대사증후군과 뇌심혈관질환 발병위험도 평가와의 연관성: 일개 사업장 남성 근로자를 대상으로 (The Relationship between Metabolic Syndrome and Korean Cardiocerebrovascular Risk Assessment: for Male Researchers in a Workplace)

  • Objectives: The prevalence of metabolic syndrome has recently increased, Payments from the Korea Labor Welfare Corporation for compensation for mortality in workers caused by cardiovascular and cerebrovascular diseases have also increased in Korea in recent years. The association of metabolic syndrome and cardiocerebrovascular disease has been investigated by several researchers in recent studies, This study was conducted in an attempt to characterize the relationship between metabolic syndrome and Korean cardiocerebrovascular disease risk assessment, and to provide basic data to group health practices for the prevention of cardiocerebrovascular disease. Methods: Health examinations were previously conducted for 1526 male researchers at a private laboratory. The prevalence by age and the odds ratio of metabolic syndrome scores into the "cardiocerebrovascular risk group" (sum of low, intermediate, and high risk groups) of the Korean cardiocerebrovascular disease risk assessment were assessed, in an effort to elucidate the associations between metabolic syndrome and cardiocere brovascular disease risk assessment. Results: The prevalence of metabolic syndrome and inclusion in the cardiocerebrovascular risk group was 11,7% and 22.1% respectively. The severity of metabolic syndrome and cardiocerebrovascular risk assessment showed that individuals in their 40's and 50's were at higher risk than those in their 30's (p<0,001). The age-adjusted odds ratio of metabolic syndrome to cardiocere brovascular risk group inclusion was 5.6. Conclusions: An active prevention program for cardiocerebrovascular disease needs to begin in the 40's, as the prevalence of metabolic syndrome and the risk group of cardiocerebrovascular risk assessment peak in the 40's age group. The odds ratio between metabolic syndrome and the cardiocerebrovascular risk group was high, which indicates that metabolic syndrome scores should be utilized as guidelines during the consultation and behavioral modification program for the workplace prevention of cardiocerebrovascular diseases in group health practices.

중년여성 근로자의 뇌심혈관질환 발병위험도 평가 (Risk Assessment for Cardiocerebrovascular Disease in Middle Age Women Workers)

  • 본 연구는 중년 여성 근로자의 뇌심혈관 질환 발병위험도를 평가하고 발병위험도에 영향을 미치는 요인을 파악하고자 일개 조선소에 근무하는 40세 이상 65세 미만의 중년 여성 근로자의 1,340명의 건강검진 자료를 바탕으로 분석하였다. 통계분석은 SPSS/WIN 18.0 프로그램을 이용하였다. 본 연구결과 정상 혈압을 가진 대상자는 1234명이고 그중 발병인자가 없는 건강군은 1,078명(80.4%), 저위험군 45명(3.4%), 중등도 위험군 150명(11.2), 고위험군 67명(5.0%)으로 나타났다. 건강군과 위험군 간에 연령, 총콜레스테롤, HDL-콜레스테롤, LDL-콜레스테롤, 흡연 유무, 뇌졸중 가족력, 심장질환 가족력, 고혈압 가족력, 당뇨 가족력이 차이가 있는 것으로 나타났다. 그리고 로지스틱 회귀분석을 통해 확인한 뇌심혈관 질환 발병위험 영향요인은 연령, HDL-콜레스테롤, LDL-콜레스테롤, 흡연, 뇌졸중 가족력, 고혈압 가족력으로 나타났다. 본 결과는 추후 중년 여성의 뇌심혈관 질환 예방 프로그램 개발의 기초자료로 활용될 것이다.

심뇌혈관질환위험군에서의 구강건강인식과 구강건강행위가 심뇌혈관질환 예방행위에 미치는 영향 (Effects Of Oral Health Awareness and Oral Health Behavior on Preventive Behavior of Cardiocerebrovascular disease in Cardiocerebrovascular Disease Risk Group)

