Cure Program Development for Caregivers of Elderly with Dementia (치매노인 부양자를 위한 치료프로그램 모형 개발)

  • The purpose of this study was to develop the cure program for family caregivers of elderly with dementia, As the lifespan has been expanded, Most of family caregivers had serious problems which are burden for caregiving(economic burden. physical & social burden, negative elderly-family caregiving relationship) and the life quality of the elderly and family caregivers and the future social welfare alternatives for taking care of the elderly. To develop the cure program for family caregivers of elderly with dementia is very important. This program can prevent to be another elderly with dementia. The results can be used as basic data to develop educational programs for family caregivers.

The Impact of Elderly Caregiving on Marital Quality

  • Although the relation of caregiving burden to well-being has been frequently examined, the effect of caregiving on marital quality has been virtually ignored. The current study explores the impact of parent care on the marriages of adult children by using a nationally representative longitudinal sample. It was hypothesized that parent care would negatively influence the marital quality of adult child caregivers, the effect of parent care would differ between women and men caregivers, and the nature of marital relationship variables would moderate or mediate the impact of care on marital quality. Overall, marital quality does not appear to suffer if caregiving load is light. However, the impact of parental caregiving on some dimensions of marital quality was conditioned by gender, gender role attitudes, and congruence between attitudes and behavior.

A Study on the Development of Self-help Program with Care Burden for Families with Demented Elderly Persons (치매노인 가족의 부양부담 경감을 위한 자조집단프로그램 개발)

  • Purpose: This study were to understand the caregiving situation of families with the demented elderly and to develop a self-help program for them. Method: A self-help program was designed to increase caregivers' confidence and skills in problem solving. In addition, it was designed to provide information about the demented elderly. The self-help program of this study was composed of nine sessions. Result: Family caregivers of elderly patients with dementia obviously experience high level of stress. which often leads the caregivers to negative outcomes such as depression, interference with social activities and health difficulties. That is, caring is stressful for caregivers because the demands of the caregiving situation tend to overwhelm the caregivers' coping resources. Therefore, families with demented elderly persons need assistances in order to be relieved from their care burden. Conclusion: The self-help program includes the understanding of dementia, the family caregiving experiences of the demented elderly persons, the time for self-care, the search for helping, the improving of interpersonal relations and communications.

Study of the Positive and Negative Caregiving Experiences in the family members who care for the psychiatric mentally ill relatives (정신장애인 가족의 긍정적, 부정적 돌봄의 경험에 관한 연구)

