A Study on the Policy Direction according to the Perception Change of the Welfare Service of the Aged

  • This study is about establishing the policy direction according to the perception change of the welfare service of the aged. To achieve this goal, this study analyzed the reprocessed result of the survey related to the welfare of the aged among the second and third community social welfare plans of D county, Southern Jeolla Province. The result shows the recipients of the welfare service of the aged have higher perception about spreading the hardware side such as the welfare facility of the aged than about practicing the software policy such as elder care services. It suggests significant implication in deciding the priority of the welfare policy of the aged from now on. But this result is confined in D county, so getting the generalized conclusion needs the comprehensive review toward more broad regions hereafter.

Determinants of Contingent Workers' Ratio in Public Health Centers (보건소 비정규직 고용 비율에 영향을 미치는 요인)

  • Objectives : This study investigates the determinants of contingent workers' ratio in public health centers. Since the economic crisis in 1997, there have been many studies on contingent workers in Korea. But, previous studies have been not conducted focusing on public health center. Methods : This study used 253 public health centers, installed and operated since December 31, 2008. in Korea as units of analysis. To examine the determinants of contingent workers' ratio, this study uses Pearson correlation and multiple regression analysis. Results : The following appeared as significant variable affecting contingent workers' ratio in public health centers; degree of the local government's financial independence(p<0.001), rate of increase/decrease in ages 65 and over(p<0.001), rate of increase/decrease in basic livelihood security recipients(p<0.01) and rate of increase/decrease in registered disabled persons(p<0.01). In contrast, internal organizational environment characteristics related variables were not statistically significant. Conclusions : Contingent workers' ratio in public health center is significantly affected by financial vulnerability of the local government and increase in demand of health care services.

Telemedicine Cooperation Experience of Nurses Working in Remote Areas (의료취약지 근무 간호인력의 원격협진 수행 경험)

  • Purpose: This study was conducted to explore the telemedicine cooperation experience of nurses working in remote areas. Methods: A focus group interviews were used to collect data. All interviews were recorded and transcribed. Content analysis was used to analyze the data. Results: The three main categories and seven sub-categories of telemedicine cooperation experience that emerged are 1) requirement of education on remote support service, 2) consideration of the recipients of medical support services and the characteristics of the area, and 3) difficulties in conducting telemedicine cooperation. Conclusion: As a result of the study, legal protection should be given priority, and it is necessary to select an area where remote cooperation is essential, to discover subjects, and to reduce the burden of work and division of manpower and duties.

The Effects of Homeownership on Welfare Perceptions (자가소유가 복지인식에 미치는 영향)

  • Homeownership is much more than a financial calculation. It is a socio-psychological phenomenon. During the last decade studies of homeownership have demonstrated a positive link between homeownership and positive social outcomes including welfare perceptions and attitudes. This study compared welfare perceptions between homeowners and renters in Korean Welfare Panel Survey using data collected in 2019. The study analyzed six continuous outcomes of welfare perceptions. Six variables indicating welfare perceptions included: (1) welfare and work disincentives; (2) welfare recipients are lazy; (3) welfare selectivism; (4) national minimum; (5) free university education and; (6) free child care and kindergarten. This study acknowledged bias arising from selection on observables. To control these biases, this study employed analytical approaches including the treatment effect model. Results of this study confirmed that attainment of homeownership is not possible for everyone. Findings from this study have provided some evidence that homeowners and renters have different perceptions on welfare listed above. Finally, issues relating to homeownership and welfare perceptions were discussed. Implications for social work practice and research were further discussed.

Community care perceived by medical welfare service providers and residents dwelling in the community (보건의료복지 서비스제공자와 지역주민의 커뮤니티케어에 대한 인식)

  • The purpose of this study was to investigate and compare health, medical and welfare service providers' and local residents' perception on community care. The participants were 68 service providers and 95 local residents. The data were analyzed using independent t-test and chi-square test. The results showed that the degree of agreement of service providers on the need for policy, and visiting services were higher. Service providers' ability to link local residents with a need for care to community resources was higher, but in the activation of self-help group in community, local residents agreed more. In add budget of policy to health insurance, local residents agreed less. In perception on priority for community care policy, high priorities were preparation of financial resources and care culture. To effectively operate community care policy, it is necessary to establish a care culture through active exchanges for reducing differences in policy and opinions of recipients.

Factors Affecting Heart Rate Variability in the Industrial Workers (사업장 근로자의 심박동 변이도에 영향을 미치는 요인)

  • The purpose of this research is to seek for efficient health maintenance device and to suggest desirable daily habit, based on the inquiry on interrelationship between workers' daily lives and their heartbeat change level. The paper survey about general features, case history and daily habits was conducted on workers during medical examination in Jeollabukdo, and examined their change of heartbeat as well. The results of research deducted from data analysis are as follows; 1. There found very positive interrelations between time-domain analysis and frequency-domain analysis, and MHR and LF/HF ratio had negative connection with other analyses. 2. The recipient showed high time-domain analysis when they are younger, have worked shorter or have spouse, and it contributes to stable sympathetic nerve and parasympathetic nerve as it stimulates autonomic nervous system. 3. According to the result of frequency-domain analysis, recipients showed higher TP and LF when they are younger, and the highest HF when they are under 34.The level of VLF was higher for university graduates than the ones who finished high school. The recipients showed higher TP and HF when they don't have spouse, and lower TP, LF and HF when they have worked longer. 4. The level of RMSSD and TSRD was high for the people who don't have case history, and HF was high when they don't have any disease in progress. 5. According to the result concerning correlation of daily habits with time-domain analysis and frequency-domain analysis, cigarette, alcohol and sleeping hours don't affect heartbeat change, but the ones who regularly workout showed higher result in every analysis. It shows that the autonomic nervous system of recipients who regularly exercise response more actively. The result mentioned above suggests that the change of heartbeat is a direct index which shows the change of autonomic nervous system, and it depends on the exercise the most. Thus, workout is proved to be the best method in order for workers to take care of their health.

