Molecular imaging is used to improve the disease diagnosis, prognosis, monitoring of treatment in living subjects. Numerous molecular targets have been developed for various cellular and molecular processes in genetic, metabolic, proteomic, and cellular biologic level. Molecular imaging modalities such as Optical Imaging, Magnetic Resonance Imaging (MRI), Positron Emission Tomography (PET), Single Photon Emission Computed Tomography (SPECT), and Computed Tomography (CT) can be used to visualize anatomic, genetic, biochemical, and physiologic changes in vivo. For in vivo cell imaging, certain cells such as cancer cells, immune cells, stem cells could be labeled by direct and indirect labeling methods to monitor cell migration, cell activity, and cell effects in cell-based therapy. In case of cancer, it could be used to investigate biological processes such as cancer metastasis and to analyze the drug treatment process. In addition, transplanted stem cells and immune cells in cell-based therapy could be visualized and tracked to confirm the fate, activity, and function of cells. In conventional molecular imaging, cells can be monitored in vivo in bulk non-invasively with optical imaging, MRI, PET, and SPECT imaging. However, single cell imaging in vivo has been a great challenge due to an extremely high sensitive detection of single cell. Recently, there has been great attention for in vivo single cell imaging due to the development of single cell study. In vivo single imaging could analyze the survival or death, movement direction, and characteristics of a single cell in live subjects. In this article, we reviewed basic principle of in vivo molecular imaging and introduced recent studies for in vivo single cell imaging based on the concept of in vivo molecular imaging.
Background: Myocardial fibrosis (MF) is an advanced pathological manifestation of many cardiovascular diseases, which can induce heart failure and malignant arrhythmias. However, the current treatment of MF lacks specific drugs. Ginsenoside Re has anti-MF effect in rat, but its mechanism is still not clear. Therefore, we investigated the anti-MF effect of ginsenoside Re by constructing mouse acute myocardial infarction (AMI) model and AngII induced cardiac fibroblasts (CFs) model. Methods: The anti-MF effect of miR-489 was investigated by transfection of miR-489 mimic and inhibitor in CFs. Effect of ginsenoside Re on MF and its related mechanisms were investigated by ultrasonographic, ELISA, histopathologic staining, transwell test, immunofluorescence, Western blot and qPCR in the mouse model of AMI and the AngII-induced CFs model. Results: MiR-489 decreased the expression of α-SMA, collagenI, collagen III and myd88, and inhibited the phosphorylation of NF-κB p65 in normal CFs and CFs treated with AngII. Ginsenoside Re could improve cardiac function, inhibit collagen deposition and CFs migration, promote the transcription of miR-489, and reduce the expression of myd88 and the phosphorylation of NF-κB p65. Conclusion: MiR-489 can effectively inhibit the pathological process of MF, and the mechanism is at least partly related to the regulation of myd88/NF-κB pathway. Ginsenoside Re can ameliorate AMI and AngII induced MF, and the mechanism is at least partially related to the regulation of miR-489/myd88/NF-κB signaling pathway. Therefore, miR-489 may be a potential target of anti-MF and ginsenoside Re may be an effective drug for the treatment of MF.
Objectives : Saposhnikoviae Radix (SR) and Glehniae Radix (GR) have been frequently used in traditional medicine to treat diseases related to 'wind' syndrome, but there have been cases where it has been mixed in a state where the plant of origin is not clear. In this study, to select materials for conducting preclinical cerebral infarction research, the network pharmacology analysis method was used to select suitable medicinal materials for the study. Methods : In this study, a Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) based network pharmacology analysis method was used, and oral bioavailability (OB), drug likeness (DL), Caco-2 and BBB permeability were utilized to select compounds with potential activity. For the values of each variable used in this study, OB ≥ 20%, DL ≥ 0.18, Caco-2 ≥ 0, and BBB ≥ -0.3 were applied, then networks of bioactive compounds, target proteins, and target diseases was constructed. STRING database was used to construct a protein-protein interaction network. Results : It was confirmed that SR rather than GR has various target proteins and target diseases based on network pharmacological analysis using TCMSP database. And it was analyzed that the bioactive compounds only in SR act more on neurovascular diseases, and both drugs are expected to be effectively used for cardiovascular diseases. Conclusions : In our future study, SR will be used in an ischemic stroke mouse model, and the mechanism of action will be explored focusing on apoptosis and cell proliferation.
