  • Based on the research of cataract, glaucoma, bluish glaucoma, we have abtained the following results. 1. Main etiologies of the cataract are deficiency of the liver and kidneys, main symptoms are no nebula, no pain, no tear and sometimes light blue and white vision appear. 2. Main etiologies of glaucoma are receives heat from the liver, stagnation of the liver and gall bladder etc, main symptoms are both forehead corner pull each ether, pain from eye ball to nasal septum, headache, extending pain of the eyes, sometimes red and white flower vision appear, etc. 3. main etiologies of the bluish glaucoma are melancholy, wrath, stagnation of the liver, internal damage, flaming up fire of the phlegm, deficiency of the liver, deficiency of blood, etc, main symptoms are numbness, visual field constriction, blindness, reduce of visual capacity, blue flower vision sometimes appear, etc. 4. Treatments are mainly uses nourish Um(陰) to reduce pathogenic fire, relieve stagnancy of Ki(氣) of the liver, nourish Um(陰) of the liver and kidneys, etc, acupuncture points are mostly contended arounded eyes such as BL10)(天柱), GV16(風府), LR3)(太衝), HT5)(通里) and in the case of nauses and vomitting PC6)(內關, ST36)(足三里) are included in addition to the above.

Risk factors, depression, quality of life and relevance of Korean adults

  • This study aimed that certain risk factors are linked to the risk of developing depression and decreasing quality of life. This study was implemented using data from the 6th and 7th Korea National Health and Nutritional Examination Survey. The National Health and Nutrition Survey consist of health surveys, screenings, and nutrition surveys. Among the risk factors, data on adult diseases such as depression, hypertension, arthritis, diabetes, cataract, glaucoma, and macular degeneration were used. In total, 12,768 adults over 20 years of age were selected, of whom 520 were diagnosed with depression. The most common risk factors in adults over 20 years of age were hypertension, arthritis, cataract, diabetes, depression, glaucoma, and macular degeneration. Their risk factors were analyzed if these were associated with depression and quality of life. The results revealed that hypertension, arthritis, diabetes, cataract, glaucoma, and macular degeneration were predictors for the occurrence of depression in adults. The factors associated with the highest risk for depression were arthritis and glaucoma. Furthermore, the study investigated the effect of certain factors on the quality of life; the factor associated with the greatest impact on quality of life was arthritis. This study verified that the aforementioned factors were related to the risk of developing depression and decreasing quality of life.

The effect of change of mandatory referral system in an ophthalmology of tertiary care medical institution (의료전달체계 변경이 3차 의료기관 안과에 미친 영향)

  • According to the change of mandatory referral system in July 1, 2000, the effect to the medical utilization of outpatient clinic and medical income in ophthalmology of tertiary care medical institute, S Hospital in Seoul was evaluated for 6 months before(1999. 12$\sim$2000. 5) and after(2000. 12$\sim$2001. 5). The results were as follows: 1. The number of outpatients was reduced by 16.6%. The number of patient with blindness low vision, retina, glaucoma increased and that of patient with accommodation refractive error, cataract decreased. 2. The number of cataract patients was reduced by 36.6%. The major location of patient's address was changed to nearer to the hospital. The number of cataract surgery reduced in 4.1%, the waiting time reduced in 42.2%, however surgery time increased in 20.2% and number of postoperative complications increased in 11.4%. 3. The income of outpatient clinic and cataract surgery reduced. Among items of outpatient clinic income, the most increased was ocular examination and the most reduced was injection and drugs. Among items of cataract surgery income, the most increased was operation fee and the most decreased was doctor's fee. In conclusion, for the patient, due to the lowered density of outpatient population more space was provided to the patients with more severe disease entity such as blindness' low vision, retina and glaucoma. For the hospital, the need for the expansion of ophthalmology was not found, however that for creation of the special clinics dealing with more severe disease entity was found. Due to reduced income and increased need of financial investment for the equipment and manpower for the more severe disease entity, the ophthalmology of tertiary care medical institute is faced with financial disaster. It is strongly suggested that the cost of medical practice of more severe disease entity be raised to achieve the success after change of mandatory referral system in ophthalmology.

Relationship among Patient Outcomes in Cataract Surgical Patient - Pilot study - (백내장 수술환자 진료결과들간의 관계 - 사전 연구를 중심으로 -)

