위절제술환자의 간호실무표준 사용이 간호의 질에 미치는 영향 (A study of the impact of using a nursing care standards on the quality of nursing care in gastrectomy patients)

  • Nursing standards determine the type and extent of services that are delivered to the patients and define quality care and communicate the institution's expectations of care. Thus, taking the standard of care and incorporating it into a welldefined indicator of excellent patient care becomes one of the first activities in setting up the nursing service's quality assurance process. The purpose of this study was to determine the impact of using a nursing care standards for the quality of nursing care in gastrectomy patients. The subjects were composed of fourty-two under going gastrectomy patients with stomach cancer in general surgery nursing care unit of K University Hospital in Pusan. The data was collected from January 3 to April 13,1996. The subjects were divided into a control group - those admitted from Jan.3 to Feb.12 and an experimental group those admitted from Feb.18 to April 13. The instruments used for this study were a nursing care standards in gastrectomy patients developed by the investigator and an evaluation tool for the quality of nursing care in abdominal surgery patients developed by Byoung-Sook Lee in 1995. The data was analized by means of chi-square test, t-test and Cronbach-alpha test with the SAS System. The result was as follows : The hypothesis, that scores of the quality of nursing care in the experimental group would be higher than that of the control group. was supported(t=-6.12, p=0.00). The detailed results of each standards of evaluation tool were as follows : The mean score of the experimental group was significantly higher than that of the control group in audit standard 1:'Collection of basic data of the patients', (t=-3.76, p=0.00). The mean score of the experimental group was significantly higher than that of the control group in audit standard 2 : 'Defining nursing diagnoses(or nursing problems)', (t= (-), p= (-) ). The mean score of the experimental group was significantly higher than that of the control group in audit standard 3:'Estabilishment of nursing care plan according to nursing diagnoses(or nursing problems)',(t= (-), p= (-) ). The mean score of the experimental group was significantly higher than that of the control group in audit stndard 4:'Implimentation of nursing care plan', (t=-2.38, p=0.01). The mean score of the experimental group was significantly higher than that of the control group in audit standard 8 : 'Increase of the knowledge of health related to surgery',(t=-2.40, p=0.01). No significant differences between the mean scores of the experimental group and that of the control group in audit standard 5 : 'Recover and maintain of the physical function', audit standard 6:'Prevention of the post-operative complication', audit standard 7 : 'Decrease of discomfort caused by operation', and audit standard 9 : 'Patient satisfaction in nursing care' were found. The standards of evaluation tool were devided into two dimension. One was process dimension which contains four standards(audit standard 1 to 4), the other was outcome dimension which contains five standards(audit standard 5 to 9). The mean score of the experimental group was significantly higher than that of the control group in process dimension (t=-12.30, p=0.00), but no significant difference between the mean scores of the experimental group and that of the control group in outcome dimension was found. From these results, it is concluded that using a nursing care standards in gastrectomy patients promotes quality of nursing care and nursing care standards of various fields are necessary for effective nursing care.

Why Screening Rates Vary between Korea and Japan-Differences between Two National Healthcare Systems

  • Both Japan and Korea provide population-based screening programs. However, screening rates are much higher in Korea than in Japan. To clarify the possible factors explaining the differences between these two countries, we analyzed the current status of the cancer screening and background healthcare systems. Population-based cancer screening in Korea is coordinated well with social health insurance under a unified insurer system. In Japan, there are over 3,000 insurers and coordinating a comprehensive strategy for cancer screening promotion has been very difficult. The public healthcare system also has influence over cancer screening. In Korea, public healthcare does not cover a wide range of services. Almost free cancer screening and subsidization for medical cost for cancers detected in population-screening provides high incentive to participation. In Japan, on the other hand, a larger coverage of medical services, low co-payment, and a lenient medical audit enables people to have cancer screening under public health insurance as well as the broad range of cancer screening. The implementation of evidence-based cancer screening programs may be largely dependent on the background healthcare system. It is important to understand the impacts of each healthcare system as a whole and to match the characteristics of a particular health system when designing an efficient cancer screening system.

