성견에 식립한 인산칼슘 피복 임플란트가 골조직 유착에 미치는 생물학적인 영향 (The Biological Effects of Calcium Phosphate Coated Implant for Osseointegration in Beagle Dogs)

  • The influence of calcium phosphate (Ca-P) coating on the bone response of titanium implants was investigated two types of titanium implants, i.e. as -machined ,as -machined with Ca-P coating, were prepared. The Ca-P coating produced by OCT Inc technique. These implants were inserted into the left and right femur of beagle dog, After implantation periods of 3 days, 1weeks, weeks, 4weeks, 8weeks, 12weeks. 24weeks, the bone-implant interface was evaluated histologically, histomorphometrically , and removal torque. Histological evaluation revealed no new bone formation around different implant materials after 2weeks of implantation. After 4 weeks, Ca-P coated implants showed a higher amount of bone contact than either of the non coated implants. After 12weeks, bone healing was almost completed. And implant were removed by reverse torque rotation with torque-measuring device. Mean torque values for 4weeks control were 2.375Kgf.cm and experimental were 2.725Kgf.cm. And mean torque values for 8weeks control were 1.25Kgf.cm and experimental were 1.0Kgf.cm On the basis of these findings, we concluded that deposition of a Ca-P coating on an implant has a beneficial effect on the bone response to this implant during the healing phase. Besides implant surface conditions the bone response is also determined by local implant site condition.

  • The purpose of this study was to compare the bone formation, maturation around HA-and titaniumcoated dental implants in dog. 5 hydroxyapatite coated IMZ implants and 5 titanium coated IMZ implants were placed into the previously extracted site in the mandible of 5 adults dogs. All dogs were injected intravenously Tetracycline, Alizalin red S, and Calcein for bone fluorescent labelling, After the experimental period of 16weeks, the dogs were sacrificed and tissue samples around the implants were obtained. Microscopic observations(ligth, polariged and fluorescence microscope), morphometric analysis, line profile with EPMA, and quantitative analysis for Ca,P, and Ti were performed. The results were as follows ; 1. Bone maturations around the implants were relatively lower than those of natural teeth. No significant differences in bone maturation and remodeling patterns were observed between the two implants groups. 2. Calcification of bone surrounding the implants was initiated in 8-11 weeks for HA-coated implants, while it took 11 weeks or more for Ti-coated implants. 3. Bone-to-implants contact ratio of 82.63% was recorded for HA-coated group and 72.25% for titanium coated group, with no significant difference between the two groups. 4. Bone around the implants exhibited reduced quantity of Ca and P in the $100{\mu}m$ region relative to natural teeth, while the rest of the regions showed no statistical differences. No significant differences were found between the two implant groups. 5. There was a separation of HA layer from the implant core and subsequent infiltration of inflammatory cells into the resulting space in the HA-coated implants, and evidences of phagocytosis of HA particles by macrophages. Bone calcification was more rapid around HA-coated implants compared to titanium-coated implants, but HA coated implants did not show any significant differences either in the degree of calcification or the bone-to-implant contact ratio over Ti coated implants. HA coated implants may have complications associated with HA absorption and separation of HA layer from the implant core.

The effect of fibronectin-coated implant on canine osseointegration

  • Purpose: The purpose of this study was to characterize the osseointegration of the fibronectin-coated implant surface. Methods: Sand-blasted, large-grit, acid-etched (SLA) surface implants, with or without a thin calcium phosphate and fibronectin coating, were placed in edentulous mandibles of dogs 8 weeks after extraction. All dogs were sacrificed forhistological and histomorphometric evaluation after 4- and 8-week healing periods. Results: All types of implants were clinically stable without any mobility. Although the bone-to-implant contact and bone density of the SLA implants coated with calcium phosphate (CaP)/fibronectin were lower than the uncoated SLA implants, there were no significant differences between the uncoated SLA surface group and the SLA surface coated with CaP/fibronectin group. Conclusions: Within the limits of this study, SLA surfaces coated with CaP/fibronectin were shown to have comparable bone-to-implant contact and bone density to uncoated SLA surfaces.

임플란트 표면의 Ca-P 코팅 방법이 MG63 골모유사세포 반응에 미치는 영향에 대한 in vitro 연구 (The effect of Ca-P coatings of anodized implant surface on response of osteoblast-like cells in vitro)

