Expression of transforming growth factor-1 in bone regeneration after the implantation of particulate dentin and plaster of Paris

  • Purpose: This study was performed to investigate the expression of the transforming growth factor (TGF)-1, in a rat calvarium defect model using particulate dentin and/or plaster of Paris, and correlate the bone regeneration process with the histologic events. Materials and Methods: Thirty-two Sprague-Dawley rats were divided into 4 groups of 8 animals each. A 1.0 cm-sized calvarial defects were made and the defect was filled with different graft materials as follows : Group A, the defects were filled with a mixture of particulate dentin and plaster of Paris with a 2:1 ratio; Group B, the defects were filled with plaster of Paris only; Group C, defects were filled with particulate dentin only; Group D, untreated control group. The animals were sacrificed by 1, 2, 4, 8 weeks after implantation. Excised wound tissues were processed for histology, immunohistochemistry and RT-PCR for the analysis of TGF-1 expression. Results: Gene expression of TGF-1 was detected for all experimental groups. The highest gene expression was observed in the specimen taken at the first week after implantation in Group A. According to the histologic and immunohistochemical studies, TGF-1 positive osteoblast-like cells were found in the early stage of healing after the implantation of particulate dentin and plaster of Paris. Conclusion: These findings suggest that TGF-1 may be related to new bone formation at the early healing process after the implantation of particulate dentin and plaster of Paris.

3D Printed Titanium Implant for the Skull Reconstruction: A Preliminary Case Study

  • The skull defect can be made after the trauma, oncologic problems or neurosurgery. The skull reconstruction has been the challenging issue in craniofacial fields for a long time. So far the skull reconstruction with autogenous bone would be the standard. Although the autogenous bone would be the ideal one for skull reconstruction, donor site morbidity would be the inevitable problem in many cases. Meanwhile various types of allogenic and alloplastic materials have been also used. However, skull reconstruction with many alloplastic material have produced no less complications including infection, exposure, and delayed wound healing. Because the 3D printing technique evolved so fast that 3D printed titanium implant were possible recently. The aim of this trial is to try to restore the original skull anatomy as possible using the 3D printed titanium implant, based on the mirrored three dimensional CT images based on the computer simulation. Preoperative computed tomography (CT) data were processed for the patient and a rapid prototyping (RP) model was produced. At the same time, the uninjured side was mirrored and superimposed onto the traumatized side, to create a mirror-image of the RP model. And we fabricated Titanium implant to reconstruct three-dimensional orbital structure in advance, using the 3D printer. This prefabricated Titanium-implant was then inserted onto the defected skull and fixed. Three dimensional printing technique of titanium material based on the computer simulation turned out to be very successful in this patient. Individualized approach for each patient could be an ideal way to manage the traumatic patients in near future.

Eight-week healing of grafted calvarial bone defects with hyperbaric oxygen therapy in rats

  • Purpose: The purpose of this study was to evaluate the synergistic effect of adjunctive hyperbaric oxygen (HBO) therapy on new bone formation and angiogenesis after 8 weeks of healing. Methods: Sprague-Dawley rats (n=28) were split into 2 groups according to the application of adjunctive HBO therapy: a group that received HBO therapy (HBO group [n=14]) and another group that did not receive HBO therapy (NHBO group [n=14]). Each group was divided into 2 subgroups according to the type of bone graft material: a biphasic calcium phosphate (BCP) subgroup and an Escherichia coli-derived recombinant human bone morphogenetic protein-2-/epigallocatechin-3-gallate-coated BCP (mBCP) subgroup. Two identical circular defects with a 6-mm diameter were made in the right and left parietal bones of each rat. One defect was grafted with bone graft material (BCP or mBCP). The other defect was not grafted. The HBO group received 2 weeks of adjunctive HBO therapy (1 hour, 5 times a week). The rats were euthanized 8 weeks after surgery. The specimens were prepared for histologic analysis. Results: New bone (%) was higher in the NHBO-mBCP group than in the NHBO-BCP and control groups (P<0.05). Blood vessel count (%) and vascular endothelial growth factor staining (%) were higher in the HBO-mBCP group than in the NHBO-mBCP group (P<0.05). Conclusions: HBO therapy did not have a positive influence on bone formation irrespective of the type of bone graft material applied after 8 weeks of healing. HBO therapy had a positive effect on angiogenic activity.