An Audit of 204 Histopathology Reports Over Three Years of Carcinoma of Cervix: Experience from a Tertiary Referral Centre

  • Background: The aim was to see compliance to minimum data set information in carcinoma cervix histopathology reports from a team of 13 pathologists; and also to analyse the distribution of parameters like tumor size, grade, depth of cervical stromal invasion, lymph node yield and pTNM stage. Materials and Methods: All pathology reports of radical hysterectomy for carcinoma cervix operated in house within a three year duration (2010-2012), (n=204) were retrieved from medical records and analyzed for the above parameters. Results: In 2010- 59 cases, in 2011- 67 cases and in 2012- 78 cases of carcinoma cervix underwent operations in our hospital. The median age was 50.5 years and the maximum T diameter was 2.8 cms in the reports of three years. Squamous carcinoma was the commonest subtype amongst all the tumors. It was noted that 60.8% of cases had cervical stromal involvement more than half the thickness of the cervical stroma. Parametrial involvement was seen in 4.82% of cases. pTNM Staging was not mentioned in 65.06% of the cases. The mean bilateral pelvic lymph node yield count in our study was 16.6 inclusive of all the three years. Conclusions: Compliance with provision of a minimum dataset in our team of 13 pathologists was generally good. Lymph node yield in our hands is reasonable, but constant striving for greater numbers should be made. pTNM staging should be more meticulously documented. Use of proformas /checklists is recommended.

Factors Predictive of Treatment by Australian Breast Surgeons of Invasive Female Breast Cancer by Mastectomy rather than Breast Conserving Surgery

  • Background: The National Breast Cancer Audit Database of the Society of Breast Surgeons of Australia and New Zealand is used by surgeons to monitor treatment quality and for research. About 60% of early invasive female breast cancers in Australia are recorded. The objectives of this study are: (1) to investigate associations of socio-demographic, health-system and clinical characteristics with treatment of invasive female breast cancer by mastectomy compared with breast conserving surgery; and (2) to consider service delivery implications. Materials and Methods: Bi-variable and multivariable analyses of associations of characteristics with surgery type for cancers diagnosed in 1998-2010. Results: Of 30,299 invasive cases analysed, 11,729 (39%) were treated by mastectomy as opposed to breast conserving surgery. This proportion did not vary by diagnostic year (p>0.200). With major city residence as the reference category, the relative rate (95% confidence limits) of mastectomy was 1.03 (0.99, 1.07) for women from inner regional areas and 1.05 (1.01, 1.10) for those from more remote areas. Low annual surgeon case load (${\leq}10$) was predictive of mastectomy, with a relative rate of 1.08 (1.03, 1.14) when compared with higher case loads. Tumour size was also predictive, with a relative rate of 1.05 (1.01, 1.10) for large cancers (40+ mm) compared with smaller cancers (<30 mm). These associations were confirmed in multiple logistic regression analysis. Conclusions: Results confirm previous studies showing higher mastectomy rates for residents of more remote areas, those treated by surgeons with low case loads, and those with large cancers. Reasons require further study, including possible effects of surgeon and woman's choice and access to radiotherapy services.

QUALITY ASSURANCE IMPLEMENTATION IN THE NATIONAL CANCER CENTRE

  • The importance of accurate dose delivery in radiotherapy is well documented. Studies have shown that a mere 5% deviation of the prescribed dose can produce an undesirable treatment outcome. Uncertainties in the dose delivery can arise at different stages of the radiotherapy process. Therefore, a good quality assurance programme will ensure the best possible results and consistency of the radiotherapeutic treatment. Quality assurance in any radiotherapy department involves the responsibility of a multi-disciplinary team of radiation oncologists, medical physicists and radiation technologists. This paper will focus on the physical and technical aspects of QA. The organizational structure and responsibility of the physics QA team is outlined and also included the types and frequencies of QA checks. For a QA program to be effective, action levels should be clearly defined and understood by all staff concerned. Data of the Singapore National Cancer Centre's participation over the last ten years with the IAEA / WHO Postal TLD Dose Inter-comparison programme is presented. The data obtained were within the international criteria. For a QA program to be successfully implemented, there must be a commitment by management to provide adequate staff, test equipment, machine time as well as continual training and education. This is in addition to the positive attitudes of all the staff. A quality audit is also necessary to serve as a check and balance to ensure that the QA is in order.

