Purpose: Hypofractionated radiotherapy (RT) is becoming a new standard in postoperative treatment of patients with early stage breast cancer after breast conservation surgery. However, data on hypofractionation in patients with advanced stage disease who undergo mastectomy followed by local and regional nodal irradiation (RNI) is lacking. In this retrospective study, we report late-term effects of 3 weeks post-mastectomy hypofractionated local and RNI with two-dimensional (2D) technique in patients with stage II and III breast cancer. Methods: Between January 1990 and December 2007, 1,770 women with breast cancer who were given radical treatment with mastectomy, systemic therapy and RT at least 10 years ago were included. RT dose was 35 Gy/15 fractions/3 weeks to chest wall by two tangential fields and 40 Gy in same fractions to supraclavicular fossa (SCF) and internal mammary nodes (IMNs). SCF and IMNs dose was prescribed at dmax and 3 cm depth, respectively. Chemotherapy and hormonal therapy was given in 64% and 74% patients, respectively. Late-term toxicities were assessed with the Radiation Therapy Oncology Group (RTOG) scores and LENT-SOMA scales (the Late Effects Normal Tissue Task Force-Subjective, Objective, Management, Analytic scales). Results: Mean age was 48 years (range, 19 to 75 years). Median follow-up was 12 years (range, 10 to 27 years). Moderate/marked arm/shoulder pain was reported by 254 (14.3%) patients. Moderate/marked shoulder stiffness was reported by 219 (12.3%) patients. Moderate/marked arm edema was seen in 131 (7.4%) patients. Brachial plexopathy was not seen in any patient. Rib fractures were noted in 6 (0.3%) patients. Late cardiac and lung toxicity was seen in 29 (1.6%) and 23 (1.3%) patients, respectively. Second malignancy developed in 105 (5.9%) patients. Conclusion: RNI with 40 Gy/15 fractions/3 weeks hypofractionation with 2D technique seems safe and comparable to historical data of conventional fractionation (ClinicalTrial.gov Registration No. NCT04175821).
Background: Liver cancer is the most frequent cancer among Thais especially people in northeastern Thailand, but there has as yet been no assessment of trend. The data of all cancers in Khon Kaen can be retrieved from data base of the Khon Kaen Cancer Registry (KKCR) which was established in 1984. Objective: To assess the incidence trend of hepatocellular carcinoma in Khon Kaen, Thailand, between 1990 and 2009. Methods: Population-based cases of liver cancer registered between 1985 and 2009 were retrieved from the KKCR data base and cases with diagnosis of hepatocellular carcinoma (HCC) with the coding C22.0 according to ICD-O were selected. Incidence trends were calculated using the Jointpoint analysis. Results: There were 7,859 cases of HCC during the study period. Males were affected two times more frequently than females. The most common age group of cases was 50 and 69 years (60.3%). Most patients were diagnosed based on radiology imaging (40.6%) while the morphology verification was 7%. The age-standardized rates (ASR) were 13.1 to 49.8 per 100,000 among males and 4.8 to 38.4 per 100,000 among females depending on year of diagnosis since 1985. Remarkably, the ASRs were clearly low during first few years of starting the registration. The overall ASRs of HCC were 30.3 per 100,000 in males (95% CI: 25.9 to 34.6) and 13.1 per 100,000 (95% CI: 10.4 to 15.8) in females. During 1990-2009, the trends in incidences have been decreasing significantly with the annual percent change (APC) of 6.2% per year (95% CI: -7.6 to -4.8) in males and by 6.5% per year in females (95% CI: -8.4 to -4.9). Conclusions: The incidence trends have been decreasing in both sexes. The recent decline in incidence may represent a falling risk.

Purpose: This study aims to contribute to the reduction of complications of breast cancer radiation therapy by analyzing skin dose differences due to Set-up error. Materials and Method: Pseudo breast was produced using a 3D printer, applied to the phantom, and images were acquired through CT. Treatment plan was carried out that the PTV, which contains 95% of the prescription dose, could be more than 95% of the volume, so that Dmax did not exceed 107% of the prescription dose. The Set-up error was evaluated by applying ±1mm/±3mm/±5mm to the X-axis, Y-axis, and Z-axis. Results: The dose-variation in skin due to Set-up error was approximately 106% to 123% compared to prescription dose, and the highest dose in skin was 49.24 Gy at 5mm Set-up error in the lateral direction of the X-axis. More than 107% of the prescription dose was the widest at 6.87 cc in skin lateral. Conclusions: If a Set-up error occurs during left breast cancer VMAT, a great difference in skin dose was shown in the lateral direction of the X-axis. If more effort is made to align the X-axis of the breast treated during CBCT registration, the dose-variation of skin will be reduced.

Background: This study was performed to evaluate the effect of gastric cancer screening through analysis of screening-related data. Methods: We investigated claims data of gastric cancer from 2009 to 2015. We evaluated whether the screening was performed to prior to registration as patients with gastric cancer. The effect of gastric cancer screening was also analyzed by gender. Results: We collected total 196,293 patients with gastric cancer. 74% of them had previous experience of gastric cancer screening. In patients with screening, early gastric cancer was 33.4% and advanced gastric cancer was 17.3%. 22,548 (15.5%) patients were diagnosed with gastric cancer within 2 years after screening. In the case of patients without screening, early gastric cancer was 15.1% and advanced gastric cancer was 25.3%. In case of men, 76% of them confirmed gastric cancer through screening, and 70.2% of women confirmed the gastric cancer. In both men and women, the rate of early gastric cancer was higher among those with screening than those without screening. Conclusion: In this study, we were able to indirectly confirm the stage shift of gastric cancer screening. However, within 2 years after screening, not a few patients with gastric cancer were diagnosed. Therefore, more studies are warranted to in the future.
