRE, ESBL, IRPA and IRAB, requires more strict control of antibiotics and causes the need of the more updated guideline for the treatment of blood stream infection.

Innate immune response to oral bacteria and the immune evasive characteristics of periodontal pathogens

  • Periodontitis is a chronic inflammation of periodontal tissue caused by subgingival plaque-associated bacteria. Periodontitis has long been understood to be the result of an excessive host response to plaque bacteria. In addition, periodontal pathogens have been regarded as the causative agents that induce a hyperinflammatory response from the host. In this brief review, host-microbe interaction of nonperiodontopathic versus periodontopathic bacteria with innate immune components encountered in the gingival sulcus will be described. In particular, we will describe the susceptibility of these microbes to antimicrobial peptides (AMPs) and phagocytosis by neutrophils, the induction of tissue-destructive mediators from neutrophils, the induction of AMPs and interleukin (IL)-8 from gingival epithelial cells, and the pattern recognition receptors that mediate the regulation of AMPs and IL-8 in gingival epithelial cells. This review indicates that true periodontal pathogens are poor activators/suppressors of a host immune response, and they evade host defense mechanisms.

Validation of Synovial Fluid Clinical Samples for Molecular Detection of Pathogens Causing Prosthetic Joint Infection Using GAPDH Housekeeping Gene as Internal Control

  • Identification of the pathogens causing infection is important in terms of patient's health management and infection control. Synovial fluids could be used as clinical samples to detect causative pathogens of prosthetic joint infections (PJIs) using molecular diagnostic assays, therefore, normalization and validation of clinical samples are necessary. Microbial culture is considered the gold standard for all infections, including PJIs. Recently, molecular diagnostic methods have been developed to overcome the limitation of microbial culture. Therefore, guideline for validating clinical samples to provide reliable results of molecular diagnostic assays for infectious diseases is required in clinical field. The present study aimed to develop an accurate validating method of synovial fluid clinical samples using GAPDH gene as an internal control to perform the quantitative PCR TaqMan probe assay to detect pathogens causing PJIs.
