엔진구동형 NiTi 파일의 근관성형효과 비교 (A COMPARISON OF THE SHAPING ABILITY OF FOUR ROTARY NICKEL-TITANIUM FILES IN SIMULATED ROOT CANALS)

  • 이 연구의 목적은 4가지 엔진구동형 NiTi 기구의 근관성형력을 비교하는 것이다. 32개의 투명레진모형을 사용하여 파일의 단면도가 S자 형태이고 "single length"방법으로 근관성형을 시행하는 Mtwo와 crown-down 방법으로 근관성형을 하는 Profile, ProTaper Universal 및 K3로 근관성형을 하였다 (n = 8). 모든 근관은 한사람의 술자가 근단공 크기가 #30이 될 때까지 성형하였다. 기구 사용 전과 후 근관 모양을 디지털 영상을 기록하였고, 컴퓨터 영상 분석 프로그램(Expression Scanner와 Adobe Photoshop CS)으로 평가하였다. 다음과 같은 결론을 얻었다. 근첨으로부터 1, 2, 3, 4, 5, 6, 7 mm 떨어진 부위의 내측과 외측으로 삭제된 레진 양을 측정하였고, 자료는 SPSS법으로 비교 분석하였다. 1. 근관형성 시간은 Mtwo가 84.50초로 가장 짧았으며, 총 작업시간은 K3가 269.37초로 가장 길었다(p < 0.05). 2. 기구의 변형과 파절은 모든 구동형 NiTi 파일에서 발생하지 않았다. 3. 근관 내측 폭경의 변화는 Profile이 1, 2 mm 부위를 제외한 다른 부위에서 가장 적었으며, ProTaper Universal은 모든 부위에서 가장 많았다(p < 0.05). 4. 근관 외측 폭경의 변화는 Mtwo이 1 mm 부위를 제외한 모든 부위에서 가장 적었으며, Profile은 3, 4 mm 부위에서 가장 많았다(p < 0.05). ProTaper Universal와 K3는 1, 2, 6, 7 mm 부위에서 가장 많았다(p < 0.05). 5. 근관중심변위율(centering ratio)은 Profile이 1, 4, 5, 6 mm 부위에서 가장 적었다(p < 0.05). 이상의 결과, 구동성 NiTi 파일을 이용한 만곡 근관의 근관형성은 치경부는 ProTaper와 같은 삭제력이 높은 파일을 사용하는 것이 근관 입구를 효과적으로 넓히고 더 나은 taper를 형성하며, 근단부는 Profile과 같은 근관 중심 변위가 적은 파일을 사용하는 것이 근관형성 시간을 단축하고 근단부의 근관 변형(transportation)을 줄일 수 있다고 판단된다.

Observation on Oil Canals of Umbelliferous Condiment Herbs of Japanese Origin

  • 일본원산(日本原産)의 미나리과향신야말(科香辛野茉)인 파드득나물, 미나리, 명일엽(明日葉) 및 갯방품의 유도(油道)의 발생(發生), 구조(構造) 및 분포(分布)에 대하여 광학현미경(光學顯微鏡)으로 관찰(觀察)하였다. 유도(油道)는 엽병(葉柄), 엽신(葉身), 경(莖), 근(根), 배축(胚軸) 및 자엽(子葉)에 존재(存在)했다. 유도(油道)는 배축(胚軸)과 자엽(子葉)의 발생초기(發生初期)에 형성(形成)되었다. 엽병(葉柄)에 있어서의 유도(油道)는 파드득나물, 미나리에서와 같이 윤상유관속(輪狀維管束)에 상반(相伴)하여 나타나는 분포(分布)와, 명일엽(明日葉), 갯방풍에서와 같이 산재위관속(散在維管束) 상반(相伴)하여 나타나는 분포(分布)로 구분(區分)할 수 있었다. 엽병(葉柄)이 비대(肥大)해짐에 따라 피층(皮層)에 형성(形成)되는 유도(油道)는 처음에 유도(油道)가 발생(發生)하고, 그 다음에 후각조직(厚角組織)과 유관속(維管束)이 발달(發達)함을 알 수 있었다. 그러나, 유관속(維管束)의 형성(形成)을 동반(桐伴)하지 않는 경우도 있었다 유도(油道)는 엽병(葉柄)에서는 주변부(周邊部)에 많이 존재(存在)했다. 엽신(葉身)에서는 엽맥(葉脈)의 향축측(向軸側)과 배축측(背軸側)에 존재(存在)했고, 주맥(主脈)에 있는 유도(油道)가 컸다. 경(莖)에서의 유도(油道)는, 파드득나물과 미나리는 피층(皮層)과 빈에, 명일엽(明日葉)과 갯방풍은 피층(皮層)과 목부주변(木部周邊)의 기본조직(基本組織)에 분포(分布)했다. 근(根)에서는, 파드득나물과 미나리는 내초에, 명일엽(明日葉)과 갯방풍은 후각성조직(厚角性組織)과 축부(縮部)에 분포(分布)했다. 유도(油道)의 횡단면(橫斷面)은 도형(圖形) 혹은 타도형(楕圖形)이었다. 분필세포(分泌細胞)는 피층(皮層)과 빈에서는 주변(周邊)의 유세포(柔細胞)보다 작았으며 사부(篩部)에서는 컸다.