  • The caregiving experiences of 100 family menbers of outpatients with schizophrenia and schizoaffective disorders were investigated for the presence of positive(positive family-patient relationship. patient' contribution to the family) and negative caregiving experience(objective and subjective burden) and their predictors. This study attempts to make the analysis of caregiving experience more useful by expanding the focus to incoporate these positive aspects of the experience of family caregiver. Objective burden consists of two elements: 'disruption of family life', 'care'(amount of caregiving related to activity of daily living). Subjective burden is defined as emotional reactions to the care giving and it comprised of 6 emotional subdimensions such as 'stigma', 'grief'. 'worry', 'pity', 'fear', 'despair'. Also we investigate the severity of patients' disturbing behaviors into two categories, positive and negative disturbing behaviors and patient' contribution to the family as a predictors of positive and negative caregiving experiences. This study use Pearson's correlation coefficient, Hierardhical regressions in the SAS Program. The results are as follows: 1. Respondents reported moderate level of objective burden 'disruption of family life' (mean = 2.48, range = 1-4), and 'care' (mean=2.54, range = 1-4), and slightly high level of total subjective burden(mean = 2.19, range = 1-4). Mean scores for the measure of the severity of behavioral disturbance indicated that the caregiver experienced negative disturbing behaviors around almost 'somtimes'(mean=2.28, range = 1-4), and positive disturbing behaviors 'almost not frequent'(mean=2.78. range=1-4). So they reported that they perceived patient's negative disturbing behaviors more than positive disturbing behaviors. Mean scores for the measure of the patient' contributions (mean = 1.99. range = 1-4) indicated that caregivers experienced these contributions a little. It means that there should be a positive aspect of possibilities of patient' family roles that can be developed in the daily life. Mean scores for the measure of the positive family-patient relationship indicated that caregivers experienced moderate level of positive family-patient relationship(mean=2.52, range = 1-4). 2. Hierardhical regression analysis 1) Hierardhical regression of 'disruption of family life' showed that the interaction between positive disturbing behaviors and patient' contributions (B = .20. p = .022) and caregiver's educational level(B=.06. p=.000) were 'significant and Hierardhical regression of 'care' showed that 'negative disturbing behaviors'(B= .35. p= .007). 'patient' contributions'(B= .28, p= .019). 'family income'(B=-.l1. p=.096) were significant. 2) Hierardhical regression of 'total subjective burden', 'stigma', 'grief', 'worry', 'pity'. 'fear', 'dispair' showed that 'positive disturbing behaviors'(B=.51. p=.000). 'negative disturbing behaviors' (B = .17, p = .026), 'caregiver's educational level'(B = .03. p=.036), 'family income'(B=.08. p=.041) were significant predictors of 'total subjective burden': 'positive disturbing behaviors'(B=.32. p=.066). 'negative disturbing behaviors'(B=.24, p=.096) 'durations of illness'(B= .03. p= .079) were significant predictors of 'stigma' 'negative disturbing behaviors'(B=.28. p=.005). 'patient sex'(B=-.32. p=.022). 'positive disturbing behaviors'(B=.28. p=.020), 'patient age'(B=.02. p=.010), 'caregiver age'(B=-01, p= .002) were significant predictors of 'grief' 'negative disturbing behaviors'(B= .28, p= .005). 'patient sex'(B= -.32. p=.039), 'caregiver age'(B=-.02, p= .023). 'caregiver's educational level'(B= .04, p = .044) were significant predictors of 'worry' 'patient sex'(B=-.46. p=.005). 'negative disturbing behaviors'(B= .28. p=.018), 'caregiver age'(B=-.01, p=.037) were significant predictors of 'pity' 'positive disturbing behaviors'(B=.83. p=.000). 'patient' contributions' (B = .22, p =.017). 'family income'(B=.09. p=.65) were significant predictors of 'fear' 'positive disturbing behaviors'(B=.49, p=.001). 'negative disturbing behaviors'(B= .24. p=.057) 'patient sex'(B=-.4l, p=.017), 'family income'(B=.14, p=.047) were significant predictors of 'dispair'. 3) Hierardhical regression of 'positive relationship' showed that 'patient contributions'(B=.32, p=.000). Family Caregivers of the Impaired Elderly: A Comparison between Spouses and Adult Children (장기요양노인 가족수발자의 부양 실태에 관한 연구: 배우자 수발자와 자녀 수발자의 비교를 중심으로)

  • This study examined how caregiving experiences of spouses and adult children were different to each other in terms of caregiver characteristics, the impairment level of the elderly, caregiving time, caregiver burden, the effects of long-term care services, etc. Data were collected from 321 spouses and 324 adult children who cared for the functionally and/or cognitively impaired elderly using long-term care services. The main results are as follows. (1) Caregiver characteristics differed significantly between spouses and adult children. (2) Adult children cared for the more severely impaired elderly in terms of IADL, cognitive impairment, and behavior problems while spouses spent more time helping in ADL activities. (3) Spouse caregivers experienced greater overall burden, worry and strain, and financial burden compared to adult child caregivers. (4) Long-term care services were effective in reducing caregiver burden and improving family relations. Additionally, relations between adult child caregivers and the elderly was more improved than relations between spouse caregivers and the elderly after using long-term care services. Based on these findings, the differential experiences between spouse caregivers and adult child caregivers were discussed.

Oral care practices for dementia patients and social support and depression in family caregivers of dementia patients (치매환자 부양가족의 치매환자 구강관리 실태 및 우울과 사회적지지)

  • Objectives: This study aims to contribute to devising systems for family caregivers of dementia patients by examining the state of oral care of dementia patients, and depression and social support among family caregivers of dementia patients. Methods: Family caregivers of dementia patients in the metropolitan area were selected in this study. The inclusion criteria were individuals who have provided care for a dementia patient at home for at least six months and those who come in contact (including phone calls) with the patient at least twice a week. Results: Oral health knowledge of the elderly, caregiving burden, depression, and social support were examined. The mean scores for oral health knowledge of the elderly and caregiving burden were $57.11{\pm}16.94$ out of 100 and $17.33{\pm}8.61$ out of 48, respectively. Further, the mean caregiving behavior score, depression score, and social support score were $8.49{\pm}13.71$ out of 100, $5.11{\pm}3.05$ out of 10, and $72.75{\pm}17.03$ out of 100, respectively. Factors affecting oral health knowledge of the elderly were examined. The results showed that the level of oral health knowledge of the elderly increased with an increasing perception of a need for oral health education (p<0.05), caregiving burden (p<0.01), and social support (p<0.01). Conclusions: These findings suggest that developing and popularizing oral care intervention programs for family caregivers of dementia patients are necessary to ensure systematic oral care for dementia patients.