Background & Objectives: Fixed-dose combinations (FDCs) offer advantages in adherence and cost-effectiveness compared to free combinations (FCs), but they can also complicate the prescribing process, potentially leading to therapeutic duplication (TD). This study aimed to identify the prescribing patterns of FDCs for dyslipidemia and investigate their associated risk of TD. Methods: This was a retrospective cohort study involving drugs that included statins, using Health Insurance Review & Assessment Service-National Patient Sample (HIRA-NPS) data from 2018. The unit of analysis was a prescription claim. The primary outcome was TD. The risk ratio of TD was calculated and adjusted for patient, prescriber, and the number of cardiovascular drugs prescribed using a multivariable Poisson model. Results: Our study included 252,797 FDC prescriptions and 515,666 FC prescriptions. Of the FDC group, 46.52% were male patients and 56.21% were aged 41 to 65. Ezetimibe was included in 71.61% of the FDC group, but only 0.25% of the FC group. TD occurred in 0.18% of the FDC group, and the adjusted risk ratio of TD in FDC prescriptions compared to FC was 6. 44 (95% CI 5. 30-7. 82). Conclusions: Prescribing FDCs for dyslipidemia was associated with a higher risk of TD compared to free combinations. Despite the relatively low absolute prevalence of TD, the findings underline the necessity for strategies to mitigate this risk when prescribing FDCs for dyslipidemia. Our study suggests the potential utility of Clinical Decision Support Systems and standardizing nomenclature in reducing medication errors, providing valuable insights for clinical practice and future research.
본 연구는 사람의 대장암에 천연물인 과라나와 그라비올라가 어떤 효능을 나타내는지를 확인하고자 항산화 활성과 항암활성을 분석하였다. 각각 에탄올로 추출하여 control, 25.0, 50.0, 75, 100.0mg/ml의 농도에서 DPPH분석한 결과 과라나는 50mg/ml에서 80.9%, 그라비올라는 71.4%의 소거율을 보여 유의적인 항산화 활성이 있음을 확인하였다. 인체 대장암 세포주인 HCT-116에서 MTT assay와 FACS에 의한 apoptotic rate로 항암효과를 조사한 결과, 과라나와 그라비올라 모두 세포증식을 강하게 억제하였으며, 특히 10mg/ml의 농도에서 과라나는 96.65±3.71, 그라비올라는 76.58±2.87%의 높은 apoptotic rate를 나타냄으로써 뚜렷한 항암 효과가 있음을 확인하였다. 이와 같은 결과는 천연 식물성 성분인 과라나와 그라비올라가 대장암의 예방과 치료에 새로운 항암제로서의 기초자료를 제공하는 것이며, 향후 국민건강 및 의료비 절감에도 기여할 수 있을 것으로 기대된다.
화학요법이 확립되고 유병율이 점진적인 감소에도 불구하고 폐결핵은 여전히 수술의 대상이 되고 있다. 이에 저자등은 폐결핵으로 수술을 시행한 예들을 분석, 검토함으로써 향후 폐결핵 치료에 도움이 되고자 한다. 방법 : 국립의료원 흥부외과에서 1991년1월부터 2000년12월까지 10년간 폐절제술을 시행한 144례를 토대로 본과에서 기왕에 발표된 4보까지와 비교관찰하여 폐결핵의 외과적 치료에 대한 최근 경향 및 그 결과를 검토,분석하였다. 방법으로 연간수술빈도 및 연령분포, 술전 결핵병소범위, 객담도말검사, 수술적응 및 술식의 종류, 합병증, 술후 추적관찰 결과들을 분석하였다. 결과 : 연간 수술빈도는 감소추세를 보였으며, 남녀성 비는 2:1로 남자환자가 많았고, 20대 이하의 감소와 50대 이상이 현저히 증가함으로써 연령의 고령화 현상을 볼 수 있다. 수술 전 병변의 범위는 대부분 중증 및 중등도에 속하였으며, 술전 항결핵제를 사용한 기간을 보면 3년 이상의 예가 16.0%에서 55.6%로 현저히 증가하는 추세를 보이고 있다. 한편 술전 결핵균에 대한 객담도말검사 양성율은 91%에서 27%로 급격한 감소 현상을 보였다. 수술적응에 있어서는 전폐파괴 및 폐부분파괴가 대부분을 차지하였으며, 폐실질 병소를 동반한 농흉이 4.0%에서 20.1%로 유의한 증가를 보이고 있다. 술식의 종류에 있어서 편측 폐절제술 및 폐엽 절제술이 대부분을 차지하고 있다. 술후 합병증으로는 농흉의 증가와 잔여 결핵 병소의 재발의 증가가 특징적으로 관찰되었다. 술후 추적관찰결과 완치율이 70% 이상을 나타내는 가운데 지속적으로 항결핵제 치료를 받고 있는 예가 5.8%에서 18.0%로 점진적 증가추세임을 나타냈다. 절론 : 최근 추세를 보면 화학치료에 내성을 보이는 환자가 증가하고, 늑막결핵의 증가와 중증의 병변환자의 증가가 특징적으로, 내과적 요법만으로 치유하기 어려운 병소를 정확히 판단하고 다제 내성균이 발생하기 전에 적극적으로 외과적 요법을 고려하는 것이 바람직하겠다.