수종의 근관형성 방법이 근관 형태에 미치는 영향 (THE EFFECT OF SOME CANAL PREPARATION TECHNIQUES ON THE SHAPE OF ROOT CANALS)

  • The purpose of this study was to compare the shape of root canal after instrumentation with some engine driven NiTi files. Thirty narrow and curved canals(15-35 degree) of mesial canals of extracted human mandibular first molars were divided into three groups. Group 1: After radicular access with Gates Glidden drill, apical shaping using step back method with Flexo file Group 2: After radicular access with Gates Glidden drill, apical shaping with Profile .04 Group 3: Canal shaping with GT file and Profile .04. Using modified Bramante technique, the root was sectioned at 2 mm from apical foramen, height of curvature, 2 mm from canal orifice. Canal centering ratio, amount of transport, amount of dentin removed, shape of canal were measured and statistical analysis is done using SPSS Program V 7.5. The results were as follows: 1. Canal centering ratio of group 3 was the lowest at coronal part, but there was no statistical difference. Centering ratio of group 2 was the lowest at curve part, and there was statistical difference between group 1(P<0.05). Centering ratio of group 2 was the lowest at apical part, but there was no statistic difference. 2. Amount of transport of group 3 was the lowest at coronal part, but there was no statistical difference. Amount of transport of group 2 was the lowest at curve part, and there was statistical difference between group 1(P<0.05). Amount of transport of group 3 was the lowest at apical part, and there was statistical difference between group 1 and group 2, group 1 and group 3(P<0.05). 3. Amount of dentin removed of group 3 was the lowest at coronal part, bur there was no statistical difference. Amount of dentin removed of group 2 was the lowest at curve part, but there was no statistical difference. Amount of dentin removed or group 2 was the lowest at apical part, and there was statistical difference between group 1 and group 2, group 1 and group 3(P<0.05). 4. The shape of the canals after instrumentation varied among the groups. The majority of canals at coronal and curve part for group 1 were round in shape(7 in 10), those at apical part were oval(8 in 10). The majority of canals at coronal part for group 2 were round in shape(7 in 10) and there was no difference in the number of shape at other part. There was no difference in the number of shape at every part for group 3. As above results, NiTi rotary instrumentation showed a trend to remain more centered in the canal than SS file instrumentation. At using NiTi file, coronal shaping with Gates Glidden drill was not statistically different from shaping with GT file. But shaping with GT file showed tapered canals, so it may be said that shaping with GT file is a safe and valuable instrumentation method.

NiTi Rotary Instrumentation이 근관만곡도 변화에 미치는 영향 (THE EFFECT OF NiTi ROTARY INSTRUMENTATION ON THE CHANGE OF APICAL ROOT CANAL CURVATURE)