Survival of Triple Negative versus Triple Positive Breast Cancers: Comparison and Contrast

  • Background: Triple negative (TN) and triple positive (TP) breast cancers both are aggressive types but TN generally has a shorter survival. Objectives: To compare the clinical characteristics and treatment outcomes for patients with TN versus TP breast cancer and to assess various prognostic factors affecting overall survival. Materials and Methods: A retrospective audit of 85 breast cancer patients was conducted in the Department of Radiation Oncology and Medical Oncology on patients from 2006 to 2013 for whom IHC for ER, PgR and Her-2 neu were available. The patients were stratified into: ER-, PR- and Her-2 neu- (Arm A, n=47) and ER+, PgR+ and Her-2 neu+ (Arm B, n=38). Results: TN subtype had higher numbers of premenopausal and advanced stage patients as compared to TP subtype. The locoregional recurrence (LRR) and distant metastatic rate was also higher in TN subtype but there was no definite pattern in both the arms. Among the prognostic factors, patients with premenopausal status and advanced stage in TN breast cancer had inferior survival (P=0.07) whereas for those with postmenopausal status and early stage there was no survival difference between the two arms. Conclusions: TN subtype tends to be more aggressive in terms of younger age and advanced stage at presentation, higher tumour grade, LRR and metastasis, suggesting need for future research efforts on providing aggressive treatment to these patients. We could attribute better outcome for TP subtype to receptor positivity enabling role of hormonal treatment and targeted therapy, although less number of patients received targeted therapy.

화학요법을 받는 암환자의 증상 및 통증조절 현황 (Current Status of Symptom and Pain Control in Cancer Patients Treated with Chemotherapy)

  • 목적 : 본 연구의 목적은 화학요법을 받는 암환자의 증상과 통증조절을 위한 약물사용 및 그 치료방법의 현황을 살펴보기 위해 실시하였다. 방법 : 2001년 8월 7일부터 24일까지 광주광역시 일개 대학교 부속병원 혈액종양 내과병동에 입원치료중인 암환자와 주사실에서 화학요법을 받고 있는 전체환자 가운데 연구에 동의하고 질문에 응답을 허락한 66명과 chart audit 및 담당의사 19명을 대상으로 하였다. 대상의 일반적 특성 및 증상의 정도와 통증조절에 사용된 약물의 종류, 투여경로, 평균 투여량, 투여방법에 대하여 조사하였다. 일반적 특성은 수와 백분율로 제시하였으며, 증상의 빈도는 평균과 표준편차로 제시하였다. 진통제의 종류, 규칙적 또는 일시적(pm) 사용여부, 투여경로는 수와 백분율로 제시하였다. 결과 : 66명의 조사대상자 중 남성 35명(53.0%), 여성 31명(47.0%)이었고, 연령은 $61{\sim}69$세가 23명(34.8%)으로 가장 많았다. 진단명은 위암 19명(28.8%), 대장 직장암 17명(25.8%), 비뇨기계암 11명(16.7%) 순으로 나타났다. 진단명 기수는 3기 14명(29.2%), 2기 12명(25.0%), 1기 11명(22.9%), 4기 11명(22.9%) 순이었고, 진단기간은 3개월 이내가 25명(37.9%)로 가장 많은 것으로 나타났다. 화학요법을 받는 암환자의 증상으로는 '쇠약'($2.4193{\pm}1.2220$), '통증'($1.9333{\pm}1.2194$), '수면장애'($1.7142{\pm}1.0384$점), '성격변화'($1.5806{\pm}0.8971$) 순으로 나타났다. 화학요법을 받는 암환자의 통증조절을 위한 진통제는 마약성 진통제 66.2%, 비마약성 진통제 33.8%를 사용하고 있었다. 진통제의 규칙적인 투여는 21%, 일시적(pm)인 투여는 31.6%, 규칙적으로 투여하면서 필요시마다 pm으로 투여한 경우는 47.4%이었다. 진통제의 투여 경로는 경구 투여한 경우 50.7%, 주사로 투여한 경우 41.8%, 패치를 사용한 경우가 7.5%로 나타났다. 결론 : 화학요법을 받는 암환자의 증상으로는 쇠약과 통증을 가장 많이 호소하고 있었다. 그리고 통증조절을 위해서는 마약성 진통제를 우선적으로 사용하였지만 임상의사들의 21%에서만 진통제에 대한 규칙적인 투여가 이루어지는 것으로 나타나 통증조절 충분하다고 볼 수 없다. 이 같은 점을 고려하여 통증의 적절한 평가와 통증조절에 대한 실무와 교육이 필요하다고 생각된다.