  • 본 연구는 심뇌혈관질환위험군의 구강건강인식과 구강건강행위가 심뇌혈관질환 예방행위에 미치는 영향을 파악하여 간호중재 프로그램의 기초자료로 제공하고자 한다. 자료수집은 전라남도에 소재한 2개의 종합병원 및 보건소, 보건진료소, 노인복지관에서 관리하고 있는 심뇌혈관질환위험군 131명을 대상으로 설문지를 통하여 조사하였다. 연구기간은 2018년 2월부터 3월까지 2개월간 시행되었다. 수집한 자료는 기술통계와 t-test, Pearson 상관분석, 단계적 다중회귀분석으로 분석하였다. 연구결과 심뇌혈관질환 예방행위는 치주질환인식인 주관적 구강건강상태(r=.261, p=.003), 구강건강의 중요성(r=.250, p=.004), 그리고 구강건강행위(r=.303, p<.001)와 양의 상관관계가 있었고, 심뇌혈관질환 예방행위에 영향을 미치는 요인은 구강건강행위(${\beta}=.29$, p<.001), 연령(${\beta}=.27$, p=.001), 주관적 구강건강상태(${\beta}=.24$, p=.003)로 나타났다. 즉, 구강건강행위가 높을수록, 연령이 높을수록, 주관적 구강건강상태가 좋을수록 심뇌혈관질환 예방행위의 점수가 높아진다고 할 수 있다. 이들 변수의 설명력은 19.3%이었다. 따라서 심뇌혈관질환위험군의 구강건강 수준을 향상시키기 위한 교육 및 중재프로그램 개발이 필요하며 구강건강 인식, 태도 및 행동과 구강건강상태 등에 관한 지속적인 추후연구가 필요할 것이다.

  • 본 연구는 심뇌혈관질환 예방프로그램이 지역사회 노인의 심혈관 위험요인과 삶의 질에 미치는 효과를 확인하고자 실시하였다. 유사실험설계의 비동등성 대조군 전후설계를 이용하였다. 연구대상자는 지역사회 노인으로 실험군 22명, 대조군 22명 총 44명을 편의표집하였고, 심뇌혈관질환 예방프로그램을 제공하고 심혈관 위험요인과 삶의 질을 측정하였다. 심뇌혈관 예방프로그램에 참여한 실험군은 대조군에 비해 심혈관 위험요인인 수축기 혈압(t=-2.419, p= .020), 이완기 혈압(t=-2.226, p= .031), 혈당(t=-2.071, p= .045)이 유의하게 감소하였고 삶의 질은 향상되었다(t=2.511, p= .016). 그러나 총콜레스테롤과 고밀도 지단백콜레스테롤은 유의한 변화가 없었다. 이에 심뇌혈관질환 예방프로그램이 지역사회 노인의 심혈관 위험요인 감소와 삶의 질 향상에 효과적임을 확인할 수 있었다.

폐경 중년여성을 위한 심뇌혈관질환 예방교육프로그램의 효과 (Effects of a Cardiocerebrovascular Disease Prevention Education Program for Postmenopausal Middle-aged Women)

  • Purpose: This study was conducted to examine effects of a cardiocerebrovascular disease (CVD) prevention education program on knowledge, self-efficacy and health behavior among postmenopausal middle-aged women. Methods: A non-equivalent control group pretest-posttest design was used. Participants were 53 postmenopausal middle-aged women who registered in two community culture centers in G metropolitan city. Experimental group (n=26) received a CVD prevention education program 8 times over 8 weeks. Knowledge, self-efficacy and health behavior of the participants were examined with self-report structured questionaries. Data were collected between October 15 and December 11, 2013, and were analyzed using chi-square test, Fisher's exact test, independent t-test, and analysis of covariance with SPSS/PC version 21.0. Results: After the intervention the experimental group showed significant increases in the knowledge of CVD symptoms (p<.001) and CVD risk factors (p<.001), level of self-efficacy (p=.028) and health behavior (p<.001) compared to the control group. However, no significant difference was found between groups for knowledge of CVD prevention (p<.133). Conclusion: Results suggest that a CVD prevention education program can be an effective strategy to improve knowledge on CVD symptoms and risk factors, self-efficacy and health behavior for postmenopausal middle-aged women.

