진행성 위암에 대한 면역 요법의 최신 지견 (Recent Progress in Immunotherapy for Advanced Gastric Cancer)

  • Immune checkpoint inhibition has been established as a new treatment option for various types of carcinoma, and many clinical trials are being actively conducted as a treatment for advanced or metastatic gastric cancer, either as a monotherapy with an immune checkpoint inhibitor or as a combination therapy with standard chemotherapy. In the CheckMate-649 clinical trial to confirm the efficacy of the combination of nivolumab and chemotherapy (FP) in advanced gastric cancer and gastroesophageal junction cancer, nivolumab group showed improvement in overall survival in programmed death ligand 1-positive cancer patients compared with placebo group. Also, the combination therapy of pembrolizumab, trastuzumab and chemotherapy (FP) in first-line treatment was tested through the KEYNOTE-811 trial. The pembrolizumab group showed 22.7% of improvement in objective response rate compared with placebo group. Accordingly, the combination of nivolumab/pembrolizumab with standard chemotherapy was approved for the first-line treatment. In KEYNOTE-059 trials for patients with progressive disease after at least two lines of chemotherapy, pembrolizumab monotherapy showed improvement in objective response rate and overall survival, and the use of pembrolizumab was approved for the third-line or more treatment. In this article, we review the result of clinical trials related to immune checkpoint inhibitors that have been recently introduced in the treatment of gastric cancer.

원발성 폐암에서 혈장 과립구 자극인자의 암표지자로서의 역할과 의의 (The Role and Significance of Biomarker for Plasma G-CSF in Patients with Primary Lung Cancer)

  • 연구배경: 폐암은 진단 당시에 완치할 수 있는 확률이 적어 예후가 불량한 종양으로 알려져 있어, 폐암 진행을 예측할 수 있는 암 표지자(tumor marker)의 발굴이 필요한 실정이다. 그러나 폐암에서 아직까지 특이적인 항원이 없고 현재까지 알려진 많은 종양관련 항원들의 민감도가 떨어지기 때문에 보편화되지 못하고 있다. 본 연구에서는 원발성 폐암 환자에서 혈장 G-CSF를 측정하고 암의 진행 및 예후와 관련이 있는지 알아보고자 하였다. 방 법: 원발성 폐암으로 진단된 100명 환자와 건강 검진에서 이상 소견이 없는 127명 정상인을 대상으로 하였다. 결 과: 정상인에서 혈장 G-CSF 농도는 12.2$\pm$3.6 pg/mL (mean$\pm$SD), 폐암환자에서는 46.0$\pm$38.0 pg/mL였다(p<0.001). 비소세포폐암에서 G-CSF 농도는 유의하게 소세포폐암보다 높았으며(p<0.05), 비소세포 폐암중 대세포 폐암이 가장 높았고, 편평세포암, 선암, 세기관지폐포암 순이었다. G-CSF 농도는 국소형보다는 진행형 비소 세포폐암에서 증가하는 경향을 보였다. 또한 타 장기로의 전이가 있을 때 유의하게 증가하였으며(p<0.05), 다발성 전이에서는 뇌, 부신, 골 전이 순으로 혈청 G-CSF 농도가 증가하는 경향이었다. 결 론: 혈장 G-CSF 농도는 폐암이 진행한 경우, 특히 타 장기로의 전이가 있을 때 유의하게 증가하였다. 그러므로 진행형 폐암의 추적관찰에 이용할 수 있으리라 사료된다.

Estimation of Superoxide Dismutase and Glutathione Peroxidase in Oral Submucous Fibrosis, Oral Leukoplakia and Oral Cancer - A Comparative Study

  • Present study was undertaken to estimate and compare erythrocyte superoxide dismutase (E-SOD) and Glutathione peroxidase (GPx) levels in oral submucous fibrosis, oral leukoplakia and oral cancer patients and age/sex matched healthy subjects, 25 in each group. Statistically significant (P<0.001) decrease in E-SOD and GPx levels were observed in OSF, oral leukoplakia and oral cancer groups as compared to the control group. Oral leukoplakia group showed lower levels in comparison with OSF (P>0.05). Oral cancer group had the lowest levels amongst the study groups. Imbalance in antioxidant enzyme status may be considered as one of the factors responsible for the pathogenesis of cancer and may serve as a potential biomarker and therapeutic target to reduce the malignant transformation in oral premalignant lesions/conditions.

