Association Between Meat Consumption and Carotid Intima-Media Thickness in Korean Adults With Metabolic Syndrome

  • Objectives: The effect of meat consumption on cardiometabolic risk has been continuously studied, but their associations are not conclusive. The aim of this study is to examine the association between the consumption of meat or red meat and carotid intima-media thickness (IMT) in healthy Korean adults. Methods: This study evaluated 2374 community-dwelling adults (933 men and 1441 women) who were free of cardiovascular disease or cancer, living in a rural area in Korea. Total meat and red meat intakes were assessed with a validated 103 item-food frequency questionnaire. Carotid IMT was evaluated ultrasonographically, IMTmax was defined as the highest value among IMT of bilateral common carotid arteries. Results: After adjustment for potential confounding factors, the mean IMTmax tended to increase in higher meat consumption groups in both men and women with metabolic syndrome (p for trend= 0.027 and 0.049, respectively), but not in participants without metabolic syndrome. Frequent meat consumption (${\geq}5$ servings/week) was significantly associated with higher IMTmax in men with metabolic syndrome (by 0.08 mm, p=0.015). Whereas, the association was not significant in women (by 0.05 mm, p=0.115). Similar but attenuated findings were shown with red meat intake. Conclusions: Our findings suggest that a higher meat consumption may be associated with a higher carotid IMT in Korean adults with metabolic syndrome. The frequent meat consumption (${\geq}5$ servings/week), compared with the others, was associated with a higher carotid IMTmax only in men with metabolic syndrome. Further research is required to explore optimal meat consumption in people with specific medical conditions.

One Sage Operation for Bilateral Carotid Arterial Obstruction and Right Coronary Artery Ostial Stenosis in Takayasu's Arteritis -Report of One Case- (Takayasu씨 대동맥염에 의한 양경동맥협착과 우관상동맥개구협착의 동시수술치험 -1례 보고-)

  • We report a case of recurred Takayasu,s arteritis.The patient was 28-year-old female underwent aorto-left common carotid and aorto-left subclavian bypass graft replacement 1 year ago.Unfortunately, she was readmitted because of newly developing angina and both eye claudication severe headache. Aorto-coronary angiogram showed complete obstruction of left common carotid artery ,stenosis of right carotid artery bifurcation and ostial stenosis of right coronary artery.Bilateral carotid arteries bypass graft with great saphenous vein and right coronary artery bypass graft with right internal mammary artery were done at same the time and she discharged after 21 days without any problem.

  • A 38-year old man was admitted to our hospital due to a $5{\times}6cm$ sized pulsating mass in the right neck. He suffered from intermittent neck pain and hoarseness for two months due to the rapidly growing mass. The radiological examinations revealed an aneurysm of the right common carotid artery near the bifurcation, and it was compressing the internal and external carotid arteries. Endarterectomy of the right internal carotid artery, aneurysmectomy of the right common carotid artery and graft interposition were done, while the cerebral circulation was maintained by an internal shunt. Intraoperative injury to the nerve tissue around the aneurysm was avoided. He was discharged on the postoperative 7th day without any complications.

A Study on Effects of the Transient Compression by Tightly Tied Necktie on Blood Flow in the Internal Jugular Veins Using 2D-PC MRA (2D-PC MRA를 이용한 넥타이의 순간적인 압박(壓迫)이 내경정맥의 혈류에 미치는 영향에 대한 연구(硏究))

  • The 25 healthy male volunteers aged from 20 to 50years old have been employed in this study. 2D-PC MRA was performed to measure the velocity of the blood flow in the internal carotid artery and internal jugular veins using 3.0T MRI Whole body (signa VH/i GE). ECTRICKS-CEMRA was performed to evaluate the pattern of blood circulation from internal carotid artery to internal jugular vein. Using 2D-PC MRA, the cross-section of the 4th and 5th cervical discs was scanned with 24cm FOV. Then the speed of blood flow was measured for internal carotid artery and internal jugular vein when the subject wears a necktie tightly and no tie. The average of maximum velocity of internal carotid arteries without a necktie was 72.13cm/sec in the right side and 74.96cm/sec in the left side(average 73.54cm/sec in both sides) while the average of maximum velocity of internal jugular veins without a necktie was -34.45cm/sec in the right side and -24.99cm/sec in the left side (-29.72cm/sec in both sides). However, when wearing a necktie tightly, the average of maximum velocity of internal carotid arteries was 61.35cm/sec in the right side and 65.19cm/sec in the left side(average 63.27cm/sec in both sides) while the average of maximum velocity of internal jugular veins was -22.14cm/sec in the right side and -17.93cm/sec in the left side(-20.03cm/sec in both sides). With the necktie tightly knotted, the average blood flow speed of both internal carotid arteries slightly decreased to 86% (63.27/73.54cm/sec) compared to no tie case in which both internal jugularveins significantly went down to 67% (-20.03/-29.72 cm/sec). Thus it is suggested that wearing a necktie affects the circulation of internal jugular veins(33% decrease in blood flow speed) more significantly than that of internal carotid artery(14% decrease in blood flow speed). Without a necktie, ECTRICKS-CEMRA showed natural blood circulation patterns of internal carotid arteries and internal jugular veins without any disturbances or compressions. However, when wearing a necktie tightly, ECTRICKS-CEMRA showed severe compression onto both internal jugular veins in all 25 volunteers. In conclusion, the result of the study showed that the tightly worn necktie instantly presses more internal jugular veins than internal carotid arteries, thereby significantly reducing the blood flow speed and leading to the temporary occlusion. Thus, the defecation or washing the face under the tightly tied necktie situations can cause the unexpected and temporary compression or occlusion of the internal jugular veins, subsequently leading to the occurrences of the stroke due to the secondary intracranial venous hypertension.