In sampling survey, nonresponse tend to occur inevitably. If we use information from respondents only, the estimates will be baised. To overcome this, various non-response imputation methods have been studied. If there are few auxiliary variables for replacing missing imputation or spatial autocorrelation exists between respondents and nonrespondents, spatial autocorrelation can be used for missing imputation. In this paper, we apply several nonresponse imputation methods including spatial imputation for the analysis of farm household economy data of the Gangwon-Do in 2002 as an example. We show that spatial imputation is more efficient than other methods through the numerical simulations.
Purpose: Endoscopic diagnosis of gastric cancer (GC) that emerges after eradication of Helicobacter pylori may be affected by unique morphological changes. Using comprehensive endoscopic imaging, which can reveal biological alterations in gastric mucosa after eradication, previous studies demonstrated that Congo red chromoendoscopy (CRE) might clearly show an acid non-secretory area (ANA) with malignant potential, while autofluorescence imaging (AFI) without drug injection or dyeing may achieve early detection or prediction of GC. We aimed to determine whether AFI might be an alternative to CRE for identification of high-risk areas of gastric carcinogenesis after eradication. Materials and Methods: We included 27 sequential patients with metachronous GC detected during endoscopic surveillance for a mean of 82.8 months after curative endoscopic resection for primary GC and eradication. After their H. pylori infection status was evaluated by clinical interviews and $^{13}C$-urea breath tests, the consistency in the extension of corpus atrophy (e.g., open-type or closed-type atrophy) between AFI and CRE was investigated as a primary endpoint. Results: Inconsistencies in atrophic extension between AFI and CRE were observed in 6 of 27 patients, although CRE revealed all GC cases in the ANA. Interobserver and intraobserver agreements in the evaluation of atrophic extension by AFI were significantly less than those for CRE. Conclusions: We demonstrated that AFI findings might be less reliable for the evaluation of gastric mucosa with malignant potential after eradication than CRE findings. Therefore, special attention should be paid when we clinically evaluate AFI findings of background gastric mucosa after eradication (University Hospital Medical Information Network Center registration number: UMIN000020849).
Background and Objectives: In previous studies, high homocysteine levels were associated with high cardiovascular mortality. However, these results were inconsistent with those of randomized controlled trials. We aimed to evaluate the causal role of homocysteine on allcause and cardiovascular mortality using Mendelian randomization (MR) analysis. Methods: This study included the 10,005 participants in the Namwon Study. In conventional observational analysis, age, sex, survey years, lifestyles, body mass index, comorbidities, and serum folate level were adjusted using multivariate Cox proportional regression. MR using 2-stage least squares regression was used to evaluate the association between genetically predicted plasma homocysteine levels and mortality. Age, sex, and survey years were adjusted for each stage. The methylenetetrahydrofolate reductase (MTHFR) polymorphism was used as an instrumental variable for predicting plasma homocysteine levels. Results: Observed homocysteine levels were positively associated with all-cause (hazard ratio [HR], 1.40; 95% confidence interval [CI], 1.26-1.54) and cardiovascular (HR, 1.62; 95% CI, 1.28-2.06) mortality when plasma homocysteine levels doubled. However, these associations were not significant in MR analysis. The HRs of doubling genetically predicted plasma homocysteine levels for all-cause and cardiovascular mortality were 0.99 (95% CI, 0.62-1.57) and 1.76 (95% CI, 0.54-5.77), respectively. Conclusions: This MR analysis did not support a causal role for elevated plasma homocysteine concentrations in premature deaths.
During image-guided radiation treatment of lung cancer patients, it is necessary to track the tumor motion because it can change during treatment as a consequence of respiratory motion and cardiac motion. In this paper, we propose a method for tracking the motion of the lung tumors based on the three-dimensional image information from planning 4D MDCT and treatment 4D CBCT images. First, to effectively track the tumor motion during treatment, the global motion of the tumor is estimated based on a tumor-specific motion model obtained from planning 4D MDCT images. Second, to increase the accuracy of the tumor motion tracking, the local motion of the tumor is estimated based on the structural information of the tumor from 4D CBCT images. To evaluate the performance of the proposed method, we estimated the tracking results of proposed method using digital phantom. The results show that the tumor localization error of local motion estimation is reduced by 45% as compared with that of global motion estimation.
With increasing of GDP, the registration number of passenger cars has exceeded 20 million last year in Korea. Especially, the registration number of the diesel engine vehicles has been increasing. However, the WHO(World Health Organization) IARC (International Agency for Research on Cancer) has reported that diesel engine exhaust gas is an one of HAPs, which has carcinogenic for human, and they have designated it to Group 1. To solve this problem, exhaust gas from diesel engines has to be controlled. Thus, it has been controlling by European regulatory standard in Korea. On the other hand, in order to meet the enhanced emission regulations, all manufacturing company applied $NO_x$ control device to vehicles such as EGR (Exhaust Gas Recirculation), SCR (Selective Catalytic Reduction) and so on. However, these devices (EGR, SCR) were operated by difference reaction mechanism respectively, and the composition of exhaust gas would be differenced from that of them. In this study, it was conducted to evaluate variety characteristics on changing of exhaust gas composition by each $NO_x$ control device, and the heavy duty diesel trucks were chosen as experimental vehicles. From the result, it revealed that vehicles (with EGR) were discharged higher THC as 52.5% than that of others (with SCR). However, it did not followed that trend, in the case of CO; it was discharged as 57.2% lower than that of others (with SCR). In the future, these data would be used to apply to efficient $NO_x$ control device for meeting to EURO 6.
