중환자를 위한 근거중심 영양지원 (Evidence-based Nutritional Support in the Intensive Care Unit)

  • Optimal nutrition serves to maintain normal organ function and to preserve body energy stores to guarantee survival during times of shortage of food. Adequate nutrition of intensive care unit (ICU) patients improves outcome, while malnutrition is strongly associated with increased morbidity and mortality rates among critically ill patients. Previously published researches showed that trials of nutritional support in critical illness rarely fulfill basic quality requirements. Nutrition support plays a vital role in the prevention and treatment of nutritional deficiencies in at-risk, critically ill patients. This paper reviewed the challenges in determining critically ill patients' nutrition requirements including nutrition assessment, determination of caloric requirements then providing them with adequate nutrition support while in the ICU with the guidelines published by Society of Critical Care Medicine and American Society for Parenteral and Enteral Nutrition. Nutrition support can be effectively enhanced by using the guidelines.

A STUDY ON THE PROTEIN AND ENERGY REQUIREMENTS OF MUSCOVY DUCKLINGS

  • Two experiments were conducted with one-day-old straight run Muscovy ducklings to determine their protein and energy requirements. In the 1st experiment, isoenergetic diets (2800 kcal ME/kg) with three dietary proteins, 18, 20 and 22% in the starter period (1-28 days) and 16, 18 and 20% in the grower and finisher period (29-84 days) were used to determine the optimum protein requirement. While, in the 2nd experiment, isonitrogenous diets (20%, C.P.) with three dietary energy, 2700, 2800 and 2900 kcal ME/kg in the starter period (1-28 days) and (18% C.P.) with 2800, 2900 and 3000 kcal ME/kg in the grower-finisher period (29-84 days) were used to determine the optimum energy requirement. It was observed that 20% C.P. in the starter period and 18% C.P. in the grower and finisher period was adequate for optimum performance, while, 2900 kal ME/kg was sufficient to meet the optimum energy requirement in both the starter, grower-finisher period as regards body weight, feed efficiency, protein efficiency and caloric efficiency are concerned.

만성적인 에탄올 섭취가 흰쥐의 엽산대사 및 혈장 호모시스테인 농도에 미치는 영향 (Effects of Chronic Ethanol Administration on Folate Metabolism and Plasma Homocysteine Concentration in the Rats)

  • The present study was conducted to investigate the effect of different levels of ethanol ingestion on 131ate metabolism and plasma homocysteine concentration in Sprague-Dawley male rats receiving 0, 10, 30% of their caloric intake as ethanol for S weeks. Diets containing 10% ethanol had no effect on plasma and red blood cell(rbc) 131a1e. However, in rats fed a 30% ethanol diet, rbc folate increased and plasma 131ate decreased significantly, In the rats maintained first on a 30% ethanol diet for S weeks and then on a control diet for 2 weeks, the levels of plasma and rbc f31ate were normalized by withdrawal of ethanol. Urinary fo1ate excretion increased markedly in rats fed 10% and 30% ethanol diets and decreased to 51% of controls by withdrawal of ethanol. Plasma homocysteine concentration increased significantly in rats fed a 30% ethanol diet. The results suggest that chronic ingestion of ethanol increased urinary 131ate excretion markedly, which may decrease plasma 131ate and deplete liver folate.

Molecular Dynamics Simulations on Melting Properties of Free Icosahedral Copper Clusters

  • We have studied the size confinement effect on the properties of melting-like transition of small icosahedral copper clusters using a classical molecular dynamics simulation based on a well fitted empirical potential. We investigated the caloric curves of icosahedron nanoclusters and the significant depression in the melting temperatures of the copper nanoclusters was compared with that of the bulk copper. A structural transitions from decahedral to icosahedral shapes were shown. As the cluster size increased, the melting temperature increased, and the latent heat increased but seem to be saturated. However, the specific heat was unrelated to the cluster size.

노인의 신체활동이 혈중지질에 미치는 영향 (The effect of a physical activity on blood cholesterol in older adults)

  • Despite well-known benefits of physical activity for older adults, most older adults remain significantly underactive. The purpose of this study was to examine the effects of a physical activity on blood cholesterol in older adults with an inclusive, choice-based physical activity promotion program to increase lifetime physical activity levels of seniors. A six-month comparison-group trial was conducted with 14 older adults(experimental group = 8, control group = 6) in community senior center. Changes in self-reported physical activity and blood cholesterol were evaluated using paired t-test. The intervention group increased estimated caloric expenditure by 858 calories/week in physical activities of any intensity (p=.050), total cholesterol(p=0.049), high density lipoprotein cholesterol(p=0.045). Control group changes were also negligible. The program led to meaningful physical activity increase. Individually tailored programs to encourage lifestyle changes in seniors may be effective and applicable to health care and community settings.

