Traumatic arterial thoracic outlet syndrome after multiple rib fractures not including the first rib in Korea: a case report

  • Arterial thoracic outlet syndrome (TOS) resulting from thoracic trauma is an exceedingly rare condition, typically caused by a fracture of the first rib or clavicle. In this report, the author presents a case of traumatic arterial TOS precipitated by multiple left rib fractures, notably excluding the first rib, following a fall from a 2-m high stepladder. The patient was treated successfully with first rib resection via a transaxillary approach, and the postoperative course was uneventful. The literature includes no known reports of traumatic arterial TOS in patients with multiple fractures that spare the first rib, making this the first documented case of its kind. In this instance, the patient sustained fractures to the fourth and fifth ribs. The TOS was likely not a direct result of the multiple rib fractures, which were located some distance from the thoracic outlet. Rather, it is hypothesized that the trauma from these fractures caused a soft tissue injury within the thoracic outlet, which ultimately led to the development of TOS.

Experimental Study on Low-income Single-Parent Family Case Management (저소득 여성 한부모가족 사례관리 실천 경험 연구)

  • This paper analyzed the experience of case management experts and single mothers in managing the cases, as the useful approach for family enrichment of a low-income single parent family. The analysis included five case managements and interview with three case management experts for single-parent families. Four of the five cases have been in progress at present. According to the analysis, the main appeals for single-parent families were economy and basic life, physical and mental health, family and social relationships, and child care and education. Furthermore, the interview with the case management experts identified the difficulty in managing single-parent family cases, especially those cases with complicated demands and problems, barriers to expert management, and required support for family enrichment. Finally, this paper analyzed the implementation process of case management for single-parent families in a specific area, meaning that the results of this paper do not represent all single-parent families. Moreover, the analyzed reports were specific to the case management implementation process. Thus, the contents, which were not recorded in the reports, were not fully reflected, and the in-depth analysis of multilateral aspects was restricted.

A Systematic Review of Acupuncture for Oculomotor Nerve Palsy

  • Acupuncture treatment for oculomotor nerve palsy has been increasing recently. This study analyzed randomized controlled trials (RCTs) and case reports, using the Cochrane risk of bias tool to investigate the efficacy of acupuncture therapy for oculomotor nerve palsy. This analysis was performed on March 7, 2019, using online databases (PubMed, Cochrane, NDSL, OASIS, CNKI) where 208 articles were retrieved. Of these, there were 18 case reports and 18 RCTs that matched the inclusion criteria, of which 32 studies used acupuncture as the primary intervention, 1 used pharmacopuncture, 1 used fire-needling, and 1 used electroacupuncture. The most commonly used acupoints were BL1, BL2, ST2, TE23, Ex-HN5, LI4, GB14, ST36, GB20 and GB1. Significant findings were reported in all RCTs. Six adverse events were reported in 3 RCTs, with no effect on the outcome. No side effects were reported in the case reports. The risk-of-bias analysis showed that the articles did not report the experimental protocol used and it was not clear whether the study was blinded. Hence, it was difficult to assess the risk of bias. Analysis of 36 studies showed that acupuncture therapy for oculomotor nerve palsy was effective in many cases. It was difficult to evaluate the potential bias.

The Review on the Trend of Teeth and Temporomandibular Joint(TMJ) Diseases Articles that Published in the Journals of Korean Medicine (국내 한의학 학술지에 게재된 치아 및 측두하악관절 질환 관련 논문들의 경향성 고찰)

  • Objective : For activation of study on filed of odontology in Korean medicine academia, we analyzed the trend of articles that published in journals of Korean medicine. Methods : Using search words of odontology, in internet reference sites we collected papers and classified those into three categories like as review article, original article, case report. Observation points of each item are as follows. Inclusive item of papers; publication year, journal, number of authors, disease. Item of review article; subject of paper, Item of original article; number of patients, period of research, remedy. Item of case report; remedy, valuation of criteria, number of cases. Results : The total number of articles searched was 88, consisting of 23 review articles, 46 original articles and 19 case reports. The percentage of 'TMJ diseases' ranked highest(47.7%) in classification by disease. The most reviewed subject was 'Remedy' (16 times). '25 or less' person ranked highest(50%) in the number of patient in original articles. Acupuncture, chuna manual medicine and herbal medicine were mostly applied in case reports. The number of 'one case' reported case reports was highest(52.6%).

