요양보호사의 서비스 질에 미치는 영향요인 (Influencing Factors on Service Quality of Care Helpers Working for the Elderly in Long Term Care)

  • Purpose: The purpose of this study was to verify influencing factors on service quality provided by care helpers working for the elderly in long term care. Methods: The data were collected using self-reported questionnaire from 221 care helpers working in long-term care facility or home care service center. The data were analyzed using multiple regression with the SPSS/WIN 17.0 program. Results: There were significant differences in service quality depending on the health status, intimacy level, pleasure level, job education, and turnover intention. Factors influencing service quality were gender, intimacy level with elderly, type of working place and job satisfaction with $R^2$ value of 17.3%. The most influencing factor was intimacy level (${\beta}$=.249), followed by job satisfaction (${\beta}$=.208), gender (${\beta}$=.170), and type of working place (${\beta}$=.146). Conclusion: The results of this study indicate that the effort to improve the service quality of care helper should be focused on helping intimacy building between the elderly and the care helper. Also, a more effective way to improve service quality would be intervening for care helpers providing institutional care.

장기요양수급 대상 보호자와의 의사소통이 요양보호사의 직무만족에 미치는 영향 (Effect of Communication with a Guardian of a Recipient of a Long-term Care Program on Job Satisfaction of Care Helpers)

  • 본 연구는 재가 장기요양시설 요양보호사가 요양서비스 제공에 있어서 보호자와의 관계로 인해 어려움을 겪고 있는 상황에서 수급노인 보호자와의 의사소통이 요양보호사 직무만족에 미치는 영향을 분석하여 요양보호사의 근무여건개선에 근거자료로 삼는데 목적이 있다. 분석결과는 다음과 같다. 첫째, 요양보호사들은 하루 4시간 근무하는 계약직으로서 급여가 적은 경우에 직무만족도가 높고, 보호자와의 의사소통도 잘되는 것으로 나타났다. 요양보호사들의 대다수가 가족요양을 목적으로 자격을 취득하였고, 특정 기관에 귀속되기 보다는 유연하게 근무하는 것을 선호하기 때문이다. 둘째, 수급노인 보호자가 남성으로 배우자 혹은 자녀인 경우, 연령이 높은 경우, 보호자로부터 부당한 요구를 받지 않는 경우에 직무만족도가 높고, 의사소통이 잘되는 것으로 나타났다. 요양보호사의 대부분이 법규에 규정된 역할이 아닌 과도한 가사지원을 보호자와 대수급자로부터 요청을 받기 때문이다. 셋째, 수급노인 보호자와 의사소통이 잘되어 관계가 좋을수록 직무만족도가 높았다. 요양보호사 직무만족에 보호자와의 의사소통이 중요한 만큼, 수급노인 보호자를 대상으로 하는 의식교육, 요양보호사를 위한 의사소통 방법에 대한 교육프로그램 운영이 필요하다. 아울러 요양보호사와 보호자가 친밀감과 의사소통을 잘할 수 있도록 요양보호사와 보호자 사이의 관계를 규정한 업무매뉴얼도 마련하여 제공할 필요가 있다.

경기도 S시 노인의 신체적 및 도구적 일상생활수행능력에 근거한 장기요양시설 보호대상자 추계 (Estimation of Elderly Needing Long-term Care in S-city Gyeonggi-do according to ADL and IADL)

  • 이 연구는 경기도 S시 노인의 신체적 및 도구적 일상생활수행능력에 따른 장기요양보호 대상을 파악하고 이에 근거한 S시의 장기요양시설의 필요 병상 및 규모를 추계하기 위한 연구이다. 장기요양시설의 필요 조사는 경기도 S시 각 동별 65세 이상 노인의 2.7%를 층화 무작위 추출을 이용하여. 최종 397명을 연구대상자로 선정하였다. 장기요양시설의 필요는 신체적 일상생활수행능력과 도구적 일상생활수행능력을 이용하여 장애정도를 평가하는 OECD과 한국보건사회연구원의 기준을 이용하였다. 연구결과 OECD의 기준에 의하면 정기요양시설 입원 대상자수기 4,716명(31.2%)이었고, 보건사회연구원 기준에 의하면 1.739명(11.5%)이었다. 따라서 S시에 필요한 장기요양시설은 1,739병상에서 4,716병상이었다 이 연구는 한 지역을 대상으로 하여 객관적인 기준에 의해 필요를 추정하고, 이에 근거하여 장기요양시설의 규모를 추계하였다는데 그 의미가 있다.