The optimal dosage of hyaluronic acid for bone regeneration in rat calvarial defects

  • Purpose: Hyaluronic acid (HA) affects angiogenesis and promotes the migration and differentiation of mesenchymal cells, thereby activating the osteogenic ability of osteoblasts. Although studies on the action of HA during bone regeneration are being actively conducted, the optimal dose of HA required for bone regeneration remains unclear. Therefore, the purpose of this study was to elucidate the most effective HA dose for bone formation using a rat critical-size defect model. Methods: Thirty rats were randomly divided into 5 groups, with 6 rats in each group. An absorbable collagen sponge soaked with HA or saline was used to fill an 8-mm defect, which was then covered with a collagen membrane. Different treatments were performed for each group as follows: (1) saline control, (2) 1 mg/mL HA, (3) 25 mg/mL HA, (4) 50 mg/mL HA, or (5) 75 mg/mL HA. After a healing period of 4 weeks, micro-computed tomography and histological analysis were performed. The obtained values were analyzed using analysis of variance and the Tukey test (P<0.05). Results: At week 4, the 75 mg/mL HA group had the highest bone volume/total volume ratio, new bone, and bone fill among the 5 groups, and these values were significantly different from those observed in the control group (P<0.01) and 1 mg/mL HA group (P<0.001). More active bone formation was observed in the higher-dose HA groups (25 mg/mL, 50 mg/mL, and 75 mg/mL HA), which included a large amount of woven bone. Conclusions: The 75 mg/mL HA group showed better bone formation than the other groups (1, 25, and 50 mg/mL HA and control).

홍화 추출물이 치주인대세포, 조골세포 활성도에 미치는 영향 (The biologic effects of safflower(Carthamus tinctorius $Linn\acute{e}$) extract and Dipsasi Radix extract on periodontal ligament cells and osteoblastic cells)

  • Safflower(Carthamus tinctorius $Linn\acute{e}$ has been traditionally used for the treatment of blood stasis, and Dipsasi Radix has been used as a drug for fracture in Chinese medicine. The purpose of present study was to examine the biologic effects of safflower extract and Disasi radix extracts on the periodontal. ligament cells and osteoblastic cells and on the wound healing of rat calvarial defect. The ethanolic extract of safflower blossom, safflower seed and Dipsasi Radix(125, 250, and 500 ${\mu}g/ml$) were prepared as test group, and PDGF-BB(lOng/ml) and unsafonifiable fraction of Zea Mays L.(125, 250, and 500 ${\mu}g/ml$) were employed as positive control. The effects of each agents on the growth and survival, ALPase activity, expression of PDGF-BB receptor, chemotactic response of PDL cell and ATCC human osteosarcoma MG63 cells in vitro were examined. The tissue regenerative effect of each extracts was evaluated by histomorphometric measuring of newly formed bone on the 8mm defect in rat calvaria after oral administration of 3 different dosages groups : 0.02, 0.1 and 0.35g/kg, per day. It was also employed the same dosages of unsaponifiable fraction of Zea Mays L. as positive controls. Safflower blossom extract, safflower seed extract, and Dipsasi Radix extract stimulate the cellular activity of MG63 cells in concentration range of $125-500{\mu}g/ml$, and safflower bolssom extract and safflower seed extract stimulate also the cellular activity of periodontal ligament cells in concentration range of $250-500{\mu}g/ml$. In activity of ALPase, $250-500{\mu}g/ml$ of safflower blossom extracts showed significant stimulating effects on MG63 cells, and the same concentration range of safflower seed extracts showed significant effect on periodontal ligament cells. In the recovery on PDGF-BB receptor expression which was depressed by $IL-1{\beta}$, $125-250{\mu}g/ml$ of safflower blossom extracts and $250-500{\mu}g/ml$ of safflower seed extracts showed significant increasing effect on MG63 cells, and $500{\mu}g/ml$ of safflower blossom extract and $250-500{\mu}g/ml$ of safflower seed extracts showed significant effect on periodontal ligament cells. In chemotactic response, among all tested group, safflower seed extracts only were chemotactic to MG63 cells and periodontal ligament cells in concentration range of $125-500{\mu}g/ml$. Also in the view of bone regeneration in rat calvarial defect model, the only group that was orally administrated 0.35g/kg, day of safflower seed extract showed significant new bone formation. These results suggested that safflower extracts might have a potential possibilities as an useful drug for adjunct to treatment for regeneration of periodontal defect.

