Although elevated serum low-density lipoprotein-cholesterol (LDL-C) is without any doubts accepted as an important risk factor for cardiovascular disease (CVD), the role of elevated triglycerides (TGs)-rich lipoproteins as an independent risk factor has until recently been quite controversial. Recent data strongly suggest that elevated TG-rich lipoproteins are an independent risk factor for CVD and that therapeutic targeting of them could possibly provide further benefit in reducing CVD morbidity, events and mortality, apart from LDL-C lowering. Today elevated TGs are treated with lifestyle interventions, and with fibrates which could be combined with omega-3 fatty acids. There are also some new drugs. Volanesorsen, is an antisense oligonucleotid that inhibits the production of the Apo C-III which is crucial in regulating TGs metabolism because it inhibits lipoprotein lipase (LPL) and hepatic lipase activity but also hepatic uptake of TGs-rich particles. Evinacumab is a monoclonal antibody against angiopoietin-like protein 3 (ANGPTL3) and it seems that it can substantially lower elevated TGs levels because ANGPTL3 also regulates TGs metabolism. Pemafibrate is a selective peroxisome proliferator-activated receptor alpha modulator which also decreases TGs, and improves other lipid parameters. It seems that it also has some other possible antiatherogenic effects. Alipogene tiparvovec is a nonreplicating adeno-associated viral vector that delivers copies of the LPL gene to muscle tissue which accelerates the clearance of TG-rich lipoproteins thus decreasing extremely high TGs levels. Pradigastat is a novel diacylglycerol acyltransferase 1 inhibitor which substantially reduces extremely high TGs levels and appears to be promising in treatment of the rare familial chylomicronemia syndrome.
Background: Recently, adolescent obesity was increased and tended to become obese adults, facing increased risk for diabetes, cardiovascular disease. Even before reaching adulthood, adolescent obesity already was experiencing hypertension, dyslipidemia, and diabetes mellitus. The purpose of this study was to find prevalence of obesity of high school students in Jeju island and association between obesity and cardiovascular risk factors and association between childhood obesity and adolescent obesity. Method: The study subjects were 6,064 students of twenty-two high school in Jeju island and health survey data was gathered from high school health examination data and hospital health examination data from Aug. 1999 to Dec. 1999. Adolescent obesity was evaluated by ideal body weight method in 1998 and body mass index. Cardiovascular risk factors were checked total cholesterol, systolic and diastolic blood pressure, fasting blood sugar. In 1,534 students, obesity at primary school is checked by primary and middle school health record. Results: Prevalence of obesity in Jeju island high school students is 12.6% in male students, 13.3% in female students by ideal body weight method and 8.7% in male students, 8.5% in female students by body mass index. Prevalence of obesity of male students in city area and district area of Jeju island were each 12.9%, 10.3% and that of male students in city area was significantly higher. Prevalence of obesity of female students in city area and district area of Jeju island were each 12.9%, 14.2% and not significantly different. Prevalence of obesity of male students in general and commercial school group were each 13.9%, 10.1% and that of general school group was significantly higher. In female students, Prevalence of obesity general and commercial school group was not significantly different. Total cholesterol, systolic and diastolic blood pressure were significantly associated with degree of obesity in male and female students(p<0.05) and fasting blood sugar was only significantly associated with degree of obesity in male students(p<0.05). In 1,514 students checked by health record, Odds ratio of obesity at primary and middle school that at high school were 12.96∼22.77 and was significantly associated. Conclusion: Prevalence of obesity of high school students in Jeju island is very high and adolescent obesity was significantly associated with cardiovascular risk factors. Mostly childhood obesity tended to become adolescent obesity and program of control of obesity at childhood and adolescence is required.
Objectives: This review is to suggest strategies to reduce risk factors of non-communicable diseases (NCD) in South Korea. Methods: Prior research findings on the burden of NCD and associated risk factors and the effectiveness of intervention programs were reviewed. Strategies regarding the control of NCD risk factors were conceived. Results: The author presented research findings from the Global Burden of Disease study on the burden of non-communicable disease (NCD) and associated risk factors in South Korea. Strengths and limitations of population and high-risk strategies for preventing NCDs were introduced. The author also reviewed the evidence on the effectiveness of multiple cardiovascular risk factor interventions and community-based intervention programs on cardiovascular diseases conducted in industrialized countries. Finally, strategies to reduce NCD risk factors in South Korea were suggested. Conclusions: The evidence-based interventions and the importance of population strategies in NCD prevention were highlighted. The author indicated that strategies employed by unhealthy commodity industries to undermine effective public health policies and programs should be actively monitored. It has been suggested that effective high-risk strategies with ecological models to address social risks rather than medical risks among disadvantaged population should be further developed in South Korea.