심뇌혈관질환 고위험군 교육프로그램개발을 위한 노인 고혈압·당뇨병환자와 교육자의 교육요구도 및 지식수준에 대한 비교분석 (Educational Needs of Elderly Hypertensive or Diabetes Patients and Educators for Education Program Development of Cardiocerebrovascular High-risk Group)

  • 본 연구는 심뇌혈관질환 고위험군 교육프로그램 개발을 위해 대구광역시 심뇌혈관질환 고위험군 등록관리 시범사업단 교육정보센터 방문한 65세 이상 고혈압?당뇨병 환자(고위험군)와 교육자를 대상으로 교육목적, 교육과정구성, 교육방법, 교육내용에 대하여 고위험군과 교육자의 교육필요도와 지식 정도 등의 교육요구도를 조사하였고, 교육내용 영역 및 내용에 대해서는 필요도와 고위험군 및 교육자가 평가한 고위험군의 지식 정도를 비교하였다. 연구대상자는 2009년 6월부터 8월까지 4개 교육정보센터 자조모임에 참석한 고위험군 60명과 교육정보센터 44명의 교육자를 대상으로 실시하였다. 가장 선호하는 형태는 소그룹(5-9명)교육, 교육 시간은 30분-1시간, 교육구성은 이론 50%와 실습 50%. 교육이수완료기간은 3개월로 응답하였다. 교육내용에 대한 필요도는 고위험군과 교육자 모두 평균 이상으로 응답하여 본 연구에서 제시한 모든 교육내용을 포함하는 것을 반영할 수 있었다. 고위험군과 교육자의 교육필요도와 지식 정도평가에 따라 교육프로그램 개발 및 교육방향을 제시하면 다음과 같다. 첫째 범주로 고위험군의 교육필요도와 지식 정도 점수 간에 차이가 없는 항목들로 대부분 기본교육과정으로, 잘 인지하고 있는 편이므로 표준화된 교육방법을 지속적으로 운영하면 될 것으로 생각된다. 둘째 범주로 고위험군의 지식 정도가 낮은 교육내용에 대해서는 기본교육과정에 포함시키고 효과적인 실습으로 모든 단계에서 반복할 수 있도록 한다. 셋째 범주로 고위험군의 지식 정도에 개인편차가 큰 항목들은 집단교육이나 소그룹교육보다 개별교육과정에 포함시켜 개별 교육 시 개인의 문제를 해결할 수 있는 기술향상을 목적으로 이루어질 수 있도록 한다. 넷째 범주로 고위험군의 지식 정도 점수와 교육자가 평가한 고위험군의 지식 정도 점수 간에 차이가 있는 항목들은 고위험군이 직접 참여할 수 있는 심화교육의 형태로 하며, 교육방법, 교육환경에 대한 표준화와 구체적인 매뉴얼을 제시하고 관리 목표를 설정하여야 할 것이다. 다섯째 범주로 고위험군과 교육자간 교육의 필요성에 대해 유의한 차이가 있는 항목들은 교육 시 이들 항목의 필요성을 강조하여야 할 것이다. 이상의 내용을 반영하고, 자가관리기술을 향상 시킬 수 있는 요소들을 추가하면 체계적인 교육 프로그램을 개발하는데 도움이 될 수 있을 것이다.

남성 운전직 근로자를 위한 소그룹기반 심뇌혈관질환 예방교육 프로그램의 개발 및 효과 (Development and Evaluation of a Small Group-based Cardiocerebrovascular Disease Prevention Education Program for Male Bus Drivers)

  • Purpose: This study was conducted to examine effects of a small group-based cardiocerebrovascular disease (CVD) prevention education program on knowledge, stage of change and health behavior among male bus drivers with CVD risk factors. Methods: A non-equivalent control group pretest-posttest design was used. Participants were 68 male bus drivers recruited from two urban bus companies. Participants from the two groups were selected by matching age, education and risk factors. Experimental group (n=34) received a small group-based CVD prevention education program 8 times over 6 weeks and 3 times through telephone interviews at 2-week intervals. Data were collected between December, 2010 and March, 2011, and were analyzed using chi-square test, t-test, and repeated measure analysis of variance with SPSS/Win18.0. Results: Experimental group showed significantly higher scores in CVD prevention knowledge (p<.001) and health behavior (p<.001) at 6 and 12 weeks after intervention. Participants in pre-contemplation and contemplation stages made progress to contemplation and action. This was significantly better at 6 and 12 weeks after intervention (p<.001). Conclusion: Results suggest that small group-based education programs for CVD prevention are effective in increasing knowledge, stage of change, and health behavior to prevent CVD among male bus drivers with CVD risk.