Comparative study of new bone formation capability of zirconia bone graft material in rabbit calvarial

  • PURPOSE. The purpose of this study was to compare the new bone formation capability of zirconia with those of other synthetic bone grafts. MATERIALS AND METHODS. Twelve rabbits were used and four 6-mm diameter transcortical defects were formed on each calvaria. Each defect was filled with Osteon II (Os), Tigran PTG (Ti), and zirconia (Zi) bone grafts. For the control group, the defects were left unfilled. The rabbits were sacrificed at 2, 4, and 8 weeks. Specimens were analyzed through micro computed tomography (CT) and histomorphometric analysis. RESULTS. The Ti and Zi groups showed significant differences in the amount of newly formed bone between 2 and 4 weeks and between 2 and 8 weeks (P<.05). The measurements of total bone using micro CT showed significant differences between the Os and Ti groups and between the Os and Zi groups at 2 and 8 weeks (P<.05). Comparing by week in each group, the Ti group showed a significant difference between 4 and 8 weeks. Histomorphometric analysis also showed significant differences in new bone formation between the control group and the experimental groups at 2, 4, and 8 weeks (P<.05). In the comparison of newly formed bone, significant differences were observed between 2 and 4 weeks and between 2 and 8 weeks (P<.05) in all groups. CONCLUSION. Zirconia bone graft material showed satisfactory results in new bone formation and zirconia could be used as a new synthetic bone graft material.

전두 사골부 수막뇌류와 안와격리증의 동시 재건 1례 (A Case of Simultaneous Correction of Frontoethmoidal Meningoencephalocele and Hypertelorism)

  • Purpose: A meningoencephalocele is a congenital malformation involving herniation of the meninges and cerebral tissue through a defect in the skull. For the patient with frontoethmoidal meningoencephalocele with hypertelorism, the removal of the meningoencephalocele without correction of the combined hypertelorism is not enough for getting a good cosmetic appearance. Correction of the hypertelorism is needed for cosmetic problem. We experienced a case of simultaneous correction of frontoethmoidal meningoencephalocele with hypertelorism. Methods: The meningoencephalocele was removed and the hypertelorism was corrected by central segment technique. The bone defects were filled with autogenous bone dusts. And the nose was reconstructed by a calvarial bone graft. Results: The patient had a good cosmetic appearance without any neurological complications without serious complications. Conclusion: We experienced a case of simultaneous correction of frontoethmoidal meningoencephalocele with hypertelorism. And a brief review of related literatures is given.

백서두개골 결손부에서 천연물유래 탄산칼슘염 골대체의 골치유에 관한 조직계측학적 평가 (Histomorphometric evaluation of bone healing with natural calcium carbonate-derived bone substitutes in rat calvarial defect)

  • Purpose: This study investigated the osteoconductivity of natural calcium carbonate-derived bone substitutes, hen eggshell (ES), and compared with those of commercial bone substitutes. Materials and Methods: Osseous defects created in the rat calvaria were filled with particulated ES(ES-1), ES with calcium-deficient hydroxyapatite surface layer (ES-2), Biocoral(Inoteb, France), and Bio-Oss(Geistlich Pharma, Wolhusen, Switzerland). After 4 and 8 weeks of healing, histomorphometic analysis was performed to evaluate the amount of newly formed mineralized bone area (NB%). Results: Histologic and histomorphometric analysis showed new bone formation and direct bony contact with the grafted materials in all groups. At 4 weeks, Biocoral group showed greater NB% compared to Bio-Oss and ES-1 groups (P<0.05). At 8 weeks, Biocoral and ES-2 groups showed significantly greater NB% compared to Bio-Oss group (P<0.05). Conclusion: These results indicate that natural calcium carbonate-derived bone substitutes with microporous calcium-deficient hydroxyapatite surface layer may be an effective materials treating osseous defects.

