한국형 Femoral IM Nail 설계를 위한 표준 Lateral 휨각도 분석 (Design of A Femoral IM Nail by analysis of Lateral Curve Angle)

  • A Femur is the longest and largest bone which supports body in human musculoskeletal structure. Therefore, it may cause heavy loss of blood when it is suffered by a simple or complex fracture, and the complication is very dangerous with a possibility of severe tissue damage. In this study, the femoral cancellous angle change is estimated in order to design the Korean femoral IM nail. Generally, it is various in the size and curvature of femoral cancellous bone depending on patient's body dimension. Therefore surgeon has difficulty in fitting this femoral IM nail to the patient in the surgical procedure. In our study, we tries to estimate femoral lateral curve angle with more precise method based on CT image of the femur and utilize this information on the design of femoral IM nail for Korean patients.

Lumbar spine 의 뼈와 Interbody cage의 접촉면에서 기계공학적 민감성 고찰 (The Mechanical Sensitivity at Interfaces between Bone and Interbody Cage of Lumbar Spine Segments)

  • 뼈의 성장에 미치는 많은 요소들 중에서 implant의 상대적인 미세운동(relative micromotion)은 뼈의 implant와의 접합을 방해하는 것으로 알려져 왔다. 그런데 이러한 상대적인 운동 및 spinal stability에 직접적으로 영향을 주는 하중조건, spinal material의 물성치, spinal geometry 및 뼈와 implant의 접촉면에서의 마찰계수를 고려하기 위하여, 하나의 titanium interbody cage 가 삽입된 human lumbar segments (L4-L5)의 유한요소 모델이 개발되었다. 이러한 유한요소 모델의 해석을 통하여 상대적인 미세운동, Posterior의 수직적인 변위, von Mises 응력 및 마찰력이 예측되었다. Cancellous bone. annulus fibers 및 ligaments의 기계적인 물성치의 감소 또는 접촉면에서의 마찰계수의 감소는 상대적인 미세운동 (relative micromotion or slip distance)을 증가 시켰다. 접촉면에서의 normal force는 뼈의 밀도 (cancellous bone density) 가 감소하거나 접촉마찰계수가 증가하면 감소했다. 특히 하중조건에 있어서, compressive preload에 대한 torsion의 추가는 접촉면의 anterior부위에서 상대적인 미세운동을 증가 시켰다. 하지만 디스크면적이 증가할수록 상대적인 미세운동은 감소했다. 결론적으로, 접촉면의 기계공학적 거동 (Relative micromotion, stress response, posterior axial displacement and contact normal force)은 접촉면의 마찰계수 뼈의 밀도, 하중조건 및 노화에 따른 형상/물성의 변화에 매우 민감함을 보이고있다.

임플란트 일차 안정성에 있어서 골질에 따른 osteotome 술식의 효과 (The Effect of Osteotome Technique on Primary Implant Stability according to Bone Quality)

  • The primary stability of implants is an important factor to predict the osseointegration. Recently, the resonance frequency analysis has been used to measure the primary stability. It is an objective method to monitor the stability of implants during healing phase. This study is to validate the differences in the effect of the osteotome method according to the bone quality as well the thickness of cortical bone. Two hundred seventy implants of 3.75mm in diameter(Neoplant, Neobiotech, Korea) were placed in 135 bovine ribs. The bone quality is classified into 3 classes according to the number of bone marrow spaces which implants would be placed, and then classified into 9 subclasses after the ribs were trimmed. Two implants were placed in 15 specimens of each class. The conclusion were as follows: 1. In case of less dense cancellous bone, the oseotome method is more effective in primary stability rather than the drilling method(p <0.05). 2. If there was cortical bone, it is more advantagous to get stronger primary stability. 3. If cancellous bone is more dense or if cortical bone exists, there is no statistical significance between drilling and osteotome method(p <0.05).

Ektaspeed Plus 방사선사진, 간접 디지털 영상 및 Digora 영상의 치근단 병소의 판독능 비교 (Detectability of Ektaspeed Plus Film, Digitized and Digora Images for Artificial Periapical Bone Lesions)

  • The comparative detectability of the artificial periapical defects among Ektaspeed Plus film. digitized and digora images was evaluated. The artifical defects were made in the cancellous bone and cancellous-cortical junction with the size of 1.0×0.8mm², 1.4×1.1mm² and 2.8×2.2mm². The defects in cancellous-cortical junction extended into cortical bone with the depth of 0, 0.5 and 1.0 mm. The results were as follows : 1. In junctional defects Ektaspeed Plus film for 2.8×12.2mm² defect showed the highest detectability. But significant difference were only found between Ektaspeed Plus films and digitized images (p<0.05). 2. Almost all defects within cacellous bone were not detected except a few digitized and Digora images for the size of 2.8×2.2mm². Digora images for them showed significant differences with Ektaspeed Plus films and digitized images (p<0.05). 3. The sensitinity of all imaging modalities were 0.9 or 1.0 in junctional defects for the size of 1.4×2.2mm² and 2.8×2.2mm². For cancellous defects, Digora image showed the highest sensitivity of 0.6 for the size of 2.8×2.2mm². 4. Significant differences for change of exposure time were found in most group of Ektaspeed Plus films and digitized images (p<0.05) . But there was no significant differences in Digora images for cacellous defects.