  • 연구목적: 본 연구에서는 양극산화 임플란트 표면에 서로 다른 두 가지 방법, Ion beam-assisted deposition (IBAD)법과 Sol-gel법으로 Ca-P 코팅한 임플란트 시편에 골모세포를 배양하였을 때 세포의 증식, 분화, 형태에 어떠한 영향을 미치는지 조사하고자 한다. 연구재료 및 방법: 지름 10 mm, 두께 2 mm 인 상업용 순수 titanium grade IV 재질의 디스크를 제작하였고, 모든 시편은 acetone, 70% ethanol, 증류수에서 각각 10분씩 세척 후 건조하였다. 모든 표면은 300 V의 constant voltage하에서 양극 산화 (anodized)시킨다. 실험군은 양극산화 임플란트 표면에 각각 IBAD법과 Solgel법으로 Ca-P 코팅하였다. 각 표면의 미세표면 거칠기(Ra)를 측정하였고, SEM을 통해 표면의 형상을 관찰하였다. 골모세포을 배양한 후 각 표면군의 세포 증식, ALP 활성도 및 RT-PCR를 통한 골세포 분화 능력 검증을 하였으며, SEM을 통해 세포의 형상도 확인하였다. 통계분석은 SPSS (version 12.0) 프로그램을 이용하여 Kruskal-Wallis Test로 각 군의 유의성을 검증하였다 ($\alpha$=0.05). 결과: IBAD법으로 Ca-P 코팅한 표면이 Sol-gel법으로 Ca-P 코팅한 표면보다 표면 거칠기 (Ra) 값이 더 크게 나타났다 (P<.05). IBAD법으로 Ca-P 코팅한 표면이 Sol-gel법으로 Ca-P 코팅한 표면 보다 세포 증식이 더 활발하고 골세포 조기 분화 정도를 확인 할 수 있는ALP 활성도 또한 더 높게 나타났다 (P<.05). SEM 관찰 결과IBAD법으로 Ca-P 코팅한 표면에 골모세포들이 친화성을 띄면서 안정적으로 부착되었다. 결론: IBAD법으로 Ca-P 코팅한 표면이 Sol-gel법으로 Ca-P 코팅한 표면보다 더 우수한 세포 반응을 보였다. IBAD법으로 Ca-P 코팅한 표면의 세포들은 증식이 잘 이루어지고 잘 분화된 골모세포 형상을 보이고 ALP 활성도 또한 높아 골 형성을 증가시켜 높은 골-임플란트 접촉을 보일 것이다.

Dissolution behavior and early bone apposition of calcium phosphate-coated machined implants

  • Purpose: Calcium phosphate (CaP)-coated implants promote osseointegration and survival rate. The aim of this study was to (1) analyze the dissolution behavior of the residual CaP particles of removed implants and (2) evaluate bone apposition of CaP-coated machined surface implants at the early healing phase. Methods: Mandibular premolars were extracted from five dogs. After eight weeks, the implants were placed according to drilling protocols: a nonmobile implant (NI) group and rotational implant (RI) group. For CaP dissolution behavior analysis, 8 implants were removed after 0, 1, 2, and 4 weeks. The surface morphology and deposition of the coatings were observed. For bone apposition analysis, block sections were obtained after 1-, 2-, and 4-week healing periods and the specimens were analyzed. Results: Calcium and phosphorus were detected in the implants that were removed immediately after insertion, and the other implants were composed mainly of titanium. There were no notable differences between the NI and RI groups in terms of the healing process. The bone-to-implant contact and bone density in the RI group showed a remarkable increase after 2 weeks of healing. Conclusions: It can be speculated that the CaP coating dissolves early in the healing phase and chemically induces early bone formation regardless of the primary stability.

Bone apposition on implants coated with calcium phosphate by ion beam assisted deposition in oversized drilled sockets: a histologic and histometric analysis in dogs

  • Purpose: The purpose of this study was to evaluate the osseointegration of calcium phosphate (CaP)-coated implants by ion beam assisted deposition with a lack of primary stability. Methods: A total of 20 CaP-coated implants were bilaterally placed in the mandible of five dogs. In the rotational implant group, the implants were inserted in oversized drilled sockets without mechanical engagement, while the conventional surgical protocol was followed in the control group. Each group was allowed to heal for 4 and 8 weeks. The bone-to-implant contact (BIC, %) was measured by a histometric analysis. Results: All of the implants were well-maintained and healing was uneventful. In the histologic observation, all of the implants tested were successfully osseointegrated with a high level of BIC at both observation intervals. There was no significant difference in BIC among any of the groups. Conclusions: Within the limitation of this study, successful osseointegration of CaP-coated implants could be achieved in unfavorable conditions without primary stability.

  • Surface characteristics of HA(hydroxyapatite) coated dental implant alloy by Sol-Gel method were investigated using potentiostat, ICP, SEM, EDX, EPMA and surface roughness tester. Surface roughness of HA coated specimen by Sol-Gel showed higher than that of PVD coated specimen. Corrosion resistance increased in the order of $1\%$ lactic acid, artificial saliva, $0.5\%$ HCI and $0.9\%$ NaCl solution. Amount of Ca element release was higher than that of V and P in the $0.5\%$ HCI and $0.9\%$ NaCl solution.

