Estimating the Willingness-To-Accept for Cash Benefit of Long-Term Care Insurance (노인장기요양보험제도의 현금급여 도입 필요성 - WTA를 통한 적정 현금급여액 추정 -)

  • Korea government has launched long-term care insurance from 2008. However, one of the most important issues, whether or not providing cash benefit, is still unresolved. In this paper, in order to provide policy guidelines for the long-term care insurance, I attempt to estimate the Willingness-To-Accept (WTA) of the cash subsidy for informal care by using Double Bounded Dichotomous Choice method, a branch of Contingent Valuation Method (CVM). In doing so, I also estimated the determinants of the preference for cash benefit. Data were obtained from face-to-face survey interviews with 300 informal care-givers at three major general hospitals in Seoul, Korea. The questionnaire was constructed with two scenarios (mild/severe symptom). The results from logistic regression analyses and the estimation of WTA indicate that informal care-givers are willing to accept the cash benefit as low as 628 thousands won for mild fragile elderly and 1,072 thousands won for severe fragile elderly. The strength of this paper is that I estimated the WTA of the cash benefit by reflecting the changes in preferences of informal care-givers. The analytic results from the this paper suggest that the cash benefit in long-term care insurance is indispensible in achieving the goal of the long-term care system.

A Study on Willingness by Doctors and Patients to Use Hospital Based Home Nursing Service (병원중심 가정간호사업의 이용의사에 관한 조사 연구)

  • This descriptive study was conducted to determine the awareness by doctors and patients of the hospital based home nursing service, and the willingness of the patients as well as the doctors to use the service if provided. The convenience samples consisted of 240 hospitalized medical-surgical patients, and 64 doctors from one' hospital. The data was collected from June 1 to June 30, 2000, by questionnairs and interviews. The results of the study were as follows: 1. 57.4% of the subjects were male patients. 35.8% were over 60 years old. and incomes ranged from 1.000,000 to 1.990,000 Won per month in 33.2% of the subjects. 2. 61.3% were unaware of home nursing. 52.7% of the subjects got information about home nursing via TV, magazines, and newspapers, and 83.4% of them showed a willingness to use the service if available. For doctors, 87%, of them said that they were aware of home nursing, and 84.4% viewed the service as necessary. However, only 39.1% responded that they are willing to refer their patients to the home nursing service if it is available. 3. Those who know about the service perceived it more advantageous; however, many do not know about it. Based on the above findings, the following suggestions are made: (1) It is necessary to hold a workshop or a seminar about hospital based home nursing service to draw attention of hospital personnel. (2) There is a need to publicize home nursing service to patients and their family members. (3) Further research is necessary to analyze cost/effectiveness of a hospital based home nursing service for the hospital.

The Effect of inpatients' Experience on Patients' Satisfaction and Willingness to Recommend Hospital (입원경험이 환자의 만족도와 병원추천의향에 미치는 영향)

  • This study examined to identify the factors influencing on inpatients' satisfaction with hospitalization and willingness to recommend hospital to others. Data from the 2018 National Patient Experience Survey were used for the analysis. Of the 593 patients experiencing inpatient services, multivariate linear regression analysis was conducted to explore the factors such as physician care, nursing care, facility and administrative support, and healthcare system on inpatients' satisfaction after controlling for their socio-demographic and health related factors. The study found that physician care, nursing care, administrative support and healthcare system were related to inpatients' satisfaction and willingness to recommend hospital. To improve inpatients' satisfaction, it is necessary to improve healthcare professionals' overall patient-centerdness attitude, user friendly hospital facilities and administrative support services and efforts to trust and satisfaction on healthcare system from the patients' perspectives.

An Analysis on Willingness to pay for Environmental-Friendly Agri-Product Food Service of Child-care Centers (보육시설 친환경농산물 급식에 대한 지불의사 분석)

  • The objectives of this study were to analysis willingness to pay (WTP) for environmental-friendly agri-product (EFAP) food service of child-care centers. To analysis preference about EFAP and WTP, parent' survey were conducted residing in Asan city. The sample size of the survey is 143 respectively. As a result, most of parents really recognized that EFAP is good for their children as well as they want to change food material from general agri-product to EFAP. Moreover, additional willingness to pay fur EFAP food service is 8,360 won on average per month. Therefore, environmental-friendly agri-policy to be more focused on the promotion of the consumer's interests and food security needs.

