Minimally Invasive Cardiac Surgery versus Conventional Median Sternotomy for Atrial Septal Defect Closure

  • Background: Median sternotomy is the standard approach for atrial septal defect (ASD) closure. However, minimally invasive cardiac surgery (MICS) has been introduced at many centers in adult/grown-up congenital heart patients. We retrospectively reviewed the results of right anterolateral thoracotomy compared with conventional median sternotomy (CMS) for ASD closure at Seoul National University Hospital. Methods: We retrospectively analyzed 60 adult patients who underwent isolated ASD closure from January 2004 to December 2013 (42 in the CMS group, 18 in the MICS group). Preoperative, operative, and postoperative data were collected and compared between the 2 groups. Results: The MICS group was younger (44.6 years vs. 32.4 years, p=0.002) and included more females (66.7% vs. 94.4%, p=0.025) than the CMS group. Operation time (188.4 minutes vs. 286.7 minutes, p<0.001), cardiopulmonary bypass time (72.7 minutes vs. 125.8 minutes, p<0.001), and aortic cross-clamp time (25.5 minutes vs. 45.6 minutes, p<0.001) were significantly longer in the MICS group. However, there were no significant differences in morbidity and mortality between groups. Only chest tube drainage in the first 24 hours (627.1 mL vs. 306.1 mL, p<0.001) exhibited a significant difference. Conclusion: MICS via right anterolateral thoracotomy is an alternative choice for ASD closure. The results demonstrated similar morbidity and mortality between groups, and favored MICS in chest tube drainage in the first 24 hours.

Variation in clinical usefulness of biomarkers of acute kidney injury in young children undergoing cardiac surgery

  • Background: Acute kidney injury (AKI) is one of the most significant postoperative complications of pediatric cardiac surgery. Because serum creatinine has limitations as a diagnostic marker of AKI, new biomarkers including neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), and interleukin-18 (IL-18) are being evaluated to overcome these limitations and detect AKI at an early stage after cardiac surgery. Purpose: This study aimed to investigate the clinical usefulness of these biomarkers in young children. Methods: Thirty patients with congenital heart diseases who underwent cardiac surgery using cardiopulmonary bypass (CPB) were selected, and their urine and blood samples were collected at baseline and 6, 24, and 48 hours after surgery. Serum creatinine and blood urea nitrogen levels as well as NGAL, KIM-1, and IL-18 levels in urine samples were measured, and clinical parameters were evaluated. Results: Of the 30 patients, 12 developed AKI within 48 hours after cardiac surgery. In the AKI group, 8 of 12 (66.6%) met AKI criteria after 24 hours, and urine KIM-1/creatinine (Cr) level (with adjustment of urine creatinine) peaked at 24 hours with significant difference from baseline level. Additionally, urine KIM-1/Cr level in the AKI group was significantly higher than in the non-AKI group at 6 hours. However, urine NGAL/Cr and IL-18/Cr levels showed no specific trend with time for 48 hours after cardiac surgery. Conclusion: It is suggested that urine KIM-1/Cr concentration could be considered a good biomarker for early AKI prediction after open cardiac surgery using CPB in young children with congenital heart diseases.

Central-Approach Surgical Repair of Coarctation of the Aorta with a Back-up Left Ventricular Assist Device for an Infant Presenting with Severe Left Ventricular Dysfunction

  • A two-month-old infant presented with coarctation of the aorta, severe left ventricular dysfunction, and moderate to severe mitral regurgitation. Through median sternotomy, the aortic arch was repaired under cardiopulmonary bypass and regional cerebral perfusion. The patient was postoperatively supported with a left ventricular assist device for five days. Left ventricular function gradually improved, eventually recovering with the concomitant regression of mitral regurgitation. Prompt surgical repair of coarctation of the aorta is indicated for patients with severe left ventricular dysfunction. A central approach for surgical repair with a back-up left ventricular assist device is a safe and effective treatment strategy for these patients.