  • Background : This study was done to assess the relationship among multiple patient outcomes of cataract surgery perioperatively, 3-4 months and 12 months after surgery. The patient outcomes include changes in visual acuity(operated eye, better eye), visual function(VF-14), patient satisfaction, subjective satisfaction with vision, and subjective overall health status. Methods : For the assessment of relationship, prospective study was performed with 92 patients who had undergone either one or both eye cataract surgery by 3 ophthalmologists practicing at a university hospital. Patients were interviewed. and clinical data were obtained. Doctors were questioned with self-entered questionnaire forms. Medical record was examined to understand surgery process. The survey was conducted at 4 stages : preoperatively, perioperatively, postoperative 3-4 months, and postoperative 12 months. Results : The correlations within patient outcomes at 4 stages - the visual acuity of operated eye and that of better eye, patient satisfaction and VF-14, subjective overall health status and relative health status as against others - were found to be positively correlated. The change in the visual acuity of operated eye and better eye was correlated with VF-14 as well as with patient satisfaction. The change was also correlated with overall health status. However, the correlations between variables were decreased as the postoperative period got longer. Conclusion : As for the postoperative clinical patient outcomes, VF-14 is acted to linker between visual acuity - clinical outcomes and overall health status - endpoint outcomes. Therefore. VF-14 is the index of patient-sided and disease-specific outcome for cataract surgery.

Endophthalmitis after Removal of an Intralenticular Foreign Body in Place without Symptoms for 20 Years (20년 동안 증상없이 잔존한 수정체 내 이물제거 후 발생한 안내염)

  • Purpose: We report a case of postoperative endophthalmitis in the cataract patient, associated with removal of an intralenticular foreign body that had remained in place without symptoms for 20 years. Case summary: A 45-year-old male visited our outpatient clinic complaining of gradual visual loss in his right eye over the past 3 months. In slit-lamp examinations, anterior capsular opacification, nuclear sclerosis, and posterior subcapsular opacity were observed in the right eye. Twenty years before, a tiny metallic projectile had hit his right eye, but slit-lamp examination at the time of injury did not reveal any intraocular foreign body. We decided to undergo cataract surgery. During phacoemulsification, a metallic foreign body was found in the lens and safely removed; then an intraocular lens was implanted. As hypopyon was evident 3 days later, we injected intravitreal antibiotics and applied fortified antibiotic eye drops to the right eye. The anterior chamber inflammation improved and the best-corrected visual acuity recovered to 1.0. Conclusions: Surgeon should be aware of that endophthalmitis could develop after cataract surgery with removal of an intralenticular foreign body that had been in place for 20 years. But did not trigger inflammation or cause any symptoms as the cataract progressed.

Protective effect of Kohl-Chikni Dawa against galactose-induced cataract in rats

  • The efficacy of Kohl-Chikni Dawa (KCD), a compound ophthalmic formulation of Unani medicine, to control cataract development was explored in rats fed 25% galactose with the diet for 6 months. When one drop of 3% KCD solution was applied (once and twice daily) in both eyes for 6 months in galactose-fed rats, a significant reduction of lens opacification was noticed. The morphological changes of the lenses were observed every month by slit-lamp biomicroscope. These results suggest that the local application of 3% KCD solution possesses anticataract effect in galactose-fed rats.

The effect of the visual impairment on the Island Elder (섬지역 노인의 시력장애 요인 실태조사)

  • In order to investigate the situation of ocular disorders involving visual functions of elder in the Island area, the survey was carried out subjective methods on 228 persons(456 eyes) over the of 60. The analysis of data resulted in as follows : 1. Among the total 456 eyes, the ocular disorders was 54.8%. 2. The causes of ocular disorders with visual impairment ; Senile cataract(44.8%), Pterygium(55.2%) 3. Age distribution of cataract showed the sixties 46.4%, the seventies 37.5% and the eighties($80{\leq}$) 16.1%. 4. As to the distribution corrected visual acuity in cataract ; 0.1 below(8.9%). 0.1~0.3(14.3%), 0.4~0.6(36.6%), 0.7 over(40.2%). 5. Age distribution of pterygium showed the sixties 60.9%, the seventies 26.1% and the eighties($80{\leq}$) 13.0%. 6. As to the distribution corrected visual acuity in pterygium ; 0.1 below(5.l%), 0.1~0.3(5.8%), 0.4~0.6(20.3%), 0.7 over(68.8%).

  • Aldose reductase (AR), the principal enzyme of the polyol pathway, has been shown to play an important role in the diabetic complications, including diabetic cataract. The inhibitors of AR, therefore, would be potential agents for prevention of diabetic complications. As part of our ongoing project directed toward the discovery of preventive and/or treatment for diabetic complications, 48 Korean herbal medicines have been investigated with an in vitro evaluation system using aldose reductase inhibitory activities. Of these, 12 herbal medicines exhibited a significant inhibitory activity against aldose reductase. Particularly, seven herbal medicines, i.e., Eurya japonica, Chrysanthemum indicum, Akebia quinata, Saururus chinensis, Catalpa bignonioides, Lonicera japonica, Vitex rotundifolia showed two times more potent inhibitory activity than the positive control 3.3-tetramethyleneglutaric acid (TMG). In addition, Catalpa bignonioides showed the retardation of cataract-opacification of the lens of the eye under diabetic condition by xylose. Therefore, this result may provide a potential therapeutic approach for preventing and treating diabetic cataracts.