양측 전완앞발목관절 탈구의 관절고정술을 위한 돼지뼈 해면골이식의 적용 (The Use of a Porcine Cancellous Bone Graft for Arthrodesis of Bilateral Antebrachiocarpal Luxation in a Dog)

  • 일곱살령, 체중 2.1 kg, 요크셔테리어종이 2개월전 부터 양측 전지의 심한 파행과 관절의 불안정을 주 증상으로 본 원에 내원하였다. 일반 신체검사 및 방사선 검사에서 양측 전완앞발목관절의 후방탈구로 진단되었다. 대부분의 작은 견종에서의 양측 전완앞발목관절 탈구 치료는 광범위한 발목관절고정술을 실시한다. 폴리메칠메타크리레이트를 적용한 개선된 외부고정법은 유용한 치료법이 될 수 있는데 본 증례에서는 앞발목관절 간극에 돼지뼈 해면골의 충진을 동시에 실시 하였다. 시술 후 임상검사 및 방사선 평가에서 돼지뼈 해면골의 적용은 자가골 해면골이식의 대안으로서 유용성이 있는것으로 평가 되었다.

Finite Element Stress Analysis according to Apical-coronal Implant Position

  • Purpose: The purpose of this study was to evaluate the influence of apical-coronal implant position on the stress distribution after occlusal and oblique loading. Materials and Methods: The cortical and cancellous bone was assumed to be isotropic, homogeneous, and linearly elastic. The implant was apposed to cortical bone in the crestal region and to cancellous bone for the remainder of the implant-bone interface. The cancellous core was surrounded by 2-mm-thick cortical bone. An axial load of 200 N was assumed and a 200-N oblique load was applied at a buccal inclination of 30 degrees to the center of the pontic and buccal cusps. The 3-D geometry modeled in Iron CAD was interfaced with ANSYS. Results: When only the stress in the bone was compared, the minimal principal stress at load Points A and B, with a axial load applied at 90 degrees or an oblique load applied at 30 degrees, for model 5. The von Mises stress in the screw of model 5 was minimal at Points A and B, for 90- and 30-degree loads. When the von Mises stress of the abutment screw was compared at Points A and B, and a 30-degree oblique load, the maximum principal stress was seen with model 2, while the minimum principal stress was with model 5. In the case of implant, the model that received maximum von Mises stress was model 1 with the load Point A and Point B, axial load applied in 90-degree, and oblique load applied in 30-degree. Discussion and Conclusions: These results suggests that implantation should be done at the supracrestal level only when necessary, since it results in higher stress than when implantation is done at or below the alveolar bone level. Within the limited this study, we recommend the use of supracrestal apical-coronal positioning in the case of clinical indications.

경골이식에 관한 임상 연구 (CLINICAL STUDY ON TIBIAL BONE GRAFT)

  • The purpose of this retrospective study was to evaluate the treatment outcome of proximal tibial graft. Thirty-nine patients were grafted with proximal tibia for the repair of maxillofacial defect. Complications can be considered incidental and neglectable clinical significance. It is concluded that the proximal tibial metaphysis offers a promising alternative sources of cancellous bone for the repair of maxillofacial defect.

  • Bone density is one of the factors in the early failure of dental implants and doctors should make a preoperative assessment of jaw bone density using patient's CT data before dental implant surgery in order to find out whether the patient has osteoporosis and osteopenia. The main goal of this study was to propose a method that based on image processing techniques in order to provide accurate information about where to drill and place an abutment screw of implants in the jaw bone for doctors and reduce human activity for the estimation of the local cancellous bone density of mandible using CT data. The experiment was performed on a computed tomography data of the jaw bone of two different individuals. We assumed that the result of the estimation of jaw bone density depends on the angle of drilling and average HU (Hounsfield Unit) values were used to evaluate the quality of local cancellous bone density of mandible. As a result of this study, we have been developed a toolbox that can be used to estimate jaw bone density automatically and found a positive correlation between the angle of the drill and time complexity but a negative correlation between the diameter of the drill and time complexity.
