Swan-Ganz Catheter를 이용한 심폐기능 이상자의 지속적 심박출량 측정 모듈 개발에 관한 연구 (The Study of continuous cardiac output measurement module development of the cardiopulmonary function patient of using the Swan-Ganz Catheter)

  • 본 논문에서는 심폐기능에 이상이 있는 중환자의 상태를 감시 및 진단하기 위한 방법으로 Swan-Ganz Catheter를 체내에 삽입하여, 지속적인 심박출량 데이터를 기존 장비들 보다 정밀하게 측정할 수 있는 모듈을 개발하고자 한다. 이전에 차가운 수액을 심장 내에 주입하여 심박출량을 측정하는 방법은 지속적인 환자의 감시 및 진단이 힘들다는 점을 보완하여 지속적인 심박출량 측정을 통해 심폐기능 문제를 가진 중환자를 보다 효과적이고 정확한 진단과 치료를 가능하게 한다.

  • Urodynamic study system is widely used for neurogenic bladder patients in various clinical setting. Generally they include 2 pressure sensors from bladder and rectum, and 1 EMG sensor. The rectal pressure catheter is often the source of data error because of gas passage and the fall out of the catheter from anus, and source of discomfort in ambulatory urodynamic system. This study is to design and calibrate the ambulatory digital urodynamic study system that can discard the rectal pressure catheter, which can make patients more comfortable and doctors can get more physiologic data. As a first step, we compared our new system with Dantec $Duet^{(r)}$ urodynamic system (Dantec, Denmark) and wanted to see the possibility of our new system.

  • Ventriculoperitoneal(V-P) shunt has been used as a popular method for surgical treatment of hydrocephalus. But complications such as infection, mechanical obstruction and failure of flow rate sometimes make painful stress to neurosurgeons and patients. Of particular, migration of distal V-P shunt catheter to extraperitoneal space has rarely been reported. Even rarer is intracardiac migration of distal V-P shunt catheter. Authors report a such case and discuss the possible mechanism and preventive method.

  • Upward migration of the peritoneal catheter of a subgaleo-peritoneal (SP) shunt and coiling into the subgaleal space is an extremely rare complication of a SP shunt. A 32-year-old male patient visited our hospital presenting with a large skull defect due to a prior craniectomy performed elsewhere. The patient underwent a cranioplasty with methylmetacrylate, but subsequently developed progressive pseudomeningocele and subgaleal cerebrospinal fluid (CSF) collection. The patient underwent CSF diversion via a SP shunt. After SP shunting, the pseudomeningocele disappeared completely. Six months later, the patient presented with progressive scalp swelling. Skull X-ray showed migration and coiling of the distal catheter of the SP shunt. The patient was treated by removing the entire shunt catheter and the dura was covered with a subgaleal flap. We would like to report our experience with a very rare complication of subgaleo-peritoneal shunting.

  • An increase in cardiac radiofrequency catheter ablation for treating refractory atrial fibrillation has resulted in an increased prevalence of complications. Among numerous complications of radiofrequency catheter ablation, atrio-esophageal fistula, although rare, is known to have fatal results. We report a case of successful management of an atrio-esophageal fistula as a complication of cardiac radiofrequency catheter ablation.

  • Epidural block is used for surgical analgesia, postoperative pain management, obstetric relief and management of chronic pain. Long-term pain control for chronic pain is achieved by development of epidural catheter placement technic. But long term placement of epidural catheteter has several problems, epidural hematoma, epidural absess and neural damage. We had successfully managed long-term placement of epidural catheter in patient with diabetic neuropathy who was susceptable to infection. The duration of epidural catheter placement was 416 days and specific complication was not occurred.

  • A cerebrospinal fluid hydrothorax is a very rare complication following ventriculoperitoneal (VP) shunt and usually reported in children. We report a case of 47-year-old woman who developed massive hydrothorax and respiratory distress following intrathoracic migration of distal shunt catheter. After the confirmation of catheter in thoracic cavity using radionuclide shuntogram, the patient was successfully treated with laparoscopic shunt catheter reposition.

  • Septic internal jugular vein-sigmoid sinus thrombosis (IJV-SST) associated with a malpositioned central venous catheter is a rare condition. It is potentially life-threatening and necessitates early diagnosis and rapid administration of appropriate medications. Unfortunately, it is difficult to diagnose due to vague clinical presentations. Several studies such as CT, MRI, and cerebral angiography should be performed and carefully examined to help make the diagnosis. We report a case of septic IJV-SST due to a malpositioned central venous catheter.

  • Unintentional foreign body ingestion is common in children, and coins are the most common foreign body ingested. Foreign body remaining in the esophagus may be associated with mucosal ulceration or esophageal obstruction and can potentially lead to significant morbidity and even death. Removal of esophageal foreign body is therefore generally recommended. Several methods are utilized including esophagoscopy in operating room, flexible endoscopy in out patient setting, fluoroscopic Foley catheter technique, and advancement using bougienage. We report two cases of esophageal foreign body(paduk stone) which were hard to be removed by usual esophagoscopic removal. Successful removal was accomplished with the aid of Fogarty catheter under the general anesthesia.

The Optical Design of Miniaturized Microscope Objective for CARS Imaging Catheter with Fiber Bundle

  • In coherent anti-Stokes Raman scattering (CARS) microscopy reported until now, conventional microscope objectives are used, so that they are limited for introduction into a living body. Gradient-index (GRIN) rod lenses might be a solution for miniaturized microscope objectives for in-vivo CARS microscopy. However, due to the inherent large amount of chromatic aberration, GRIN rod lenses cannot be utilized for this purpose. CARS imaging catheter, composed of miniaturized microscope objective and fiber bundle, can be introduced into a living body for minimally invasive diagnosis. In order to design the catheter, we have to first investigate design requirements. And then, the optical design is processed with design strategies and intensive computing power to achieve the design requirements. We report the miniaturized objective lens system with diffraction-limited performance and completely corrected chromatic aberrations for an in-vivo CARS imaging catheter.
