  • Phase-contrast(PC) methods have been used for quantitative measurements of velocity and volume flow rate. In addition, phase contrast cine magnetic resonance imaging (MRI) combines the flow dependent contrast of PC MRI with the ability of cardiac cine imaging to produce images throughout the cardiac cycle. In this method, the through-plane velocity has been encoded generally. However, the accuracy of the flow data can be reduced by the effect of flow direction, finite slice thickness, resolution, pulsatile flow pattern, and so on. In this study we calculated the error caused by misalignment of tomographic plane and flow directon. To reduce this error and encode the velocity for more complex flow, we suggested 3 directional velocity encoding method.

MRI에서 비허혈성 심근병증으로 오인된 좌심실의 림프종: 증례 보고 (Non-mass-forming Lymphoma of the Left Ventricle Mimicking Non-ischemic Cardiomyopathy on MR Imaging: A Case Report)

  • 40세 남성에서 상대정맥의 폐쇄와 우폐동맥 협착을 동반한 종격동 종괴가 발견되어 병리학적 소견에 경화성 종격동염으로 진단되었고, 6개월 후 심장 자기공명영상(MRI)에서 비허혈성 심근병증으로 의심된 심장림프종의 예를 보고하고자 한다. 심장 초음파에서 제한적 양상의 심장 생리현상을 보였고 구혈율이 매우 감소되어 있었다. MRI에서 전반적인 심근비대와 조영제 주입 후 광범위한 지연조영증강이 있어 비허혈성 심근병증이 의심되었으며 심장근육 생검 후 림프종으로 확진되었다. 조영제 주입후 MRI에서 심근 기저부는 침범되지 않았고 전반적인 심내막 보존이 있었으므로 아밀로이드증과 구별되는 양상이고 종괴를 형성하지 않았기 때문에 심장종양 보다는 심근병증에 더 유사한 영상 소견을 보인 특이한 증례로서 심장림프종의 자기공명영상에서의 특징을 문헌고찰과 함께 보고하고자 한다.

  • Magnetic Resonance Imaging (MRI) is one of the most advanced imaging techniques in clinical and research medicine. However, clinical application of MRI to the lung or thorax has been limited due to various drawbacks. Low signal intensity of the lung and cardiac and respiratory movements are the most serious problems with MRI in thorax. Nevertheless, MRI is superior to CT in some selected patients with thoracic diseases. The role of clinical MRI in thoracic disease has been widened with improvement of MR equipments and development of new pulse sequences. Otherwise, functional assessment of lung by MRI has been studied for the last decade. These include perfusion MRI with or without contrast enhancement and ventilation MRI with oxygen-enhancement or hyperpolarized noble gas, $^3He$ and $^{129}Xe$.

다중 생체 신호를 이용한 심장 자기공명영상 스마트 트리거 시스템 (Multi-biological Signal-based Smart Trigger System for Cardiac MRI)

  • In cardiac magnetic resonance imaging (CMRI), heart and respiratory motions are one of main obstacles in obtaining diagnostic quality of images. To synchronize CMRI to the physiological motions, ECG and respiratory gatings are commonly used. In this paper multi-biological signal (ECG, respiratory, and SPO2) based smart trigger system is proposed. By using multi-biological signal, the proposed system is robust to the induced noise such as eddy current when gradient pulsing is continuously applied during the examination. Digital conversion of the multi-biological signal makes the system flexible in implementing smart and intelligent algorithm to detect cardiac and respiratory motion and to reject arrhythmia of the heart. The digital data is used for real-time trigger, as well as signal display, and data storage which may be used for retrospective signal processing.

전이학습을 수행한 신경망을 사용한 압축센싱 심장 자기공명영상 (Compressed-Sensing Cardiac CINE MRI using Neural Network with Transfer Learning)

  • 전이학습을 수행한 심층 인공신경망을 압축센싱 심혈관 자기공명영상에 적용하였다. 전이학습은 선행학습 신경망의 구조나 필터 커널, 가중치를 현재의 학습이나 응용에 활용하는 방법이다. 전이학습은 학습 속도를 향상시키고, 학습 데이터가 제한적일 때 신경망의 일반화에 도움이 된다. 8명의 건강한 지원자가 참여한 심장 자기공명영상 실험에서 전이학습을 수행한 신경망은 단독학습 신경망에 비해 학습시간이 5배 이상 단축되었다. 시험 데이터에 대해서도 전이학습을 수행한 신경망은 전이학습을 수행하지 않은 신경망에 비하여 낮은 정규화 평균제곱오차와 향상된 재구성 영상화질을 보였다.