  • During cleaning and shaping of narrow and curved canals, it is very difficult or nearly impossible to maintain the original canal shape. Procedural accidents such as, ledge, zipping, perforation, and instrument breakage are frequently occurred and even may lead to failure of endodontic therapy. To prevent these kinds of accidents, various instrumentation techniques and materials have been introduced. Recently some nickel titanium (NiTi) files are introduced and it is reported that These NiTi files created rounder preparations with less transportation than conventional instruments in curved canals. This study compared the change of the canal curvature and procedural accidents after instrumentation produced by stainless steel K-flexo file, and NiTi rotary files (Profile 29 and Quantec 2000). Thirty narrow and curved canals (25-45 degree) of extracted human molars were randomly divided into three groups. In group 1, canals were instrumented using a step-back and watch-winding/pull motion with K-flexo files. In group 2, canals were prepared with Profile 29. Group 3, canals were prepared with Quantec 2000 files. Before and after preparation of canals, periapical radiographs were taken and scanned. The change of canal curvature were measured using Photoshop 4.0 program and the incidence of procedural accidents were also evaluated. The results were as follows: 1. All group showed some loss of canal curvature after instrumentation. 2. Average loss of canal curvature was $6.70{\pm}5.31$ degree for group 1, $3.80{\pm}2.57$ degree for group 2, and $5.40{\pm}4.83$ degree for group 3. All group There was significant change in curvature between before and after instrumentation (p<0.05). But there was no statistical difference amoung 3 groups. 3. In group I, there were no procedural accidents, such as ledging, perforation, or instrument fracture. In group 2, two cases of ledge and one case of instrument fracture were produced Goup 3, each one case of ledge, perforation and instrument fracture were occurred. Whthin the limits of above results, It seems that NiTi rotary instrumentation is not All Mighty and if we use uncarefully, it is more dangerous to produce some procedural accidents than conventional hand files. But more studies should be taken to evaluate the exact effects of NiTi rotary instrumentations.

Endodontic management of a C-shaped maxillary first molar with three independent buccal root canals by using cone-beam computed tomography

  • The aim of this study was to present a method for endodontic management of a maxillary first molar with unusual C-shaped morphology of the buccal root verified by cone-beam computed tomography (CBCT) images. This rare anatomical variation was confirmed using CBCT, and nonsurgical endodontic treatment was performed by meticulous evaluation of the pulpal floor. Posttreatment image revealed 3 independent canals in the buccal root obturated efficiently to the accepted lengths in all 3 canals. Our study describes a unique C-shaped variation of the root canal system in a maxillary first molar, involving the 3 buccal canals. In addition, our study highlights the usefulness of CBCT imaging for accurate diagnosis and management of this unusual canal morphology.

Cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider, and the One G glide path instruments in double-curvature canals

  • Objectives: The aim of this study was to compare the cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider and One G glide path instruments in artificial doublecurvature canals. Materials and Methods: This study included 15 WaveOne Gold Glider (size 15/0.08), 15 ProGlider (size 16/0.08), and 15 One G (size 16/0.06) nickel titanium files. The files were used in accordance with the manufacturer's instructions until they were broken in artificial double-curvature canals made of stainless steel. The time to fracture was recorded via a digital stopwatch and the number of rotations until fracture was also calculated. The data were statistically analyzed via the Kruskal-Wallis test. Results: The highest average number of rotations until fracture of the files was found for the WaveOne Gold Glider, followed by ProGlider and One G in order. Statistically significant differences were present between all groups of files (p < 0.05). Conclusions: In our study, the resistance of the WaveOne Gold Glider nickel-titanium (Ni-Ti) file to cyclic fatigue in S-shaped curved canals was found to be higher than that of the ProGlider and One G Ni-Ti files.

상아세관에서 세균부착에 관한 주사전자현미경적 연구 (A SCANNING ELECTRON MICROSCOPIC STUDY OF BACTERIAL ATTACHMENT IN DENTINAL TUBULES)

  • Microorganisms are implicated the endodontic treatment failures. Persistent endodontic infection may be the result of retention of microorganisms in the dentin of the root canal walls. Dentinal tubules of the root canal walls have been shown to harbor microorganisms. The purpose of this study was to investigate the invasion of microorganism into the root dentin and dentinal tubules. The effects of irrigation solutions and smear layer on bacterial colonization of root canal were evaluated using a scanning electron microscopy. Canals of extracted human teeth with single and straight canals were stepback prepared using normal saline. Tooth samples were divided into four groups according to the irrigation solutions -5 % sodium hypochlorite and normal saline-and smear layer treatment. The smear layer was removed by 5% NaOCl and 20% EDTA for 10 min respectively. After sterilization, they were incubated with each strains of Streptococcus sanguis, Enterococcus faecalis, Staphylococcus aureus and Escherichia coli. Sodium hypochlorite solution reduced the adhesion of microorganisms effectively compared to normal saline. The smear layer inhibited colonization of E. faecalis, S. aureus and E. coli in the root canals due to their blocking of dentianl tubules. But S. sanguis invaded dentinal tubules in the root canals without smear layer. It was suggested that bacterial attachment might be different according to the strains. Sodium hypochlorite inhibited bacterial attachment in the dentinal tubules dramatically. The absence or presence of smear layer affected bacterial invasion of the dentinal tubules.