대구 및 그 인접지역의 질병 발생 경향을 추정하고, 영남대학교 의과대학 부속병원의 약제수급 계획을 위한 참고자료를 얻기 위하여, 1985년 3월부터 동년 8월까지 당 병원 외래에 내원한 환자들을 위하여 내려진 약물 처방을 전산 집계하여 다음과 같은 결과를 얻었다. 가장 빈번히 처방된 약물로 부터 100종의 약물을 처방빈도 순으로 나열하였던 바, diazepam, aluminum compound, acetaminophen, isoniazid, metoclopramide, $polaranine^{(R)}$, carboxymethylcystein, ephedrine, codeine, caroverine (이하생략) 등의 순으로 나타났으며, 이와 같이 선정된 100종의 약물을 임상용도별로 분류하여 총 처방회수 521,855회에 대한 백분율을 조사한 결과는 다음과 같다. 중추신경계약물 (20.57%)이 가장 많았고, 다음은 위장관 및 평활근에 작용하는 약물 (18.64%), 호흡기계약물 (16.11%), 항균제 (15.12%), 심혈관 계약물 (5.64%)의 순으로 많았으며, 그 다음은 소염제 (4.33%), 단독비타민제 (3.76%), 호르몬 및 항호르몬제 (3.29%), 감기치료제 (3.12%), 이뇨제 (2.81%), 간기능이상치료제 (2.02%), 자율신경계약물 (1.89%), 항히스타민제 (1.02%) 및 소독약 (0.74%) 등의 순으로 나타났다.
Prevention of thrombombolism after rosthetic cardiac valve replacement is essential for the patients. About 90% of patients are free of major and minor thromboembolic complications 5 year after replacement of cardiac valves with prosthetic devices when they are under control of anticoagulant therapy. Ticlopidine is a drug that alter platelet function to have an antithrombotic effect. It is an antiaggregating agent which inhibits primary platelet function to have an antithrombotic effect. It is an antiaggregating agent which inhibits primary platelet aggregation induced by ADP and increases the production of prostaglandin $D_{2}$. Aspirin in small doses inhibits platelet synthesis of prostaglandins by irreversibly blocking the enzyme cyclo-oxygenase. Platelet secretion and aggregation are impaired with Ticlopidine and Aspirin. the thromboembolic event sof 54 patient s who were treated with Ticlopidine and Aspirin after cardiac valve replacement were evaluated and compared with that of 79 patients who were treated with Wafarin and Aspirin after the same type of operation. The follow-up period ranged from 4 to 110 months (mean of 48 months). there were 11 major thromboembolic episodes including three deaths in the warfarin goup during mean follow-up period of 56 months. two cases of CVA and one hemoarthrosis were noted due to overdose of Warfarin. Inticlopidine group, there was only one fatal thromboembolic epdisode three month after mitral valve replacement during mean follow-up period of 18 months. Two episodes of hypermenorrhea resulting anemia ere noted in the ticlopidine group. We measured the parameters of platelet function in aggreagation curve of platelet with platelet aggregometer (chrono-log Aggregometer, Model No. 430) Aggregation test was performed with three final concentrations of epinephrine in 10 uM/L, ADP in 5uM/L. 28 patients with prosthetic cardiac valves and 35 healthy volunteers were subgrouped as follows to analyze the effect of antithrombotic drugs used. Group I ; 11 patients treated with 250-500 mg of ticlopidine and 0.5gm of Aspirin as a daily single dose after cardiac valve replacement (14 St. Jude Medical and 1 Carpentier-Edwards, 9 patients with atrial fibrillation