Cranial Base Reconstruction and Secondary Frontal Advancement for Meningoencephalocele Following LeFort III Osteotomy in a Patient with Crouzon Syndrome: Case Report

  • Patients with Crouzon syndrome have increased risks of cerebrospinal fluid rhinorrhea and meningoencephalocele after LeFort III osteotomy. We report a rare case of meningoencephalocele following LeFort III midface advancement in a patient with Crouzon syndrome. Over 10 years since it was incidentally found during transnasal endoscopic orbital decompression, the untreated meningoencephalocele eventually led to intermittent clear nasal discharge, frontal headache, and seizure. Computed tomography and magnetic resonance imaging demonstrated meningoencephalocele in the left frontal-ethmoid-maxillary sinus through a focal defect of the anterior cranial base. Through bifrontal craniotomy, the meningoencephalocele was removed and the anterior cranial base was reconstructed with a pericranial flap and split calvarial bone graft. Secondary frontal advancement was concurrently performed to relieve suspicious increased intracranial pressure, limit visual deterioration, and improve the forehead shape. Surgeons should be aware that patients with Crouzon syndrome have the potential for an unrecognized dural injury during LeFort III osteotomy due to anatomical differences such as inferior displacement and thinning of the anterior cranial base.

Ca-P 박막 이종골과 제 1형 교원질이 토끼 두개골 결손부의 골재생에 미치는 영향 (The Effects of Calcium-Phosphate Coated Xenogenic Bone and Type I Collagen for Bone Regeneration on the Calvarial Defects in Rabbits)

  • 골재생을 위해 사용되는 골이식재로 자가골, 동종골, 이종골 등이 있다. 자가골은 가장 예지성이 높은 골이식재이지만, 부가적인 수술, 환자의 동통과 불편, 채취하는 양의 제한, 비용의 증가 등의 단점이 있다. 따라서 많은 연구자들은 오랫동안 자가골을 대체할 골이식재 개발에 힘써왔고, 다양한 연구가 있었다. 소로부터 유래한 이종골은 천연 다공성의 골 무기질로서, 인간의 골의 구조와 유사하면서, 골 전도성이 있고, 생체 적합성이 뛰어나다고 보고되었다. 이에 최근에 개발된 Ca-P 박막이 이종골과 조작성을 용이하게 하기 위해 부가적으로 type I collagen을 혼합한 골이식재를 토끼 두개골 결손부에 매식하여 골형성 능력 및 주변 조직의 반응을 보고자 하였다. 총 16마리의 New Zealand white rabbits를 사용하였고, 두개골에 4부위의 결손부를 형성한 후, 다음과 같이 적용하였다. 이식재를 넣지 않은 군을 음성대조군으로, 자가골 분말을 이식한 군을 양성대조군으로, Ca-P 박막 탈단백 우골 분말을 이식한 군을 실험1군으로, Ca-P 박막 탈단백 우골 분말과 type I collagen을 같은 부피로 혼합하여 이식한 군을 실험2군으로 하였다. 1, 2, 4, 8주째 4마리씩 희생하여, H-E 염색과 Masson's trichrome 염색을 시행한 후, 광학현미경을 사용하여 조직학적으로 관찰하였다. 토끼 두개골 결손부에 이식한 Ca-P 박막 탈단백 우골은 골성회복초기에 골결손부 변연에서 골전도성을 보였지만, 완전한 골성회복을 이루지 못하였고, 신생골과 직접적인 유합을 보이지 않았다. 또, collagen의 부가적인 사용은 조작성은 가장 우수했으나, 조직소견상 신생골의 형성을 보이지는 않았다. 반면 자가골을 이식한 부위는 신생골 형성양과 밀도에 있어서 가장 우수한 결과를 보였다.