A study on caregiving situations and resources for adjustment among family caregivers of the elderly with dementia (치매노인 가족의 부양상황과 적응자원에 관한 연구)

  • The purpose of this study was to identify caregiving situations and to examine resources for adjustment among family caregivers of the elderly with dementia. Thereby the study can provide the basic information for the development of education programs for family caregivers through in-depth interview. The main result of this study was as follows : 1) The major problems identified by family caregivers I caring for the elderly with dementia were related to care recipient's toileting and to care recipient's confusion and wandering. 2) The family caregiver expressed feelings of burden including chronic fatigue, depression, guilt and frustration. 3) The factors influencing the adjustment of family caregivers for demented elderly were quality of relationship, motivation for caregiving, circumstances of caregiving, social support and coping strategies.

  • The purposes of this study were to understand caregiving situation and to develop education program for families with the demented elderly,. Family caregivers of elderly patients with dementia clearly experience high levels of stress which often leads to such negative caregiver outcomes as depression interference with social activities and health difficulties. That is providing care is stressful for caregivers because the demands of the caregiving situation threaten to overwhelm the caregivers' coping resources. Therfore the families with demented elderly persons need assistances in order to alleviated burden of caregiving, In this study the educational program was designed to increase caregivers' confidence in problem-solving, And it was designed to provided information to demented elderly. The educational program of this study was given 5 session. And it was included the understanding of dementia the family caregiving experiences of the demented elderly persons the time for elf-care the search for helping the improving of interpersonal relations and communications.

Factors Related to Family Caregivers' Burden with the Community-Dwelling Disabled Elderly under the Long-Term Care Insurance System (장기요양 재가서비스 이용자를 돌보는 가족주부양자의 부양부담감에 영향을 미치는 요인)

  • Background: Informal care is increasingly recognized as placing a significant burden on the lives of family caregivers. The purpose of this study is to investigate factors related to family caregivers' burden with the community-dwelling disabled elderly under the long-term care insurance system, using the Stress Process Model developed by Pearlin (1990). Methods: Total 1,233 family caregivers with the disabled elderly, using the long-term care services in their home from May to June 2009, completed questionnaires finally. The questionnaire of this study consists of a total of 32 questions, including 11 questions related to background and context, 17 questions related to objective stressors, and 4 questions related to coping resourses. Family caregivers' burden is measured by the Korean Revised Caregiving Appraisal Scale (K-RCAS, Cronbach's alpha=0.86). To investigate factors related to family caregivers' burden, multiple regression analysis was conducted. Results: The average score of caregivers' burden was 22.0 (${\pm}6.12$). In multiple regression analysis, there were statistically significant factors affecting on the family caregivers' burden, that are related to background and context (region, living status, education level, relationship with beneficiary), objective stressors (duration of caregiving), coping resourses (caregiver's health status). Conclusion: This study found that family caregivers experience a considerable burden. The findings suggest that policies must be taken to relieve family caregivers of their duties temporarily, and to support them with counselling and education.

Grandparents Raising Grandchildren (조부모-손자녀가족 조부모의 손자녀양육)

  • The number of grandparents who have responsibility for raising their grandchildren is growing. The present study examines the extent of this phenomenon and some of the issues these grandparents face in surrogate parenting. The sample consists of 69 grandparents(63 grandmothers and 6 grandfathers) of grandchildren who have lived with them at least 6 months without their parents. The grandparents raising grandchildren reported that caregiving bought negative changes in relations with their own children, economic situation and health condition. They concerned most about the issue of good mannered behavior of grandchildren and making good friends and needed social supports in terms of house cleaning and cooking and transportation for after school activities. Caregiver burden was related to the number of family members, the degrees of life changes, economic hardship due to caregiving, grandchildren's school performance and the relations with granparents' own children.