건강가정지원센터 아이 돌봄 활동가의 '돌봄 노동' 방향성 연구 (A Study on 'Care Work' Directionality of Child Care Helper in Healthy Family Support Centers)

  • The purpose of this study is to examine the motive of participating in the care work, the overall cognition survey for the care work and the infant care practice areas required in the care work for the child care helpers who activate in the care work, and search for methods of specializing infant care and care experiences on the results. Results of study showed that firstly, middle- or the late middle-age women participated in the care worker recognized care work as productive labor to create economic value and that secondly, the child care helpers' recognition degree of self-development and job satisfaction through performance of care work was wholly high level and that there was will of participation in training for self-development but actual participation rate was low due to restrictions by general conditions. Thirdly, the infant care practice areas and their particular contents that the child care helpers recognized in the care work necessary were highly shown in the whole. The requested level of education for safety management, play guidance, humanity guidance for infant and child was high. Point to discuss is that occupation stability and occupation image must be raised through the public job-hunting system for the child care helpers. In addition, a tailor-made education support suitable for the child care helpers' career stage is needed. Education support by the performance of infant care practice area and the educational requirements must be provided to major rearers to ensure that they can function as behavioral and emotional supporters. Finally, a support system for continuous self-management.

자가 신장 운동이 요양보호사의 근골격계 증상 및 직무 스트레스에 미치는 효과 (Effect of a Self-Stretching Exercise on Musculoskeletal Symptom and Job Stress for Care Helpers)

  • PURPOSE: The purpose of this study was to research the effect of self-Stretching Exercise on musculoskeletal Symptom and job Stress for care helpers who work at geriatric hospital and come up to NIOSH standard. METHODS: The effect of self-Stretching Exercise was measured using 'musculoskeletal symptom survey table' and 'Job stress measurement scale for korean' from the Korea Occupational Safety and Health Agency(KOSHA) for 40 care helpers. The paired t-test and independent t-test was used for statistical analysis. RESULTS: The pracitce of Self-Stretching Exercise was showed significant effect reducing the stress for care helper's total job Stress and inappropriate compensation, job instability, job requirement, relationship conflict, physical environment(p<.05), and significant difference for their total score, relationship conflict, structure and organization, physical environment, inappropriate compensation improvement on variations on job stress in groups(p<.05). However, self-Stretching Exercis was showed no significant effect on physcial burden. CONCLUSION: The self-Stretching Exercise was the most effect way to reduce Musculoskeletal Symptom and job Stress for a care helpers through the guidance of physical therapist.

서울시 아이돌보미의 직무 만족도에 관한 연구 (A Study on the Job Satisfaction of Child Care Helpers in Seoul City)

  • This study aimed to investigate the job satisfaction of child care helpers who are service providers in a child care support project. For this study, 302 child care helpers in ten districts within Seoul participated in the present study. A survey was conducted to collect data concerning their job satisfaction. Data were analyzed with descriptive statistics and regression using the SPSSWin 16.0 version. The results of this study were as follows. Generally, child care helpers had a high degree of job satisfaction. In addition, they exhibited the highest degree of satisfaction in relationships and the lowest satisfaction in wages. Furthermore, they demonstrated varying degrees of job satisfaction depending on their individual characteristics, activity characteristics, and activity recognition. The results of the present study can provide useful implications in regards to successful child care support projects.