Factors Affecting Treatment Adherence of Kidney Transplantation Recipients (신장이식 환자의 치료지시이행에 영향을 미치는 요인)

  • This study is an explanatory research conducted to analyze the influencing factors of treatment adherence in kidney transplant recipients. The subjects were 132 renal transplant recipients who visited the outpatient department in a university hospital. Data were collected from July 17, 2017, to August 22, 2017, and analyzed using SPSS WIN 24.0. Frequency, percentage, mean and standard deviation, variance analysis, correlation, and regression analysis were performed. The results of the study showed that there was a significant difference in the treatment adherence based on the age, religion, and the time passed since kidney transplantation. The study also found that the treatment adherence had significant positive correlations with social support (r=.54, p<.001), family support (r=.43, p<.001), health provider's support (r=.57, p<.001), and self-care knowledge (r=.21, p=.015). The factors influencing the treatment adherence were health provider's support, the time passed since kidney transplantation, spouse, and religion. The final explanatory power of the model was 41.9%. In conclusion, intervention strategies should be introduced to promote the support of healthcare providers in order to improve the adherence of the kidney transplantation patients.

Evaluation of u-Healthcare Demonstration Project in Sungnam (성남 u-헬스 시범사업의 평가)

  • To test if the developed ubiquitous health care devices working well and vital information could be collected and monitored systematically through internet and to test if the devices and services could be used further. Kyungwon University, KT Co., Gil Medical Center, LIG Nex1 Co., and Sujeong Health Center conducted an ubiquitous health care demonstration project in Sujeong-Gu, Sungnam, Korea from Mar. 5 to May 16. We developed and applied several medical devices to monitor health of the elderly in their houses through internet. The devices were sphygmomanometer, glucometer, body fat scale, Health Pad, and activity sensor. We distributed the devices to 20 recipients of home care and 7 diabetes patients. After received the devices and were explained how to use them, they used the devices in their houses. The vital signs of the residents were monitored through internet. A nurse monitored and consulted their vital signs in the monitoring center in Kyungwon University during the demonstration period. The consultant called them and consulted on their blood pressure, blood sugar level, and body fat after a few seconds they used the devices as well as provision of recommended contents such as diets and activities through Health Pad. To investigate cognition and satisfaction of the participants for the devices, we surveyed the participants at the end of the demonstration period. For the change in blood pressure, blood sugar level, and activities, we conducted statistical test. After the demonstration period. cognition and satisfaction for the devices and change in blood pressure, blood sugar level, and activities were evaluated. Most of the participants were acknowledged how to use the device and satisfied with the use of the devices. The internet monitoring and services are considered to be promising because most of the participants were satisfied especially because somebody was monitoring their health status. However some weaknesses such as short battery life of the activity sensor, lack of connection of consultations with hospitals, and low understanding on usage of some of the devices need to be complemented.

The Effects of National Health Insurance Denture Coverage Policies for the Elderly on the Unmet Dental Needs of the Edentulous Elderly

  • As the elderly population increases, they are increasingly affected by oral health problems. Therefore, efforts are being made to improve the oral health of older people, alleviate mental discomfort, and reduce unmet dental needs. This study was conducted to confirm the relationship between the National Health Insurance Elderly Denture Coverage and the unmet dental need for the edentulous elderly, as part of the protection policy. We analyzed the 2011 and 2013 Community Health Survey data of the edentulous elderly, aged 75 years or older, before 2012. In order to more precisely confirm the effects of the denture donation policy on unmet dental care, basic life recipients who were subject to the free elderly prosthetic project were excluded from the analysis. The final analysis included 20,400 subjects. According to our investigation of the factors that affect the unmet dental needs of the elderly, the National Health Insurance Elderly Denture Coverage did not affect unmet dental needs. The statistically significant variables that affected the unmet dental needs of the elderly were education and income levels, which are representative socioeconomic status variables. The lower the level of education, the unhealthier the dental care experience, and income levels showed a similar tendency. The elderly who have a low socioeconomic status are more likely to experience unmet dental needs because they lack the knowledge and socioeconomic ability to pay for dental care. Therefore, the policy for health protection of the entire elderly population should be continuously expanded. In addition, the socioeconomically vulnerable groups may have health problems due to the restriction of medical use, which may lead to quality of life deterioration.

Development of Job Description of HIV/AIDS Counselling Nurse in Korea (에이즈 상담간호사 자격제도 개발을 위한 직무 분석 - 포커스 그룹 연구방법과 데이컴 기법을 중심으로 -)

  • Purpose: The aim of this study was to analyze the needs of HIV/AIDS care and to develop the job description of HIV/AIDS Counselling nurse in Korea. Methods: The needs assessment was done by focus group interview with HIV/AIDS care recipients who are 16 persons living with HIV/AIDS and four their family members, seven persons from HIV/AIDS high risk group, and five nurses working on HIV/AIDS clinics. Based on the result of needs assessment, job description was developed using the DACUM. Mail survey was done to identify the frequency, importance, and difficulty of duties, tasks, and task elements. Results: The job description was classified under 8 duties, 36 tasks, and 290 task elements. Duties were categorized as needs assessment, health promotion and quality of life, improvement of treatment compliance, symptom care, health education, resource network, administrative activity, and career development. The importance of all duties and tasks showed high score from the survey, but the frequency and the performance level were middle range. Conclusion: It is suggested that the special training program based on the job description needs to be developed. In the political aspects, the introduction of HIV/AIDS nurse specialist certification could be considered.