Functional Exploration of Vestibulo-Ocular Reflex by a Caloric Stimulation

  • In this paper, we proposed the bidirectional mono-thermic test for the functional exploration of the vestibules by considering a temeprature variation in the semicirular canals constant for a short time and an inclination of the semicircular canal plan relative to the vertical. Through the proposed test we showed the evidence of an eventual asymmetry between the responses of right and left vestibules. In view of clinical practice, the proposed test has the advantages of saving the test time, of using only one temperature for the ear irrigation and of making only head movements to the given angle for the stimulation.

A Study on the Quantitative Functional Exploration of Vestivular Oculomotor System

  • This study describes the battery functional test in order to explore vestibular oculomotor system. These tests are the following : a) acceleration test by using a pseudorandom stimulus. b) caloric test by considering both the constant temperature variation in the semicircular canals for a short time and the only head movement, to the fore and the back, for the ear ir- rigation, which gave us two types of stimulus (cold and warm) according to the only head position. c) optokinetic test by continuous constant velocity displacement of a large image fill- ing the entire subject's visual field. With relation to the clinical practices, some results of research programs which are at present in progress are presented.

Sirt1 and the Mitochondria

  • Sirt1 is the most prominent and extensively studied member of sirtuins, the family of mammalian class III histone deacetylases heavily implicated in health span and longevity. Although primarily a nuclear protein, Sirt1's deacetylation of Peroxisome proliferator-activated receptor Gamma Coactivator-$1{\alpha}$ (PGC-$1{\alpha}$) has been extensively implicated in metabolic control and mitochondrial biogenesis, which was proposed to partially underlie Sirt1's role in caloric restriction and impacts on longevity. The notion of Sirt1's regulation of PGC-$1{\alpha}$ activity and its role in mitochondrial biogenesis has, however, been controversial. Interestingly, Sirt1 also appears to be important for the turnover of defective mitochondria by mitophagy. I discuss here evidences for Sirt1's regulation of mitochondrial biogenesis and turnover, in relation to PGC-$1{\alpha}$ deacetylation and various aspects of cellular physiology and disease.

순차식 촉매연소 시스템 (Sequential Catalytic Combustion System)

  • Compared to conventional flame combustion, catalytic combustion had the advantage of oxidation of V.O.C. gas which was high voluminous, low caloric mixture flow. However, the temperature of mixture gas should be over the one of catalytic reaction start and the control of reaction on the catalytic surface tends to be vulnerable. To overcome these obstacles, composition of both catalytic combustor and heat exchanger was devised and named the sequential catalytic combustion system. In this system, only trigger unit needed preheating process for transient starting time. Once trigger unit was ignited, the next unit w3s supplied heat to ignite from that and same process was performed to the last one sequentially. When it come to steady state, whole mixture gas was oxidated at each unit simultaneously and preheating for trigger unit was not needed any more. System of 100 kcalh/hr capacity was devised and operated successfully.

초임계 유체를 위한 분자 클러스터 기반의 격자모델 (A Lattice Model Based on Molecular Clusters for Supercritical Fluids)

  • A semi-empirical fluctuation term is presented to improve a classical equation of state (EOS) for volumetric properties in the critical region. The term is based on the two assumptions: (1) The Helmholtz energy is individually divided into classical and long-range density fluctuation contribution (2) All molecules form cluster near the critical region due to long-range density fluctuation. To formulate such molecular cluster, we extended the Veytsman statistics originally developed for the cluster due to hydrogen bonding. The probability function in the statistics is modified to represent the characteristics of long-range density fluctuation vanishing far from critical region. The proposed fluctuation contribution was incorporated into the Sanchez-Lacombe EOS and the combined model with 6 adjustable parameters has been tested against experimental VLE data. The combined model is found to well represent flatten critical isotherm for methane and top of the coexistence curve for the tested components. The prediction results for caloric data are in good agreement with the experimental data.