음파 및 초음파기구를 이용한 근관형성법의 효율성에 관한 실험적 연구 (AN EXPERIMENTAL STUDY OF THE EFFICACY OF SONIC AND ULTRASONIC ROOT CANAL PREPARATION TECHNIQUES)

  • The purpose of this study was to evaluate the canal preparation time, deviation of the apex (Zipping), and debridement effect of the canal of three canal preparation techniques. Thirty extracted 1st and 2nd molars were divided into 3 groups and each group was enlarged by ultrasonic, sonic or hand instrument. The specimens were examined under scanning electron microscope and light microscope. The results were as follows: 1. In the canal preparation time, ultrasonic group was the shortest, followed by sonic, hand group. All pairs of groups were significantly different at the 0.05 level. (P < 0.05) 2. In the deviation of the canal, no two groups were significantly different at the 0.05 level. (P > 0.05) 3. In the evaluation of debris scores, ultrasonic and sonic groups were better debridement effect than hand group in the fine canals. In the large canals, ultrasonic group was the best results, followed by sonic, hand group. 4. In the evaluation of smear layer scores, ultrasonic group was the best and sonic and hand group were same effect in the fine canals. In the large canals, three groups were same effect. 5. In the effect of the removal of predentin and pulpal debris, in the regardless of canal size, three groups were same and pulpal debris was not completely removed by either technique.

A comparison of the shaping ability of reciprocating NiTi instruments in simulated curved canals

  • Objectives: The study was to compare the shaping ability of Reciproc (VDW) and WaveOne (Dentsply Maillefer) instruments compared with ProTaper, Profile and hand instrument during the preparation of simulated root canals. Materials and Methods: Five groups (n = 5) were established. Reciproc, WaveOne, ProTaper, Profile and K file (K-flexo file) were used to prepare the resin simulated canals. A series of preoperative and postoperative images were taken by a microscope and superimposed in 2 different layers. The amount of resin removed from both the inner and the outer sides of the canal was measured to the level of 10 mm from the apical tip, with a 1 mm increment. Results: The mean of resin removal from the inner canal wall was not different from the outer canal wall for Reciproc and WaveOne groups at apical third (1 - 3 mm level). There was no difference in the change of working length and maintenance of canal curvature. NiTi instruments are superior to stainless-steel K file in their shaping ability. Conclusions: Within the limitation of this present study, Reciproc and WaveOne instruments maintained the original canal curvature in curved canals better than ProTaper and Profile, which tend to transport towards the outer canal wall of the curve in the apical part of the canal.

Glyoxide의 근관정화효과에 관한 연구 (A STUDY ON THE CLEANSING EFFECT OF GLYOXIDE ON THE DENTINAL WALLS OF ROOT CANAL)

  • The purpose of this study was to evaluate the cleansing effect of Glyoxide on the dentinal walls of the root canal. Fourty teeth were divided into four groups and the canals in each group were individually enlarged with K-file by step-back technic and irrigated with one of four irrigants. The four used irrigants were Glyoxide (Marion lab. U.S.A.) in combination with 3.5% sodium hypochlorite, 3.5% sodium hypochlorite, 3.5% sodium hypochlorite in combination with 3% hydrogen peroxide and normal saline solution. All the irrigants were used in conjunction with instrumentation as they would be during clinical conditions. After final irrigation, the canals were dried with paper points and the teeth were split longitudinally. The cleaness of canal walls according to the size and the level of canals were evaluated under steroscope by t analysed statistically The results were as follows. 1. The use of 3.5% sodium hyphochlorite in combination with 3.0% hydrogen peroxide revealed the most clean canal surface regardless of the size of canal at apical third of root canal (p < 0.05). 2. Glyoxide in combination with 3.5% sodium hyphchlorite showed no significant difference in cleaning effect of canal surface compared with 3.5% sodium hypochlorite and normal saline solution at the apical third of narrow canal. 3. Glyoxide in combination with 3.5% sodium hypochlorite revealed no significant difference in debridement of canal walls compared with 3.5% sodium hypochlorite in combination with 3% hydrogen peroxide and saline solution at the middle third of narrow and large canals.