The Review on the Acne Related Articles Published in the Journal of Korean Medicine Ophthalmology, Otolaryngology & Dermatology (한방안이비인후피부과학회지에 게재된 여드름과 관련된 논문에 관한 고찰)

  • Objective : This study was carried out to analyze the trends of Acne related articles that have been published in the Journal of Korean Medicine Ophthalmology, Otolaryngology & Dermatology(JKOOD). Method : We studied 29 research papers relevant to Acne that published in JKOOD from 1998 to 2013. All the paper were classified into three categories; original article, review article and case report. We sub-classified the original articles into three categories; descriptive study, analytic study and experimental study. We analyzed the case report according to various types of treatment. Results : 1. The number of searched journals is 29 papers; 17 original ordicles, 4 review articles, 8 case reports. 2. Classification of 17 original articles into three categories; 10 experimental studies, 7 descriptive studies, 0 analytic study. 3. Method of Evaluation; Korean Acne Grading System(KAGS) is used frequently in descriptive studies and experimental studies. 4. Treatment in Case reports; 7 Case reports used External medical treatments. Conclusions : It is needed to develop the methods of grading system in acne related to Oriental medicine to obtain objectivity of studies. The number of analytic study, clinical trial and experimental study is need to increase.

Mediastinal thymolipoma: a case report (흉선지방종 치험 1례 보)

  • Thymolipoma is a rare benign tumor consisting of fat and thymic tissue. Approximately 60 case reports have been published. On 1984, one patient with proven mediastinal thymolipoma was treated surgically at the Department of Thoracic & Cardiovascular surgery, Seoul National University Hospital. The following case is the first reported example of thymolipoma occurring in Korea.

  • Surgical fires require an ignition source, oxidizer, and fuel. The pulsed dye laser (PDL) has been shown to be effective in the treatment of hypertrophic scars and keloids in dermatology. With the increasing number of applications of $CO_2$, laser as ignition source has been associated with operating room fires in the otorhinolaryngologic field. There have been many case reports on PDL-induced fires in dermatology, but until now, there were no reports in the larynx. We describe a 57-year-old patient diagnosed with laryngeal hyperkeratosis treated by PDL-assisted laryngomicrosurgery. In this case, we experienced incidental PDL tip flaring during pulsed dye laser vaporization.

A Reports on Chronic Dermatitis Treated by fasting Therapy (慢性 皮膚疾患 患者에 對한 節食療法 治驗例)

  • A Reports were done on 3 cases of proriasis, 1 case of chronic urticaria which were admition and treated by fasting therapy in Dept. surgery, opthalmology & otolaryngology, college of Oriental Medicine, Kyungsan University, Pohang, Korea, from March 1999 to October 2000. The results were as follows: 1. In the case chronic urticaria, symptom and sign(jtching, wheal, erythema), except of pigment, were showed conciderable improvement. 2. In the case proriasis, it is different of person, bellow 5cm sized erythematous popules were disappeared during fasting therapy, above 5cm sized erythematous papules and plaque were improved by using medication after fasting therapy. Silver-white colored scales was improved about $50-60\%$, itching was disappeared during fasting therapy. 3. After fasting therapy there was a recurrence, but about $40-50\%$ decreased symptoms were showed. 4. Besides upper diagnosis and treatment based on overall of symptoms and signs considered by documentary investigation, medications by Sasang prescription also can be expected good treatment effect.

  • I have taught various case audit underwriting methods to underwriters for many years. Underwriting requires specialized medical knowledge and experience of comprehensive individual risk analysis. Decisions in individualized underwriting depend on the insurance company's management philosophy and vision, marketing strategies, culture, product line etc. They also depend on the information acquired through proposal forms, inspection reports, laboratory results and disclosures. Underwriters match this acquired information to each insurance product, search appropriate medical resources, assess the probabilities of future claims, and decide what, if any, further medical information should be gathered. Sources include doctors' reports, medical records, biopsies, blood and urine tests, CT, MRI, Holter monitoring, sonography etc. Insurance medical directors help and teach underwriters to analyze individual medical risks and make effective.