This study examined the contribution of the risk factors that cause cardiovascular disease in Koreans. The subjects were 20,012 adults aged 30~74 years who were enrolled from the 2005~2013 Korean National Health and Nutrition Examination Survey, had filled the risk factors data, and had no cardiovascular disease history. Data analysis was conducted using SAS 9.3, and a weighted composite sample design analysis was performed. The contribution was obtained by applying a squared semi-partial correlation. As a result, they were higher in the order of age (63.9%), smoking (17.3%), systolic blood pressure (7.9%), diabetes (6.9%), antihypertensive medication use (3.0%), and BMI (0.9%) in men. In women, the contributions were in the order of age (54.1%), systolic blood pressure (23.5%), diabetes (13.6%), antihypertensive medication use (5.1%), smoking (3.1%), and BMI (0.6%%). According to age bracket analysis, smoking (51.8~33.7%) was the biggest risk factor, followed in order by age (26.6%) in their 30s and systolic blood pressure (17.3~21.0%) in those older than 40 years in men. In women, the systolic blood pressure (56.2~40.5%) was the main contributor to the risk factors, followed by age (24.0%) in their 30s and diabetes (13.6~32.6%) in those older than 40 years. The contribution of the income level was similar. Therefore, to prevent cardiovascular disease, various programs that reflect the contributions of the risk factors should be developed. Moreover, it is recommended to use the results shown in this study as basic data.
Objectives The purpose of this study was to investigate the relationship between osteoporosis and cardiovascular risk factors according to Sasang constitution. Methods A total of 1773 participants, over 40 years old, were examined as part of a community-based cohort in Wonju city in Gangwon province of South Korea, from June 2006 to August 2009. We measured bone mineral density by Achilles ultrasonometer, serum levels of lipids, fasting blood sugar, insulin and other cardiovascular risk factors like blood pressure and waist circumference. Constitution was verified by a Sasang constitution specialist using the results of PSSC(Phonetic System for Sasang Constitution), facial photos and a simplified Sasang constitutional questionnaire. Results The prevalence rate was 11.2% in total participants, and 10.6% in Soyangin, 10.4% in Taeeumin and 13.9% in Soeumin. In general characteristics, educational and economic property and exercise were concerned with osteoporosis. Systolic blood pressure, pulse rate and total cholesterol were significantly high in osteoporosis group. Age and menopause were the key risk factors for osteoporosis. There was a significantly high prevalence in Soeumin for osteoporosis in men alone. Low physical activity was a major risk factor for osteoporosis. Waist circumference and hip circumference had significantly high odds ratio and weight had significantly low odds ratio. By Sasang constitution, Soyangin has the negative correlation with free fatty acid, Taeeumin has the negative correlation with waist circumference, pulse rate, systolic blood pressure, total cholesterol and low density lipoprotein and Soeumin has the positive correlation with body fat amount. Conclusions Regimens on osteoporosis should be considered according to Sasang constitution. Cardiovascular diseases should be considered according to Sasang constitution. Soeumin should be cautious of osteoporosis and gain weights and reasonable amount of fat food. Soyangin had better do exercise lower body and eat little food containing triglyceride. Taeeumin had better lose weights and eat little cholesterol food.

The purpose of the study was to assess the effects of a 12 weeks aged garlic extract (AGE) regimen with regular exercise on cardiovascular disease (CVD) risk in postmenopausal women. A total of 30 postmenopausal women ($54.4{\pm}5.4$ years) were randomly divided into the following four groups: Placebo (Placebo; n = 6), AGE intake (AGEI; n = 8), exercise and placebo (Ex + Placebo; n = 8), exercise and AGE (Ex + AGE; n = 8) groups. The AGE group consume 80 mg per day, and exercise groups performed moderate exercise (aerobic and resistance) three times per week. After 12 weeks of treatment, body composition, lipid profile, and CVD risk factors were analyzed. Body weight was significantly decreased in AGEI, Ex + Placebo, and Ex + AGE groups compared to baseline. Body fat % was significantly decreased in the AGEI and Ex + Placebo groups. Body mass index (BMI) was significantly decreased in the AGEI, Ex + Placebo, and Ex + AGE groups. Fat-free mass was significantly decreased in the AGEI group. Total cholesterol (TC) was significantly lower in the Ex + Placebo compared to the Placebo group. AGE supplementation or exercise effectively reduced low-density lipoprotein (LDL-C). Triglyceride (TG) was significantly increased in the AGEI group. Malondialdehyde (MDA) levels were significantly decreased in the AGEI, Ex + Placebo, and Ex + AGE compared to the placebo group. AGE supplementation reduced homocysteine levels regardless of whether the women also exercised. The present results suggest that AGE supplementation reduces cardiovascular risk factors independently of exercise in postmenopausal women.