among them) Group II ; 10 patients treated with 3-5 mg of Warfarin and 0.75 gm of Aspirin daily to prolong prothrombin time around 20 seconds for more than 6 months and single Aspirin dose was maintained afterward as a life-long regimes(3 St. Jude Medical, 1 Hall-Kaster and 7 Carpentier-Edwards valve, 9 patients in atrial fibrilation). Group III ; 7 patients who quit anticoagulant treatment (Warfarin + Aspirin) 6-12 months after the regime as group II (3 St. Jude Medical. 1 bjork-Shiley, 1 Hall-Kaster, 3 Carpentier-Edwards valve, 2 of them are with atrial fibrillation). Group IV ; 35 healthy vounteers (28 males and 7 females). The following results were obtained. 1. The mean maximal platelet aggregability in Group I induced by 10uM/L epinephrine was 15.6%, and 17.5 and 18.7% in BM in proportion to the induction by 5 and 10 uM/L ADP. 2. The mean maximal platelet aggregability in Group II induced by 10uM/L epinephrine was 16.5%, and 27.4 and 44.7% in BM in proportion to the induction by 5 and 10uM/L ADP. 3. The mean maximal platelet aggregability in group III induced by 10uM/L epinephrine was 65%, and 56.5 and 51.8% in BM in proportion to the induction by 5 and 10 uM/L ADP. 4. The mean maximal platelet aggregability in the normal subjects induced by 10 uM/L epinephrine was 64%, and 65 and 69% in Bm inproportion to the induction by 5 and 10 uM/L ADP. 5. Reversible change of platelet aggregation curve induced by 5 and 10uM/L was noted all of the patients in Group I. conclusion : Ticlopidine is an antiaggregating agent which inhibits primary platelet aggregation induced by ADP, and increases the production of prostaglandin $D_{2}$. Ticlopidine and Aspirin produced a significant inhibition of platelet in the presence of ADP and epinephrine in our study. Acccording to our brief experience, 250 mg of ticlopidine and low dose of Aspirin resulted synergistic superior effect to each drug alone in prevention of thromboembolism after prosthetic cardiac valve replacement.
연구목적 클로자핀은 조현병 치료에 널리 처방되는 항정신병약물로, 대사성 부작용 등으로 인한 심뇌혈관질환의 위험을 증가시키는 것으로 알려져 있다. 하지만, 클로자핀이 심뇌혈관질환의 발생에 중요한 영향을 미치는 또 다른 요소인 혈소판 활성 정도에 어떤 영향을 미치는지에 대해서는 알려진 바가 거의 없다. 본 연구에서는 클로자핀 투여 전후 평균혈소판요소 (mean platelet component, MPC) 값 비교를 통하여 조현병 환자에서 클로자핀이 혈소판 활성에 미치는 영향을 알아보고자 하였다. 방 법 2003년 9월 1일부터 2007년 4월 30일까지의 기간 동안 건양대학교 병원 정신건강의학과에서 클로자핀을 새롭게 투여 받기 시작한 환자들을 대상으로 의무기록을 후향적으로 검토 하였다. 최종 통계 분석에는 14명이 포함되었다. 평균혈소판요소는 Bayer ADVIA $120^{(R)}$ system를 이용하여 측정하였다. 결 과 14명의 연구 대상자 중, 남성은 4명(28.60%), 여성은 10명(71.40%)이었으며, 평균 나이는 $37.50{\pm}11.64$세였다. 유병 기간은 평균 $91.00{\pm}93.96$개월 이었으며 피험자들이 마지막 혈액 검사를 한 시점에 복용하였던 클로자핀 용량의 평균은 $337.50{\pm}109.52mg$이었다. 클로자핀 투여 전과 투여 후의 평균혈소판요소 값은 각각 $26.12{\pm}2.22g/dL$과 $25.14{\pm}2.08g/dL$ 이었다. 윌콕슨 부호-순위 검정에서 클로자핀 투여 후 MPC 값이 유의미하게 감소하였다(V=16, p=0.024). 결 론 본 연구의 결과는 클로자핀 투여가 혈소판을 활성화하며, 이로 인해 혈전색전성 질환 등의 발생에 부정적인 영향을 미칠 수 있음을 시사한다. 또한 클로자핀 투여 시 혈소판 활성에 대한 평가를 포함하여 뇌혈관질환의 위험성에 대한 주의 깊은 모니터링이 필요함을 시사한다.