Anomalous Origin of the Coronary Artery from the Pulmonary Artery in Children and Adults: A Pictorial Review of Cardiac Imaging Findings

  • Anomalous origin of the coronary artery from the pulmonary artery is a rare and potentially fatal congenital heart defect. Up to 90% of infants with an anomaly involving the left coronary artery die within the first year of life if left untreated. Patients who survive beyond infancy are at risk of sudden cardiac death. Cardiac CT and MRI are increasingly being used for the accurate diagnosis of this anomaly for prompt surgical restoration of the dual coronary artery system. Moreover, life-long imaging surveillance after surgery is necessary for these patients. In this pictorial review, multimodal cardiac imaging findings of this rare and potentially fatal coronary artery anomaly are comprehensively discussed, and representative images are provided to facilitate the understanding of this anomaly.

  • Heart related special images are classified as Cardiac US, XA, CT, MRI. Several Problem is caused by image compression, control and medical support point, so most big hospitals have created a Cadiac PACS differentially in past years. For this reason, create a conflict in inner colleague and patient, protector that result from 2 data processing server operating independently in 1 medical center area. For this reason, we sugges an alternative model of best medical control process together with understand the current situation on medical facility.

원발성 심장 종양의 수술적 치료 - 27례 보고- (Surgical Treatment of Primary Cardiac Tumor -Report of 27 cases -)

  • 대상 및 방법: 가톨릭대학교 의과대학 흉부외과학 교실에서는 1979년 부터 1996년까지 27명의 원발성 심장 종양 환자를 수술하였으며, 환자의 연령은 21세부터 67세까지로 평균 45.1세이었다. 술후 병리학적 진단결과 24례는 점액종, 2례는 연골 육종, 1례는 혈관 육종이며, 가장 흔한 술전 증상은 심부전이었다. 진단은 심장초음파로 확진되었고 호발 부위는 난원와였다. 종양 주위를 세심히 관찰후 심방 중격을 포함하여 완전 절제를 시행하였으며 술후 대부분의 환자 에서 증상 호전이 있었다. 결과: 1례에서 수술 직후 저심박출증으로 사망하였고, 2례에서 국소 재발로 인해 만기 사망하였다. 결론: 결론적으로, 수술시 완전 절제가 중요하며 술전 심장 초음파 검사상 양성으로 보일지라도 악성의 가능성을 배제해서는 안되며 CT나 MRI가 진단에 도움이 되며 수술시에 악성이 의심되면 광범위한 절제와 더불어 술후 항암제및 방사선 요법이 도움이 되리라 생각된다.

심장 자기공명영상을 이용한 팔로사징 완전 교정술 후 우심실 기능에 대한 연구 (Investigation of right ventricle function in patients with tetralogy of Fallot after total correction using cardiac magnetic resonance imaging)

  • Background: We investigated the difference in right ventricle (RV) volume and ejection fraction (EF) according to the pulmonary valve (PV) annular extension technique during Tetralogy of Fallot (TOF) total correction. Methods: We divided patients who underwent the procedure from 1993 to 2003 into two groups according to PV extension technique (group I: PV annular extension, group II: no PV annular extension) during TOF total correction. We then analyzed the three segmental (RV inlet, trabecular and outlet) and whole RV volume and EF by cardiac magnetic resonance imaging (MRI). Results: Fourteen patients were included in this study (group I: 10 patients, group II: four patients; male: nine patients, female: five patients). Cardiac MRI was conducted after a 16.1 years TOF total correction follow-up period. There was no statistical difference in RV segmental volume index or EF between groups (all p>0.05). Moreover, the total RV volume index and EF did not differ significantly between groups (all p>0.05). Conclusion: The RV volume and EF of the PV annular extension group did not differ from that of the PV annular extension group. Thus, PV annular preservation technique did not show the surgical advantage compared to PV annular extension technique in this study.

Imaging Findings of Coronary Artery Fistula in Children: A Pictorial Review

  • Coronary artery fistula, defined as an abnormal communication between the coronary arteries and a cardiac chamber (most commonly) or a thoracic great vessel, may result in hemodynamically significant problems due to vascular shunting in children. Echocardiography, cardiac catheterization, cardiac MRI, and cardiac CT may be used to evaluate coronary artery fistula in children. Recently, CT has played a pivotal role for the accurate diagnosis of coronary artery fistula in children. Surgical or interventional treatment is performed for hemodynamically significant coronary artery fistulas. In this pictorial review, the detailed imaging findings of coronary artery fistula in children are described.