Purpose: Cardiovascular risk factor modification is important for patients with coronary artery disease to prevent poor progression of the disease. Without an understanding and an awareness of risk factors, patients with coronary artery disease are not able to reduce their risk by the lifestyle modification. The aims of this study were to assess patient's awareness of risk factors and to identify predictors of awareness of risk factors. Methods: A descriptive correlational study using a cross-sectional survey method was performed. The sites of the study were three hospitals in Seoul, Korea. The sample consisted of 214 subjects. The awareness of risk factors and other subjects' characteristics were measured by a questionnaire developed for this study. Results: A range of subjects (gender: 72.0%; hyperlipidemia: 51.9%; hypertension: 40.7%; obese: 37.4%; age: 35.5%; smoking: 22.4%; diabetes: 19.2%; family history: 18.2%) were not able to accurately identify the risk factors. Among predictors, gender (${\beta}$=-.17) and diabetics (${\beta}$=-.25) had statistically significant influences on awareness of risk factors. Conclusion: Our findings highlight the lack of awareness of risk factors for coronary artery disease. The findings have important implications for nursing practice in terms of guiding educational strategies for the modification of risk factors for coronary heart disease.
Coronary artery calcification is associated with cardiovascular risk factors and metabolic syndrome, and several studies have already reported that coronary artery calcification score are closely related to the amount of atherosclerotic plaques. This study was conducted on 109 patients who underwent coronary calcium CT who visited the comprehensive health examination center in Daegu city during the period from December 2020 to February 2021. we would like to investigate the relationship between coronary artery calcification score and blood factors. As a result of the study, the abnormal group increased the risk of calcification by 1.113 times compared to the normal group in the waist circumference factor. In the fasting glucose factor, the abnormal group increased the risk of calcification by 1.036 times compared to the normal group, and in the triglyceride factor, the abnormal group was normal. As the risk of calcification increased 1.008 times compared to the group, the waist circumference factor, fasting glucose factor, and triglyceride factor were found to be factors affecting coronary artery calcification score. The risk of developing calcification is primarily associated with waist circumference, anemia and triglycerides, and health care and health checks are expected to help reduce the incidence of cardiovascular disease and reduce medical costs.
Background: Elderly patients with gastrointestinal (GI) and cardiovascular (CV) risk factors may be more easily exposed to NSAID-related side effects (SEs). Based on the ACG guideline of year 2009, the aim of the study is to evaluate proper use of NSAIDs and gastroprotective drugs according to the degree of GI and CV risk strengths in the patients. Methods: Retrospectively surveyed 410 elderly patients with NSAIDs for more than 30 days at a general hospital in Korea. GI risk factor includes age, ulcer history, high-dose NSIADs, concurrent aspirin use, steroids or anticoagulants. CV risk factor includes angina, myocardial infarction, cerebral infarction, atrial fibrillation or coronary intervention requiring low-dose aspirin. These factors were classified as high/low cardiovascular groups and high/moderate/low GI groups. Results: There were 14 patients in high CV risk group and high GI risk group. The group was recommended not to use NSAIDs as it is not adequate. There were 101 patients in high CV risk group and moderate GI risk group. This group was recommended to use naproxen and PPI/misoprostol. But all patients except one were not adequate. There were 9 patients in low CV risk group and high GI risk group. This group was recommended to use selective COX-2 inhibitor and PPI/misoprostol. 5 cases were proper while 4 cases did not. There were 285 patients in low CV risk and moderate GI risk group who were recommended to use non selective NSAIDs and PPI/misoprostol or selective COX-2 inhibitor only. 103 patients were proper while 182 patients not adequate. Overall, the SEs were higher in those cases for inadequate use of drugs comparing to the adequate. CV SEs were statistically significant. However, SEs for each risk groups were different. For the case of low CV risk group and high/moderate GI risk group, the inadequate use of drugs makes the SE high and the other groups are not. Also, it was not statistically significant. Conclusions: In elderly patients, the inappropriate use of NSAIDs can increase the risk of the disease. Therefore, GI and CV risk must be considered simultaneously, and the proper use of NSAIDs and gastroprotective drugs for each risk groups should be reconsidered.
Purpose: This study was conducted to identify the clinical characteristics and risk factors on the occurrence of variant angina, and to examine the predicting factors on the vascular endothelial dysfunction of the patients with variant angina. Methods: A total of 134 patients diagnosed with variant angina were recruited from 2006 to 2008. The degrees of endothelial dysfunction were measured and recorded by the researcher using the values of flow-mediated vasodilation of their brachial arteries and Nitroglycerine-mediated dilation. Subjects' demographic data and risk factors were gathered after obtaining informed consent, and their electronic medical records were reviewed to collect laboratory data. Results: The mean age was $54.2{\pm}9.6$ years and 52% was male patients. More than 50% of the male patients were cigarette smokers and had hypercholesterolemia. 84% of the male patients and 70% of the female patients had more than one risk factor of cardiovascular disease. A stepwise multiple regression analysis showed that smoking and hypercholesterolemia predicted the decrease of flow-mediated vasodilation (Adjusted $R^2$ = .204, p < .001). Conclusion: Tailored educational interventions for smoking cessation and cholesterol management are needed to prevent recurrence of angina attack for patients with variant angina and to prevent cardiovascular disease for middle-aged workers.