Classification of Genes Based on Age-Related Differential Expression in Breast Cancer

  • Transcriptome analysis has been widely used to make biomarker panels to diagnose cancers. In breast cancer, the age of the patient has been known to be associated with clinical features. As clinical transcriptome data have accumulated significantly, we classified all human genes based on age-specific differential expression between normal and breast cancer cells using public data. We retrieved the values for gene expression levels in breast cancer and matched normal cells from The Cancer Genome Atlas. We divided genes into two classes by paired t test without considering age in the first classification. We carried out a secondary classification of genes for each class into eight groups, based on the patterns of the p-values, which were calculated for each of the three age groups we defined. Through this two-step classification, gene expression was eventually grouped into 16 classes. We showed that this classification method could be applied to establish a more accurate prediction model to diagnose breast cancer by comparing the performance of prediction models with different combinations of genes. We expect that our scheme of classification could be used for other types of cancer data.

Analysis of G3BP1 and VEZT Expression in Gastric Cancer and Their Possible Correlation with Tumor Clinicopathological Factors

  • Purpose: This study aimed to analyze G3BP1 and VEZT expression profiles in patients with gastric cancer, and examine the possible relationship between the expressions of each gene and clinicopathological factors. Materials and Methods: Expression of these genes in formalin-fixed paraffin embedded (FFPE) tissues, collected from 40 patients with gastric cancer and 40 healthy controls, was analyzed. Differences in gene expression among patient and normal samples were identified using the GraphPad Prism 5 software. For the analysis of real-time polymerase chain reaction products, GelQuantNET software was used. Results: Our findings demonstrated that both VEZT and G3BP1 mRNA expression levels were downregulated in gastric cancer samples compared with those in the normal controls. No significant relationship was found between the expression of these genes and gender (P-value, 0.4835 vs. 0.6350), but there were significant changes associated with age (P-value, 0.0004 vs. 0.0001) and stage of disease (P-value, 0.0019 vs. 0.0001). In addition, there was a direct relationship between VEZT gene expression and metastasis (P-value, 0.0462), in contrast to G3BP1 that did not demonstrate any significant correlation (P-value, 0.1833). Conclusions: The results suggest that expression profiling of VEZT and G3BP1 can be used for diagnosis of gastric cancer, and specifically, VEZT gene could be considered as a biomarker for the detection of gastric cancer progression.

Dickkopf-1 Levels in Turkish Patients with Bladder Cancer and its Association with Clinicopathological Features

  • Background: Evidence indicates that Dickkopf-1 (DKK-1) levels may be a biomarker for cancer risk. The aim of this study was to assess DKK-1 and its correlation with clinic-pathological features in patients with bladder cancer. Materials and Methods: DKK-1 levels were determined in serum samples from 90 patients with bladder cancer before transurethral tumor resection. The concentrations of DKK-1 were determined by using enzyme linked immune-sorbent assay (ELISA). Results: Elevated preoperative DKK-1 levels were associated with tumor stage (p<0.001), grade (p<0.001) and histological grade (p<0.001). Conclusions: The results of our study demonstrated that the level of serum DKK-1 is correlated with both disease progression and increase in the tumor grade. Preoperative serum DKK-1 elevation may thus represent a novel marker for the determination of bladder cancer and the detection of patients with a likely poor clinical outcome.

Survey of Her2-neu Expression and its Correlation with Histology of Gastric Carcinoma and Gastroesophageal Junction Adenocarcinoma

  • Background: There is increasing evidence that HER2-neu is an important biomarker in gastric carcinomas (GC) and gastroesophageal junction (GEJ) adenocarcinomas. The aim of this study was to evaluate HER2-neu expression and also some clinicopathological features of these neoplasms. Materials and Methods: We selected 211 paraffin-embedded blocks, 193 GC and 18 GEJ. Then 4 micron sections were prepared for staining with hematoxylin and eosin and also for IHC (Her2-neu). The Chi-square test was used for significance between expression of HER2-neu and clinicopathological parameters. Results: In patients with advanced cancer of GC and GEJ, HER2-neu overexpression was more associated with the intestinal cancer subtype. Conclusions: This could be a guide to new complementary therapy for affected patients.