심뇌혈관질환 위험요인을 가진 남성 사무직근로자를 위한 배우자참여 건강 코칭의 효과 (Effects of Spouse-Participated Health Coaching for Male Office Workers with Cardiocerebrovascular Risk Factors)

  • Purpose: This study aimed to examine the effect of spouses participating in health coaching on stage of the change, health behaviors, and physiological indicators among male office workers with cardiocerebrovascular disease (CVD) risk factors and compare the findings with trainers who provided health coaching only to workers. Methods: A quasi-experimental pretest-posttest design was used. Convenience sampling was used to recruit participants from a manufacturing research and development company in the city of Gyeonggi province. The health coaching program for the experimental group (n=26) included individual counseling sessions according to workers' stage of change, and provision of customized health information materials on CVD prevention to workers and their spouses for 12 weeks through mobile phone and email. Results: After 12 weeks of intervention, the total score for health behavior, and scores on the sub-areas of exercise and health checkups significantly improved in the experimental group, but there were no significant differences in the scores of stage of the change and physical indicators. The results of a paired t-test showed a significant decrease in the body mass index, abdominal circumference, systolic blood pressure, diastolic blood pressure, total cholesterol and triglyceride values, and a significant increase in the high-density lipoprotein cholesterol value in the experimental group after the intervention. Conclusion: To improve the health of male workers with CVD risk factors in the workplace, sharing health information with their spouses has proven to be more effective than health coaching for only workers. Therefore, it is important to develop strategies to encourage spousal participation when planning workplace health education for changing health-related behaviors.

남성 사무직근로자의 심뇌혈관질환 위험요인에 따른 건강행위, 웰니스 상태 및 건강행위변화단계의 관계 (Relationships among Health Behavior, Wellness Condition, and Stage of Change in Health Behavior by Cardiocerebrovascular Risk in Male Office Workers)

  • Purpose: The purpose of this study was to investigate the relationships among health behavior, wellness condition, and stage of change in health behavior by risk of cardiocerebrovascular diseases (CVD) in male office workers. Methods: A total of 205 male office workers participated in the 2017 National Health Examination at a manufacturing/ R&D business in Seongnam and completed self-reported questionnaires. Results: There were significant differences in health behavior scores by risk of CVD (F=4.78, p=.009) and statistically significant differences in no smoking (F=5.86, p=.003), exercise (F=5.49, p=.005), and health checkup (F=4.39, p=.014). There were statistically significant differences in health behavior (t=-4.14, p<.001) and wellness condition (t=-2.61, p=.010) by the stage of change in health behavior. Health behavior had a weak positive correlation with wellness condition (r=.36, p<.001). Logistic regression analysis showed that, when adjusted for age and employment period, the probability of becoming attention or risk group was 11% lower for quitting smoking (OR 0.89, 95% CI 0.81~0.97, p=.006) and 18% lower for regular exercise (OR 0.82, 95% CI 0.70~0.95, p=.009). Conclusion: The direction of health management at the workplace should be changed to promote the wellness of all workers, not the management of the disease, and a health promotion program should be continued to emphasize health behaviors such as smoking cessation and regular exercise.

일 지역 남성 운전직 근로자들의 심뇌혈관질환 발병위험도, 예방 관련 지식, 변화단계 및 건강행위 (Incidence Risk of Cardiocerebrovascular Disease, Preventive Knowledge, Stage of Change and Health Behavior among Male Bus Drivers)

  • Purpose: This study was designed to identify the incidence risk of cardicerebrovascular disease (CVD) among male bus drivers, and to examine and compare the predictors of their health behavior according to the level of CVD incidence risk. Methods: The convenience sample of 222 male bus drivers were recruited from a bus company located in Jeonnam province. Data were collected from self-reported questionnaires and annual medical examination records from 2010. The CVD incidence risk was calculated based on the risk criteria for industrial workers. Results: The 26.6% and 26.1% of the participants were in the moderate and high risk group, respectively. The 72% of the participants were in the precontemplation stage and reported no intention to change their unhealthy lifestyles. Stepwise multiple regression analyses showed that current smoking, excessive alcohol drinking, physical inactivity and lack of knowledge were negative predictors of good health behavior in the normal/low risk group (Adj $R^2$=.443). Heavy alcohol drinking, current smoking, physical inactivity and dyslipidemia were reported by the moderate/high risk group (Adj $R^2$=.427). Conclusion: This study suggested that targeted education and counseling are needed to modify unhealthy lifestyles such as alcohol consumption, smoking and exercise among middle aged male drivers. Especially, dyslipidemia should be managed among those who are at risk for CVD.