Early Clinical Experience with Sutureless Aortic Valve Replacement for Severe Aortic Stenosis

  • Background: Sutureless aortic valve replacement (SU-AVR) has been developed as an alternative surgical treatment for patients with symptomatic severe aortic stenosis (AS). The aim of this study was to evaluate the clinical outcomes of SU-AVR through an assessment of hemodynamic performance and safety. Methods: From December 2014 to June 2016, a total of 12 consecutive patients with severe AS underwent SU-AVR. The endpoints were overall survival and valve-related complications (paravalvular leakage, valve thrombosis, migration, endocarditis, and permanent pacemaker implantation). The mean follow-up duration was $18.1{\pm}8.6months$. Results: The mean age of the patients was $77.1{\pm}5.8years$ and their mean Society of Thoracic Surgeons score was $9.2{\pm}17.7$. The mean cardiopulmonary bypass and aortic cross-clamp times were $94.5{\pm}37.3$ minutes and $54.9{\pm}12.5minutes$, respectively. Follow-up echocardiography showed good prosthesis function with low transvalvular pressure gradients (mean, $13.9{\pm}8.6mm\;Hg$ and peak, $27.2{\pm}15.0mm\;Hg$) at a mean of $9.9{\pm}4.2months$. No cases of primary paravalvular leakage, valve thrombosis, migration, or endocarditis were reported. A new permanent pacemaker was implanted in 1 patient (8.3%). The 1-year overall survival rate was $83.3%{\pm}10.8%$. Conclusion: Our initial experience with SU-AVR demonstrated excellent early clinical outcomes with good hemodynamic results. However, there was a high incidence of permanent pacemaker implantation compared to the rate for conventional AVR, which is a problem that should be solved.

Outcomes of Reoperative Valve Replacement in Patients with Prosthetic Valve Endocarditis: A 20-Year Experience

  • Background: Prosthetic valve endocarditis (PVE) is a serious complication of cardiac valve replacement, and many patients with PVE require reoperation. The aim of this study was to review our institutional 20-year experience of surgical reoperative valve replacement in patients with PVE. Methods: A retrospective study was performed on 84 patients (mean age, $54.8{\pm}12.7years$; 51 males) who were diagnosed with PVE and underwent reoperative valve replacement from January 1995 to December 2016. Results: PVE was found in 1 valve in 61 cases (72.6%), and in 2 or more valves in 23 cases (27.4%). The median follow-up duration was 47.3 months (range, 0 to 250 months). Postoperative complications occurred in 39 patients (46.4%). Reinfection occurred in 6 cases, all within 1 year. The freedom from reinfection rate at 5 years was $91.0%{\pm}3.5%$. The overall survival rates at 5 and 10 years were $64.4%{\pm}5.8%$ and $54.3%{\pm}7.3%$, respectively. In stepwise multivariable Cox proportional hazard models, older age (hazard ratio [HR], 1.48; 95% confidence interval [CI], 1.05 to 2.10; p=0.027) and cardiopulmonary bypass (CPB) time (HR, 1.03; 95% CI, 1.00 to 1.01; p=0.033) emerged as independent risk factors for death. Conclusion: Older age and a longer CPB time were associated with an increased risk of overall mortality in PVE patients.

양에서 시행한 이동작동기 형태(MOVING ACTUATOR TYPE) 인공심장의 삽입실험 (Experimental Implantation of Moving Actuator Type Total Artificial Heart in Sheep)

  • We recently developed a new model of moving actuator type totally implantable artificial heart[TIAH , based on the reverse position of the aortic and pulmonary conduits. This concept was proposed by one of surgeons in our team[Joon-Ryang Rho, M.D. to facilitate anatomical fitting of TIAHs. The moving actuator type electromechanical TIAH consisted of the left and right blood sacs, and the moving actuator including a motor. The inverted umbrella type polyurethane valves were used in the blood pumps. The aortic conduit was positioned anterior to the pulmonary conduit, which was the opposite relation to the conventional configuration of other total artificial hearts. We also adapted slip-in connectors for the aortic and pulmonary conduits. Two sheep , weighing 60-69 kg, were used for implantation. After small cervical incision and trans-sternal bilateral thoracotomy, cardiopulmonary bypass [CPB was administered using an American Optical 5-head pump and a membrane oxygenator[Univox-IC, Bentley . The anterior and posterior vena cavae were drained separately for venous return. An arterial return cannula was inserted into the right common carotid artery. During CPB, almost all of the ventricular myocardium was excised down to the atrioventricular groove and the artificial heart was implanted. We achieved 3-day survival in the first sheep and 2-day survival in the second. The day after operation the first sheep was successfully extubated and the second sheep was weaned from a respirator with good condition. After extubation, the first sheep walked around in the cage and fed herself. Serial laboratory and hemodynamic examinations were done during the experiments. In both sheep, pulmonary dysfunction was gradually developed, which was accompanied by acute renal failure. The animals were sacrificed and autopsy was done. Unexpected pregnnacy was incidentally found in both sheep. To our knowledge this is the first report of significant survival cases in the orthotopic implantation of electric TIAH using sheep.