The Want, its Determinants and the Willingness to Pay of the Long Term Care Service (장기요양 서비스를 누가, 얼마나, 얼마에 원하고 있는가? - 장기요양 서비스의 욕구와 결정요인 및 지불의사금액 -)

  • Before introducing the national long-term care insurance in 2008, the want for long term care service has to be estimated and analysed. This study estimates the demand and analyses what determines the want of long term care service. This study investigated data of 3f6 elderlies, that was collected by age stratified random sampling. The elderies resided in Onyang 4 - dong (urban area) and Dogo-myun (rural area) In the city of Asan. The researchers visited the elderlies and their care giver, and assessed their demand for the long term care service and examined physical, mental, socio-economic status by the assessment tools for Korean Long-Term Care System. $64\%$ of the those who are entitled to be served refuse the long term care service. $26.7\%$ of them wants for home care service and $7.9\%$ want facility care service. It is estimated that the want of home care service are three or four times as much as that of facility care service. The demand for long term care service is 5.155 times higher for those who live in rural area (p=0.000), 3.040 times higher for those who do not have spouse(p=0.057), and 3.356 times higher for the people who is in medicaid than medical insurance(p=0.029). However, income(p=0.782), means(p=0.614), living alone(p=0.223), number of family to live with (p=0.341) and age of the elderly(p=0.420) are not related with the demand of long term care service. The assessment tools for Korean Long-Term Care System for need evaluation of the long term care service can reflect the demand well.(p=0.024) If medical care will cover $80\%$ of total cost, the willingness to pay of the out of pocket money of the people with medical insurance is 67,400 Korean Won(66.77 US$) for the home care service and 182,500 Korean Won(180.78 US$) for the facility care service. There is possibility that long term care demand is still small after Introducing the long term care Insurance due to the care given by family members. When developing service delivery system of long term care insurance, rural area has to be given more consideration than urban area because of the higher demand. The people who do not have spouse or are in medicaid have to be given special consideration as well.

  • The purpose of this study is to identify the effect of the innovation characteristics that AI Elderly Care Robots have on elderly consumers and to verify the acceptance or mediated resistance by the elderly of the innovation characteristics. Research was conducted throughout this study of the characteristics of the elderly so that the study results could contribute to the improvement of the quality and manufacture of AI Elderly Care Robots by lowering the psychological resistance of the elderly. Survey questions were answered by senior citizens aged 65 and older. The SPSS25 and AMOS22 programs were used for analysis. The analysis confirmed that innovation resistance has a significant mediated effect on the characteristics of elderly consumers and their willingness to accept the innovation characteristics of the AI Elderly Care Robots. Through this study, Ram's innovation resistance model has been verified empirically and it is estimated that by considering the specificities of elderly consumers and using the information to contribute to the development and modification of AI Elderly Care Robots will increase the willingness of the elderly to accept the innovation characteristics of the AI Elderly Care Robots.

Relationship between Characteristics of Lengthy Hospital Stay Patients, Knowledge of Transfer Needs and Their Willingness to Transfer - Strategies for the Effective Transfer of Lengthy Hospital Stay Patients - (장기재원환자의 특성 및 전원 인지도와 전원 의향과의 관계 - 장기재원환자의 효율적 전원을 위한 전략 제시 -)

  • Background : It is very common in Korea to take care of non-acute patients in an acute setting, due to the lack of long-term facilities. Long term hospitalization increase medical expenses and decreases the bed utilization, which can affect the urgent and emergent admissions, and eventually jeopardize the hospital financially. In this study, strategies for effective transfers to the lower levels of care, and to decrease the length of stay were presented by surveying and analyzing the patient's knowledge of the transfer needs, and the willingness to transfer those whose hospital length of stay was more than 30days. Method : The survey is subject to a group of 251 patients who have been hospitalized over 30 days in a general hospital in Seoul. Excluding those that were in the Intensive Care Unit and psychiatric ward, 214 in-patients were used as participants. They were surveyed from April 9, 2002 to April 17, 2002. One hundred and thirty seven out of 214 were responded which made the response rate 64%. Data were analyzed by SAS and SPSS. Result : Multi-variable Logistic Regression Analysis showed a significant effect in medical expenses, knowledge of referral system and the information of the receiving hospital. The financial burden in medical expenses made the patient 10.7 times more willing to be transferred, knowledge of the referral system made them 5 times more willing to be transferred, and the information of receiving hospital makes 6.5 times more willing to be transferred. Reasons for willing to be transferred to a lower level of care were the phase of physical therapy, the distance from home, the attending physician's advice and being unable to be treated as an out patient. Reasons for refusing to be transferred were the following. The attending physician's competency, not being ready to be discharged, not trusting the receiving hospital's competency due to the lack of information, or never hearing about the referring system by the attending physician. Conclusion : Based on this, strategies for the effective transfer to the lower levels of care were suggested. It is desirable for the attending physician to be actively involved by making an effort to explain the transfer need, and referring to the Healthcare Coordinating Center, which can help the patient make the right decision. Nationwide networking for the referral system is the another key factor that may need to be suggested as an alternative to decrease the medical expenses. Collaborating with the Home Health Agency for the early discharge planning and the Social Service Department for financial aid are also needed. It is recommended that the hospital should expedite the transfer process by prioritizing the cost and the information as medical expenses, knowledge of referring system and the information of the receiving hospital, are the most important factors to the willingness to transfer to a lower level of care.