가정간호환자의 일상생활작동수행능력 변화에 대한 연구 (A study on Changes in ADL Functioning of Residents in Taejon City)

  • This study was done to investigate the ADL differences between before and after home care. For this survey, the Barthel index, an ADU(activities of daily living) assessment, and general history questions were asked. Functional performance, i.e. ADL, was studied in a population a total of 56 men and women aged 65 and older from the city of Taejon. Among the independent subjects, women, 73 years of age and older, married status, 1-2 times taking home care per month, the case answering 'Quite' about satisfactory of home care, elderly dwelling with others and who have helper and spouse, elderly having a snack regularly, are statistically significant. Also ADL differences were found in grooming, getting in and out chair, getting on and off toilet, walking 500 meters on the level. Further studies should evaluate the activities of daily living to predict important disability-related outcomes.

노인장기요양보험제도에서의 가족수발자의 경험 (The Experiences of Family Caregivers under the Long-term Care Insurance)

  • Purpose: The purpose of this study was to explore the experiences of family caregivers who care for the elderly under Long-term Care Insurance. Methods: Data were collected using focus group interviews and analyzed using a phenomenological approach. The four focus groups consisted of eight caregivers, two social workers and three nurses in B city, Korea. Results: Five themes were identified: 'Obtaining a care-helper certification for employment', 'Taking care of the elderly in their homes', 'Difficulties due to life changes', 'Difficulties due to reduced wages' and 'Dissatisfaction with the Long-term Care Insurance operating system'. Conclusion: The results of this study demonstrate that the long-term care system for family caregivers faces many systematic challenges in providing care for the elderly harmoniously in their home. To help them succeed in their tasks, Long-term Care Insurance system must offer respite and support programs to family caregivers.

방문 요양보호사의 대인관계가 직무만족에 미치는 영향 (The Effects of Interpersonal Relationships on Job Satisfaction of Home-visiting Care Helpers)

  • 본 연구는 방문 요양보호사의 대인관계가 직무만족에 미치는 영향을 알아보기 위한 것으로, 방문 요양보호사들의 대인관계는 대인서비스적 업무 특성을 고려하여 수급자, 보호자, 동료, 기관장과의 관계로 구분하였다. 대전광역시의 방문요양서비스 제공기관에 근무하는 요양보호사 336명을 연구대상으로 하였으며, 수집된 자료는 SPSS 22.0프로그램을 이용하여 신뢰도분석, 빈도분석, 상관관계분석, 위계적 회귀분석을 실시하였다. 최종 분석 결과, 인구사회학적 특성의 영향력은 유의미하지 않았으나, 업무수행관련 특성 중 업무비중 우선순위, 이직경험, 직업선택이유는 직무만족에 유의미한 영향을 주는 것으로 나타났다. 특히 보호자와의 관계를 제외한 대인관계 요인은 영향력이 가장 큰 변수로서 직무만족에 유의미한 영향을 미치는 것으로 나타났다. 영향력은 기관장과의 관계, 수급자와의 관계, 동료와의 관계 순으로 컸으며, 관계가 좋을수록 직무만족도가 높았다. 본 연구결과는 방문 요양보호사의 직무만족을 향상시키기 위해, 기관 내외적으로 원만한 대인관계를 형성 할 수 있는 환경을 조성할 필요가 있음을 보여주고 있다.

노인장기요양보험 제도 하에 제공되는 방문간호 서비스의 중요도와 성과도 (Importance and Performances of Visiting Nurse Services Provided under the Long Term Care Insurance System for the Elderly)

  • Purpose: This study was attempted to provide quality improvement and revitalization of visiting nurse services by Importance-Performance Analysis of persons being provided with the services. Methods: The subjects were 350 people being provided with visiting nurse services from a long term home care institution in the kangwon Province during the period of data collection between Aug. 16 and Sep. 15, 2011. Data analysis was conducted through t-test, ANOVA, and IPA by using the SPSS/WIN 12.0 program. Results: As a result of the Importance-Performance Analysis of visiting nurse services, it turned out that the strength on reliability, assurance, and empathy should be maintained with good work and the aspects of responsiveness appeared to be intensively improved, and the tangibility proved to be subject to improvement. Conclusion: For the activation of visiting nurse services, methods for improving the performance in the intensive care area and management strategy establishment which highlights the advantages of strength maintenance area are needed.