만성 신부전으로 인한 무뇨증 환자에서의 판막치환술 - 1례 보고 - (Valve Replacement in an Anuric Patient with Chronic Renal Failure - 1 Case Report -)

  • 오늘날 말기 신부전 환자에서의 심장수술은 보편화 되는 추세이며, 만성 신부전 환자들의 수가 늘어남에 따라 이들 환자에 대한 적절한 치료법 개발이 필요하게 되었다. 이 환자들에게는 관상동맥질환 뿐만 아니라 판막질환도 드물지 않다. 이들에게 시행되는 복막투석은 체외순환에 다소의 지장은 있을수 있지만 수술 전후에 적절한 대책을 준비한다면 더 이상 심장수술의 걸림돌이 되지 않는다. 저자는 심한 승모판막 폐쇄부전과 만성신부전을 동반한 33세의 무뇨증 여자환자에서 기계판막치환술을 시행하였다. 환자는 수술전에 복막투석을 주기적으로 함으로써 수분 및 전해질 균형을 적절히 유지시켰고, 수술후에도 혈액 생화학적 검사결과를 수시로 예의검토하면서 복막투석을 계속시행함으로써 무사히 회복될 수 있었다.

신생아의 복잡심장기형에서 심장이식을 위한 동물실험 (Animal Experiments of Heart Transplantation for Complicated Congenital Heart Disease in Neonate)

  • 배경: 복잡심장기형의 경우 완전교정술식의 높은 난이도와 사망율로 말미암아 심장이식이 고려되고 있다. 그러나 정상심장을 이식하기에 곤란한 복잡기형의 경우 수술전 자세한 계획이 매우 중요하다고 하겠다. 신생아의 복잡심장기형에서 심장이식을 고안하기위한 동물실험을 하였다. 대상 및 방법: 8마리 신생돼지를 사용하여 4차례 심장이식실험을 하였다. 실험 1: 성인에서의 심장이식과 같은 방법으로 시행하였다. 실험 2: 좌심형성부전증후군(Hypoplastic Left Heart Syndrome)의 경우를 모의실험하기 위하여 공여돈의 대동맥궁을 이용한 성형수술을 하였다. 실험 3: 폐동맥성형술을 동반한 심장이식을 하였다. 실험 4: 이식후 장기 생존을 위한 실험을 시도하였다. 결과: 신생돈에서의 심장이식수술은 큰 어려움없이 시행할 수 있었다. 실험후 인공심폐순환에서 벗어날 수 있었으나 2마리는 폐동맥연결부위와 좌심방연결부위의 출혈로 사망하였다. 결론: 이상의 실험으로 대혈관의 위치에 따라 세심한 수술계획을 한다면 거의 실행하지 못할 복잡심장기형이 없을 것으로 보이며 신생아의 경우는 특히 출혈에 대해 주의를 해야 할 것으로 생각되었다.

소아개심술시 아프로티닌이 술후 출혈 및 혈액응고계에 미치는 영향 (Effects of Aprotinin on Postoperative Bleeding and Blood Coagulation System in Pediatric Open Heart Surgery)

  • 1994년 12월부터 1995년 4월까지 서울대학교병원 소아흉부외과에서 개심술을 시행받는 95명의 환아 들을 무작위로 아프로티닌을 쓴 환아군(n=47)과 쓰지않은 환아군(n=48)으로 분류하여 아프로티닌의 효과에 대하여 연구하였다. 아프로티닌은 50,000K/U/kg를 인공심폐기의 충전용액에 단일 투여하였다. 술전, 마취후 5분, 심폐기 관류 5분후, 심폐기 관류 35분후, 재관류 5분후, 재관류 3시간후 및 24시간후에 혈액을 채 취하여 헤모글로빈, 헤마토크릿, BUW, creatinine, 섬유소원, 전해질 농도, 활성 응고시간, 프 로트롬빈 시간, 제3항 트롬빈 등을 측정하였다. 또한 술후 24시간 동안 흥관을 통한 배액 양, 수혈 혈액의 양을 체중으로 나누어 비교하였다. 두 환자군간에 술후 24시간 동안 수혈한 충전 적혈구 양을 제외한 다른 모든 검사에서 통계 학적 의의는 없었으나 아프로티닌의 부작용은 발견되지 않았고 출혈로 인한 재수술도 없었다.