Expression of Annexin A3 in Gastric Cancer and its Correlation with Proliferation and Apoptosis

  • Annexin A3 has been identified as a novel biomarker in different types of cancers. However, little is known about its clinical significances and and biological roles in gastric cancer. In this study, we assessed annexin A3 expression in 80 patients with gastric cancer and explore its correlation with prognosis Moreover, correlations with Ki-67, Bcl-2 and Bax were also investigated. Expression of annexin A3 was increased in gastric cancer compared with that in normal gastric tissues. Annexin A3 expression was significantly associated with tumor volume and TNM stage (p<0.05). and inversely correlation with prognosis of patients. More interestingly, expression of annexin A3 was positive correlated with Ki-67 and Bcl-2 expression. Our study showed annexin A3 might be a potential prognostic marker for gastric cancer and involved in tumorigenesis by regulating apoptosis and proliferation.

Carboplatin/Paclitaxel과 한방치료를 병행하며 부분 관해 된 폐전이 동반 말기 난소암 1례 (A Case Report of Partial Remission of End-stage Ovarian Cancer Patient with Lung Metastasis Treated with Carboplatin/Paclitaxel and Traditional Korean Medicine)

  • Objectives: The purpose of this study is to report the effect of combination treatment of Traditional Korean medicine (TKM) and chemotherapy on a ovarian cancer with lung metastasis patient. Methods: One ovarian cancer with lung metastasis patient was treated by TKM in conjunction with Carboplatin/paclitaxel since Feb. 2020. Repeat cycle every 3 weeks for 6 times. The patient has been treated with TKM at the same time. To evaluate the patient, symptoms were measured by Numeric Rating Scale (NRS), Eastern Cooperative Oncology Group (ECOG) and tumor size was measured by scanning with Computed Tomography (CT). Blood tests including cancer biomarker were conducted during treatment. Adverse events were evaluated by the National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), version 5.0. Results: After treatment with Carboplatin/paclitaxel and TKM during 2 months, the size of the ovarian cancer was decreased(Partial Response, PR), size and malignant pleural effusion at right lung disappeared. And no evidence of newly developed metastatic lesions. After 2 months, the tumor response was stable disease while improving the performance and other symptoms. Conclusions: This case provides us conjunctive treatment with Conventional and Eastern medicine may have substantial benefit for patients with end-stage ovarian cancer.

B세포림프종의 임상적 악성도 표지자로서 혈청 Thymidine Kinase 1의 유용성 (Usefulness of Serum Thymidine Kinase 1 as a Biomarker for Aggressive Clinical Behavior in B-cell Lymphoma)

  • 배경: Thymidine kinase 1 (TK1)은 세포 주기의 중요한 조절 효소로 암세포의 증식 시에 증가하는 것으로 알려져 있으며, 현재까지 혈액종양과 다양한 고형암에서 진단 또는 치료 후 모니터링과 예후 예측에 중요한 표지자로 보고되고 있다. 본 연구에서는 한국인에서 혈청 TK1 정량분석을 통하여 건강인의 혈청 TK1 참고치를 설정하고자 하였으며 B세포림프종 환자를 대상으로 임상적 악성도 표지자로서 혈청 TK1 검사의 유용성을 평가하고자 하였다. 방법: 72명의 B세포림프종 환자와 143명의 건강대조군의 혈청검체에서 화학발광면역측정법으로 혈청 TK1 농도를 측정하였다. 건강대조군에서 혈청 TK1의 참고치를 설정하였고, 환자군과 건강대조군에서 측정된 혈청 TK1 결과를 이용해 ROC 분석을 통한 기준치를 구하여 상대적으로 높은 혈청 TK1 정량값과 B세포림프종의 임상 지표들과의 상관성을 비교하였다. 결과: 전체 건강대조군의 혈청 TK1의 95 percentile에 따른 참고범위는 5.4-21.8 U/L였다. B세포림프종 환자군과 건강대조군의 혈청 TK1 수치 비교에서 평균${\pm}$표준편차는 각각 $40.6{\pm}68.5U/L$$11.8{\pm}4.4U/L$로 나타났으며 두 그룹 간에 유의한 차이를 보였다(P<0.001). ROC 분석 후, 혈청 TK1 수치 15.2 U/L를 이용하였을 때 민감도 59.7%, 특이도 83.2%, AUC 0.73을 보여 B세포림프종 환자를 선별할 수 있는 기준치로 설정하였다. 상대적으로 높은 혈청 TK1 수치(${\geq}15.2U/L$)는 병기, 골수침범, IPI 점수, LD 수치, 낮은 Hb (<12 g/dL), 림프구 수와 상관성이 있는 것으로 나타났다. 결론: B세포림프종 환자에서 측정되는 혈청 TK1 수치는 B세포림프종의 임상적 악성도 표지자로서 유용하게 사용될 수 있을 것이다.