Direct Carotid Cavernous Fistula of an Adult-Type Persistent Primitive Trigeminal Artery with Multiple Vascular Variations

  • We report a case of spontaneous right carotid-cavernous fistula (CCF) in a proximal segment of persistent primitive trigeminal artery (PPTA) and combined vascular anomalies such as left duplicated hypoplastic proximal posterior cerebral arteries and a variation of anterior choroidal artery supplying temporal and occipital lobe. A 45-year-old male presented with progressive right exophthalmos, diplopia, and ocular pain. With manual compression of the internal carotid artery, a cerebral angiography revealed a right CCF from a PPTA. Treatment involved the placement of detachable non-fibered and fibered coils, and use of a hyperglide balloon to protect against coil herniation into the internal carotid artery. A final angiograph revealed complete occlusion of PPTA resulted in no contrast filling of CCF.

NUMERICAL ANALYSIS OF BLOOD FLOW DYNAMICS AND WALL MECHANICS IN A COMPLIANT CAROTID BIFURCATION MODEL (혈관 유연성을 고려한 경동맥 분기부 모델 혈류역학 해석)

  • Blood flow simulations in an idealized carotid bifurcation model with considering wall compliance were carried out to investigate the effect of wall elasticity on the wall shear stress and wall solid stress. Canonical waveforms of flowrates and pressure in the carotid arteries were imposed for the boundary conditions. Comparing to rigid wall model, generally, we could find an increased recirculation region at the carotid bulb and an overall reduced wall shear stress. Also, there was appreciable change of flowrate and pressure waveform in longitudinal direction. Solid and wall shear stress concentration occurs at the bifurcation apex.

Surgical Treatment of Takayasu`s Arteritis; Report of One Case (Takayasu`s arteritis의 수술치험 1례)

  • Takaysu`s arteritis is an arteritis of unknown etiology involving larger elastic arteries. The end stage pathologic feature is vascular obstructive change and the resulting clinical manifestations are local ischemic symptoms such as syncope, visual disturbance, claudication of extremities, hypertension, and angina. Recently we have experienced one case of Takayasu`s arteritis involving aortic arch, left common carotid artery and left subclavian artery. The patient was 27 year-old female and she was admitted because of headache and neck pain. Aortogram revealed fusiform dilatation of left common carotid artery with focal narrowing on it`s distal portion. The patient underwent surgical resection and replacement of Dacron tube graft between distal and proximal left common carotid artery. 3 months after operation, she was readmitted because of shoulder pain and headache. Aortogram revealed focal narrowing of proximal left common carotid artery and total obstruction of left subclavian artery which caused subclavian steel syndrome. Aorto-left common carotid and aorto-left subclavian bypass graft replacement were done.

  • Takayasu`s arteritis is an arteritis of unknown etiology involving larger elastic arteries such as aorta and its branches, pulmonary arteries, and rarely coronary arteries. The late pathologic feature is vascular obstructive change and the resulting clinical manifestations are local ischemic symptoms such as syncope, visual disturbance, claudication of extremities, hypertension, and angina. the disease occurs predominantly in females, with the age of onset between 10 and 30 years. Recently we have experienced one case of Takayasu`s arteritis involving aortic arch and all its major branches. The patient was 36 year-old female and she was admitted because of headache, blurred vision, and easy fatigability and motor weakness of upper extremities. Aortogram revealed total obstruction of both carotid arteries at the site of its origin and partial irregular obstructive change in the innominate artery and both subclavian arteries. Bypass graft surgery using Gore-Tex grafts was performed with successful result.

Axillo-Axillar bypass with Gore-Tex graft for a pulseless disease. (액와동맥간 Bypass Graft 를 이용한 무맥증 수술치험 1례)

  • The patient was 17 years old female who complained of coldness of right arm and occasional dizziness, since 4 months prior to admission. On physical examination, the right radial, brachial and common carotid arterial pulses were not palpable. Aortography revealed narrowing of innominate and right common carotid arteries, and complete obstruction of right subclavian artery. The right axillary artery was faintly visualized on the delayed film. Axillo-axillar bypass was done using Gore-Tex graft of 8 mm I.D.. By bilateral subclavicular incision, both side axillary arteries was exposed. End to side anastomosis was made between graft and right axillary artery and the graft was brought out to the left side, subcutaneously, over the sternum, and the anastomosis was made between graft and left axillary artery. Postoperatively, both radial pulses were equally palpable. On follow-up visiting, there were no subjective symptoms and blood pressure in both arms was equal.