Passive Smoking and Breast Cancer - a Suspicious Link

  • Background: Breast cancer is the most common malignancy of women in the world. The disease is caused by infectious and non-infectious, environmental and lifestyle factors. Tobacco smoke has been one of the most widely studied environmental factors wiith possible relevance to breast cancer. The purpose of this study was to assess the impact of tobacco smoking in breast cancer patients in a hospital based cohort and to establish prognostic implications if any. Materials and Methods: A retrospective audit of 100 women with pathological diagnosis of invasive breast cancer was included in this study. The verbal questionnaire elicited information on current and previous history of exposure to smoking in addition to active smoking. All analyses were adjusted for potential confounders, including stage at presentation, alcohol intake, hormonal replacement therapy, oral contraceptive intake, obesity and menopausal status. Results: The mean age at presentation of breast cancer was $51.4{\pm}10.86$ years. Mean age of presentation was $53.1{\pm}11.5$ and $45.7{\pm}11.9$ years in never smokers and passive smokers, respectively. Age at presentation varied widely in patients exposed to tobacco smoke for >10 years in childhood from $40.3{\pm}12.0$ years to $47.7{\pm}13.9$ in patients exposed for > 20years as adults. Among passive smokers, 60.9% were premenopausal and 39.1% of patients were postmenopausal. In never smokers, 71.4% were post menopausal. Expression of receptors in non-smokers vs passive smokers was comparable with no significant differences. Metastatic potential in lung parenchyma was slightlyelevated in passive smokers as compared to never smokers although statistically non-significant. Conclusions: An inverse relationship exists between the intensity and duration of smoking and the age at presentation and poor prognostic factors. The results strongly suggest efforts should be taken to prevent smoking, encourage quitting and restrict exposure to second hand smoke in India.

Characteristics Associated with Survival in Patients Receiving Continuous Deep Sedation in a Hospice Care Unit

  • Continuous deep sedation (CDS) is an extreme form of palliative sedation to relieve refractory symptoms at the end of life. In this study, we shared our experiences with CDS and examined the clinical characteristics associated with survival in patients with terminal cancer who received CDS. We conducted a chart audit of 106 consecutive patients with terminal cancer who received CDS at a single hospice care unit between January 2014 and December 2016. Survival was defined as the first day of admission to the date of death. The associations between clinical characteristics and survival were presented as hazard ratios and 95% confidence intervals using a Cox proportional hazard model. The mean age of participants was 65.2 years, and 33.0% (n=35) were women. Diazepam was the most commonly administered drug, and haloperidol or lorazepam were also used if needed. One sedative was enough for a majority of the patients. Stepwise multivariate analysis identified poor functioning, a high Palliative Prognostic Index score, hyperbilirubinemia, high serum ferritin levels, and a low number of sedatives as independent poor prognostic factors. Our experiences and findings are expected to be helpful for shared decision-making and further research on palliative sedation.

위암 수술의 질 관리 (Quality Assurance of Gastric Cancer Surgery)

  • 질 관리란 진료 행위의 표준화 작업과 행위의 다양한 평가를 통해서 더 나은 치료 결과를 얻고자 이루어지는 체계적이고 종합적인 작업이다. 일반적으로 외과 영역의 질 관리는 항암화학치료나 방사선치료에 비해 적용이 어려운 것으로 알려져 있다. 하지만 최근 다기관 전향적 임상시험이 활발히 진행되면서, 두 가지의 수술법을 비교하거나 수술 전후 보조요법의 유용성을 확인하는 임상 연구에서 있어서 수술의 질 관리의 중요성이 점점 더 강조되고 있다. 위암의 경우 몇몇 대규모 전향적 임상시험이 시행되었으나 수술의 질 관리가 만족할 만한 수준으로 유지된 경우는 많지 않다. 유럽에서 시행되었던 D1-D2 림프절 절제술 비교 임상시험은 수술의 질 관리에 많은 노력을 기울인 연구였으나 우리나라의 실정과는 차이가 있는 반면, 현재 일본에서 시행 중인 D2와 D2 및 대동맥 주위림프절 절제술을 비교하는 임상시험에서는 우리에게도 적용 가능한 질 관리 프로그램이 다수 포함되어 있다. 향후 위암 수술에서의 다기관 무작위 전향적 임상시험을 성공적으로 수행하기 위해서는 learning curve 와 연간 증례 수를 고려한 치료자의 선택, 다양한 교육 및 의견 교류를 통한 수술 방법의 표준화, 체계적이고 통일된 전향적 데이터베이스 프로그램의 개발, 표준화된 수술 후 환자 관리 프로그램의 개발, 표준화된 수술 후 환자 관리 프로그램의 개발, 병리 검사 및 병리 기록의 표준화 등이 요구된다. 하지만 지나치게 세세한 표준화 작업 및 너무 많은 감시 프로그램은 환자 및 치료자 대상군 확보를 어렵게 만들고 프로토콜 위반율을 높일 수 있다는 점을 염두에 두어야 할 것이다.