Influence of Implant Surface Coated with pH Buffering Agent on Early Osseointegration

  • Purpose: Surface treatment with pH buffering agent has been developed to achieve higher and faster osseointegration. The aim of this study was to evaluate its influence by measuring removal torque and analyzing histological characteristics. Materials and Methods: Titanium implants with following surfaces were used in this study: sand-blasted acid-etched (SA) surface (SA group as control I group), SA surface in calcium chloride aqueous solution (CA group as control II group) and SA surface coated with pH buffering agent (pH group as test group). Removal torque test after 2 weeks and bone-to-implant contact and bone area analyses at 2 and 4 weeks were performed. Result: The rotational torque values at 2 weeks were significantly higher in pH group ($107.5{\pm}6.2Ncm$, P<0.05). The mean values of bone-to-implant contact at 2 and 4 weeks were both higher in pH group ($93.0%{\pm}6.4%$ at 2 weeks, $88.6%{\pm}5.5%$ at 4 weeks) than in SA group ($49.7%{\pm}9.7%$ at 2 weeks, $47.3%{\pm}20.1%$ at 4 weeks) and CA group ($73.7%{\pm}12.4%$ at 2 weeks, $72.5%{\pm}10.9%$ at 4 weeks) with significances (P<0.05). The means of bone area showed significantly higher numbers in pH group ($39.5%{\pm}11.3%$ at 2 weeks, $71.9%{\pm}10.9%$ at 4 weeks, P<0.05). Conclusion: Our findings demonstrated that surface modification with pH buffering agent improved early osseointegration with superior biomechanical property.

Bioactive characteristics of an implant surface coated with a pH buffering agent: an in vitro study

  • Purpose: The purpose of this study was to evaluate the effectiveness of conventional sandblasted, large-grit, acid-etched (SLA) surface coated with a pH buffering solution based on surface wettability, blood protein adhesion, osteoblast affinity, and platelet adhesion and activation. Methods: Titanium discs and implants with conventional SLA surface (SA), SLA surface in an aqueous calcium chloride solution (CA), and SLA surface with a pH buffering agent (SOI) were prepared. The wetting velocity was measured by the number of threads wetted by blood over an interval of time. Serum albumin adsorption was tested using the bicinchoninic acid assay and by measuring fluorescence intensity. Osteoblast activity assays (osteoblast adhesion, proliferation, differentiation, mineralization, and migration) were also performed, and platelet adhesion and activation assays were conducted. Results: In both the wetting velocity test and the serum albumin adsorption assay, the SOI surface displayed a significantly higher wetting velocity than the SA surface (P=0.000 and P=0.000, respectively). In the osteoblast adhesion, proliferation, differentiation, and mineralization tests, the mean values for SOI were all higher than those for SA and CA. On the osteoblast migration, platelet adhesion, and activation tests, SOI also showed significantly higher values than SA (P=0.040, P=0.000, and P=0.000, respectively). Conclusions: SOI exhibited higher hydrophilicity and affinity for proteins, cells, and platelets than SA. Within the limits of this study, it may be concluded that coating an implant with a pH buffering agent can induce the attachment of platelets, proteins, and cells to the implant surface. Further studies should be conducted to directly compare SOI with other conventional surfaces with regard to its safety and effectiveness in clinical settings.

  • Statement of problem: Surface texture of the implant is one of the important factors of the implant success, especially in the immediate implant loading. Many methods of the surface treatment of implant have developed and introduced. Purpose : This study was to evaluate the effects of the Ca-P coating implant crystallized the hydroxyapatite on the surface by the removal torque test and the histomorphometric analysis in vivo. Material and methods: 135 screw type implants, 4.0mm in length and 3.75mm in diameter were used in this study. Implants were divided into 3 groups and treated in the different mothods. Group I was not treated, Group II was treated in the SLA method, and Group III was treated in the Ca-P coating with the anodizing method and the hydroxyapatite was crystallized on the surface with the hydrothermal treatment. Firstly, the surface roughness of each group was measured, 45 rabbits were used in this experiment. Two implants were inserted on right tibial metaphysis and one implant was inserted on left side with the alternating order. After the healing periods of 3, 5, and 12 weeks, the rabbits were sacrificed to evaluate the osseointergration by the removal torque test and the histomorphometric analysis. Results : 1. In the analysis for the surface roughness, Group II showed the highest roughness. And Group III showed higher secondly. There was a significant difference one another statistically 2. In the removal torque test, Group III and II were significantly higher than Group I. There was no statistical difference between Group III and Group II. 3. For all Groups, the removal torque values at 12th week were significantly higher than at 3rd and 5th week. 4. In histomorphometric analysis, the bone implant contact rates of Group III and II were higher than that of Group I at 3rd and 5th week. There was a significant difference at 5th week. 5. In histomorphometric analysis, the bone implant contact rate of Group III and II increased from 3rd week to 5th week, but decreased at 12th week. In Group I, the contact rate at 12th week was significantly higher than at 3rd week and 5th week.