Willingness to pay for eco-friendly products: case of cosmetics

  • Environmental concern has been an important issue for a few decades, and the extent of consumer demand for eco-friendly consumption has been increased. This study seeks to investigate consumers' willingness to pay (WTP) a premium for eco-friendly cosmetics. This study evaluates how much more a consumer is willing to pay for eco-friendly cosmetics and examines significant factors influencing consumers' WTP for eco-friendly cosmetics. Consumers' WTP is measured using four different ecofriendly cosmetics: low-priced skin care cosmetics, low-priced makeup cosmetics, high-priced skin care cosmetics, and high-priced makeup cosmetics. This study uses the contingent valuation method (CVM) to estimate consumer's WTP for eco-friendly cosmetics. Survey questions were designed using both dichotomous choice and payment card method of CVM. Through face to face interviews and on-line surveys, the data were collected from women between 20 and 49 years old residing in Seoul and Kyeonggi area, Korea, in May 2010. A total of 226 questionnaires (132 from interviews and 94 from on-line) were included for the analytical sample in this study. The data were analyzed using descriptive analysis, T-test and Log-Logit analysis. The findings are as follows: First, the WTP measured by dichotomous choice method was estimated using the Log-Logit analysis. The results showed that the estimated WTP for low-priced skin care cosmetics was 19,152 won, which was 27.7% higher than the reference price, 15,000 won. For low-priced makeup cosmetics, the estimated WTP was 18,524 won, and its green premium was 21.0%. The estimated WTP for high-priced skin care cosmetics was 59,128 won, which was 18.3% higher than the reference price, 50,000 won. For high-priced makeup cosmetics was 57,666 won, and its green premium was 15.3%. Second, the WTP measure by payment card method was estimated using descriptive analysis. The results showed that the respondents were willing to pay 17,955 won for low-priced skin care cosmetics, which was 19.7% higher than the reference price, 15,000 won and 17,595 won for low-priced makeup cosmetics, which was 17.3% higher than the reference price. For high-priced skin care cosmetics, the average WTP was 56,950 won which was 13.9% higher than the reference price, 50,000 won. For high-priced makeup cosmetics, the average WTP was 55,650 won, which was 11.3% higher than the reference price. Overall, the WTP was higher in order of low-priced skin care, low-priced makeup, high-priced skin care, and high-priced makeup. It means that consumers decide degree of premium based on the price and the attributes of eco-friendly products. Third, the findings showed that age, monthly income, and having children or not were statistically significant factors that influenced consumers' willingness to pay for eco-friendly cosmetics. Other explanatory variables such as education, marital status, job, purchase experience of eco-friendly products, and environmental concerns did not show any statistical significance. The major contribution of this study is the investigation of the value of green attributes of the products by using CVM. Unlike most previous researches, this research used two methods of CVM, the dichotomous choice and the payment card, so it enhanced the reliability of research. According to this study, consumers showed price sensitivity when they pay green premium. These findings can be used as useful information to establish marketing strategies for green cosmetics.

Demand and willing to pay for oral hygiene service in long-term care insurance of elderly (노인장기요양보험 재가급여 수급자의 구강위생서비스 요구도와 비용지불의사)

  • Objectives: This study was conducted to identify the demand and willingness to pay for oral hygiene services among elderly people with long-term care insurance. Methods: Our study was a cross-sectional analysis. Subjects comprised 126 elderly individuals from long-term home-care centers. A total of 28 centers were selected through convenience sampling from among 78 centers in ${\bigcirc}{\bigcirc}$. For analysis, semi-structured questionnaires that required about 20-30 minutes to complete were used. Analysis was performed using SPSS 23.0 software. Results: The overall demand for oral hygiene services was 44.4%, and willingness to pay was 31.0%. Thirty-three people (58.9%) of elderly those who have demand for an oral hygiene service were willing to pay for the service, and 64 people (91.4%) who did not have a demand were not willing to pay for it. Among those with partial dependence on brushing, 65.6% had demand for oral hygiene services and 50.0% were willing to pay costs. Among basic livelihood beneficiaries, 69.6% were willing to pay for oral hygiene services; general subjects and relievers were less willing to pay. Conclusions: The overall demand for oral hygiene services among elderly people was 44.4%, and the willingness to pay was as low as 31.0%.

The Moderating Effect of Optimism on the Relationship between Depression and Willingness for Self-Sufficiency in Self-Sufficiency Program Participating Workers (자활사업 참여 근로자의 우울과 자활의지 간의 관계에서 낙관성의 조절효과)

  • The purpose of this study was to verify the moderating effects of optimism on the relationship between depression and willingness for self-sufficiency in self-sufficiency program participating workers. For this study, a sample of 775 self-sufficiency program participating workers completed depression questionnaire, optimism scale and willingness for self-sufficiency scale. The data was analyzed using the Baron & Kenny's moderating effect verification method. The results of the analysis showed that depression had a significant negative effect on willingness for self-sufficiency, and optimism had a significant positive effect on willingness for self-sufficiency. The results verified the moderating effects of optimism on the relationship between depression and willingness for self-sufficiency. On the basis of the results, We discussed that optimism can reduce the negative effect of depression on willingness for self-sufficiency and improve willingness for self-sufficiency. We discussed the optimism promotion program as an intervention method to improve willingness for self-sufficiency in self-sufficiency program participating workers.