연구 배경 : 폐결핵은 아직도 노인들에게 있어서 높은 유병률을 보이며 심각한 임상문제로 남아 있다. 그러나 노인 폐결핵은 젊은 연령 군에 비해서 증상이 비전형적이고 약제에 대한 부작용이 많아서 조기 진단 및 치료가 어려운 경우가 많다. 본 연구는 노인 폐결핵의 진단 및 치료에 있어서의 임상 양상이 젊은 연령 군과 비교하여 차이가 있는지 알아보기 위해 시행되었다. 연구 방법 : 1991년 11월에서 1997년 1월까지 서울 시립 보라매병원에서 폐결핵으로 진단, 치료받은 65세 미만인 젊은 연령 군 125명과 65셰 이상인 노인 군 70명올 대상으로 의무기록을 후향적으로 분석하였다. 결 과 : 노인 폐결핵 환자들은 젊은 연령 군에 비하여 식욕 감소(12% vs 31.4%), 전신 쇠약감(20.0% vs 54.3%), 호흡곤란(21.6% vs. 37.1%), 의식 변화(0.8% vs. 15.7%) 등의 비전형적인 증상으로 내원한 경우가 유의하게 많았다. 반변에 젊은 연령인 군에서는 노인 군에 비하여 객혈(32.8% vs. 10.0%), 발열(58.4% vs. 35.7%)의 빈도가 높았다. 기저질환에 있어 노인 군은 젊은 연령 군에 비하여 심혈관 질환과 만성폐쇄성 폐질환의 빈도가 유의하게 높았다. 흉부 X-선 소견 상 폐결핵 병변의 위치는 양군간에 차이가 없었으나 병변의 모양은 노인 군에서 폐렴이나 폐암으로 오인되는 경우가 많았다(8.8% vs. 30.0 %, p<0.05). 양군 간에 객담 항산균 포말 및 배양 검사상 차이는 없었으며, 치료에 대한 반응이나 치료 중 이탈률에도 차이가 없었다. 노인 군에서 젊은 연령 군에 비하여 약제 부작용이 많았고(13.6% vs. 45.7%, p<0.05), 치료 도중 약제 변경도 노인 군에서 많았다(4.9% vs. 25.7%, p<0.05). 젊은 연령 군 125명 중 1명, 노인 군 70명 중 6명이 결핵으로 인해 사망하였다. 결 론 : 노인 폐결핵은 임상적, 방사선학적으로 비전형적인 소견을 보이는 경우가 있으므로 노인에서 폐결핵이 의심될 때는 즉시 객담 도말 및 배양 검사를 시행해야 한다. 노인 폐결핵 환자는 약재 부작용이 사망률도 높았으나 치료에 대한 반응은 대체로 좋고 반응도 빨랐다.

① 이용자는 회원가입 신청 또는 회원정보 변경 시 실명으로 모든 사항을 사실에 근거하여 작성하여야 하며,
허위 또는 타인의 정보를 등록할 경우 일체의 권리를 주장할 수 없습니다.
② 당 사이트가 관계법령 및 개인정보 보호정책에 의거하여 그 책임을 지는 경우를 제외하고 회원에게 부여된
ID의 비밀번호 관리소홀, 부정사용에 의하여 발생하는 모든 결과에 대한 책임은 회원에게 있습니다.

제 4 장 서비스의 이용
제 12 조 (서비스 이용 시간)

원칙으로 합니다. 단, 당 사이트는 시스템 정기점검, 증설 및 교체를 위해 당 사이트가 정한 날이나 시간에
서비스를 일시 중단할 수 있으며, 예정되어 있는 작업으로 인한 서비스 일시중단은 당 사이트 홈페이지를
통해 사전에 공지합니다.
② 당 사이트는 서비스를 특정범위로 분할하여 각 범위별로 이용가능시간을 별도로 지정할 수 있습니다. 다만
이 경우 그 내용을 공지합니다.
제 13 조 (홈페이지 저작권)
① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
